1.Determinations of symptomatical fractured vertebrae in treating osteoporotic vertebral compression fractures with vertebroplasty and pitfalls as well as preventive strategy

[Objective]Osteoporotic vertebral compression fractures(OVCFs)are commonly multiple fractures and this study is aimed to explore how to determine the symptomatical fractured vertebrae as well as the pitfalls in treating OVCFs using percutaneous vertebroplasty(PVP).[Method]Forty-eight OVCFs patients(78 vertebrae)undergoing PVP were retrospectively analyzed.All the patients had anteroposterior and lateral plain X-rays as well as T1W1,T2W1 and fat-compressing(STIR)MR images preoperatively.The symptomatical fractured vertebrae were determined with combination of regional pain、X-rays and MR images.Of all the patients,36 were injected unilaterally while 12 were injected bilaterally.The mean injected volume of PMMA were 3-7 ml.[Result]The back pain of all the patients were relieved to different degrees postoperatively.The back pain completely disappeared in 30 patients while the left back pain was not improved though the right back pain disappeared in 2 patients who were injected via right approach.The back pain almost disappeared but the bilateral rib pain was not improved in 1 patient with T8 vertebral fracture.No severe complications including pulmonary emboli occurred.[Conclusion]The symptomatical fractured vertebrae should be determined comprehensively not only based on preoperative X-rays but also on the MR images,especially fat-compressing MR images.Only when the fractured vertebrae demonstrate low intensity on T1W1 MR image and high-intensity on T2W1 or fat-compressing MR image can we consider them new fractures.Otherwhise,the fractured vertebrae are considered old fractures and they see no necessity to be injected.The injection should reach the most severely fractured part and if necessary the bilateral approaches are considered.PMMA should be injected after the bone cements are solid enough so as not to develop complications such as pulmonary emboli.

Purpose:To invastigate the changes of the related signaling pathways and it's mechanism of reguation of mitogen activated protein kinase cascade(MAPK) by SUM149 cells induced plasmid of dominant-negative E-cadherin mutant H-2k~d-E-cadherin.Methods:Using western blot method,we examined the signal pathway of mitogen activated protein kinase(MAPK) between the dominant-negative mutant E-cadherin transfected SUM149 cells and controls.Results:Compared with the controls,the phosphorylated extracellular signal regulated kinase P44/42 expresion of the SUM149 cells which expressed higher dominant-negative mutant E-cadherin was significantly down-regulated.Conclusions:In this cell line model,the down-regulation of P44/42 may involve the regulation of MMPs.
3.Adjacent segment disease after anterior cervical interbody fusion

[Objective]The present study was undertaken to investigate the incidence of symptomatic adjacent segment disease after anterior cervical interbody fusion(ACIF),and to identify the factors which are related to the development of this disease.[Method]From 1981 to 1997,a total of 160 patients underwent anterior cervical in terbody fusion for intervertebral disc herniation and cervical spondylosis.A total of 112 patients were followed up clinically and radiologically for more than two years.Of them,74 were men and 38 were women,the average age at operation was 51 years(ranged,31~70 years).Of the 112 patients,66 had one,44 had two and 2 had three levels of fusion.Follow-up evaluation was primarily viaclinical visited.The post-operative course of any symptoms,the findings of neurological examination and serial follow-up radiographs were performed in all patients.The diagnosis of symptomatic adjacent segment disease was based on the presence of new radiculopathy or myelopathy symptoms referable to an adjacent level,and the presence of a compressive lesion at an adjacent level by magnetic resonance imaging or myelography.We evaluated the correlation between the incidence of symptomatic adjacent segment disease and the following clinical parameters(age at operation sex,number of the levels fused)and radiological parameters(preoperative cervical spine alignment,preoperative range of motion of C_(2~7) cervical spine,antero-posterior spinal canal diameter,preoperative existence of an adjacent segment degeneration on plain radiograph,myelography and magnetic resonance imaging(MRI).[Result]The average length of follow-up was 9.4 years(ranged,2 to 19 years).Symptomatic adjacent segment disease developed in 19 out of 112 patients(19%)followed.A Kaplan-Meier survival analysis was performed in order to follow the disease-free survival of the entire series of patients.The disease free survival rates were 89% at 5 years,84% at 10 years and 67% at 17 years.The incidences of indentation of dura matter on pre-operative myelography or disc protrusion on MRI at the adjacent level were significantly higher in disease cases(P=(0.0 087),(0.0 299),respectively;Chi-square test).Howerer,the other parameters did not show a statistically significant difference.There were 7 cases(37%)who had failure of non-operative treatment and additional operations were performed.[Conclusion]The incidence of symptomatic adjacent segment disease after ACIF was higher when pre-operative myelography or MRI revealed asymptomatic disc degeneration at that level regardless of the number of the level number fused,pre-operative alignment,spinal canal diameter of fusion alignment.
4.Isolation and Purification of Adhesin of Candida Albicans and its Effect on Adherence to Host Cells

Objective To investigate the adherence mechanism of Candida albicans to host cells in molecular level. Methods and Results Yeast cell wall protein of Candida albicans were extracted, and purified fibronectin (Fn) adhesin was obtained from yeast cell wall extract. Fn adhesin was analyzed by sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS PAGE), the molecular weights of Fn adhesin were 60kd and 105kd. In order to find out the effect of the protein on adherence to host cells, human buccal epithelial cells and Candida albicans cells were incubated at 37℃ for 2 hours on a rotator, and yeasts adhering to the surfaces of epithelial cells were assayed microscopically. The adherence rate of C.albicans to buccal epithelial cells was 44 7?6 28%, and after treating the epithelial cells with Fn adhesin, the adherence rate reduced to 28 3?6 18%, P
5.Hand-Assisted Laparoscopic Splenectomy for Massive Splenomegaly: Comparative Study

Objective To explore the safety, feasibility, and techniques of hand-assisted laparoscopic splenectomy (HSLS) for massive splenomegaly. Methods Between January 2005 and December 2006, 40 patients with massive splenomegaly owning to portal hypertension were treated with HSLS or open splenectomy (OS). The two groups were comparable in age, sex, hepatic function by Child classification, and size of the spleen. Results No serious complications occurred in both the groups. Compared with the OS group, patients in the HALS group had more blood loss [ (312?61) ml vs (235?105) ml, t=2.583, P=0.014], longer operation time [(95?20) min vs (73?16) min, t=3.832, P=0.000], earlier recovery of intestinal function [(48?1) h vs (98?1) h, t=-153.093, P=0.000], and shorter postoperative hospital stay [(6?2) d vs (10?2) d, t=-6.124, P=0.000]. Conclusions HSLS is feasible and safe for patients with massive splenomegaly. Despite a longer operation time, the procedure is superior in postoperative recovery.
6.Patients′self-care management after eyelid reconstructing with free flap-skin

Objective To explore the method of self-care management education for patients who have received the eyelid reconstruction surgery by transplanting with free flap-skin. Methods There were 11 cases (15 eyes) who achieved self-care management and continued nursing, including the preoperative, postoperative nursing care of the donor-skin area, eye nursing, diet guidance, medication guidance, safety protection and its like. Results None of the cases revealed infection and all the grafts showed good biological activity among the 11 cases (15 eyes). Conclusions After self-care management education and continued nursing, the averaged hospitalization days of the patients are shortened; the cost is reduced, and the successful rate of the eyelid′s construction by free skin-flap transplantation is enhanced. Self-care management education and continued nursing care played an important role in promoting the healing and improving the successful rate of eyelid′s skin-flap transplantation.
7.Effects of antioxidant and NF-κB on the induction of iNOS gene in rat pulmonary microvascular endothelial cells in vitro

AIM:To investigate the role of NF-κB in the activation of inducible nitric oxide synthase (iNOS) gene by tumor necrosis factor alpha (TNF α) and lipopolysaccharide (LPS) in endothelial cells and effect of antioxidant on the induction of iNOS. METHODS:Rat pulmonary microvascular endothelial cell (RPMEC) was cultured and the cells were identified with antiendothelial cell antibody CD31 using immunohistochemistry(ABC). The concentration of nitrite in the culture media was determined based on Griess reaction. iNOS mRNA was analyzed using RT-PCR and Northern blot. NF-κB in cell nuclei was detected with electrophoresis mobility shift assay (EMSA). RESULTS:A marked production of nitrite in RPMECs was found after 24 hours treatment with TNF α(105 U/L) and LPS (1 mg/L) (P<0.01). The level of iNOS mRNA increased significantly after adding TNF α(105 U/L) and LPS (1 mg/L) to the cell media for 2 hours (P<0.05). Pretreatment with cycloheximide (CHX, 10 mg/L) or antioxidant, PDTC (0.1 mmol/L) or NAC (20 mmol/L) significantly decreased nitrite production and iNOS mRNA expression induced by TNF α(105 U/L) and LPS (1 mg/L) (P<0.05). Furthermore, there was a dose-effect relationship between PDTC/NAC and inhibitory effect. TNF α (105 U/L) and LPS (1 mg/L) triggered the activation and translocation of NF-κB. This effect was blocked by adding PDTC (0.1 mmol/L) or NAC (20 mmol/L) to the cell media for 1.5 hours.CONCLUSION:1.TNFα and LPS may induce iNOS gene expression at transcriptional or posttranscriptional level. The upregulation of iNOS depends on new protein synthesis. 2. The induction of iNOS gene expression by TNFα and LPS is dependent on the activation of NF-κB. 3. Antioxidants may inhibit the induction of iNOS gene through the inhibition of NF-κB activation.
8.Effects of Haikangling Mixture,a Traditional Chinese Medicine on Immunological Function in Immunosuppressed Mice

OBJECTIVE:To explore the effects of Haikangling Mixture on immunological function in immunosuppressed mice induced by cyclophosphamide. METHODS: Mice were given Haikangling Mixture intragastrically for 14d, and the level of hemolysin was determined by sheep erythrocyte (SRBC) immunization to exploer the influence of Haikangling on humoral immunity in mice; splenic lymphocytes were measured by MTT to observe the influence on cell immunity; the clearance of macrophage were measured by carbon particle clearance method to explore the influence on non-specificity immunity. RESULTS: The level of hemolysin, the cell reproductive activity, splenic lymphocytes and the phagotrophy of mononuclear macrophage were improved obviously by Haikangling Mixture in immunosuppressed mice induced by cyclophosphamide. CONCLUSION: Haikangling Mixture can enhance the immunological function in immunosuppressed mice induced by cyclophosphamide.
9.The role of mu-opioid receptors in seizure susceptibility induced by kainic acid

Objective To investigate the role of hippocampal mu-opioid receptors (MORs) in the formation of seizure susceptibility induced by kainic acid (KA). Methods The SD rats were injected subcutaneously with a convulsive dose of KA (10 mg/kg). Then mini-osmotic pumps were used for intrahippocampal injection with a selective MOR antagonist ?-FNA, and a selective MOR agonist PL017 in these rats. 7 days later, the seizure susceptibility was checked by a subthreshold dose of KA (5 mg/kg). Results All animals showed motor seizures over Stage 4 after a convulsive dose KA injection. 7 days later, 100% animals showed seizures over Stage 4 in KA, CSF + KA and PL017+KA groups after injection of subthreshold dose of KA, but only 28.5% in PL017+KA group. Mean latency of PL017+KA group was (10.1?3.0) minutes, shorter significantly than that of KA and CSF+KA groups (0.010.05). METHODS:Nitrite was determined based on Griess reaction. iNOS mRNA was analyzed using Northern blot. NF ?B in the cell nucleole was detected with electrophoretic mobility shift assay.RESULTS:(1)NO production and iNOS mRNA expression induced by LPS and TNF? was blocked by pyrrolidine dithiocarbamate(PDTC) or N-acetylcysteine(NAC). (2)LPS and TNF? triggered the activation and translocation of NF ?B, and PDTC or NAC inhibited the activation of NF ?B induced by LPS and TNF?.CONCLUSIONS:(1)The induction of iNOS gene by TNF? and LPS is dependent on the activation of NF ?B.(2)Antioxidants may inhibit the induction of iNOS gene through the inhibition of NF ?B activation.
