2.The biological advance in acute lymphoblastic leukemia of t (4;11) in infants

Infant acute lymphoblastic leukemia B (B-ALL) accounts for 10% of childhood ALL. Eighty percent of infant B-ALL was caused byMLL gene rearrangement (MLL-r). The overall survival rate of ALL was less than 35% in infants with MLL-r. Among infant ALL with MLL-r, infants with positivefusion geneMLL-AF4 (MA4) formed by chromosome t (4;11) had even poor prognosis. Studies in monozygotic twins and archived blood spot at birth had veriifed that fusion gene MA4 originated from antenatal. Whole genome sequencing found that t (4;11) alone might be sufifcient to spawn leukemia. This paper is going to summarize the advances in biological characteristics such as clinical features, cellular origin, genomics and disease models of normalMLL gene and infant B-ALL withMA4.
3.Comparison of surgical results between single-hole laparoscopy and conventional laparoscopy in ovarian cystectomy

Objective:To investigate the effect of single-port laparoscopy and conventional laparoscopic surgery in ovarian cystectomy.Methods:The clinical data of 139 patients with laparoscopic surgery for ovarian cyst admitted to the same department of gynecology from December 2016 to December 2019 in Suzhou Hospital Affiliated of Anhui Medical University were retrospectively analyzed, and they were divided into single-port laparoscopic surgery group (62 cases) and conventional laparoscopic surgery group (77 cases) according to different surgical methods. The clinical indictors, the levels of estrogen, T lymphocyte subgroups, the complications rate and incision satisfaction rate between two groups were compared.Results:The operation in both groups was smooth and there was no conversion to open surgery. Compared with the conventional laparoscopic surgery group, the surgery time, intraoperative blood loss in single-port laparoscopic surgery group were higher, the hospital stays in single-port laparoscopic surgery group was shorter, but there were no significant differences ( P>0.05). The levels of follicle-stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E 2) in two groups before operation had no significant differences ( P>0.05). At 30 d after operation, the levels of FSH, LH and E 2 in single-port laparoscopic surgery group were higher than those in conventional laparoscopic surgery group, but the differences were no statistically significant ( P>0.05). Before operation, the levels of CD 4+, CD 8+ and CD 4+/CD 8+ in two groups has no significant differences ( P>0.05). At 3 d after operation, the levels of CD 4+, CD 8+ and CD 4+/CD 8+ in single-port laparoscopic surgery group were higher than those in conventional laparoscopic surgery group: 0.51 ± 0.03 vs. 0.47 ± 0.02, 0.41 ± 0.23 vs. 0.39 ± 0.05, 1.36 ± 0.02 vs. 1.24 ± 0.02, and the differences were statistically significant ( P<0.05). The rate of complication in two groups had no significant difference ( P>0.05). At 3 d after operation, the scores of visual analogue scale (VAS) of pain in single-port laparoscopic surgery group were lower than that in conventional laparoscopic surgery group: (1.12 ± 0.21) scores vs. (3.56 ± 0.65) scores, and the difference was statistically significant ( P<0.05). The incision satisfaction rate in single-port laparoscopic surgery group was significantly higher than that in conventional laparoscopic surgery group: 90.32%(56/62) vs. 72.73%(56/77), and the difference was statistically significant ( P<0.05). Conclusions:Single-port laparoscopy can be safely and smoothly implemented in ovarian cystectomy, and can reduce postoperative pain, improve cell immunity, and have better cosmetic result.
4.Efficacy of hirudin in treating immunoglobulin A nephropathy with hematuria: a randomized controlled trial

OBJECTIVE: To investigate the curative effects and side effects of hirudin in treating immunoglobulin A nephropathy (IgAN) with hematuria and minimal proteinuria in a short-term. METHODS: Two hundred and sixty-two histologically confirmed cases of IgAN with hematuria and minimal proteinuria from 1998 to 2007 were randomly divided into hirudin-treated group (peroral administration of Maixuekang capsules) and dipyridamole-treated group (peroral administration of dipyridamole). In the two groups, contrast analysis of conformation and counts of erythrocytes in urine, urine protein quantitation in 24 hours, levels of serum creatinine (Scr) and creatinine clearance rate (Ccr), blood lipid, five items of blood clotting and side effects was performed. RESULTS: After six-month treatment, the anisotrophy rate and the counts of erythrocytes in urine, and the urine protein quantitation in 24 hours in hirudin-treated group were decreased distinctly as compared with pre-treatment (P<0.01) and dipyridamole-treated group (P<0.05). On the other hand, Ccr was increased obviously in hirudin-treated group as compared with pre-treatment and dipyridamole-treated group (P<0.01). The blood lipid was also ameliorated in hirudin-treated group, but there was no significant difference. The anticoagulation effect of hirudin was better than dipyridamole (P<0.01). Efficacy assessment showed that the total response rate, complete remission rate and predominance remission rate in hirudin-treated group were higher than those in dipyridamole-treated group. Few side effects were found in both groups, and the rate of adverse reaction in gastrointestinal tract was lower in hirudin-treated group as compared with that in dipyridamole-treated group (P<0.05). CONCLUSION: Compared with dipyridamole, hirudin has superiority in kidney protection and decreasing the anisotrophy rate, counts of erythrocytes in urine and the urine protein.
5.Research on the clinical characteristics and prognosis of children with chronic myeloid leukemia in the blast phase

Objective:To explore the clinical characteristics and prognosis of children with chronic myeloid leukemia in the blast phase (CML-BP) .Methods:The clinical characteristics, treatment measures, and survival outcomes of 28 children with CML-BP were analyzed in our hospital from January 2008 to November 2022.Results:The male to female ratio of the 28 children with CML-BP was 1.15∶1. The median age of diagnosis of CML-BP was 10 years, and the median follow-up time was 79 months. During the diagnosis of CML, four children were in the BP, one was in the accelerated phase (AP) and 23 children were in the chronic phase (CP). Among the 23 children with CML-CP, 75% had progressed directly from CP to BP without experiencing the AP. Among the children diagnosed with CML-BP, 71.4% were classified as chronic myeloid leukemia lymphoid blast phase (CML-LBP), 25.0% belonged to the chronic myeloid leukemia myeloid blast phase (CML-MBP), and 3.6% belonged to the chronic myeloid leukemia mixed phenotype acute leukemia (CML-MPAL). Treatment with hemaopoietic stem cell transplantation (HSCT) after tyosine kinase inhibitor (TKI) combined with chemotherapy was administered to 19 children, two children received HSCT after TKI alone, and seven children received TKI combined with chemotherapy but without HSCT. The 5-year overall survival of the 28 children with CML-BP was 59.3%.Conclusion:The direct progression of BP from CP is greater in children with CML-BP compared with adults, and the overall prognosis of children with CML-BP is poor.

7.Construction a Risk Prediction Model of IgA Nephropathy Proteinuria Treated by Traditional Chinese Medicine Based on Random Survival Forest Model

Objective Constructing a risk prediction model of IgA nephropathy proteinuria treated by traditional Chinese medicine based on random survival forest model,Screening prognostic risk factors of IgA nephropathy proteinuria.Methods Collecting retrospectively clinical data of 129 cases diagnosed with IgA nephropathy,randomly divided them into training set(60%)and test set(40%).The risk prediction model of IgA nephropathy proteinuria was constructed in the training set with the random survival forest model,and the prognostic risk factors were screened by VIMP method.The accuracy of risk prediction model was validated in the test set with time-dependent ROC curve(tdROC).Results According to the result of VIMP,the prognostic risk factors for IgA nephropathy proteinuria are in the order of eGFR,hypertension,traditional Chinese medicine,24 hUPRO>1 g,genomo sclerosis ratio,Lee grading,fat,hyperlipidemia,hypertrophymia,hyparmane ledmia,Anemia,age and gender.The eGFR was negatively and non-linearly associated with the risk rate of developing persistent proteinuria.Glomerulosclerosis ratio greater than 0.3 is approximately linearly and positively associated with the risk rate of persistent proteinuria.Conclusion Random survival forest model has good predictive performance in the risk prediction model of IgA nephropathy proteinuria treated by traditional Chinese medicine.This risk model can determine the result of IgA nephropathy treated by traditional Chinese medicine,and which is helpful for clinical follow-up monitoring and formulation of individualized treatment plans.
8.Effects of β-sitosterol on the function of synovial fibroblasts in rheumatoid arthritis and its mechanism

OBJECTIVE To investigate the effects of β-sitosterol on the function of rheumatoid arthritis (RA) fibroblastic synoviocytes MH7A cells and its mechanism. METHODS Network pharmacology was adopted to screen the targets of β-sitosterol and the targets for the treatment of RA. After the intersection of them, topological analysis was performed to find the most critical target in the treatment of RA. MH7A cells were treated with different concentrations (0, 5, 10, 20, 40 μmol/L) of β-sitosterol, and CCK-8 was used to assay cell viability for screening the optimal concentration of β-sitosterol. MH7A cells were induced by 10 ng/mL TNF-α in vitro and treated with β-sitosterol (the optimum concentration). CCK-8 and EdU were used to detect the ability of cell proliferation. Scratch experiment and Transwell invasion assay were used to analyze cell migration and invasion. The levels of interleukin-1β (IL-1β) and IL-6 in cell supernatant were detected by enzyme-linked immunosorbent assay (ELISA). The mRNA and protein expressions of peroxisome proliferator-activated receptor α (PPARα) were measured by qRT-PCR and Western blot, respectively. The siRNA targeting PPARα was transfected into MH7A cells, and the effects of β-sitosterol on cell proliferation, migration, invasion, the secretion of inflammatory factors and the expression of PPARα after PPARα knockdown were detected by the above experimental methods. RESULTS PPARα was the most critical target of β-sitosterol in the treatment of RA. The optimal concentration of β-sitosterol was 20 μmol/L. Compared with model group, β-sitosterol decreased the viability of MH7A cells, and the number of proliferating cells also decreased significantly (P<0.05); the cell migration rate and the number of cell invasion decreased significantly (P<0.05). The levels of IL-1β and IL-6 were also significantly decreased (P<0.05), and the mRNA 15 and protein expression levels of PPARα were significantly increased (P<0.05). Compared with negative control small interfering RNA group, after PPARα knockdown, the cell viability increased by about 35.6% (P<0.05), the number of cell proliferation, the cell migration rate and the number of cell invasion increased significantly (P<0.05), and the levels of IL-1β and IL-6 also increased significantly (P<0.05). CONCLUSIONS β-sitosterol could effectively inhibit the proliferation, migration, invasion and secretion of inflammatory factors in MH7A cells, the mechanism of which may be associated with activating PPARα pathway.
9.Effect of Yishen Huoxue Prescription on Endothelial Structure and Function in Mice with Renal Interstitial Fibrosis

ObjectiveTo explore the mechanism of Yishen Huoxue prescription in renal interstitial fibrosis (RIF) from the perspective of endothelial cell and cell energy metabolism. MethodThe model was successfully established by unilateral ureteral obstruction (UUO). Seventy-five SPF C57BL/6 mice were randomly divided into a model group, a resveratrol group (50 mg·kg-1·d-1), three Yishen Huoxue prescription low, medium, and high-dose groups (7.1, 14.2, 28.4 g·kg-1·d-1), with 15 mice in each group. In addition, another 15 mice were used to prepare sham operation model. Mice in the sham operation group and the model group were gavaged with equal volume of normal saline. All mice were sacrificed on 7, 14, and 21 d after modeling. The protein expression of platelet endothelial cell adhesion molecule 31 (CD31) was detected by immunohistochemical S-P method. The expression of α-smooth muscle actin (α-SMA), collagen Ⅳ (Col-Ⅳ), angiopoietin 1(Ang-1) and tyrosine kinase receptors 2 (Tie-2), vascular endothelial growth factor (VEGF), vascular endothelial cadherin (VE-cadherin), and occludin in renal tissues was detected by Western blotting. The mRNA expressions of Ang-1/Tie-2, VEGF, VE-cadherin, and occludin in renal tissues were detected by Real-time fluorescence quantitative polymerase chain reaction(Real-time PCR), and the levels of reactive oxygen species (ROS) in mice were detected by enzyme linked immunosorbent assay (ELISA). ResultAs compared with the sham operation group, the expression of CD31 in renal tissues of the model group was significantly decreased and worsened with the extension of modeling time (P<0.05), α-SAM and Col-Ⅳ protein expression levels were significantly increased (P<0.01), but the expression of CD31 was stable in 14-21 d. ROS levels were significantly increased (P<0.01), and the protein and mRNA expressions of Ang-1/Tie-2, VEGF, VE-cadherin, and occludin were significantly down-regulated (P<0.01). As compared with the model group, the expression of CD31 was increased (P<0.05), and α-SAM and Col-Ⅳ in the resveratrol group and the medium and high-dose Yishen Huoxue prescription groups were significantly decreased (P<0.01). The ROS content was significantly decreased (P<0.01), and the protein and mRNA expressions of Ang-1/Tie-2, VEGF, VE-cadherin, and occludin were up-regulated (P<0.01), As compared with the resveratrol group, the protein expressions of Ang-1/Tie-2, VEGF, VE-cadherin, and occludin in the medium and low-dose Yishen Huoxue prescription groups were significantly different (P<0.01). There was no significant difference in the mRNA expressions of CD31 and Ang-1/Tie-2 in the high-dose Yishen Huoxue prescription group, and no significant difference in the ROS level in the medium-dose Yishen Huoxue prescription group. ConclusionThe anti-RIF effect of Yishen Huoxue prescription may be related to promoting vascular endothelial repair, regulating mitochondrial ROS to reduce oxidative stress, protecting the integrity of renal endothelial structure, delaying cell apoptosis, and maintaining cell energy metabolism.
10.Protective role of berberine on ulcerative colitis through modulating enteric glial cells-intestinal epithelial cells-immune cells interactions.

Ulcerative colitis (UC) manifests as an etiologically complicated and relapsing gastrointestinal disease. The enteric nervous system (ENS) plays a pivotal role in rectifying and orchestrating the inflammatory responses in gut tract. Berberine, an isoquinoline alkaloid, is known as its anti-inflammatory and therapeutic effects in experimental colitis. However, little research focused on its regulatory function on ENS. Therefore, we set out to explore the pathological role of neurogenic inflammation in UC and the modulating effects of berberine on neuro-immune interactions. Functional defects of enteric glial cells (EGCs), with decreased glial fibrillary acidic protein (GFAP) and increased substance P expression, were observed in DSS-induced murine UC. Administration of berberine can obviously ameliorate the disease severity and restore the mucosal barrier homeostasis of UC, closely accompanying by maintaining the residence of EGCs and attenuating inflammatory infiltrations and immune cells overactivation. , berberine showed direct protective effects on monoculture of EGCs, bone marrow-derived dendritic cells (BMDCs), T cells, and intestinal epithelial cells (IECs) in the simulated inflammatory conditions. Furthermore, berberine could modulate gut EGCs-IECs-immune cell interactions in the co-culture systems. In summary, our study indicated the EGCs-IECs-immune cell interactions might function as a crucial paradigm in mucosal inflammation and provided an infusive mechanism of berberine in regulating enteric neurogenic inflammation.
