OBJECTIVE:To investigate the effect of preoperative use of high-dose simvastatin on the pulmonary artery pres-sure in congenital heart disease with left-to-right shunt. METHODS:86 patients with left-to-right shunt congenital heart disease who were planned to implement interventional occlusion therapy were randomly divided into control group(43 cases)and observa-tion group(43 cases). Patients in both groups were given digitalis,diuretics,anticoagulants and other basic treatments;On this basis, control group received Sildenafil citrate tablet,25 mg,3 times a day,orally given 1 h before a meal;observation group received Simvastatin tablet 80 mg,once every evening,orally. The treatment course for both groups was 7 d. Mean pulmonary hypertension (mPAP),N terminal prohormone brain natriuretic peptide (NT-proBNP),arterial oxygen saturation (SaO2),arterial oxygen ten-sion(PaO2),pulmonary blood circulation ratio(Qq/Qs),pulmonary pressure variation(Rp/Rs),pulmonary vascular resistance in-dex(PVRi)before and after treatment in 2 groups were observed,and related indicators of liver functions [alanine aminotransfer-ase (ALT),aspartate aminotransferase(AST)] and myocardial enzymes [lactate dehydrogenase (LDH),creatine kinase MB(CK-MB)] were detected. The incidence of adverse reactions were deteted. RESULTS:Before treatment,there were no significant differ-ences in mPAP,NT-proBNP,SaO2,PaO2,Qq/Qs,Rp/Rs,PVRi,ALT,AST,LDH and CKMB levels in 2 groups(P>0.05). Af-ter treatment,mPAP,NT-proBNP,Rp/Rs and PVRi levels were significantly lower than before,SaO2,PaO2,and Qq/Qs levels were significantly higher than before and Qq/Qs level in observation group was higher than these in control group.The difference was statistically significant(P<0.05). There was 1 patient showed severe liver injury in observation group and exited the test;the other 85 patients showed no obvious adverse reactions,with successful surgery and good prognosis. CONCLUSIONS:The preoper-ative use of high-dose simvastatin shows obvious effect on controlling mean pulmonary hypertension,reducing mPAP,NT-proB-NP,Rp/Rs and PVRi levels and increasing SaO2,PaO2,and Qq/Qs levels before operation,with good safety.

Objective To investigate the expression of inflammtory factor genes change induced by sodium sulfite.Methods MTS assay was used to detect the cell toxicity to human embryonic kidney cell line 293(HEK293) of 0,0.0025,0.01,0.039,0.156,0.625,2.5,10 mmol/L sodium sulfite,morphological changes were observed with inversion microscope and RT-PCR was used to study the expression of mRNA changes of TNF-?,MCP-1 and IL-8.Results Cytotoxicity analysis showed that treatment of cells with 0.625,2.5,10 mmol/L Na2SO3 could significantly decrease the OD value,with the OD value of(0.354 75 ?0.021 24),(0.600 50?0.012 77),(0.784 75?0.009 85) respectively,compared with control group(2.514 5?0.202 265).When treated with ≤0.156 mmol/L Na2SO3,it sould not significantly affect cell viability,with the OD value of(2.473 75?0.069 99)-(2.625 00? 0.120 29).Morphological observation showed that exposure of ≥0.625 mmol/L Na2SO3 could decrease cell numbers significantly and living cells seemed narrower and longer than the usual way with fewer evection.But lower concentrations of Na2SO3(≤0.156 mmol/L) did not change cell numbers and cell morphology.RT-PCR result showed that treatment of 0.039-10 mmol/L Na2SO3 could not induce the expression of TNF-?,MCP-1 and IL-8.Conclusion Na2SO3 can cause significant inhibition and injury in HEK293,but can not up regulate the expression of mRNA of TNF-?,MCP-1 and IL-8,and there is no obvious relation between them.

AIM:To study the effect of chronic corticosterone ( CORT) injection on the depression-like behav-iors and the brain glycogen level in mice.METHODS:Male C57BL/6N mice (n=40) were randomly divided into nor-mal control group and model group.The mice in model group were subcutaneously consecutively injected with CORT for 4 weeks.The mouse model of chronic stress depression was constructed.The forced swim test and open field experiment were conducted to prove chronic stress model.The serum level of CORT in the mice was measured by radioimmunoassay.The protein levels of hippocampal synaptophysin ( SYP) and brain-derived neurotrophic factor ( BDNF) were detected by West-ern blot.Hippocampus glycogen, glycogen synthase and glycogen phosphorylase were determined by indirect fluorescence measurement.RESULTS:Compared with normal control group, the immobility time of the forced swim test in model group was significantly lengthened (P<0.01), and the ability of spontaneous activity was reduced (P<0.01), indicating that chronic CORT injection induced depression-like behaviors in mice.The CORT level increased significantly (P<0.01) in model group.CORT injection decreased the protein expression of hippocampal SYP and BDNF (P<0.01), reduced hipp-ocampal glycogen level (P<0.05) and glycogen synthase activity (P<0.05), and increased glycogen phosphorylase ac-tivity (P<0.05).CONCLUSION:Chronic CORT injection causes hippocampal neuron damage and induces the depres-sion-like behaviors of mice, which may be associated with decreasing hippocampal glycogen level by CORT.

Objective To study the effects of long-term injection of corticosterone (CORT) on the hippocampal astrocytes and synaptic plasticity in mice.Methods Male C57BL/6N mice (n=40) were randomly divided into control group and model group.Mice in the model group were treated with subcutaneous injection of CORT for 4 weeks to generate chronic stress depression model.Chronic stress model was proved to be established by tail suspension test and sucrose preference test and serum level of CORT was determined by radioimmunoassay.Protein expression levels of hippocampal synaptophysin (SYP) and postsynaptic density-95 (PSD-95) were detected by Western blot.Somal volume and protrusion length of glial fibrillary acidic protein-positive astrocytes were assayed by immunocytochemistry and quantitative stereological techniques.Results Immobility time in model group ((137.95±6.22) s) was significantly extended in comparison with that in the control group ((114.05 ± 4.12) s) (P＜ 0.01).Sucrose preference in the model group (62.42 ± 6.75)％ was lower than that in the control group (86.52±5.08)％ (P＜0.01).CORT levels in model group ((11.48±0.62) ng/ml) was significantly increased compared with that in the control group ((1.11±0.05) ng/ml) (P＜0.01).Hippocampal SYP (0.54±0.04) and PSD-95 (0.57±0.07) expression levels in model group were lower than those in the control group (0.99±0.14),(1.03±0.10) (P＜0.01).Somal volume in the model group ((132.04±9.23) μm3) and protrusion length ((1.39± 0.05) × 107 μm) was lower than that in the control group ((168.49±9.01)μm3),(1.77±0.10) × 107 μm) (P＜0.05,P＜0.01).SYP and PSD-95 expression level was found to be positively correlated with the reduction of the protrusion length(r=0.660,P＜0.01;r=0.614,P＜0.01)in astrocytes.Conclusion The results suggest that depression-like behavior-in-duced by long-term CORT treatment is possibly caused by the alternation of the synaptic plasticity associated with the reduction of the reducing the protrusion length.

Objective:To study TGC on tumor apopotosis and to apply the experimenting basis of TGC's developing and utilizing.Methods:The flow cytometer(FCM) was used for determination apoptosis.The protein expression of Bcl-2 and the mRNA expression of c-myc were evaluated by immunohistochemistry and in situ hybridization.Apoptosis small body were observed by EU.Results:The TGC group appear the Ap apex,the number of cell of G0-G1 period reduce obviously,the cells of S period increase obviously.Bcl-2 and the c-myc mRNA expression levels were decreased in TGC group.The structure of inner membrane was intacted with EU.Chromatin gathered side of cell,the nucleus turns into the nucleus band and nucleus project,there were many cells of the apoptosis and meniscus or the garland shape(the apoptosis corpuscle formatiol) in TGC group.Conclusion:The total glucosides of radix paeoniae induces the tumor cell apoptosising,its mechanism maybe closely related with adjusting expression levels of the Bcl-2 and the c-myc mRNA.

Objective Experiment the tumor animal model of the H22 Cancer of the liver mile,study the inhibition effect of chitosan oligosaccharide and bifidobacteria on tumour.Methods the growth of tumour were observed after chitosan oligosaccharide and bifidobacteria were injected into tumor-bearing mice;then the immunify funcfion weremcasured.Results Growth of tumour were inhibited by chitosan oligosaccharide and bifidobacteria,they can increase the content of IL-2 and IFN-?and the weight of spleen and thymus of the mice.Conclusions chitosan oligosaccharide and bifidobacteria could inhibit the growth of tumour,inproue immunity function.

Objective To investigate the association betweent clinical markers of N‐terminal probrain natriuretie peptide (NT‐proBNP) ,cardiac troponin I (cTnI) and C‐reaction protein (CRP) in the patients with acute respiratory distress syndrome (ARDS) caused by severe pneumonia and to reveal the pathophysiologic process of myocardial injury after ARDS .Methods Totally 33 patients diagnosed as ARDS caused by severe pneumonia were divided into 3 groups:mild group ,moderate group and severe group according to the New Berlin definition of ARDS .Arterial blood gas analysis ,NT‐proBNP ,cTnI and CRP were observed be‐fore the treatment .Statistics were used for the analyses .Results The NT‐proBNP level in the severe group was significantly higher than that in the mild and moderate groups (P<0 .01);the NT‐proBNP level in the moderate group was also higher than that in the mild group (P< 0 .05) .Besides ,the cTnI level in the severe group was significantly higher than that in the mild and moderate groups (P<0 .01);however ,the cTnI level had no statistical difference between mild and moderate groups (P> 0 .05) .The CRP level in the severe group was significantly higher than that in the mild (P<0 .05) .Moreover ,the NT‐proBNP level of 33 patients was positively associated with the both CRP and cTnI level (P<0 .05) .Conclusion The seriously inflammatory response mediates myocardial injury probably because of ARDS .Combined NT‐proBNP and cTnI testing is important for evaluating the condition of ARDS at the early stage .

Objective:To explore the feasibility of using Hirsch index(H index) to evaluate the performance of academic papers published by medical talents.Methods:67 medical talents of Wuxi People′s Hospital Affiliated to Nanjing Medical University were enrolled in the study. The numbers of papers published and citations of these talents which have been included by PubMed, Chinese Citation Database, CNKI, Wanfang, and Baidu Scholar. The H value of these talents was calculated according to the list method proposed by Rousseau R. The H index was compared with the traditional evaluation indexes of scientific research performance, such as the number of papers published, the total number of citations and the average number of citations. Pearson test was used to explore the correlation between H index and traditional evaluation index.Results:The H value of 67 talents ranged from 1 to 14. Pearson test showed that the H index was significantly correlated with the total number of papers, the total number of citations and the average number of citations at the level of 0.01, and their Pearson value was 0.746, 0.932, and 0.648 respectively.Conclusions:As a new scientific research performance evaluation system, the H index is recommended in the evaluation of medical personnel, but it needs further and sustained optimization in terms of database inclusion timespan, research work timespan, and authors ranking limit among others.

Objective To explore the effect of antenatal taurine supplementation on cerebral apoptosis and the expression of glial cell line-derived neurotrophic factor (GDNF) and caspase-3 in fetal rats with fetal growth restriction (FGR).Methods Fifteen pregnant Sprague-Dawley rats were randomly divided into three groups:control group,FGR model group (model group) and FGR with antenatal taurine supplementation group (taurine group).Taurine was added into the diet of taurine group at a dose of 300 mg/(kg · d) from the 12th day of gestation until natural delivery.Two appropriate for gestational age (AGA) newborn rats were randomly selected from each mother in control group and two FGR fetal rats were randomly selected from each mother both in model and taurine groups.Apoptosis of neural cells in the brain was detected by terminal deoxynucleotidyl transferase mediated nick end labeling (TUNEL).Changes in protein expression of GDNF and caspase-3 were detected by immunohistochemistry.Levene method,one-way ANOVA and SNK-test,or Kruskal Wallis rank sum test and Tamhane’ s test were applied for statistical analysis.Results (1)The total amounts of fetal rats in control group,model group and taurine group were 65,60 and 59.The mean body weight of fetal rats were (6.36±0.44) g,(4.55 ± 0.45) g and (5.11±0.67) g,respectively.All fetal rats developed FGR in model group,while 76.3％(45/59) of fetal rats were FGR in taurine group.Therefore,FGR model was successfully established.(2) In control group,there were few expression of TUNEL positive cells in cerebral cortex.A large amount of TUNEL positive cells were found in the cortex,hippocampal and white matter area in model group,but less positive cells were identified in taurine group than in model group.The amount of apoptotic brain cells in the three groups were (0.46 ± 0.11),(14.76 ± 3.42) and (6.78 ± 1.93),respectively(H=429.80,P=0.000).(3)There were only small amount of GDNF positive cells in cerebral cortex in control group and more in model group.The amount of GDNF positive cells was further increased in taurine group.The amount of GDNF positive cells in cerebral cortex in the three groups were (93.56± 6.73),(120.36± 6.23)and(139.56± 5.28),respectively (H=715.17,P=0.000).(4) Few caspase-3 positive cells were found in cerebral cortex in control group.A large amount of positive cells were found in model group and less positive cells were found in taurine group,which was still more than those in control group.The amounts of caspase-3 positive cells in the three groups were (7.50±2.31),(151.32±24.43)and(37.28±11.21),respectively (F=132.54,P=0.000).Conclusions The number of apoptotic neural cells in brain tissue of baby rats with FGR were significantly increased,which can be significantly alleviated by maternal antenatal taurine supplementation through upregulation of GDNF and downregulation of caspase-3 expression.
