1.Perioperative nursing of continuous thrombolysis treatment through the femoral artery to treat patients with diabetic foot ulcers

Objective To investigate the points of perioperative nursing of patients with diabetic foot ulcers (DFUs)treated with continuous thrombolysis.Methods A total of 78 cases of DFUs patients treated with continuous infusion of urokinase through femoral artery from February 2006 to August 2011 were recruited into the study.Besides,psychological care,education guidance,and materials preparation were given to them before the operation.Meanwhile,a module of “Nursing observation and assessment indicators to DFUs treated with interventional thrombolysis” was set up and used to give a dynamic evaluation.Results Foot ulcers of all the subjects were healed,and no case occurred serious complications such as bleeding or infection and so on.Conclusions Our results indicate that professional psychological care,education guidance,and fully materials preparation in perioperative period lay the foundation of clinical efficacy.The establishment and application of “Nursing observation and assessment indicators to DFUs treated with interventional thrombolysis” is the key to the success of thrombolytic therapy because it can effectively reduce postoperative adverse events and promote ulcer healing.
2.CLIP for the evaluation of liver cancer patients after hepatectomy

Objective To evaluate the prognostic value of the Cancer of the Liver Italian Program (CLIP) score system in a study of hepatocellular carcinoma (HCC) patients who underwent radical resection. Methods Clinical pathological and follow-up data of 157 HCC patients, who underwent radical resection in our hospital from 1996 to 2004, were reveiwed retrospectively. Patients were divided into four groups according to CLIP scores. Disease-free survival rate was compared between groups, and within groups, the disease-free survival rate of patients receiving anatomic liver resection was compared with that of irregular liver resection. Results The overall 1,3,5-year disease-free survival rate was 63.6%、45.2%、35.7%,and there were significant differences among the four groups. For the zero score group, the recurrence rate of patients undergoing anatomic liver resection was lower than that of irregular liver resection (P = 0.003). Conclusions CLIP score system can be used to evaluate the recurrence rate of HCC patients receiving hepatectomy. CLIP score system can be used as a guidance for liver resection.
3.Study on gene polymorphism in patients with aspirin resistance

Objective To explore the correlation between aspirin resistance and COX-1, P2Y1, GPIa and GPIIIa gene polymorphism. Methods 35 case with aspirin resistant were selected and were given aspirin enteric coated tablets for oral treatment, 14 days.After the determination COX-1, P2Y1 and GPIa and GpIIIa gene polymorphism of each patient were analysis.Results The genotype AA of COX1 (A-842G) locus and CC of COX1 (C50T) locus was higher than other genotype (P<0.05);AG of P2Y1 (A1622G) locus genotype and CC of P2Y1 (C893T) locus genotype was higher than other genotype (P<0.05);CT of GPIa (C807T) locus genotype and GA of GPIa (G873A) locus genotype was higher than other genotype (P<0.05);PLA1/A1 of GPⅢa(T1565C)locus genotype was higher than other genotype (P<0.05).Conclusion P2Y1 (C893T), GPIa (C807T) allele is associated with aspirin resistance,With COX-1 (A-842G, C50T), GPIa (G873A), GP III A (T1565C), P2Y1 (A1622G) allele is more likely to induce the occurrence of aspirin resistance.
4.Efficacy of antiviral treatment in chronic HBV infected patients with mild hepatic dysfunction and marked pathological injury

Objective To evaluate the efficacy of entecavir treatment in chronic hepatitis B virus ( HBV ) infected patients with mild hepatic dysfunction and marked pathological injury.Methods One hundred and fifty five chronic hepatitis B ( CHB) patients with HBV DNA>1.0 ×104 U/mL admitted in Lishui People’ s Hospital during January 2008 to October 2011 were enrolled in the study.Patients were divided into three groups: those with serum ALT <2 ×ULN and liver inflammation injury ≥G2 and/or fibrosis stage≥S2 were in observation group ( n=75 ); patients with ALT ( 2-5 ) ×ULN were in control group 1 (n=38);patients with ALT>5 ×ULN were in control group 2 (n=42).All patients were given entecavir (0.5 mg, 1/d, p.o) treatment.ALT normalization rates, HBV DNA negative rates, HBeAg negative conversion rates and seroconversion rates at 12-, 24-,48-, 96-and 144-week were observed and compared among groups.Variance analysis andχ2 test were performed for measurement data and numeration data, respectively.Results ALT normalization rates in observation group were 86.7%, 90.7%, 90.7%, 92.0%and 96.0%at 12-, 24-, 48-, 96-and 144-week, which were higher than those in control group 1 (χ2 =2.04, 2.15, 2.78, 2.69 and 2.47, P <0.01), but no statistically significant difference was observed between observation group and control group 2 (χ2 =2.53,2.42,2.09,2.24 and 2.32,P>0.05) . HBV DNA negative rates in observation group were 70.7%, 78.7%and 82.7%at 12-, 24-and 48-week, which were higher than those in control group 1 (χ2 =4.56, 4.23 and 4.28, P<0.05), but no statistically significant difference was observed between observation group and control group 2 (χ2 =2.75, 2.62 and 2.98, P>0.05).HBeAg negative conversion rates in observation group were 6.6%, 21.3%and 25.3%at 48-, 96-, and 144-week, which were higher than those in control group 1 (χ2 =4.68, 4.78 and 5.01, P<0.05), but no statistically significant difference was observed between observation group and control group 2 (χ2 =2.24, 2.57 and 2.13, P>0.05).HBeAg seroconversion rate in observation group was 4.0%at 24-week, which were higher than that in control group 1 (χ2 =2.87, P <0.05), but the seroconversion rates at 96-and 144-week were lower than those in control group 2 (χ2 =2.92 and 3.14, P<0.05).Conclusion The efficacy of entecavir treatment for HBV infected patients with mild hepatic dysfunction and marked pathological injury is satisfactory.
5.Therapeutic Effect and Safety of Repaglinide and Gliclazide in the Treatment of Elderly Type 2 Diabetes Mellitus

Objective:To evaluate the clinical efficacy and safety of repaglinide and gliclazide in the treatment of elderly type 2 di-abetes mellitus (T2DM). Methods:A total of 225 T2DM patients with the age over 60 were enrolled and randomly divided into repa-glinide group (n=112) and gliclazide group (n=113). The repaglinide group was given repaglinide 0. 5~2 mg, po, tid, while the gliclazide group was given gliclazide modified release tablets 30~120 mg, po, qd,both with the treatment course of 12 weeks. The level of FPG, 2h PG, HbAlC, TC and TG and the side effects were observed and the clinical efficacy was compared between the two groups. Results:The level of FPG, 2h PG, HbAlc and TC in the two groups showed significant decrease after the treatment (P<0. 01 or P<0. 05), and the level of 2h PG was decreased more significantly in the repaglinide group (P<0. 05). There were no significant differ-ences in the total effective rate (91. 8% vs 86. 2%), hypoglycemia incidence (4. 5% vs 8. 2%) and ADR incidence (6. 4% vs 10. 1%) in the two groups (P>0. 05). Conclusion:Both repaglinide and gliclazide show safe and reliable efficacy in the treatment of elderly T2DM.

Objective To assess whether the T~ -786 →C mutation of endothelial nitric oxide synthase (eNOS) gene is associated with endothelial dysfunction in type 2 diabetes. Methods A total of 162 type 2 diabetic men without angiopathy were studied. Objective To investigate molecular changes in growing rats masticatory muscle after functional mandibular protrusion.Methods SD rats were randomly divided into eight groups as control 3d,7d,14d,21d and experimental 3d,7d,14d,21d.The RTPCR and qPCR methods were used to find expression and any differences of mRNA of MMPs in growing rats masticatory muscle after functional mandibular protrusion. (Results)(① Expression) of MMP-2,MMP-9 mRNA expressed in all growing rats masticatory muscle.② Expression differences of MMP-2 and MMP-9 mRNA in 3d,21d were found between control and experimental groups after functional mandibular protrusion.Conclusion ①MMP-2 and MMP-9 mRNA participate in masticatory muscle normal development.(② MMPs) mRNA involve changes in growing rats masticatory muscle after functional mandibular protrusion.

Objective: To observe the effect of urapidil (agonist of 5-HT 1A receptor ) on morphine withdrawal syndrome Method: A physical dependent model in mice was established by subcutaneous injection of quantitative morphine The intensity of withdrawal syndrome was evaluated by observing jumping latency, jumping times and lose of body weight after intraperitoneal injection of naloxone (5 mg/kg) The effect of urapidil was observed after intraperitoneal injection it with 3 different dosages (100, 200, 400 mg/kg) Contents of NO in plasma and brain tissue were measured by nitrate reduction method Results: Morphine withdrawal syndrome was inhibited by urapidil at any dosage Content of NO in brain tissue reduced by urapidil 400 mg/kg, while plasma NO increased Conclusion: Urapidil inhibits morphine withdrawal syndrome in mice, increase the content of NO in their brain tissue
10.Clinical Significance of CD11b,CD18,IL-8 and IFN-? in Patients with Psoriasis

Objective To explore the clinical significance of cytokines expression including CD11b,CD18,IL-8 and IFN-? in patients with psoriasis.Methods 55 patients with psoriases,including vulgaris,pustulosa,arthropathica and erthrodermic ones,were enrolled into the case group and 40 healthy persons as control.The expressions of CD11b,CD18 were detected by flow cytometry,while IL-8 and IFN-? with ELISA.Results The expression of CD11b,CD18,IL-8 and IFN-? in patients with psoriasis was significantly higher than those in control.Conclusions The ultra expression of CD11b,CD18,IL-8 and IFN-? might relate with the immunopathology of lymphocyte cell activation in psoriasis.This might provide the dependable basis for its treatment.
