BACKGROUND:During cervical posterior expansive open-door laminoplasty for multisegmental cervical spondylosis, spinal canal restenosis, loss of cervical lordosis, and axial symptoms are the important factors affecting curative effects. It is very necessary to maintain spinal canal expanded state and to reduce interference of the posterior cervical structure in the clinical treatment. OBJECTIVE:To observe clinical outcomes and short-term fol ow-up effect of posterior expansive open-door laminoplasty via titanium miniplate in treatment of cervical spondylotic myelopathy. METHODS:A total of 67 patients with cervical spondylotic myelopathy who underwent posterior expansive open-door laminoplasty at the Zhongshan Hospital, Xiamen University from April 2006 to April 2013 were retrospectively analyzed. Titanium miniplate group (n=27) received titanium miniplate fixation. Suture group (n=40) received traditional suture suspension. Al patients had decompression ranged from C 3-7 . Operation time, intraoperative blood loss, improvement rate of Japanese Orthopedic Association score during fol ow-up, value of cervical curvature, axial symptoms, and lamina opened angle were compared between the two groups. RESULTS AND CONCLUSION:No significant difference in operation time, intraoperative blood loss and improvement rate of Japanese Orthopedic Association score was detectable between two groups (P>0.05). Cervical curvature changes in both groups:loss of cervical curvature at 6 months postoperation was not significant in the titanium miniplate group, but cervical curvature partial y lost in the suture group, and significant differences in the cervical curvature were detected between the two groups (P<0.05). The incidence of axial symptoms was significantly lower in the titanium miniplate group than in the suture group at 6 months after surgery (P<0.05). During final fol ow-up, no significant difference in the lamina open-angle was detected between titanium miniplate group (35.2±6.2)° and suture group (34.0±4.7)° (P>0.05). These data suggested that posterior expansive open-door laminoplasty for treatment of cervical spondylotic myelopathy using both titanium miniplate and suture methods can obtain good clinical outcomes. However, titanium miniplate fixation can relieve postoperative axial symptoms and prevent loss of cervical curvature.

Objective To describe the MR imaging features and pathologic findings of hepatic angiomyolipoma (HAML), and to evaluate the utility of MR imaging in diagnosis of this tumor. Methods The MR imaging features of eight patients with HAMLs confirmed pathologically were analyzed retrospectively and correlated with pathologic findings. Results Eight patients had a solitary hepatic mass, five (mixed type HAML) of which were diagnosed correctly, and two (1 myomatous type HAML and 1 angiomatous type HAML) of which were diagnosed as hepatic adenoma and focal nodular hyperplasia, and one (mixed type HAML) with hemorrhage and multiple cystic change was mistaken for malignant tumor. Six cases of mixed type HAML exhibited heterogeneously high signal intensity on T 1 and T 2WI. The high signal intensity on T 1WI in five cases disappeared completely after using fat saturation, and most part of the high signal intensity in one case disappeared and some of the high signal intensity remained unchanged after using fat saturation, and multiple cystic change within this mass was detected. One myomatous type HAML and one angiomatous type HAML showed hypointensity on T 1WI and hyperintensity on T 2WI. Multiphase dynamic Gd DTPA enhanced MR scanning was performed in six patients and all those masses showed strong enhancement during arterial phase, and four of which showed moderate enhancement and two slight enhancement during portal venous and delayed phase. Pseudocapsule detected in five cases showed slight enhancement on portal venous and (or) delayed phase. Conclusion MR imaging can reveal the characteristic findings of mixed type HAML and accurate preoperative diagnosis can be made.

【Objective】 To investigate the effect of cold agglutination on blood group typing. 【Methods】 37℃ water bath, absorption elution test and 2-mercaptoethanol method were used to eliminate the influence of cold agglutination. Forward and reverse blood group typing, cross matching, DAT and IAT experiments were then performed on red blood cells and serum after treatment. 【Results】 Before treatment, obvious discrepancy in forward /reverse typing and nontypable cross matching in 16 blood samples were noticed due to cold agglutination. After corresponding treatments, all samples were consistent or negative in forward/reverse typing, cross matching and antibody screening. No adverse reactions to cross matching blood transfusion occurred in patients, and the increase of hemoglobin was in line with the effective standard of transfusion. 【Conclusion】 37℃ water bath, absorption elution test and 2-mercaptoethanol method can be used to eliminate the interference caused by cold agglutination to obtain correct typing results. The strong reactivity caused by cold agglutination in AIHA patients were different from other cases, which deserved our attention.
