1.Qualitative study on the experience and needs of caregivers of patients with hematopoietic stem cell transplantation

Objective To investigate the experience and needs of caregivers of patients with hematopoietic stem cell transplantion (HSCT) , in order to provide the basis for targeted intervention. Methods Using qualitative phenomenological research methods, semi- structured interviews of 10 caregivers of HSCT patients were conducted, then the data were summarized and analyzed. Results Four themes were extracted: heavy caregiving burden, adaptation to the caregiving role, professional support needs, humanistic care needs. Conclusions The medical staff should strengthen the health education on HSCT caregivers to improve their ability and their own health concerns, alleviate their psychological pressure, and mobilize social forces to reduce the burden of care.

OBJECTIVE To know more clearly about the situation of Hepatitis B virus markers in clinical medical workers and take further interventional strategies to protect high risk medical workers.METHODS Hepatitis B virus markers in doctors,nurses and medical checkers who have contacted with patients′ blood,body fluid,or other occupational hazard situation,were detected by of ELISA.RESULTS Among the 587 medical workers detected,311 were with deficiency of active immunity(52.98%),196 were HBV infectors(33.39%).CONCLUSIONS Medical workers are in high risk groups of HBV infection.Medical institutions should attend to their self-protection and encourage them to take HBV vaccine to prevent iatrogenic transmission.
4.The protective effect of anti-macrophage migration inhibitory factor monoclonal antibody on oleic-acid-induced acute lung injury

AIM: To investigate the protective effect of anti-macrophage migration factor monoclonal antibody (anti-MIF MAb) on oleic-acid-induced acute lung injury (ALI) rats and its influence on the expression level of MIF and intercellular adhesion molecule-1(ICAM-1). METHODS: The rats were subjected to injection of oleic acid (oleic acid group) or saline solution (control group). One hours before administration of oleic acid, the rats were intraperitoneally injected with anti-MIF antibody (5 mg/kg) as the treatment group. After injecting oleic acid or saline for 4 hours, the PaO_2, lung permeability index (LPI), the number of macrophage and the level of soluble ICAM-1 (sICAM-1) in the bronchial alveolar lavage fluid (BALF) were measured. The expression level of MIF mRNA and ICAM-1 mRNA in the lung were detected by in situ hybridization, and the degree of macrophage infiltration and the expression of MIF were evaluated by double staining immunocytochemistry. 5.CD4+CD25+CD127low/- regulatory T cells and related cytokines in patients with psoriasis vulgaris

Objective To explore the role of CD4+CD25+CD127low/-regulatory T cells (Tregs) in the pathogenesis of psoriasis vulgaris(PV). Methods Flow cytometry analysis was used to detect the amount of Tregs in peripheral blood and ELISA to test the levels of IL-10 and TGF-β1 in blood serum; the suppressive function of Tregs on autologous CD4+CD25-T cells was determined by MTT method. Results No significant difference was found in the proportion of Tregs in PV patients and healthy controls(P>0.05). There was a diminished suppression of Tregs from patients on autologous CD4+CD25- responder T cell proliferation in PV patients when compared with that in controls (P < 0.01). The serum level of IL-10 in patients was lower than that in controls (P < 0.01) while that of TGF-β1 in PV patients was significantly higher than that in controls(P < 0.01). Conclusion Abnormal function of Tregs and low secretion of IL-10 in PV patients might be related to the pathogenesis of psoriasis.
6.Pathogenic and clinical presentation of bullous rash in hand, foot and mouth disease.

OBJECTIVETo investigate the pathogenic and clinical presentation and laboratory tests of bullous rash in hand, foot and mouth disease (HFMD) in Xi'an from January 2013 to December 2014 by retrospective analysis.
METHODA total of 224 specimens were collected from clinically diagnosed HFMD cases who were characterized by widespread mucocutaneous bullous reactions in Xi'an Children's Hospital from January 2013 to December 2014, the identification and subtyping of the isolates were conducted with real-time fluorescent quantitative RT-PCR. A retrospective analysis was performed to analyze the clinical presentation, laboratory tests and late follow-up problems of the HFMD.
RESULTIn the clinically diagnosed HFMD cases who were characterized by widespread mucocutaneous bullous reactions, 207 were caused by coxsackievirus A6 (CA6), accounting for 92. 4% of all cases with bullous, 4 were caused by enterovirus 71 (EV71), accounting for 1.8%, 10 were caused by coxsackievirus A16 (CA16), accounting for 4. 5%; 4 cases were negative for these viruses. In the cases positive for intestinal virus-nucleic acid, 130 were male, 90 were female; male to female ratio was 1. 44: 1, 203 were <5 years old, accounting for 92. 3%. Leukocytosis was found in 75 cases (34. 1%); high-sensitivity C-reactive protein (hsCRP) increased in 200 cases (90. 9%); elevated myocardial enzyme CK-MB was found in 35 cases (15. 9%), alanine aminotransferase increased in 15 cases (6. 8%); 187 cases had fever (85. 0%). None of the cases had serious complications such as encephalitis or myocarditis. In the course of the critical phase bullous rash or large vesicle-like changes, obvious itching, and facial rash appeared. After the fluid in the bullae was absorbed or the bullae ruptured or became ulcerated, scar formation and large areas of exfoliation occurred, with no effusion on the newly formed epithelium in the base, without significant pigmentation on later follow-up. In the late follow up process, 52 cases in CA6-positive patients (25. 1%) developed onychomadesis within 2-4 weeks after onset, 1 to 8 nails, an average of 4. 3 fell off, new nails grew, the nail bed showed no structural abnormalities and hyperplasia after falling off, the surface was smooth, had no hypertrophy, left no sequelae.
CONCLUSIONThe pathogen in HFMD characterized by widespread bullous reactions was mainly the CA6, this kind of HFMD was mainly mild type, with significant itching, later the bullae may have scar formation and skin exfoliation, in some cases onychomadesis may occur.

8.Research of prognosis assessment by four kinds of scoring systems in the patients of ventilator-associated pneumonia

Objective To study the significance of the prognosis assessment by acute physiology and chronic health evaluation (APACHE Ⅱ ),sequential organ failure assessment (SOFA) score,clinical pulmonary infection score(CPIS) and multiple organ dysfunction syndrome (MODS) score in the patients of ventilator-associated pneumonia (VAP). Methods The clinical data of 68 cases with VAP in the ICU or RICU were studied. APACHE Ⅱ , SOFA and MODS scores on admission and APACHE Ⅱ , SOFA, CPIS and MODS scores on the first 24-hour of VAP diagnosis were recorded. The area under the receiver operating characteristic curve(AUROC ) and Logistic regression were used to estimate the prognostic ability by the four kinds of scoring systems. Results The APACHE Ⅱ , CPIS, MODS and SOFA scores on the first 24-hour of VAP diagnosis were significantly higher in non-survivors than those in survivors. AUROC of APACHE Ⅱ ,SOFA,MODS and CPIS respectively were 0.80,0.75,0.73,0.71. Logistic regression analysis showed that only APACHE Ⅱ> 18 scores on the first 24-hour of VAP diagnosis was an independent predictor of the mortality (OR: 5.7,95% CI: 1.9 - 20.0, P = 0.013). Conclusion The APACHE Ⅱ on the first 24-hour of VAP diagnosis may be a useful index in predicting progress of patients with VAP.

Objective To modify a classic two-vessel occlusion (2VO) modeling method in order to decrease the systematic errors in the behavioral experiments such as Morris water maze.Methods Thirty-two adult male Wistar rats were randomly allocated into classic 2VO model,modified model,sham operation and sham ligation groups (n =8 in each group).Only the bilateral common carotid arteries were ligated in the classic 2VO model group; the common carotid arteries were clipped intermittently,and the origins of pterygopalatine arteries of the internal carotid arteries were high selectively ligated in the modified model group; the common carotid arteries were only ligated intermittently in the sham ligation group; and only the common carotid arteries and the upper segment of pterygopalatine artery branches were separated in the sham operation group.The rat behavior was evaluated using the pupillary light reflex,Morris water maze and eight-arm radial maze.HE staining was used to observe the histological changes.Results The Morris water maze escape latency (F =72.169 - 163.102,all P < 0.001) and the number of reference memory errors of eight-arm radial maze (F =33.515-74.726,all P <0.001) in the modified model and the classic 2VO model groups were longer and higher than those in the sham operation group.The pupillary light reflex of the rats was lost in the classic 2VO model group and the pupillary light reflex of the rats was normal in other groups.The reaching platform time in the classic 2VO model group was significantly longer than that in the modified model and sham operation groups (P <0.001).The percentage of target quadrant dwell time was also decreased significantly (at day 7 after procedure:F =13.770,P <0.001 ; at day 90 after procedure:F =14.780,P <0.001).HE staining showed pathological changes such as the cells decrease in hippocampal CA1 region and leukoaraiosis in the modified model and the classic 2VO model groups.In addition,there were more vacuole-like changes in the rat optic nerve region in the classic 2VO model group,while there were no such changes in the modified model group.Conclusions Establishing vascular dementia model with permanent occlusion of bilateral internal carotid arteries after intermittent occlusion of bilateral carotid arteries could avoid severe visual impairment in rats.In the Morris water maze and eight-arm maze test,the modified model rats showed significant decrease in learning and memory abilities and had hippocampal damage.
10.Effect of traditional Chinese medicine wine combired with electromagnetic therapy for the neck,shoulder, back and leg pain

Objectives To explore the effect of traditional Chinese medicine wine combired with electromagnetic therapy for the neck,shoulder,back and leg pain. Methods Eighty cases of patients with neck,shoulder,back and leg pain were randomly divided into 2 groups,with 40 cases in each group,treatment group was given the traditional Chinese medicine wine and electromagnetic therapy,the control group was prescribed(only with electromagnetic therapy),use the numerical rating scale to observe the effects of two groups before therapy,3 days and 7 days after therapy . Result After treatment,the pain of the treatment group was relieved significantly better than that of the control group(P<0.05). Conclusion Traditional Chinese medicine wine combired with electromagnetic therapy for the neck,shoulder,back and leg pain has satisfactory effect,and is worthy to promotion.
