1.Current situation of glove perforation during surgical operation

Objective To realize the current situation of glove perforation during surgical operation process,and provide basis for preventing surgical glove perforation.Methods In January 2014,an investigation on glove perfo-ration was conducted among operation personnel in 9 operating rooms in a hospital,glove perforation rates,sites, causes and noticed ways were analyzed.Results A total of 2 909 person-time was investigated,147 person-time oc-curred glove perforation,person-time rate of glove perforation was 5.05%;153 of 5 818 gloves (2.63%)were per-forated,6 person-time occurred perforation of double gloves;gloves perforation rates of each specialized surgery were significantly different (χ2=87.945,P<0.001),cardiac surgery(11.84%)and plastic surgery(10.78%)had the hight perforation rates.The common sites of perforation were index finger,thumb,and middle finger,the main cause of perforation was sharp damage by stitches and devices.Most perforations were noticed intraoperatively,and seldom found postoperatively by naked eyes;gloves of different brands had different perforation rates(χ2=33.845, P<0.001).Conclusion In order to reduce and prevent the perforation during operation process,glove brands with good quality should be chosen,and measures for the prevention of glove perforation should be formulated according to the types of surgery.
2.THE INCIDENCE OF TEMPOROMANDIBULAR JOINT DISORDER IN AVIATORS FLYING DIFFERENT TYPES OF AIRCRAFT

To explore the impact of acceleration load on the temporomandibularjoint in aviators and its clinical significance.617 aviators flying aircraft of different types without previous history of temporomandibularjoint disease were studied. Abnormal manifestations of the temporomandibularjointswere analysed. The incidence of temporomandibular joint symptoms was markedly higher in the fighter plane pilots than that in the transportation planegroup. Load induced by acceleration could be one of the main causative factors of temporomandibularjoint disorder.
3.Analysis of the Effect of Double-wavelength Diode Laser for Relase of Pain

Objective: Adopting quantified index to observe and analyze the effect of acute and chronic pain through double-wavelength diode laser. Methods: Using diode laser with the wavelength of 650nm / 810nm to irradiate the special tissues or aching points of patient, selecting different frequency, intensity and time to make treatment. Results: VAS scoring: P<0.05, the total efficient rate is 96%. Conclusions: Treatment with double-wavelength diode laser to various acute and chronic pain is just a definitely good curative effect, and it can release the effects of patient sleeping, daily life, communication with people and life interests caused by pain, and finally enhancing the quality of life.
4.Clinical features and the efficient therapy of 66 acute promyelocytic leukemia patients with high initial white blood cell count

Objective To investigate the clinical features and the efficient therapy of acute promyelocytic leukemia(APL)with high initial white blood cell(WBC)count.Methods The clinical features of 66 newly diagnosed APL patients with high initial WBC and 152 patients without high initial WBC from the First Affiliated Hospital of Soochow University between October 1993 and August 2006 were retrospectively analyzed.Additionally,66 patients with high WBC were divided into different groups according to the different induction therapy and the therapeutic effects were compared.Results The early death rate and the incidence of disseminated intravascular coagulation(DIC)and retinoic acid syndrome(RAS)were 30.3%,57.6% and 31.8% respectively for the group with high initial WBC,which were higher than the indexes of 7.2%,38.1% and 21.05% respectively for the group without high initial WBC(P
5.Electrochemical sensor for acetaminophen based on layer-by-layer self assembly technique.

A novel type of carbon nanotube-coated Au nanoparticle and [bmim]BF4 composite modified glassy carbon electrode was fabricated by a layer-by-layer self-assembly technique. The electrochemical performance of acetaminophen (ACOP) on the modified electrode was investigated by cyclic voltammetry. The Nafion/GNPs/RTIL/MWNTs/GC electrode showed an excellent electrocatalytic activity for the oxidation of ACOP and accelerated electron transfer between the electrode and ACOP. For ACOP, the reversible electrochemical process was observed on the Nafion/GNPs/RTIL/MWNTs/GC electrode, while irreversible electrochemical process occurred on the GC electrode. For the Nafion/GNPs/RTIL/MWNTs/GC electrode, the anodic peak potential of ACOP was moved from 0.562 V to 0.413 V, with a potential drop of 149 mV. At the same time, the reduction peak potential was 0.384 V, and the potential difference was only 29 mV. It was shown that the modified electrode possessed higher electrocatalytic activity and more sensitive effect for the detection of ACOP than both MWNTs/GC electrode and GC electrode. The effects of the different experimental conditions on the electrochemical behaviors of ACOP were explored. Under the optimum conditions of preparation and experimental, the linear calibration curves of ACOP were obtained in a wide range of 2 x 10(-1) to 4.0 x 10(-4) mol x L(-1) with a correlation coefficient 0.999 2 and a detection limit of 2.6 x 10(-8) mol x L(-1) (the ratio of signal to noise, 3:1). The recovery rate was 97.9%-100.8%. This method can be used to determine ACOP in paracetamol tablets with satisfactory results.
6.Functional Research of Cassette Chromosome Recombinase C Mediated Staphylococcal Cassette Chromosome mec Istand Excision

Objective To construct 2 recombinant plasmids carrying cassette chromosome recombinase C(ccrC)and ccrAB respectively and introduce the plasmids into methicillin-resistant Staphlococcus aureus(MRSA)strain 81/0432,and to observe the precise excision of Staphylococcal cassette chromosome mec(SCCmec)island from bacterial chromosome.Methods ccrC and ccrAB genes were amplified with chromosomal DNAs isolated from MRSA strains 81/0342 and N31S as PCR templates.We constructed recombinant plasmids pSR5C and pSR2AB by cloning ccrC and ccrAB genes into temperature-sensitive plasmid pYI3,after introducing them into MRSA strains 81/0432 and N315 by electroporation.PCR was performed to identify SCCmec excision from the bacterial chromosome.The transformants were serial passaged for 10 days,and then the drug resistance of these rransformants was detected by replica experiment.Results The fragment length of ccrC gene was 1.9 kb,smaller than the fragment length of ccrAB from N315.The recombinant plasmids of pSR5C and pSR2AB were successfully constructed.After these 2 recombinant plasmids were introduced into MRSA strain 81/0342,type-V SCCmec island was excised from the chromosome and formed a closed circular structure in the bacteria.However,type-Ⅱ SCCmec island could be excised only in N31S strain after the expression of pSR2AB.Replica experiment verified that transformed strains of 0342(pSR2AB),0342(pSR5C),and N315 (pSR2AB)were mostly susceptible to ceftizoxim or tobramycin after the excision of SCCmec island.Conclusion cciC could serve as a recombinase as ccrAB,which could mediate precise excision of SCCmec island from the chromosome of type-V MRSA strain.This study shows that type-V SCCmec island is widely disseminated between Staphylococcus aweus strains in community setting.
7.Clinical study on vocal cords spontaneous rehabilitation after CO2 laser surgery.

OBJECTIVE:
To study the spontaneous rehabilitation and phonation quality of vocal cords after different types of CO2 laser microsurgery.
METHOD:
Surgical procedures based on Remacle system Type I, Type II, Type III, Type IV and Type V a respectively. Three hundred and fifteen cases with hoarseness based on strobe laryngoscopy results were prospectively assigned to different group according to vocal lesions apperence,vocal vibration and imaging of larynx CT/MRI. Each group holded 63 cases. The investigation included the vocal cords morphological features,the patients' subjective feelings and objective results of vocal cords.
RESULT:
There are no severe complications for all patients in perioperative period. Vocal scar found in Type I ,1 case; Type II, 9 cases ;Type III, 47 cases; Type IV, 61 cases and Type Va 63 cases respectively after surgery. The difference of Vocal scar formation after surgery between surgical procedures are statistical significance (χ2 = 222.24, P < 0.05). Hoarseness improved after the surgery in 59 cases of Type I , 51 cases of Type II, 43 cases of Type III, 21 cases of Type IV and 17 cases of Type Va. There are statistically significance (χ2 = 89.46, P < 0.05) between different surgical procedures. The parameters of strobe laryngoscope: there are statistical significance on jitter between procedures (F 44.51, P < 0.05), but without difference within Type I and Type II (P > 0.05). This happened in shimmer parameter and the maximum phonation time (MPT) as jitter. There are no statistical significance between Type IV and Type Va on MPT (P > 0.05).
CONCLUSION
Morphological and functional rehabilitation of vocal cord will be affected obviously when the body layer is injured. The depth and range of the CO2 laser microsurgery are the key factors affecting the vocal rehabilitation.

8.Treatment of recurrent laryngeal papilloma by submucosal resection and the effect on prognosis.

OBJECTIVE:
To investigate the efficacy of submucosal resection by CO2 laser in the treatment of recurrent laryngeal papilloma and the effect on prognosis.
METHOD:
A total of 11 patients diagnosed as recurrent laryngeal papilloma were included in this review. Papilloma was marked before operation and checked under fibro-laryngoscope. Papilloma was resected completely including the submucosal tissure with CO2 laser or microequipment. In widespread papilloma, false membrane in raw surface were cleared 7-10 days after operation. Surgical specimens (including membrane) were detected by routine pathology, HPV typing and immunohistochemical pathologic examination. The patients were checked once a month in the first 3 months after operation, and then once for every 3 months. Once the hoarseness and other symptoms aggravated or the disease was recurrent, the patients were treated immediately.
RESULT:
HPV viral DNA was found in 10/11 cases, with HPV11 (7/11 cases) and HPV6 (3/11 cases). Cases with regards to follow-up, from 6 months to 1 year, 3 cases were followed up 1 year after operation, without recurrence. Five patients including 2 children were followed up 6 to 12 months after operation, without recurrence. Two children underwent 2 or 3 operations, were followed-up more than 6 months withouting recurrence.
CONCLUSION
Papilloma submucosal resection could decrease postoperative recurrence and is worth to be further investigated.

10.Enhancing effect of DHEA ON DbcAMP-induced cell differentiation

Aim To observe whether DHEA has enhancing effect on DbcAMP -induced differentiation of NG108-15 cells, including neurite outgrowth, and study its possible mechanisms. Methods NG108-15 cells (a h ybrid cell line of mouse neuroblastoma and rat glioma) were used as a substitute for primary culture neuron in vitro. The morphology of NG108-15 cells was o bserved and neurite outgrowth was determined in an inversed microscope after treatme nt with various drugs. Gelatin-substrate gel electrophoresis was used to detect gelatinases (MMP-9 and MMP-2). Results ① DHEA and DbcAMP inhibited NG108-15 proliferation.②DHEA had enhancing effect on the promoting activity of neuronal differentiation and neurite outgrowth by DbcAMP. DbcAMP could increase neurite elongation of NG108-15 cells. Compared with this, the combined treatment with DHEA and DbcAMP significantly enhanced the neurite outgrowth of NG108-15 cells, including neurite length and numbers of cells with neurite, in a DHEA dose-dependent manner. ③ MMPs were involved in neuronal differentiation. DbcAMP induced the increase in MMP-9 and MMP-2 activities and such elevation was enhanced by DHEA in a dose-dependent manner. Conclusion DHEA enhances the effect of DbcAMP in promoting the neurite outgrowth of NG108-15 cells, which might be related to the increase in MMP-9 and MMP-2 activities.
