2.Research progress of autoimmune gastritis
Journal of Chinese Physician 2021;23(3):461-465
Autoimmune gastritis(AIG) is a CD4 + T cell-mediated organ specific autoimmune disease of the stomach, characterized by the inherent gland atrophy in the stomach and intestinal metaplasia and/or pseudopyloric gland metaplasia. The diversity and nonspecificity of clinical features make the diagnosis difficult. In the past, AIG was thought to occur in the West, but in recent years, more and more studies have shown that the prevalence rate of AIG in China is not low. However, the disease has not attracted the attention of domestic clinical workers. Therefore, this paper reviews the latest research progress in the etiology, clinical characteristics, relationship with other diseases, diagnosis and treatment of AIG, so as to improve the understanding of clinical workers and improve the treatment rate and prognosis of AIG.

We established methods to isolate human amniotic fluid-derived progenitor cells (hAFPCs), and analyze the ability of hAFPCs to secrete human coagulation factor IX (hFIX) after gene modification. The hAFPCs were manually isolated by selection for attachment to gelatin coated culture dish. hFIX cDNA was transfected into hAPFCs by using a lentiviral vector. The hFIX protein concentration and activity produced from hAFPCs were determined by enzyme-linked immunosorbent assay (ELISA) and clotting assay. The isolated spindle-shaped cells showed fibroblastoid morphology after three culture passages. The doubling time in culture was 39.05 hours. Immunocytochemistry staining of the fibroblast-like cells from amniotic fluid detected expression of stem cell markers such as SSEA4 and TRA1-60. Quantitative PCR analysis demonstrated the expression of NANOG, OCT4 and SOX2 mRNAs. Transfected hAFPCs could produce and secrete hFIX into the culture medium. The observed concentration of secreted hFIX was 20.37% +/- 2.77% two days after passage, with clotting activity of 16.42% +/- 1.78%. The amount of hFIX:Ag reached a plateau of 50.35% +/- 5.42%, with clotting activity 45.34% +/- 4.67%. In conclusion, this study established method to isolate and culture amniotic fluid progenitor cells. Transfected hAFPCs can produce hFIX at stable levels in vitro, and clotting activity increases with higher hFIX concentration. Genetically engineered hAFPC are a potential method for prenatal treatment of hemophilia B.

This study was aimed to explore the relationship between the Shen-Gui-Fang(SGF) formula of different proportions and treatments for diseases with the method of bibliometrics. A total of 49 formulae of SGF from ancient books of traditional Chinese medicine (TCM) were retrieved and the database was established in order to analyze the origin of herbal medicine and the relationship with the ratio of the key medicinal material, as well as the treatment for major diseases. The results showed that 49 SGF formulae were mainly used in the treatment of internal disorders for 16 times (32.65%), gynecological disorders for 17 times (34.69%) and pediatric disorders for 8 times (16.32%). Guistanded forAngelica sinensis. There were 3 types ofShen, which werePanax ginsengfor 40 times,Codonopsis pilosulafor 8 times, andSophora flavescensfor 1 time. The ratio betweenP. ginsengandA. sinensiswas less or equal to 1. The ratio betweenC. pilosulaandA. sinensiswas usually more than 1. The formulae with the highest frequency were in the treatment of internal medicine and gynecological diseases. Statistical analysis was used to find the application features and rules of the combination ofP. ginsengandA. sinensisin SGF formula, in order to provide theoretical evidences and references for clinical medication selection and new medication research.

Objective To evaluate the activation patterns in the cortexes of expressive aphasics after stroke so as to explore the pathogenic mechanism of expressive aphasia. Methods Blood oxygen level dependent functional magnetic resonance imaging (BOLD-fMRI) was the method of choice. It was administered to 9 subjects with expressive aphasia at 1 to 3 months post-stroke onset and to 10 healthy controls.Active areas in the patients' brains were observed using a block-designed picture-naming task, and language function was tested with the China Rehabilitation Research Center's aphasia examination (CRRCAE).The control group received BOLD-fMRI only.SPM8 software was used to process the fMRI data. Results Differences were observed in the mapping of activated areas between the two groups, but many activated areas showed no difference.Significant differences in activation were observed in areas associated with vision, language and cognition, including the bilateral inferior frontal gyrus, the bilateral superior temporal gyrus, the bilateral insula, the bilateral basal ganglia, the left superior frontal gyrus, the left middle frontal gyrus, the left precentral gyrus, the left thalamus, and the left middle temporal gyrus. All the patients had activated cortex regions associated with visual processing in the left and/or right hemisphere, such as the middle frontal gyrus, the middle temporal gyrus, the lingual gyrus and the fusiform gyrus. The activation volumes in the left hemisphere were significantly smaller than those in normal adults.Regions related to language such as the left inferior frontal gyrus (Broca's area),the left middle frontal gyrus, and the right inferior frontal gyrus (the mirror region of Broca's area) were activated in some of them. While the activation frequency, activated volume and activation intensity generally were all less in the patients than in the controls, the activation intensity in the right superior temporal gyrus, the bilateral superior parietal lobule and the left inferior temporal gyrus were stronger. Conclusions Language production may be associated with multiple, interconnected regions. The right hemisphere participates in natural language processing.Aphasia damages both linguistic and cognitive areas, reducing activation in Broca's aphasia.Activation areas in the left hemisphere and the right inferior frontal gyrus decrease significantly, while some regions in the right hemisphere are relatively more activated. The right inferior frontal gyrus may play a different role in language recovery at different periods of aphasia after stroke.
