1.Clinical application effects of artificial back percussion sputum excretion: a Meta-analysis

Objective?To systematically evaluate the clinical effects of artificial back percussion sputum excretion. Methods?Randomized controlled trial (RCT) on artificial back percussion sputum excretion and other methods to promote sputum excretion were included by searching the Cochrane Library, Pubmed, FMRS, Ovid, BMJ, ClinicalKey, Springer for R&D, CNKI and WanFang data from setting up the database to November 2017. Two researchers screened documents, extracted data, evaluated the quality of included documentson their own. The RevMan 5.3 software was used to meta-analysis. The outcome indexes which failed to carry out Meta-analysis were conducted descriptive analysis. Results?A total of 10 RCTs were included involving 541 patients. Meta-analysis results showed that the artificial back percussion could increase the volume of sputum in 24 hours [WMD=2.17,95%CI(1.50,2.84),P<0.000 01], improve patients' degree of blood oxygen saturation [WMD=2.68,95%CI(2.36,2.99),P<0.000 01], reduce breathing rate [WMD=-2.56, 95%CI(-3.06,-2.05),P<0.000 01], improve blood oxygen partial pressure (PaO2) [WMD=8.07,95%CI(6.19, 9.95),P<0.000 01]as well as partial pressure of carbon dioxide (PaCO2) [WMD=-3.77,95%CI(-5.51,-2.03), P<0.000 01], decrease the frequency of cough [WMD=-2.46,95%CI (-3.26,-1.66),P<0.000 01]and the fade time of lung rale [WMD=-4.59,95%CI(-5.36,-3.82),P< 0.000 01] compared with non-intervention group. Conclusions?The artificial back percussion sputum excretion has initial effects on improving the effects of sputum excretion, breathing related indexes such as degree of blood oxygen saturation, breathing rate blood PaO2as well as PaCO2, decrease the frequency of cough and the fade time of lung rale.
2.Cancer-related fatigue in patients with advanced malignant tumours:current status and influencing factors

Objective To investigate the status quo of cancer-related fatigue(CRF)in patients with advanced malignant tumours during anti-tumour treatment,and explore the influencing factors so as to provide a reference for nursing intervention.Methods Between January and August 2022,a total of 279 patients with advanced malignant tumours who received anti-tumour therapies in the Department of Oncology of a general hospital in Beijing were selected as study subjects using convenience sampling method.General data questionnaire,cancer fatigue scale,chemotherapy-related gastrointestinal symptoms inventory,and nutritional risk screening 2002 were used for the investigation.Logistic regression analysis was conducted to determine the influencing factors of cancer-related fatigue.Results Toally 279 patients finished the study.A total of 204(73.12%)patients had cancer-related fatigue.Binary logistic regression analysis showed that BMI,education,monthly family income and diabetes were the influencing factors in cancer-related fatigue(all P<0.05).Conclusions The incidence of cancer-related fatigue is high in patients with advanced malignant tumours during anti-tumour therapy.Low BMI,poor education,low monthly family income and diabetes are the risk factors in cancer-related fatigue.Targeted interventions should be implemented based on the risk factors so as to reduce the incidence of cancer-related fatigue.
3.Analysis of the current status and risk factors of workplace violence among nurses in Class Ⅲ Grade A hospitals of Beijing

?? [Abstract]? Objective? To investigate the current status of workplace violence (WPV) among nurses in 3A-level hospitals of Beijing and analyze the risk factors. Methods? Convenient sampling method was adopted to select nurses from three ClassⅢ Grade A hospitals in Beijing from October 2018 to January 2019. The nurses were given by general information questionnaire and Workplace Violence Questionnaire. With the effective response rate of 98.6%, a total of 1 072 questionnaires were collected with 1 057 ones valid. Chi-square analysis and Logistic regression analysis were used to analyze the influential factors of workplace violence. Results 50.80% (537/1 057) of respondents reported that they had experienced WPV in the past with high incidence of scolding and language abuse. Most WPV happened in the ward (45.25%) during the morning shift(72.81%) . Majority of the violators are male(67.60%), middle-aged(67.41%) and family members of the patients (61.64%). Univariate analysis showed that the difference of working length, department, job position, educational level, form of employment, job position and monthly number of night shift between the nurses and their WPV incidence are correlated and have statistical significance(P< 0.05). The result of Logistic regression analysis indicated that the nurse's department, educational background, job position, and opinions towards questions including "whether heard of WPV before" "whether do you think WPV is unavoidable" "whether do you think WPV is normal" "whether have you benefited from relevant training about WPV" "whether to report upon WPV" "whether the solution of the WPV from the hospital in the past is effective" all had impact on the WPV incidence. Conclusions? WPV against nurses are common in ClassⅢ Grade A hospitals of Beijing. The department of out-patient and emergency, and the position of nurse-in-charge are risk factors; whereas high education level, positive perceptions and attitudes towards WPV; and the hospital's effective treatment upon the past WPV are protective factors. It is suggested that hospitals and nursing administrators should adopt specific strategies to reduce the incidence of WPV.
4.Current situation and influencing factors of instrumental activities of daily living among community-dwelling elderly people in China

Objective:To explore the current status of instrumental activities of daily living among community-dwelling older adults (≥ 75 years old) in China, and analyze its influencing factors.Methods:This study was a cross-sectional study. From January to May 2023, multi-stage sampling was used to select 1 832 older adults from 27 community health service centers in 9 provinces and cities across China as the research subject. A survey was conducted on older adults using the General Information Questionnaire and the Lawton-Brody Instrumental Activities of Daily Living Scale. Binomial Logistic regression was used to analyze the influencing factors of instrumental activities of daily living among older adults in the community. A total of 1 832 questionnaires were distributed, and 1 746 valid questionnaires were collected, with an effective response rate of 95.30%.Results:Among 1 746 older adults in the community, 1 194 (68.4%) had functional dependence of instrumental activities of daily living. Binomial Logistic regression showed that age, educational level, type of residence, type of chronic disease, regular exercise, self-reported social support, self-reported visual impairment, and self-reported hearing impairment were the influencing factors for the functional dependence of instrumental activities of daily living among older adults in the community ( P<0.05) . Conclusions:The instrumental activities of daily living among older adults in communities in China needs to be improved. Nurses can identify high-risk populations in the early stages and develop intervention measures based on the influencing factors of older adults' instrumental activities of daily living.
5.Scoping review on the application of gamification design in exercise interventions for elderly patients with cognitive impairment

Objective:To conduct a scoping review on gamified exercise interventions for elderly patients with cognitive impairment to provide insights for the application of gamification design in exercise programs for this population.Methods:Guided by the scoping review framework proposed by Arksey and O'Malley, a systematic search was conducted in databases including China National Knowledge Infrastructure, Wanfang, VIP, SinoMed, PubMed, Cochrane Library, CINAHL, Web of Science, Scopus, ProQuest, and Wiley Online Library. The search covered the period from the establishment of the databases to June 12, 2024. An integrative analysis was performed on the included literature.Results:A total of 12 studies were included. The most commonly used gamification elements were achievement- and progress-oriented features, while the most frequently applied behavior change theories were behavior change techniques and social cognitive theory.Conclusions:Gamification-based exercise interventions can improve adherence to exercise programs in elderly patients with cognitive impairment, promote physical activity, and enhance cognitive function. Future research should focus on combining various theories with different gamification elements to encourage proactive exercise behavior in elderly individuals with cognitive impairment.
6.Investigation on the status of nurses′ knowledge about Prevention and Treatment of Pressure Ulcers: Quick Reference Guide (2014)

Objective To understand the status of nurses′ knowledge about Prevention and Treatment of Pressure Ulcers: Quick Reference Guide (2014).Methods Totally 378 nurses were investigated by a self-designed questionnaire about pressure prevention and treatment knowledge.Results The score of the questionnaire was 12-100 scores with average score(67.10±15.24), and the average accuracy was 67.10%; there was no significant difference between nurses of different ages, working ages, education background, professional title or position (P>0.05); the accuracy 10 terms were lower than 60%.Conclusions Nurses′ should be trained specifically and timely to update and improve their knowledge about pressure ulcers, and the corresponding training should be organized according to requirements.
7.Current situation and training demands for geriatric nursing ability of clinical nursing teachers

Objective:To investigate the current situation of geriatric nursing ability of clinical nursing teachers and analyze the influencing factors, and to identify demands of geriatric nursing training.Methods:From May to June 2023, a cross-sectional study was conducted among 2 408 clinical nursing teachers in 50 tertiary hospitals from 9 provinces and cities in East China, South China, Central China, North China, and Southwest China using the general information questionnaire, Geriatric Nursing Competence Scale for Clinical Nurses, and Geriatric Nursing Training Demands Questionnaire.Results:Among the clinical nursing teachers surveyed, there were 269 (11.17%), 938 (38.95%), and 1 201 (49.88%) teachers at low, medium, and high levels of geriatric nursing ability, respectively. The results of multinomial Logistic regression analysis showed that age, years of working in geriatric nursing, geriatric nursing related training, satisfaction with clinical nursing work, and interest in geriatric nursing work were common influencing factors in the low and medium level groups ( P<0.05), and professional title was a specific influencing factor in the medium level group ( P<0.05). The top three demands of geriatric nursing training content were geriatric nursing service model, knowledge of chronic disease management, and comprehensive geriatric assessment techniques. The top three demands of training forms were live streaming, field visits, and offline lectures. Conclusions:For clinical nursing teachers, demand-oriented geriatric nursing training should be strengthened, and attention should be paid to the satisfaction and interest in nursing work, so as to improve the teachers' geriatric nursing ability and strengthen the level of geriatric service.
8.Research progress on the implementation of discharge preparation service in aged patients

The implementation of discharge preparation services can ensure the continuity of care for patients in different places of residence, shorten the length of hospital stay, reduce the rate of patient readmission, and increase the satisfaction of patients and their families. This article reviews the concept, implementation principles, service content, implementation process, domestic and foreign application status and implementation effects of discharge preparation services in aged patients, aiming to promote the application and practice of discharge preparation services in aged patients as well as their implementation in China.
9.Status and relationship of nursing work environment and nurses' occupational commitment in eastern, middle and western China

Objective:To understand the status quo of nursing work environment and nurses' occupational commitment in eastern, middle and western China and further analyze the effect of nursing work environment on nurses' occupational commitment.Methods:From March to December 2020, the convenient sampling method was adopted to select 3 142 nurses from 11 tertiary and 6 secondary hospitals in 8 provinces/municipalities/autonomous regions in the eastern, middle and western China as research objects. The general information questionnaire, Chinese version of Practice Environment Scale and Chinese version of Nurses' Occupational Commitment Scale were used for questionnaire surveys.Results:A total of 3 142 questionnaires were collected and 124 invalid questionnaires were excluded, and 3 018 valid questionnaires were collected. The total mean score of Nurses' Occupational Commitment Scale was (3.54±0.67) . From highest to lowest, scores of each dimension were normative commitment (3.83±0.84) , emotional commitment (3.78±0.87) , economic cost commitment (3.67±0.81) , emotional cost commitment (3.40±0.94) and opportunity commitment (2.88±0.85) . The total mean score of Practice Environment Scale was (3.02±0.49) . The scores of Nurse's Occupational Commitment Scale of nurses with different hospital levels, departments, working years, professional titles, educational backgrounds, employment forms, average monthly night shifts and average monthly income showed statistically significant differences ( P<0.05) . Moreover, the score of Nurses' Occupational Commitment Scale for specialist nurses was higher than that of non-specialist nurses, and the difference was statistically significant ( P<0.01) . The results of multiple stratified regression showed that low hospital levels (β'=-0.058) , specialized nurses (β'=0.069) and fewer average monthly night shifts (β'=-0.194) were influencing factors that affected nurses' occupational commitments. After controlling for general data, nurses' participation in hospital affairs (β'=0.301) , adequate human and material resources (β'=0.260) and medical cooperation (β'=0.132) in the nursing environment were the influencing factors of nurses' occupational commitment ( P<0.01) . Conclusions:The overall occupational commitment of nurses in eastern, middle and western China is at a medium level, which is affected by whether they are specialist nurses, average monthly night shifts, levels of hospitals and the working environment of nursing. Managers should conduct scientific guidance and management for these influencing factors to improve nurses' occupational commitment.
10.The mediating effect of nurses' professional commitment between practice environment and turnover intention

Objective:To understand the status quo of nurses' professional commitment, practice environment, and turnover intention in the eastern, middle and western parts of China, and to explore the mediating effects of nurses' professional commitment between practice environment and turnover intention.Methods:In this cross-sectional study, a total of 3 142 nurses from 11 ClassⅢ hospitals and 6 ClassⅡ hospitals in the eastern, middle and western parts of China were selected by convenient sampling from March to December 2019. They were investigated with the general information questionnaire, Practice Environment Scale (PES) , Nurses' Professional Commitment Scale, and Nurses' Turnover Intention Questionnaire. Totally 3 142 questionnaires were distributed, and 3 018 valid questionnaires were collected, accounting for an effective recovery rate of 96.1%.Results:The average scores of the PES, Nurses' Professional Commitment Scale, and Nurses' Turnover Intention Questionnaire in the 3 018 nurses were (3.02±0.49) , (3.54±0.67) , and (2.72±0.99) . There was a correlation among nurses' professional commitment, practice environment, and turnover intention ( P<0.01) . The hierarchical regression analysis showed that after controlling for general data, sufficient human and material dimensions in the PES and the emotional commitment, normative commitment, and opportunity commitment dimensions in the Nurses' Professional Commitment Scale were independent influencing factors of nurses' turnover intention. The mediating effect analysis revealed that the mediating effect of nurses' professional commitment between practice environment and turnover intention was 0.267, accounting for 57.3% of the total effect. Conclusions:Nurses' professional commitment shows mediating effects between nurses' practice environment and turnover intentions, suggesting that nursing managers can reduce nurses' turnover intention and stabilize the nursing team by constructing a healthy practice environment or improving nurses' professional commitment.
