1.Application of intracavitary electrocardiography in the insertion of the groshong peripherally inserted central catheters

Objective To assess the application of intracavitary electrocardiography (IC-ECG) during the insertion of the Groshong peripherally inserted central catheters (PICC).Methods A total of 168 patients (aged 20-76 years) with gynecologic tumors who required the insertion of Groshong PICC because of various clinical conditions between January 2013 and December 2014 were selected into this study in Zhongnan Hospital of Wuhan University.During PICC insertion, the P-wave of IC-ECG helped general judgments about the position and direction of the tip of catheters, hence guiding the intubation.Precise position was determined by Xray following the fastening of catheters, based on which the accuracy and sensitivity of IC-ECG in the insertion of Groshong PICC were evaluated.Results All the cases presented typical changes of P-wave.However, 15 cases exhibited no such alternations in the first placement however deep or shallow the catheters were, in whom the typical P-wave was induced after withdrawal and replacement of the catheters with adjustment of patient position.According to the X-ray (gold standard), the accuracy and sensitivity of IC-ECG in diagnosing position of tip in the superior vena cava (SVC) were 98.8% and 98.8%, respectively.The accuracy of diagnosing the position at the optimum location in the SVC reached 97.0%.Conclusions A high peaked P-wave of similar shape could be observed in IC-ECG in the insertion of Groshong PICC.IC-ECG could remarkably increase the positioning accuracy of PICC tip into the SVC;meanwhile, the optimal inserted length of the catheters could be determined by monitoring the changes of the P-wave with IC-ECG during insertion.
2.Effectiveness of using bandage for punctured wound of postoperative patients with neurosurgical in-tervention treatment

Ohjective To know the proper wraping method for postoperative patients with neurosurgical intervention treatment. Methods Divided 94 patients with neurosurgical intervention treatment into the ban-dage group and the gauze group randomly, there were 47 cases in the each group. Bandage and gauze was used in the two groups for punctured wound respectively, and then compared the condition of complications and puncture location between the two groups. Results The incidence rate of puncture-associated comphcations in the bandage group was sinificant lower than that of in the gauze group. Conclusions Wraping by bandage for postoperative patients with neurosurgical intervention treatment is more effective than gauze method.
3.The Effect of Diet on Expression of Protein Tyrosine Phosphatase 1B in Liver of Insulin Resistant Rats

Objective To investigate the effect of diet on the expression of protein tyrosine phosphatase (PTP) 1B in liver of insulin resistant rats induced by high fat diet. Methods 30 male Wistar rats were randomly divided into the control group (n=10) which was given basic diet, and the model group (n=20) which was given high fat diet. After 4 weeks, the model group was randomly divided into 2 subgroups:insulin resistant group which continued to receive high fat diet, and diet-treated group which accepted diet intervention for 6 weeks. Objective To investigate the change on the extent of coagulated lesion under various interrupting methods of hepatic blood flow in vivo animal livers, and the clinical effect of combining tran scathcter arterial chernoembolization(TACE) with percutaneous microwave coagulation therapy (PMCT) for primary and metastatic hepatic tumors. Methods Using FORSEA MTC-3-500s microwave coagulator, we performed microwave tissue coagulation in vivo liver of 6 pigs at 60 W, 2 min and of 20 rabbits al 40 W, 2 min with or without the interruption of hepatic blood flow. 27 patients with primary hepatic carcinoma(30 nodules) and 8 patients with metastatic carcinoma( 12 nodules) underwent the combination therapy of 1 - 4 sessions of TACK followed by within 3-10 days by 1 - 3 sessions of PMCT guided by ultrasonography and/or CT. The 42 lesions measured from 2. 3 cm to 15.6 cm were taken place. Objective To analyse effects of moxifloxacin solution for aerosol inhalation on inflammatory mediators and blood gas analysis index in patients with chronic obstructive pulmonary disease with respiratory failure.Methods 60 patients who were diagnosed with chronic obstructive pulmonary disease with respiratory failure in our hospital were collected.All patients were randomly divided into experimental group and control group,30 cases in each group.On the basis of conventional treatment, control group were given sulfate for inhalation shating amine alcohol solution 2mg per times, 3 times per day atomization inhalation treatment,and experimental group was given 2 mg/times, 4 times per day atomization inhalation treatment,two were treated for 2 weeks.After the treatment, the serum levels of CRP,TNF-α,IL-10,IL-18 and arterial blood PO2 ,PCO2 levels were detected in all patients.Results Compared with control group post-treatment, the serum levels of CRP, IL-18 and TNF-αwere lower in experimental group (P<0.05), and the serum level of IL-10 was higher;the arterial blood level of PO2 was higher,and the arterial blood level of PCO2was lower in experimental group (P<0.05). Conclusion Effect of moxifloxacin solution for aerosol inhalation can significantly reduce the serum CRP,TNF-αand IL-18 levels, improve serum IL-10 level in patients with chronic obstructive pulmonary disease with respiratory failure, ameliorate PO2 and PCO2 .

7.Clinic evaluation of interventional treatment for renal artery aneurysm

Objective To evaluate the safety and efficacy of interventional procedure for treating the renal artery aneurysm (RAA).Methods From Jan 2009 to Apr 2014,17 patients,who were diagnosed as RAA and accepted the interventional therapy,were reviewed in our hospital.The mean age in those patients,including 7 males and 10 females,was (46.4±10.3) years old (range from 20 to 67 years old).The related symptoms included backache in 4 cases,abdominal pain in 4 cases,intermittent hematuria in 2 cases,chyluria in one case,oligouria in one case.9 cases were diagnosed as multiple RAA and 8 cases were confirmed as signle cases.In 17 cases,31 aneurysms were found,including 26 true aneurysms,5 pseudoaneurysums,17 sacculated aneurysms,4 spindle-like aneurysms,4 irregular shape aneurysms,4 parenchyma aneurysm and 2 dissecting aneurysm.8 aneurysms located in the main renal artery,19 aneurysms located in the branch of renal artery,4 aneurysums located in the renal parenchyma.Intracavitary coil embolization was used in 4 patients.We carried out parent artery embolization in 3 patients.A combination of the former techniques was performed in 6 cases.Covered stent placement was operated in one case.Combination of the intracavitary coil embolization and nude stent placement were performed in 2 patients.We used two techniques in one patient with multiple artery aneurysms in both sides.Results The interventional treatment of RAAs succeed at the first operation in 16 of 17 patients.17 cases were followed-up from 3 to 53 months (mean 23 months).No severe complications or death cases occurred in this study.Urine occult blood in 3 patients turned to negative after one week.Primary symptoms such as gross hematuria,abdominal pain,lumbodorsalgia,fever vanished or obviously eased after a month.Laboratory tests showed that normal level in SCr,BUN,routine urinalysis 3 months,6 months and 1 year later.No tendency of stent and coil stent shifting was found in 16 patients and the parent arteries were patent in 8 cases,with reexamination bv ultrasonic or computed tomography angiography (CTA).Conclusions Interventional techniques are minimally-invasive,safe and effective methods for treating the RAAs.
8.Effect of sevoflurane preconditioning on heme oxygenase-1 expression in lung tissues during one lung ventilation in rats

ObjectiveTo investigate the effect of sevoflurane preconditioning on heme oxygenase-1 (HO-1) expression in lung tissues during one lung ventilation (OLV) in rats.MethodsTwenty-four male SD rats weighing 220-250 g were randomly divided into 4 groups ( n =6 each):control group (group C) ; two lung ventilation group (group T); OLV group (group O) and sevoflurane preconditioning+ OLV group (group SO).The animals were anesthetized with intraperitoneal pentobarbital sodium 40 mg/kg.In group T,the animals were tracheal intubated and bilateral lungs were ventilated for 1 h (VT 10 ml/kg,RR 60 bpt/min,I∶E 1∶2) and PETCO2 was maintained at 35-50 mm Hg.In groups O and SO,the animals were tracheal intubated and OLV was performed for 1 h (VT 5 ml/kg,RR 80 bpt/min,I:E 1:2) and PETCO2 was maintained at 35-50 mm Hg.In group SO,2.4%sevoflurane was inhaled for 30 min before OLV and then washed out by inhalation of oxygen for 15 min.The left lung tissues were removed in groups C and T,and the bilateral lung tissues were removed in groups.O and SO for microscopic examination and determination of wet/dry lung weight ratio (W/D ratio) and expression of HO-1 (by Western blot).Results Compared with group C,W/D ratio was significantly increased and the expression of HO-1 was up-regulated in left lung tissues in group T and in bilateral lung tissues in groups O and SO ( P <0.05).Compared with group T,W/D ratio was significantly increased and the expression of HO-1 was up-regulated in bilateral lung tissues in group O and in right lung tissues in group SO ( P < 0.05).Compared with group O,W/D ratio was significantly decreased,the expression of HO-1 was up-regulated (P < 0.05) and the pathological changes were reduced in bilateral lung tissues in group SO.ConclusionThe mechanism by which sevoflurane preconditioning reduces OLV-induced lung injury is related to up-regulation of HO-1 expression.
9.Effects of Compound Uncaria Hypotensive Tablets on Expressions of MCP-1 and MMP-9 of Vascular Remodeling in Spontaneously Hypertensive Rats

Objective To observe the effects of Compound Uncaria Hypotensive Tablets on expressions of MCP-1 and MMP-9 of vascular remodeling in spontaneously hypertensive rats (SHR); To discuss it possible mechanism of action. Methods Totally 24 12-week old male SHR were randomly divided into SHR model group, Compound Uncaria Hypotensive Tablets group, and positive medicine group, with 8 rats in each group. Another 8 WKY rats were set as normal control group. Medication groups were given relevant medicine for gavage for successive 6 weeks. Noninvasive tail cuff method was used to observe blood pressure; morphological changes in thoracic aorta and renal artery were observed by HE staining; immunohistochemistry was used to detect the protein expressions of MCP-1 and MMP-9 in thoracic aortic wall. Results Protein expressions of MCP-1 and MMP-9 in thoracic aortic wall of SHR model group were significantly higher than those of normal control group (P<0.01); Compared with the SHR model group, protein expressions of MCP-1 and MMP-9 in thoracic aortic wall decreased significantly in the medication groups (P<0.05, P<0.01); Compared the Compound Uncaria Hypotensive Tablets group and positive medicine group, there was no obvious difference in protein expressions of MCP-1 and MMP-9 in thoracic aortic wall. Conclusion Compound Uncaria Hypotensive Tablets can reduce the blood pressure of SHR, reduce inflammation reaction, and regulate vascular remodeling, which mechanism may be related to down-regulation of expressions of MCP-1 and MMP-9 in SHR aortic endothelial cells.
10.Comparison of the artifacts on magnetic resonance imaging caused by different porcelain-fused-to-metals

BACKGROUND:The patients with porcelain-fused-to-metal crowns who do examination of magnetic resonance imaging (MRI) can cause artifacts. In recent years, researching for MRI artifacts of different porcelain-fused-to-metal materials has been some progress, but there are less quantitatively reports on the MRI artifacts of different porcelain-fused-to-metal materials.
OBJECTIVE:To evaluate the artifact sizes shown on fast spin-echo T 2-weighted sequence caused by different kinds of porcelain-fused-to-metal crowns.
METHODS:Forty-eight lower right first molar crown patients who had MRI examination in MRI room were enrol ed. The patients were divided into nickel-chromium al oy group, cobalt-chromium al oy group and titanium crown group. Al patients were examined with fast spin-echo T 2-weighted sequences by means of 1.5 T MRI apparatus. MRI artifacts areas of same sequence on the MRI images of different porcelain-fused-to-metal materials were analyzed with variance test.
RESULTS AND CONCLUSION:Forty-five cases appearing to have high signal samples with clearly curved boundary zone that can be measured were selected, 15 cases for each material. Different artifact sizes were produced on the same sequence of different porcelain-fused-to-metal materials, which were (321.67±33.29) mm in the nickel-chromium al oy group, (263.53±34.95) mm2 in the cobalt-chromium al oy group, and (143.67±31.13) mm2 in the titanium crown group. There were significant differences between groups (P<0.05). The artifact size is smal est for the titanium crown and largest for the nickel-chromium al oy crown.
