1.Study on clinical effect of glargine insulin combined with glimepiride in newly diagnosed T2DM patients

Objective To study the efficacy and safety of glargine insulin combined with glimepiride therapy in type 2 diabetes mellitus(T2DM).Methods According to the random number table,100 T2DM patients with unsatisfactory efficacy of oral hypoglycemic agents were randomly divided into treament group and control group, 50 cases in each group.Then,the treatment group was dealed with glargine insulin and glimepiride,the control group were treated with insulin glargine for our treatment protocol.12 weeks after treament,the fasting blood glucose(FPG), 2 hours blood glucose(2hPG),body mass index(BMI),glycosylated hemoglobin(HbA1c),islet β-cell function index(HOMA -β),insulin resistance index(Homa -IR),insulin dosage(INS),time requirds,weight gain and so on were observed.Meanwhile,The occurrence condition of hypoglycemia was also observed.Results In control groups, the post -treatment indexes of FPG,2hPG,HbAlc,HOMA -βand HOMA -IR[(5.7 ±0.8)mmol/L,(8.6 ± 2.5)mmol/L,(6.5 ±0.7)%,(40.35 ±3.12),(2.68 ±1.41 )]were much better than those in pre -treatment [(11.7 ±1.6)mmol/L,(15.1 ±6.1 )mmol/L,(9.2 ±1.1 )%,(19.01 ±2.79),(3.42 ±1.47)](t =23.717, 6.971,14.642,36.052,2.568,P <0.01 or 0.05 for both).In the treatment group,the post -treatment indexes of FPG,2hPG,HbAlc,HOMA -β,HOMA -IR[(5.6 ±0.7)mmol/L,(7.3 ±2.3)mmol/L,(6.3 ±1.0)%,(54.18 ± 3.23),(2.12 ±1.26 )]improved obviously than those in pre -treatment [(11.8 ±1.7 )mmol/L,(15.8 ± 5.2)mmol/L,(9.3 ±1.2)%,(18.71 ±3.12),(3.36 ±1.56)](t =23.846,10.570,13.580,55.849,4.372,P <0.01 for both).After treatment,the indexes of 2hPG,HOMA -β,HOMA -IR,insulin dose,time requirds and ampli-tude of weight gain[(7.3 ±2.3)mmol/L,(54.18 ±3.23),(2.12 ±1.26),(18.06 ±1.43)U /d,(10.1 ±2.4)d, (1.1 ±0.4kg)]etc in the treatment group were significantly ameliorated than those in the control group[(8.6 ± 2.5)mmol/L,(40.35 ±3.12),(2.68 ±1.41 ),(22.46 ±1.77)U /d,(13.2 ±2.6)d,(2.1 ±0.6)kg](t =23.717,6.971,14.642,36.052,2.568,P <0.01 or 0.05 for both).Hypoglycemia events of treatment group were 4 cases(8.0%)and the same in the control group were 13 cases(26.0%),there were statistical significance between the two groups(χ2 =5.535,P <0.05).Conclusion Treatment of glargine insulin combined with glimepiride can improve blood glucose level of T2DM patients,recover their islet β-cell function,relieve the IR,shorten the time requirds,as well as reduce the insulin dosage,range of weight gain and rate of hypoglycemia events.Hence,treatment protocol of glargine insulin combined with glimepiride is an effective clinical treatment for newly diagnosed T2DMpatients.

By computer research, the literature of clinical and experimental studies on moxibustion therapy for chronic gastritis in the recent fifteen years was collected, sorted and analyzed, in order to understand the general situation about the clinical and experimental studies on moxibustion therapy for chronic gastritis, summarize the regularity of moxibustion therapy for chronic gastritis, so as to guide the treatment of chronic gastritis.
3.Determination of Metamizole Sodium and Chlorphenamine Maleate in Zhongganling Tablets by Solid-Phase Extracting HPLC

Objective: To establish a method for the determination of metamizole sodium and chlorphenamine malete in zhongganling Tablets. Methods: The sample was determined by ion pair HPLC after it was purified on Sep Pak C 18 microcolumn. The chromatographic conditions included: Hypersil DBS C 18 chromatographic column (250mm?4.6mm, i.d.5?m) as an anlaytical column, methanol mixed solution of sodium heptanesulfonate and glacial acetic acid (600∶400) as a mobile phase, the detection wavelength at 264nm and 1.0mL?min -1 of flow rate. Results: The average recoveries of metamizole sodium and chlorphenamine maleate were 99.6% (RSD was 2.1% and n was 6) and 98.0% (RSD was 1.5% and n was 6), respectively. Conclusion: Metamizole sodium and chlorphenamine maleate can be determined respectively by HPLC with the same mobile phase when Sep Pak C 18 microcolumn solid phase extraction method is used to substitute for the traditional sample pretreatment methods refluxing, extracting and concentrating, and sodium heptanesulfonate ion pair reagent in acid condition is selected.
4.Determination of taurocholic acid in Shedanchuanbei Oral Liquid and snake bile by SPE-HPLC

Objective: To establish the method for determination of taurocholic acid in the Traditional Chinese Medicine Shedanchuanbei Oral Liquid and snake bile. Methods: The sample was prepared as mixed solution containing methanol and KH 2PO 4. The mixed solution was injected into Sep Pak C 18 cartridge for the purpose of sample purity. In this processing, the substances which having strong retain action and could harm analytic column were hold in the Sep Pak C 18 cartridge. The eluting solution that the Sep Pak C 18 cartridge had be over loading for taurocholic acid was used as the test solution. The test solution was measured by RP HPLC. The chromatographic conditions were as followed: Supelcosil LC 8 column(150mm?4.6nm,5?m) as analytic column, detect wavelength at 203nm, and MeOH 0.4%KH 2PO 4 mixed solution(56∶44, V/V ) as mobile phase. The inject volume was 50?L. Results: The linear response range of sodium taurocholate was from 0.0253mg?mL -1 to 0.253mg?mL -1 , and the correlation coefficient was 0.9999. The average recovery rate was 101.3%, RSD was 0.40%( n =6). Conclusion: This method was simple, efficient and suitable to the quality control for Shedanchuanbei Oral Liquid and snake bile.

7.Effects of Jianpi Yangxue Qufeng Formula on Aquaporin-3 in Model Mice Skin Tissue with Chronic Eczema

Objective To observe the effects of Jianpi Yangxue Qufeng Formula (JPYXQF) on the AQP3 in mice with chronic eczema, and explore mechanism of action. Methods Fifty healthy male mice were randomly divided into 5 groups, namely normal group, model group, positive medicine group and JPYXQF high and low dose groups. Low-dose DNCB and Sennae Fominm were used to establish mice models of chronic eczema with spleen deficiency. JPYXQF groups were treated by JPYXQF for gavage, while the positive medicine group was treated by levocetirizine hydrochloride for gavage. The expression of AQP3 in mice skin tissue was detected by immunohistochemical method. At the same time, the pathological changes of skin were observed. Results The pathology of mice skin lesion showed that JPYXQF has certain recovery effects on the inflammation injury of skin lesion. Compared with the normal group, expression of AQP3 over expressed in model group. Compared with the model group, the expression of AQP3 in all treatment groups significantly decreased, and the staining intensity decreased. In the model group, the average optical density of AQP3 was significantly higher than that in the normal group (P<0.01). Compared with the model group, the treatment groups can reduce the expression of AQP3 in mice skin tissues (P<0.05). Conclusion JPYXQF can reduce the over expression of AQP3 in skin lesion, which is probably its mechanism for the treatment of chronic eczema.
8.Effects of Acupuncture at Myofascial Trigger Points on Spastic Foot Drop and Inversion after Stroke

Objective To study the effects of acupuncture at myofascial trigger points on spastic foot drop and inversion after stroke. Methods From May, 2014 to May, 2016, 50 stroke patients were randomly divided into control group (n=25) and observation group (n=25). Both groups accepted routine rehabilitation, while the observation group accepted acupuncture at myofascial trigger points per day in addi-tion. They were assessed with Visual Analogue Scale (VAS) of pain, modified Ashworth Scale (MAS), range of motion (ROM) of ankle, sim-plified Fugl-Meyer Assessment (FMA) for lower limbs and maximum walking speed (MWS) in ten metres before and six weeks after treat-ment. Results The scores of VAS, MAS, and FMA, the ROM of ankle, and MWS improved after treatment (t>6.845, P<0.001), and im-proved more in the observation group than in the control group (t>5.586, P<0.001). Conclusion Acupuncture at myofascial trigger points can release spasm to reduce foot drop and inversion in patients with stroke.
9.Clinical effect of bilateral craniotomy in treatment of severe traumatic brain injury

Objective To investigate the effect of craniotomy bilateral in treatment of severe traumatic brain injury. Methods Selected 50 patients underwent bilateral craniotomy in treatment of severe brain trauma as research subjects, recorded intracranial pressure changes at the first 1 d, the first 3 d, 7 d after operation, and the follow-up after 6 months, the body recovery scores were compared. Results The intracranial pressure in patients was on a downward trend postoperative 1 d, 3 d, 7 d. Three patients died, accounting for 6.00%;Good recovery in 24 cases, accounting for 48.00%; Moderate disability in 16 cases, accounting for 32.00%; Six cases of severe disability, accounting for 12.00%, in a vegetative state in one case, accounting for 2.00%. Conclusion The bilateral craniotomy should be used early on severe traumatic brain injury patients with bilateral craniotomy indications, it can effectively improve the treatment effect, reduce morbidity and mortality, treatment of severe traumatic brain injury is an effective method, but need to strictly control surgical indications and perioperative good job.
10.Research on the relationship between the various factors of traumatic brain injury patients before surgery and intraoperative surgical results

Objective To investigate the relationship between the various factors of traumatic brain injury patients un-dergoing surgery before the results of surgery. Methods A retrospective analysis from June 2011 to December 2014 in our hospital's clinical data line occupancy brain decompression surgery, 92 cases of traumatic brain injury patients,including a good number of surgical outcomes of 56 patients,the number of 36 patients with a poor prognosis cases. By Pearson univariate and multivariate Logistic regression analysis determine factors affecting the surgical results in pa-tients with traumatic brain injury. Results(1)By Pearson univariate analysis,good prognosis group and a poor progno-sis group in age, GCS score, systolic blood pressure, SpO2 respect, the differences were statistically significant (P<0.05);(2) the analysis of the single-factor results to substitute Logistic regression model, ultimately affecting the sur-gical results obtained in patients undergoing brain injury factors age(β=0.419,SE=0.192,Wald=8.293,P<0.05, OR=1.827,95%CI for 1.362~2.209), GCS score (β=0.753,SE=0.368,Wald=9.091,P<0.05,OR=2.238,95% CI of 1.827~3.114), systolic blood pressure(β=0.746,SE=0.273,Wald=7.082,P<0.05,OR=2.372,95%CI of 1.402~4.156),SpO2(β=0.810,SE=0.556,Wald=10.281,P<0.05,OR=2.591,95%CI of 2.211~3.391). Conclusion The factors affecting the brain injury surgical results are mainly age, GCS score, systolic blood pressure and SpO2 levels, preoperative should observe carefully these factors to improve prognosis in patients with traumatic brain injury situation.
