1.Effects of Fasudil on the expression of Nogo-A and NF200 in neonatal hypoxic-ischemic brain damage rats

Objective To study the effects of Fasudil on expression of Nogo-A and NF200 in neonatal hypoxic-ischemic brain damage(HIBD) rats.Methods One hundred and twenty 7-day-old Wistar rats were divided into 3 groups with random number table:Sham operation group (n =40),HIBD group (n =40) and Fasudil group (n =40).Sham group only separated from the common carotid artery,without ligation,direct suture the incision does not do hypoxia; HIBD group were injected with saline; Fasudil group was injected with fasudil(10 mg/kg).The rats were killed at 6 h,12 h,24 h,72 h,7 d,after administration.The pathological changes were observed by means of HE.The expression of Nogo-A and NF200 was studied with immunohistochemical staining.Results 1.Naked eye observation:Sham group bilateral symmetrical cerebral hemispheres; HIBD group of brain edema aggravated,the visible hemisphere focal necrosis; fasudil treatment group of edema than HIBD group ease.2.HE stain:the structure and shape of brain in Sham operation group were normal.In HIBD group,the cells became edema,karyopyknosis,lyse,and the inflammatory cells became more.The number of edema cells and karyopyknosis decreased in Fasudil group.3.Immunohistochemical stain:there were less expressions of Nogo-A in Sham operation group.It increased slightly after 12 h in HIBD group but decreased later.The expression of Nogo-A in Fasudil group was less than the other two groups at any time except 6 h (P <0.01).There was more expression in HIBD and Fasudil group compared with Sham operation group(P <0.01).NF200 was less expression in Sham operation group.NF200 appeared after 6 h and became less after 12 h.The expression of NF200 was at 24 h and later became more.The expression of NF200 in Fasudil group was more with HIBD group at each different time (P < 0.01).The expressions of NF200 in Fasudil and HIBD group were more compared with Sham operation group.Conclusions Fasudil can rehabilitate the damaged axon and promote nerve regeneration through controlling the Rho/Rock and make the expression of NF200 increase.
2.A qualitative research on the feelings of nursing students in simulation training for doctor-nurse cooperation emergency

Objective To explore the feeling and experience of the nursing undergraduate students in the simulation training for the doctor-nurse cooperation emergency before their internship,in order to provide reference for making excellent teaching and practical plan of emergency nursing.Methods Using phenomenology method,the experience records of thirty-six nursing students were collected.The experience was recorded by Colaizzi method.Results Five themes were summarized by Colaizzi method:preliminary acquaintance of doctor-nurse cooperation; reflection on division of labor cooperation; scan self-ability; focus on interpersonal communication;comprehension of emergency characteristics.Conclusions Simulation training for doctor-nurse cooperation emergency can give nursing students' opportunity of cooperative learning; Simulation training for doctor-nurse cooperation emergency can arouse nursing students' interest of active learning; Simulation training for doctor-nurse cooperation emergency can develop nursing students'habit of reflective learning;Simulation training for doctor-nurse cooperation emergency can improve nursing students' ability of integrative learnng.
3.Expression and significance of C D19+CD5+B cells and interleukin-10 in patients with systemic lupus ;erythematosus

Objective To access the expression and clinical significance of CD19+CD5+B cells and interleukin (IL)-10 in patients with systemic lupus erythematosus (SLE). Methods Forty-six SLE patients and ten healthy controls were recruited in the Second Affiliated Hospital of Shanxi Medical University. CD19+CD5+B cells subsets were detected with flow cytometry. IL-10 level in serum were detected with enzyme linked immunosorbent assay (ELISA). The correlation between the expression of CD19+CD5+B cells and serum level of IL-10 with ESR, systemic lupus erythematosus disease activity index (SLEDAI) score and complement was analyzed. Pair-wise comparison of means of groups was conducted with one-way ANOVA. Comparison between the two groups was conducted by LSD-t test. Correlations between variables were carried out using Spearman's rank correlation test. Results The percentage of CD19+CD5+B cells in peripheral blood of SLE patients [(5.7±2.1)%] was significantly lower than those in healthy control group [(19.1±2.9)%](t=2.431, P=0.005), and it had a negative correlation with SLEDAI score (r=-0.292, P=0.049). The IL-10 serum level of SLE patients [(18.8±13.5) pg/ml] was significantly higher than the healthy control group [(8.3±2.9) pg/ml] (t=3.021, P=0.003), and it had a positive correlation with SLEDAI score (r=0.322, P=0.029). Conclusion CD19+CD5+B cells and IL-10 may participate in the occurrence and development of SLE. The changes of CD19+CD5+B cells and IL-10 in the peripheral blood might contribute to the pathogenesis and correlate with the disease activity of SLE.

5.A qualitative study on the current situation of hospital-institution-community comprehensive prevention of stroke

Objective:To deeply explore the real work of primary, secondary and tertiary prevention of stroke in general hospitals, rehabilitation and pension institutions and community health service centers (stations), and to provide reference for improving the level of comprehensive prevention of stroke in the medical system.Methods:From July to November 2022, face-to-face semi-structured interviews with 18 medical staff in general hospitals, 10 medical staff in rehabilitation and pension institutions, and 8 medical staff in community health service centers (stations) were conducted by phenomenological research methods. Colaizzi 7-step analysis method was used to analyze the interview data, and Nvivo12 software was used to code the interview data.Results:Four themes were extracted: hospitals, institutions and communities all played an important role in the comprehensive prevention of stroke and actively cooperated with each other; the public was still lack of comprehensive prevention awareness and correct cognition of stroke; lack of professional medical resources hindered the development of comprehensive stroke prevention; incomplete collaborative mechanism of comprehensive prevention of stroke in hospitals, institutions and communities.Conclusions:In the current comprehensive prevention of stroke, there are still problems such as weak public awareness of prevention, insufficient professional medical resources, and imperfect coordination mechanism. In view of the advantages of medical institutions at all levels, it is necessary to clarify the responsibilities of the system at all levels and explore efficient and sustainable coordination mechanism, so as to improve the comprehensive prevention level of stroke in the medical system.
6.Factors influencing the efficacy of residual thyroid clearance with 131I for the first time after surgery in patients with differentiated thyroid cancer

Objective:To explore the influencing factors of residual thyroid clearance with 131I after surgery in patients with differentiated thyroid cancer (DTC). Methods:A retrospective analysis was conducted on the clinical data of 100 DTC patients admitted to the Hunan Provincial People′s Hospital from January 2018 to February 2021 who underwent 131I treatment for the first time. The success rates of first thyroidectomy using different doses of 131I, different pathological types, and different treatment times were compared, and logistic regression analysis was conducted to investigate the influencing factors of the efficacy of first postoperative 131I thyroidectomy in DTC patients. Results:A total of 54 patients successfully cleared residual thyroid, 46 patients failed to clear residual thyroid, and the success rate of clearing residual thyroid was 54%. The success rates of first clearance of residual thyroid in patients with 131I doses of 80 mCi, 90 mCi, and 100 mCi were 37.50%(12/32), 52.78%(19/36), and 71.88%(23/32), respectively, with statistically significant differences among the groups ( P<0.05); The success rates of first removal of residual thyroid in patients with follicular carcinoma, mixed papillary follicular carcinoma, and papillary carcinoma were 65.71%(23/35), 39.13%(9/23), and 52.38%(22/42), respectively. There was no statistically significant difference between the groups ( P>0.05); The success rates of first removal of residual thyroid in the group1 of patients (treatment time<3 months), the group2 of patients (treatment time 3-12 months), and the group3 of patients (treatment time>12 months) were 68.09%(32/47), 44.44%(16/36), and 35.30%(6/17), respectively. There was no statistically significant difference between the groups ( P>0.05); There was no statistically significant difference in the success rate of clearing residual thyroid in DTC patients of different genders, ages, pathological stages, and thyroid stimulating hormone (TSH) levels (all P>0.05); The difference in the success rate of clearing residual thyroid in DTC patients with different metastatic conditions and stimulating thyroid globulin (sTg) was statistically significant (all P<0.05); sTg, postoperative lymph node metastasis, and postoperative distant metastasis were independent risk factors for the efficacy of residual thyroid clearance in DTC patients for the first time after surgery (all P<0.05). Conclusions:The influencing factors for the efficacy of the first 131I in removing residual thyroid include differences in 131I dosage, presence or absence of metastatic lesions during treatment, Tg levels, etc. Reducing Tg levels is an important factor in improving remission rate, and controlling lymph nodes and distant metastasis is a key factor for the successful efficacy of the first 131I in removing residual thyroid.
7.A qualitative research on the working experience of home caregivers based on long-term care insurance service

Objective:On the basis of introducing and piloting long-term care insurance services and providing home care for older adults with disabilities in Suzhou, this paper aims to explore the working experience of home caregivers in the caring environment, understand their difficulties and issues, explore their true feelings and the relevant causes, and to draw lessons from their experience, providing constructive suggestions for the establishment of caregivers as well as policy recommendations for the government.Methods:The qualitative research method was used to collect the field experience data of 9 home caregivers in the caring environemnt through observation and semi-structured interview. Colaizzi phenomenological analysis was used to process and analyze the data.Result:1. Heavy workload and lack of incentive mechanism; 2. Care experience is diverse, with a sense of achievement and happiness, but lack of family′s trust and identification; 3. Professional trainings and peer support are needed.Conclusion:1. Improving the working environment of caregivers and reducing the burden of care; 2.Improve the employment treatment of home care workers, and actively improve the relevant incentive mechanism;3. Paying attention to the emotional experience of caregivers and helping them adapt to their role; 4.Standardize professional knowledge and skills training, provide risk assessment tools, and improve the quality of care.
8.Appraisal preventive effects of meridian stretch stocking on postgynecological pelvic surgery to LEDVT

Objective To explore the effects and influences of meridian stretch stocking for prevention of lower extremity deep vein thrombosis ( LEDVT ) after gynecological pelvic surgery. Methods A total of 210 patients,who conducted pelvic surgery at Department of Obstetrics and Gynecology, Wuhan Third Hospital from April 2010 to February 2013, were randomly divided into routine group (Group A), the common stretch stocking group ( Group B) and meridian stretch stocking group ( Group C) , 70 cases in each group,according to their check-in time. The patients of Group A followed routine nursing methods, and the patients of Group B complied with normal stretch stocking while the patients of Group C adopted meridian stretch stocking. The usage of stretch stockings was on the basis of conventional nursing methods and applied immediately after the surgery. Then a series of comparisons were made in terms of lower extremity venous blood flow velocity, foot dorsum skin temperature and the incidence of LEDVT, coagulation and fibrinolytic function, etc. Results Group C possessed the lowest LEDVT incidence in three groups and the differences were statistically significant (χ2 =6. 667,P<0. 05). Meanwhile Group C had the fastest posterior divisions superficial vein and popliteal fossa vein blood flow velocity with statistical significance compared with the other two groups (P<0. 01). The lower limb dorsal skin temperature of Group C was higher than that of Group A and Group B after two and four hours′ surgery ( P<0. 01). There was no statistical difference in blood coagulation and fibrinolytic function of the three groups (P>0. 05). Conclusions Meridian stretch stocking can advance venous blood flow velocity, improve lower limb dorsal skin temperature and reduce LEDVT incidence of patients with gynecological pelvic surgery.
9.Cognition on humanistic care of nursing undergraduates in the process of early clinical experience

Objective? To explore the cognition on humanistic care of nursing undergraduates in the process of early clinical experience. Methods? We selected 26 nursing undergraduates with Grade 2016 in school of nursing of Soochow University as subjects by convenience sampling. Then, we analyzed their clinical experience diary and results of symposium. Results? Students had a preliminary understanding and self-examination on humanistic care by clinical experience. A total of three themes were refined including perceived care, regurgitation-feeding other and reflection care. Conclusions? Early clinical experience could improve humanistic care of students, which is worth to be promoted and referred.
10.Effects of platelet factor 4 on expression of bone marrow heparan sulfate in syngenic bone marrow transplantation mice.

To explore the effects of platelet factor 4(PF4) on hematopoietic reconstitution and its mechanism in syngenic bone marrow transplantation (BMT). The syngenic BMT mice models were established. 20 and 26 h before irradiation, the mice were injected 20 micrograms/kg PF4 or PBS twice into abdominal cavity, then the donor bone marrow nuclear cells (BMNC) were transplanted. On the 7th day, spleen clone forming units (CFU-S) were counted. On the 7th, 14th and 21st day after BMT, the BMNC and megakaryoryocytes in bone marrow tissue were counted and the percentage of hematopoietic tissue and expression level of heparan sulfate in bone marrow tissue were assessed. In PF4-treated groups, the CFU-S counts on the 7th day were higher than those in BMT groups after BMT. The BMNC and megakaryoryocyte counts and the percentage of hematopoietic tissue and heparan sulfate expression level were higher than those in BMT group on the 7th, 14th and 21st day after BMT (P < 0.01 or P < 0.05). PF4 could accelerate hematopoietic reconstitution of syngenic bone marrow transplantation. The promotion of the heparan sulfate expression in bone marrow may be one of mechanisms of PF4.
