1.Application of biomedical simulation in ultrasound teaching

Traditional medical education focus on expository teaching,rote learning,and apprenticeship training for clinical skill.Although these teaching approaches can be beneficial for imparting knowledge and clinical skill,they appear to be insufficient for team cooperation,knowledge integration,and standard training.Biomedical simulation teaching is a new method for imparting knowledge,and has achieved good results in current medical education.This paper aims to introduce the definition and characteristic of biomedical simulation,and the application and significance of biomedical simulation in ultrasound teaching are also outlined and discussed.
2.Sonographic features and clinical significance of the complications of the thyroglossal cysts

OBJECTIVE To assess the ultrasonographic features and clinical significance of the complications of the thyroglossal cysts. METHODS We retrospectively reviewed the ultrasonographic findings in 41 cases with complications of the thyroglossal cyst, which was confirmed surgically and pathologically. 43 cases with simple thyroglossal cyst consisted of the control group. The location, size, internal echo, internal septa, wall thickness, posterior acoustic feature, boundary, vascularity, and fistulas of the lesion were analyzed and compared with the control group. RESULTS Most of the inflammatory thyroglossal cyst showed thickening walls, indistinct boundaries, posterior echo enhancement, and peripheral vascularity on ultrasound images. There were significant differences of the thickness of the cyst wall, indistinct boundary, and peripheral vascularity between the two groups. CONCLUSION The characteristic findings of thyroglossal cyst with inflammation are thickened wall, indistinct boundary, and peripheral vascularities. Ultrasonography can be useful tool in determination of the appropriate time of the operation to reduce the recurrence rate.
3.Correlation of expression of ER, PR and c-erbB-2 with ultrasonographic features in invasive ductal cancer of breast

Objective To analyze the relationship of the expression level of estrogen receptor (ER), progesterone receptor (PR) and human epidermal growth factor receptor-2 (HER-2, c-erbB-2) of breast invasive ductal carcinoma (IDC) with ultrasonographic characteristics. Methods Totally 104 patients with IDCs confirmed pathologically were involved in this study. ER, PR and c-erbB-2 expression in the IDC specimens was determined by immunohistochemical staining technique. The correlation between the ultrasonographic features and the positive expression of ER, PR or c-erbB-2 was analyzed. Results The positive expression rate of ER and PR in the group with tumor spiculation sign and posterior acoustic attenuation was higher than that in the group without. The positive expression rate of ER differed significantly (P<0.05) while that of PR did not (P>0.05). The over-expression rate of c-erbB-2 in the group of microcalcification, sufficient blood flow and axillary lymph node metastasis was higher than that in the group of non-microcalcification, deficient blood flow or without axillary lymph node metastasis (P<0.05). The expression of ER, PR and c-erbB-2 was not related to the size of tumor (P>0.05). Conclusion The expression of ER and c-erbB-2 is closely related to the ultrasonographic characteristics of IDC, which may, to some extent, reflect the expression level of ER and c-erbB-2.
4.Differentiation diagnosis of prostate nodules in peripheral zone by contrast-enhanced transrectal ultrasonography

Objective To evaluate quantitative parameters of contrast-enhanced transrectal ultrasonography for differential diagnosis of prostate nodules in peripheral zone.Methods Forty-seven patients suspected of prostate cancer for peripheral zone nodules on ultrasonographic imaging were enrolled in this study.Time intensity curves of contrast-enhanced ultrasound were analyzed in all patients.Results The full-width at half maximum (FWHM) of malignant lesions in peripheral zone was shorter than that of adjacent peripheral zone (47.1 s±21.1 s vs 74.2 s±29.7 s, P=0.01).The peak intensity (PI) of benign nodules in peripheral zone was lower than that of adjacent peripheral zone 11.9 dB±7.7 dB vs 17.5 dB±4.5 dB, P=0.02).Conclusions It is helpful for differentiation diagnosis of peripheral zone nodule through analyzing FWHM and PI on contrast-enhanced transrectal ultmsonography imaging.
5.Extraction Technology Optimization for Organic Acids in Crataegi Fructus by Central Composite De-sign-response Surface Method

OBJECTIVE:To optimize the extraction technology for organic acids in Crataegi fructus. METHODS:Based on sin-gle factor test,using liquid-solid ratio,extraction time and ethanol volume fraction as independent variables,extraction yield of or-ganic acids as dependent variable,central composite design-response surface method was used to optimize the extraction technology of organic acids in Crataegi fructus. RESULTS:The optimal extraction technology was as follow as liquid-solid ratio of 18.5:1, adding 75% ethanol,reflux extraction twice,2.0 h each time. Average extraction yield of organic acids in verification test was 5.22%(RSD=2.70%,n=3),with 1.75% relative error of the predicted value(5.13%). CONCLUSIONS:Optimized extraction technology for organic acids in Crataegi fructus is simple,with good reproducibility and predictability.
6.Effect of saxagliptin on peripheral blood neutrophil-to-lymphocyte ratio in elder patients with type 2 diabetes mellitus

To explore the clinical effect of saxagliptin on release of peripheral blood neutrophil-to-lymphocyte ratio(NLR)in elder patients with type 2 diabetes mellitus. A total of 110 newly-diagnosed elder patients with type 2 diabetes were assigned into 2 groups(both n=55). Trial group was treated with exercise-diet therapy and saxagliptin at 5 mg/d(SG group),while control group with exercise-diet therapy and repaglinide(RG group). NLR were evaluated in both groups before treatment without significant difference(P>0.05). After 12 weeks'treatment, NLR were both lowered. Decrease of NLR in SG group was more significant than that of RG group(P<0.05).
7.High frequency ultrasound inspection of T7-8 paravertebral space and adjacent structures

Objective To observe value of high frequency ultrasound for inspection of T7-8 paravertebral space and adjacent structures.Methods Color Doppler ultrasonic diagnostic apparatus with linear array transducer (frequency 3-12 MHz) were used.Taking shoulder blade,ribs,thoracic spine and transverse process as anatomical marks,T7-8 paravertebral spaces of 30 normal adult (a total of 60 side) were examined.Ultrasonographic features of T7-8 paravertebral space and the adjacent structures were observed.Results T7-8 thoracic paravertebral showed as similar triangular shape with ultrasonography,interior solid homogeneously hypoechoic was noticed.Adjacent muscles of T7-8 paravertebral space included the trapezius,latissimus dorsi,spinalis,semispinalis,multifidus,rotatores and intercostal muscles.Color Doppler or power Doppler flow imaging could demonstrate posterior intercostal artery in the paravertebral space.Conclusion High frequency ultrasound can clearly show T7-8 paravertebral space and adjacent structures,thus providing ultrasonic references for clinical diagnosis and treatment of lesions of thoracic space and adjacent structures.
8.Multi-center investigation and the relative factors of the clinical nurse occupational low back pain

Objective To understand the present situation of occupational low back pain of clinical nurses, and analyze its influence factors.Methods A questionnaire survey was conducted among 1300 clinical nurses from 7 Class Ⅲ Grade A general hospitals in 5 provincial capital cities, in the region of north China, central China, southern China, southwest and northwest, from April to December 2016. The survey tools included general demographic characteristics questionnaire, Visual Analogue Scale (VAS) and Chinese version of Fear-Avoidance Belief Questionnaire (FABQ-CHI). A total of 1300 questionnaires were distributed, and 1279 valid were collected. SPSS 21.0 was applied for statistical analysis.Results In the 1279 nurses, 115 (8.99%) had no low back pain, and 1164 (91.01%) was positive. The majority of nurses suffered with mild pain, account for 64.69%. The average score of FABQ-CHI in negative nurses was (50.11±17.5), and that in positive nurses was (55.24±8.2), the difference was statistically significant (t=14.56,P< 0.001). Univariate analysis showed that there were significant differences in VAS scores among nurses of different ages, marital status, fertility status, professional titles, working years, and whether or not they participated in training (P<0.01). Multivariate analysis showed that age (OR=1.40) and professional title (OR=1.84) were the influencing factors of low back pain.Conclusions The incidence of occupational low back pain in nurses is high, and it is related to individual factors, work related factors and psychological factors. It is suggested that great attention should be paid to the popularization of knowledge on prevention and treatment of low back pain and the training of occupational protection skills for clinical nurses. Especially, we should pay attention to the correlation between psychological factors and nurses' occupational low back pain, gradually improve the working environment and bad living habits of nurses, reduce the occurrence of low back pain, and improve the job satisfaction of clinical nurses.
9.Sonographic appearance immunoglobulin G4-related submandibular gland disease in adult patients

Objective To analyze and summarize the sonographic features of immunoglobulin G4(IgG4)-related submandibular gland disease.Methods The retrospective analysis on the sonographic results of 22 lesions in 19 patients with IgG4-related submandibular gland disease were conducted,including submandibular gland size,shape,lesion range,internal echo,blood flow,and peripheral lymph nodes.Results The sonographic features of 22 lesions of IgG4-related submandibular gland disease were classified into localized nodule changes,diffuse nodule changes,and tumor-forming changes.The localized nodule appearance,includes 2 lesions in 2 patients,shows a hypo-echoic area with a coarse echotexture in the superficial site of the gland and rich blood flow.In the diffuse nodule appearance,includes 12 lesions in 11 patients,the gland shows an irregular net or honeycomb structure.Further,the gland shows echo enhancement,thickening,uneven,and rich blood flow with the features of multiple,variously sized hypoechoic foci and surrounding parallel echogenic foci.The tumor-forming appearance,includes 8 lesions in 7 patients,shows the decrease of echogenic heterogeneity,coarse gland texture,multiple parallel echogenicity,and rich blood flow.Conclusions Ultrasonography can be used as the preferred diagnostic method for IgG4-related submandibular gland disease.When a patient is suspected of having IgG4-related submandibular gland disease,the tissues and organs often involved in IgG4 related diseases should be further examined,meanwhile conducting ultrasound-guided biopsy.
10.A comparison of binocular and monocular surgery on the effect of eye-covering pretreatment on emergence delirium after general anesthesia in pre-school aged children

Objective:To evaluate the effect of eye-covering pretreatment on acute delirium in ophthalmology preschool-age children who underwent binocular and monocular surgery by general anesthesia.Methods:The 300 preschool-age children who underwent general anesthesia of elective ophthalmic surgery in the Second Affiliated Hospital, Zhejiang University School of Medicine, from August 2019 to February 2021 were selected as the research object. They were divided into control group and blindfold group with 150 cases each by random number-table. Children in the control group received regular education on cartoon animation videos before surgery; children in the blindfold group received eye-covering pretreatment on the basis of cartoon animation videos(monocular surgery with monocular cover, binocular surgery with binocular cover). The Modified Yale Preoperative Anxiety Scale (m-YPAS) , the Nursing Delirium Screening Scale(NU-DESC), the incidence rate of delirium and the score of postoperative nursing difficulty were compared between two groups.Results:The 271 cases were completed in this study, including 129 cases(monocular surgery 66 cases, binocular surgery 63 cases) in the blindfold group and 142 cases (monocular surgery 73 cases, binocular surgery 69 cases) in the control group. The preoperative m-YPAS score, the postoperative NU-DESC score, the incidence rate of acute delirium and postoperative nursing care difficulty score of monocular surgery in the blindfold group , monocular surgery was (40.28 ± 15.02) points, 1.00 (0.00, 2.00) points, 27.3%(18/66), 1.00 (1.00, 2.00) points,and binocular surgery was (41.69 ± 16.35) points, 1.00 (0.00, 2.00), 39.7%(25/63), 1.00(1.00, 2.00); in the control group, monocular surgery was (46.28 ± 15.76) points, 2.00 (1.00, 3.00) points, 67.1% (49/73), 2.00 (1.00, 3.00) points, and binocular surgery was (47.77 ± 14.82) points, 3.00 (2.00, 4.00) points, 82.6% (57/69) and 2.00 (1.50, 3.00) points respectively. The difference between the two groups was statistically significant ( t= -2.29, -2.24, Z values were -5.74 - -2.95, χ2= 32.94, 25.78, all P<0.05). The preoperative m-YPAS score, the postoperative NU-DESC score, the incidence rate of acute delirium and postoperative nursing care difficulty score of monocular surgery patients in the blindfold group had no significantly statistical difference with that of binocular surgery patient (all P>0.05) . Conclusions:Monocular/ binocular eye-covering pretreatment can effectively decrease the preoperative m-YPAS score, the postoperative NU-DESC score, incidence rate of acute delirium and the postoperative nursing care difficulty in preschool-age children who underwent general anesthesia both monocular or binocular surgery. There was no difference in the application effect of monocular or binocular surgery.
