Objective To observe the influences of Shenzhi Huoxue capsule to prognosis of patients with hypertensive intracerebral hemorrhage after hard channel puncture and drainage.MethodsTo divide 106 patients with hypertensive intracerebral hemorrhage from May 2013 to April 2015 into two groups.The observation group with 53 cases received Shenzhi Huoxue capsule after hard channel puncture and drainage, the control group did not receive Shenzhi Huoxue capsule.To observe the complications rate, the number of deaths during hospitalization, the number of persons with disabilities after treatment, GOS score, BI index, SSS score and NCSE score of two groups.Results(During treatment, the infection, liver and renal dysfunction and rebleeding rates of observation group were lower than control group, and the infection ratio was significant (P<0.05).(During treatment, the proportion of deaths of observation group was 18.87%,lower than control group with 26.42%, but the differences were not statistically significant.The proportion of disabilities of observation group was 26.42%,lower than control group with 45.28%, and the differences were statistically significant (P<0.05).After treatment, GOS score and BI index of two groups were higher than before treatment(P<0.05).GOS score and BI index of the observation group were (12.85±0.81,83.95±4.37)score, higher than the control group with (10.05±0.73,74.95±4.06)score, and the differences all were statistically significant (P<0.05).After treatment, the SSS score and NCSE score of two groups were higher than before treatment(P<0.05),the SSS score and NCSE score of observation group were (47.10±4.18,66.72±3.99)score, higher than the control group with (38.42±3.05,59.34±3.45)score, and the differences were statistically significant (P<0.05).ConclusionAdding Shenzhi Huoxue capsule to patients with hypertensive intracerebral hemorrhage after hard channel puncture and drainage can reduce the number of persons with disabilities and improve life self-care ability and cognitive ability.

OBJECTIVE: To investigated the effects of Src family kinases on the expression of mRNA and protein of Nav1.1 in spiral ganglion neurons. METHOD: RT-PCR and Western blot techniques respectively explored the level of expression of mRNA and protein of Nav1.1 in spiral ganglion neurons by Src family kinases inhibitor. RESULT: An application of the inhibitor of Src family kinases which was PP2 (10 micromol/L) and SU6656 (2 micromol/L) gived rise to the mRNA decreasing to 26% +/- 0.8% and 36% +/- 1.5% respectively (P < 0.05), and protein reducing to 39% +/- 12.5% and 53% +/- 1.7% severally (P < 0.05). CONCLUSION Administration of the inhibitor of Src family kinases could decrease the expression of mRNA and protein of Nav1.1 in spiral ganglion neurons.

Objective Observe the effect of of U8 similar bioelectricity stimulant therapy of infertility with gracile endometrium on endometrium reception .Methods 80 cases of patients in 252 natrual period with infertility caused by poor endometrial develop-ment for unknown reasons were randomly divided into two groups .The treatment group was intervened by U8 similar bioelectricity stimulant other day from the eighth day of menstruation until the day to inject HCG .After ovulation ,patients were sustained by progestin with a dose of 20 mg per day .The control group was given estradiol valerate with a dose of 2mg per day from the eighth day of menstruation until the day to inject HCG .After ovulation ,patients were sustained by progestin with a dose of 20mg per day as well .To compare the generral conditions on HCG injection day of the two groups anterior-posterior :both groupsal thickness and types ,parameters of intramembrane and submucosal uterine blood flow ,biochemical pregnancy rates and clinical pregnancy rates . Results The endometrial thickness and types on HCG injection day in treatment group were improved significantly (P<0 .05) . The parameters of intramembrane and submucosal uterine blood flow were better than the control ,there was a significant difference ( P<0 .05) .And the clinical pregnancy rate of treatment group was higher than that of control group there was a significant differ-ence(P<0 .05) .Conclusion U8 similar bioelectricity stimulation can promote the grow th of endometrium ,improve the endometrial receptivity and the clinical pregnancy rate .

Objective To study the effect of Na-2SeO-3 on experimental carcinogenesis of stomach and on p53 and p16 expression. Methods Weaning male Wistar rats were divided into four groups randomly:high selenium group(4mg/L),low selenium(2mg/L)group,experiment control group and normal control group.Wistar rat gastric cancer was induced by N-methyl-N′-nitro-N-nitrosoguanidine(MNNG,20mg/kg)given daily for 10days.Na 2SeO 3 was given by piped drinking before one week of MNNG administration.Rats were killed at the 43th week.The surface characters of gastric mucosa were observed with noked eyes.Histopathologic changes were observed by methods of HE staining and Alcian Blue Periodic acid Schiff reaction(AB-PAS).Changes of p53 and p16 were detected by SP immunohistochemical method.The immunohistochemical results were quantitatively analysed by image analysis.Statistical analysis was taken by SPSS. Results Dietary Na-2SeO-3(2mg/L,4mg/L)aggravated gastric erosion and hemorrhage and promoted intestinal metaphasia of gastric cancer (P0.05).Conclusion These results suggest that dietary Na-2SeO-3 by piped drinking might not decrease incidence of Wistar rat gastric cancer induced by MNNG.The mechanism may be associated with the mutations of p53 and abnormal expression of p16 gene.

Objective To analyze the common causes leading to lung infection following renal transplantation and provide targeted preventive measures to reduce the incidence of lung infection.Methods The clinical data of 561 recipients who underwent renal transplantation from January 2006 to February 2011 were retrospectively analyzed.The recipients were divided into two groups:group Ⅰ,from January 2006 to December 2009 (n =416) ; group Ⅱ,from January 2010 to February 2011 (n =145).The causes possibly leading to lung infection which took place 3 days before the appearance of the clinical symptoms were offered by the patients who suffered lung infection of group Ⅰ.And then the causes were summarized and analyzed to formulate the specific and comprehensive measures to prevent the infection.Finally the measures were applied to recipients in group Ⅱ from January 2010.After applying the measures for 14 months,the incidence of lung infection in group Ⅱ was counted and compared with that in group Ⅰ to see the preventive effect.Results There were 58 cases of lung infection in group Ⅰ (58/416,13.9％) and 12 cases in group Ⅱ (12/145,8.3％). There was significant difference in the incidence of lung infection between two groups (x2 =4.0361,P＜0.05).All of the recipients with lung infection were hospitalized in six months after the transplantation.The causes leading to lung infection of 58 cases in group Ⅰ were as follows:6 cases due to being excessively tired,3 cases due to guest visiting,12 cases due to abrupt change of weather,9 cases due to exposure to public place,8 cases due to returning to hospital,6 cases due to close contact with children,5 cases due to close contact with animals,and the other 9 cases without specific causes found.Conclusion The incidence of lung infection following renal transplantation can be notably reduced by the application of targeted and concrete health propaganda education and preventive measures based on analysis on the specific causes of infection.

OBJECTIVE: To investigate mRNA expression of GABAa receptor(GABAaR) subunits in the rat cochlear spiral ganglion neurons (SGN) and explore the effect of sodium salicylate (SS) on the expression of GABAaR subunits. METHOD: The realtime fluorescent quantitative PCR (FQ-PCR) was used to detect mRNA expression of twelve GABAaR subunits in the newborn rat SGN and then investigate mRNA expression of GABAaR subunits after treatment with 5 mmol/L SS for 15 min, 30 min, 1 h, 3 h and 6 h in the primary culture SGN. RESULT: (1) GABAaR subunits of α1-6, β1-3, and γ1-3 were detected in the SGN, and the expression of GABAaR subunits was lower than those in the cerebral cortex. In the subunit α family of GABAaR, the expression rank was α2>α3/α5>α4>a1>α6, and the expression of α3 and α5 had no difference (P>0. 05). In the subunit β family, the expression rank was β3>β2>β1. In the subunit γ family, the expression rank was γ1>γ2>γ3. (2) The expression of all subunits of GABAa receptor was obviously fluctuated excepting subunit α5 after treatment with SS. At 15 min post-SS, α1, α2 , β1 and γ1-3 were upregulated, and α3 was downregulated; At 30 min post-SS, α3, β1 and β3 were upregulated, and γ1 was downregulated; At 1 h post-SS, β2 was upregulated and γ3 was downregulated; At 3 h post-SS, β1 and β2 were upregulated, and α3 and γ2 were downregulated; At 6 h post-SS, αl, α3 ,β2, β3 and γ1 were upregulated, and α2, α4 and β1 were downregulated. CONCLUSION The mRNA of GABAaR was expressed in the rat SGN, and the expression of GABAaR subunits was lower in SGN than the cerebral cortex. SS could alter the GABAaR expression quantity in rat SGN; Most of the subunits expression were elevated obviously in the early post SS (15 min), followed by a slight fluctuation.

Objective Through testicular sperm aspiration (TESA),testicular biopsy,and detection of serum reproductive hormone levels and testicular volume in azoospermia patients,to explore the correlation between spermatogenic function of testis and serum FSH,LH,INHB levels,and testicular volume.Methods 76 cases of azoospermia patients were collected as the research objects.Chemiluminescence method was employed to detect the levels of serum reproductive hormone,and testis model was used to detect testicular volume.Routine disinfection was given to make TESA.According to TESA results,patients were divided into sperm group (group A),and azoospermia group (group B).According to testicular biopsy results,patients were divided into normal spermatogenic function group (group C),sperm maturation block group(group D),permatogenic dysfunction group (group E),and sertoli-cell-only syndrome group (group F).At the same time,40 cases of healthy male were selected as the control group (group G),and they received sperm routine examination,serum reproductive hormone and testicular volume detection.Results The level of serum FSH,LH,INHB and testicular volume in group A and C had no significant difference compared with that in group G(P＞0.05).The level of serum FSH and LH was significantly higher in group C than that in group G.The level of serum INHB and testicular volume was significantly lower than that in group G (P＜0.05).The level of serum FSH in group E was higher than in group G.The level of serum INHB in group D and E was lower than that in group G,and the difference was statistically significant (P＜0.05).Spearman correlation analysis showed that the spermatogenic function of testis was negatively correlated with serum FSH and LH levels,and positively correlated with serum INHB level and testicular volume (P＜0.05).Conclusion The detection of serum FSH,LH,INHB levels and testicular volume has important clinical value for predicting testicular spermatogenie function in azoospermia patients,and can be used to guide clinical testicular sperm aspiration.

Objective:To improve the level of clinical diagnosis and treatment by analyzing the clinical features and relevant factors of cryptococcosis neoformans in patients with connective tissue disease(CTD).Methods:Twelve patients with CTD and cryptococcosis neoformans infection in Peking University People's Hospital from January 2010 to April 2021 were retrospectively enrolled. Clinical and laboratory data, treatment and outcome were collected and analyzed. Independent sample t-test or Rank-sum test was used. Results:The age of the patients ranged from 18 to 85 years old(mean 51 years old), all of whom were female. None of them were exposed to pigeons and their feces. Of the 12 patients, 3 patients suffered from rheumatoid arthritis, 7 patients had systemic lupus erythematosus, 1 patient was diagnosed with primary Sj?gren 's syndrome, and 1 patient was diagnosed as undifferentiated connective tissue disease. Four cases were cryptococcal meningitis, 8 were pulmonary cryptococcosis. None of the 12 patients had immunodeficiency virus infection. All 12 patients were given glucocorticoid alone or combined with immunosuppressive or biological agents. All were detected with positive cryptococcus neoformans antigen in serum; 6 got lumbar puncture, 2 cases were positive for ink stain, cerebrospinal fluid (CSF) culture were positive in 2, in whom 3 had high intracranial pressure, in which the highest one was more than 600 mmH 2O (1 mmH 2O=0.009 8 kPa); 7 cases underwent lung biopsy. Among these patients, all were positive for cryptococcosis neoformans in lung tissue pathological examination; 6 had the number of peripheral lymphocytes less than 1.0×10 9/L, and 2 were detected for the number of CD4 + T cell, which was significantly decreased. As for the initial anti-fungal drug therapy, all cases were treated with fluconazole intravenously; 2 were treated with combined amphotericin, 1 was treated with combined fluorocytosine, 1 was treated with amphotericin and fluorocytosine. Then oral flu-conazole was prescribed as sequential therapy. The whole treatmentcourse ranged from 4 to 21 months. Eleven patients were cured, and 1 was relieved. Conclusion:Patients with connective tissue disease complicated with cryptococcus neoformans infection have atypical clinical symptoms. Treatment with immunosuppressive drugs and glucocorticoids are related causes. Patients with decreased peripheral blood lymphocytes, especially CD4 + T cell, are more susceptible to infection. Early diagnosis and timely treatment are the key to improve the prognosis and cure of the disease.

Objective: To analyze the correlation between the continuous opening of patent ductus arteriosus(PDA) in preterm infants and platelet parameters in the first 24 hours of life. Methods: The preterm infants (gestational age <34 weeks) admitted to Neonatal Intensive Care Unit(NICU)of the Affiliated Xuzhou Hospital of Southeast University from November 2012 to July 2018 were enrolled.The following data were collected retrospectively: the platelet parameters in the first 24 hours of life, clinical factors possibly related to continuous opening of PDA, and echocardiography examination fin-dings on the 4^th-7^th day after birth.According to the diagnostic criteria of PDA, all preterm infants were divided into symptomatic PDA(sPDA) group, non-sPDA (nsPDA) group, and non-PDA (nPDA) group.SPSS 20.0 software was used for data analysis.Data were analyzed by Chi-square test, LSD or Tambane′s T2 of One-Way analysis of variance, and binary Logistic regression analysis of the receiver operating characteristic (ROC) curve. Results: Totally 760 preterm infants were chosen, and among them there were 67 cases (8.8%) in sPDA group, 106 cases (14.0%) in nsPDA group, and 587 cases (77.2%) in nPDA group.There were significant diffe-rences in the terms of gestation age, birth weight, platelet counts (PLT), and plateletcrit (PCT) in the first 24 hours of life among 3 groups (all P<0.05). The smaller gestation age, the lower birth weight, the lower PLT and PCT in the first 24 hours of life, and the higher incidence of PDA in preterm infants.There were no significant differences in the platelet distribution width, mean platelet volume, and platelet large cell ratio in the first 24 hours of life among 3 groups (all P>0.05). The low lower birth weight and PCT in the first 24 hours of life were independent risk factors for the occurrence of sPDA in preterm infants (P=0.013, 0.000). The risk of occurrence of sPDA in preterm infants will be increased by 3.279-fold (95%CI: 2.369-4.479) if PCT in the first 24 hours of life is decreased by 0.10%.The area under the ROC curves of PCT and PLT in the first 24 hours of life for prediction of sPDA in preterm infants was 0.757 (95%CI: 0.712-0.814) and 0.718 (95%CI: 0.671-0.768), respectively.The best cutoff values of PCT and PLT were 0.178% (sensitivity was 75.7%, specificity was 71.9%) and 207.5×10⁹/L (sensitivity was 71.4%, specificity was 63.2%). Conclusions The continuous opening of PDA in preterm infants may have correlation with the platelet.The low PCT, rather than PLT, in the first 24 hours of life was an independent risk factor and has medium predictive value for the occurrence of sPDA in preterm infants on the 4^th-7^th day after birth.

Objective To study the possible mechanisms,efficacy and safety of acetaminophen and platelet-rich plasma (PRP) on promoting the closure of ductus arteriosus in preterm infants.Method From January 2016 to May 2018,preterm infants (gestational age＜34 weeks) with symptomatic patent ductus arteriosus (sPDA) admitted to our neonatal intensive care unitwere enrolled prospectively.15 mg/kg of acetaminophen was orally given every 6 hours for three days (acetaminophen group).If acetaminophen was contraindicated (PRP group) or acetaminophen therapy failed (PRP rescue group),PRP transfusion was given at a single dose of 20 ml/kg.Echocardiogram,platelet-derived growth factors (PDGF) and urinary prostaglandin E2 (PGE2) were examined before and 72 hours after treatment.The data were analyzed using ANOVA,student's t test,Kruskal-Wallis H method and chi-square test.Result A total of 70 cases were enrolled.61 cases were treated with acetaminophen.The success rate (67.2％,41/61) was similar to PRP group (6/9) (P=1.000),and significantly higher than PRP rescue group (6/17) (P=0.017).The complications in acetaminophen group included 22 cases of hyperbilirubinemia (36.1％),5 cases of upper gastrointestinal hemorrhage (8.2％),4 cases of positive fecal occult blood test (6.6％),3 cases of oliguria (4.9％),1 case of Grade Ⅲ ～ Ⅳ intraventricular hemorrhage (IVH) (1.6％),and 1 case of≥ Ⅱ stage necrotizing enterocolitis (NEC) (1.6％).A total of 3 cases of hyperbilirubinemia were in PRP group.One case of hyperbilirubinemia,one oliguria,one Grade Ⅲ～ Ⅳ IVH and one ≥ Ⅱ stage NEC were in PRP rescue group.The urinary PGE2 level in post-treatment was lower than pre-treatment in acetaminophen group (t=6.878,P＜0.001).The blood platelet count and PDGF level in post-treatment were higher than pre-treatment in PRP group (t=-2.496,-8.906;P=-0.037,＜0.001) and PRP rescue group (t=-3.374,-2.503;P=0.004,0.024).Conclusion Oral acetaminophen and PRP transfusion had similar efficacy in promoting the closure of ductus arteriosus in preterm infants.If oral acetaminophen was contraindicated or failed,PRP transfusion could be one of the candidates for rescue therapy to promote the closure of ductus arteriosus in preterm infants.
