1.Pancreatic Dysfunction and Influencing Factors of Glucose Metabolism in children with β-thalassemia Major After Allogeneic Hematopoietic Stem Cell Transplantation

ObjectiveTo investigate and explore the characteristics and influencing factors of glucose metabolism in children with β-thalassemia major (β-TM) after allogeneic hematopoietic stem cell transplantation (allo-HSCT). MethodsThe follow-up data of 41 patients with β-TM who underwent HSCT at Hematopoietic Stem Cell Transplantation Department of Children's Medical Center of Sun Yat-sen Memorial Hospital, Sun Yat-sen University were retrospectively analyzed. Their glucose metabolism characteristics were evaluated through laboratory tests and the related influencing factors were analyzed. ResultsIn the study, 41.46% (17/41) of patients developed abnormal glucose homeostasis after HSCT. Among them, 82.35% (14/17) characterized by insulin resistance, but no cases of diabetes mellitus were found. The results of insulin releasing test and oral glucose tolerance test(OGTT) showed that 45.00% (9/20) of patients had abnormal insulin releasing curve and 40.0% (8/20) had delayed serum glucose peak. The average age of HSCT in abnormal glucose homeostasis group was significantly older than that in the normal glucose homeostasis group [(8.8±3.9) years old vs (6.0±3.1) years old, P=0.015]. ConclusionsPatients with β-TM after HSCT may develop abnormal glucose homeostasis, consists largely of insulin resistance. The elder age of HSCT (≥7 years old) is a risk factor for abnormal glucose homeostasis in β-TM patients after HSCT. It is recommended to regularly monitor glucose metabolism indicators in β-TM children after HSCT, especially in elderly transplant recipients.
2.Basic information on radiological health technical service institutions in Shaanxi Province, China

Objective To understand the basic information on radiological health technical service institutions in Shaanxi Province, China and analyze the main problems, and to strengthen the capacity building of radiological health technical service institutions in Shaanxi Province. Methods The radiological health technical service institutions registered in Shaanxi Province were investigated and analyzed by means of a questionnaire and literature review. Results There were 20 radiological health technical service institutions in Shaanxi Province, including 7 state-owned ones and 13 privately owned ones; of all the institutions, there were 3 disease control institutions, and 15 institutions obtaining the qualification of radiation protection evaluation, radiation health protection testing, and individual dose monitoring at the same time. The total number of radiation workers in the institution was 237, including 48.5% aged less than 35, 68.8% with bachelor degree or above, 54.4% with intermediate professional title or above, 26.5% with a major related to radiation protection, and 79.3% with national and provincial training. The institutions were equipped with 309 testing instruments, of which the radiation diagnostic performance testing equipment accounted for the largest proportion (52.4%). Conclusion The service scope and capacity of radiological health technical service institutions in Shaanxi Province can meet the current needs of the province, but the institutions need to be improved in terms of personnel, equipment, and service quality. The quality control of institutions needs to be strengthened to standardize the service behavior in the whole province.

Objective:To understand the current situation of nurses in 52 hospitals in China on mastery of knowledge about skin injury in the elderly based on the background of mixed-mode homogenization training.Methods:Using the convenient sampling method, a total of 1 067 nurses from 52 hospitals in China were selected as the research objects in January 2021. A self-designed questionnaire on knowledge of skin injury in the elderly was used to investigate the nurses through the questionnaire star and univariate analysis was used to analyze the influencing factors. A total of 1 067 questionnaires were distributed and 1 067 valid questionnaires were recovered, and the effective recovery rate was 100%.Results:The knowledge scores of pressure injury, incontinence-associated dermatitis, skin tear and xerosis cutis among 1067 nurses were (95.66±7.37) , (95.65±9.15) , (91.37±15.45) and (87.67±15.91) , respectively. The results of univariate analysis showed that hospital grade was the influencing factor of nurses' knowledge score of pressure injury, skin tear and incontinence-associated dermatitis ( P<0.05) , educational background was the influencing factor of nurses' knowledge score of skin tear ( P<0.05) , professional title was the influencing factor of nurses' knowledge scores of pressure injury, incontinence-associated dermatitis and xerosis cutis ( P<0.05) . Conclusions:Hospitals at all levels need to strengthen the theoretical and practical knowledge training for nurses on skin xerosis and skin tear in the elderly, especially for nurses with primary titles and lower education in grassroots hospitals.
4.Ginsenoside 20(S)-Rg3 upregulates tumor suppressor VHL gene expression by suppressing DNMT3A-mediated promoter methylation in ovarian cancer cells.

OBJECTIVE:
To explore the mechanism by which ginsenoside 20(S)-Rg3 upregulates the expression of tumor suppressor von Hippel-Lindau (VHL) gene in ovarian cancer cells.
METHODS:
Ovarian cancer cell line SKOV3 treated with 20(S)-Rg3 were examined for mRNA and protein levels of VHL, DNMT1, DNMT3A and DNMT3B by real-time PCR and Western blotting, respectively. The changes in VHL mRNA expression in SKOV3 cells in response to treatment with 5-Aza-CdR, a DNA methyltransferase inhibitor, were detected using real-time PCR. VHL gene promoter methylation was examined with methylation-specific PCR and VHL expression levels were determined with real-time PCR and Western blotting in non-treated or 20(S)-Rg3-treated SKOV3 cells and in 20(S)-Rg3-treated DNMT3A-overexpressing SKOV3 cells. VHL and DNMT3A protein levels were detected by immunohistochemistry in subcutaneous SKOV3 cell xenografts in nude mice.
RESULTS:
Treatment of SKOV3 cells with 20(S)-Rg3 significantly upregulated VHL and downregulated DNMT3A expressions at both the mRNA and protein levels (
CONCLUSIONS
Ginsenoside 20(S)-Rg3 upregulates VHL expression in ovarian cancer cells by suppressing DNMT3A-mediated DNA methylation.

Objective To study and discuss influencing factors of the elderly with ST elevation myocardial infarction (STEMI) underwent emergency percutaneous coronary intervention (PCI) after myocardial perfusion.Methods A total of 168 STEMI patients in our hospitalwere divided into total perfusion (CMP) group,reperfusion (PMP) group and non perfusion (NMP) group according to the integral contrast perfusion (APS) scores,including 54 cases in CMP group,96 cases in PMP group and 18 cases in NMP group.General information,coronary angiography and PCI results were compared,and the influencing factors of myocardial perfusion after primary PCI were compared by Logistic regression analysis.Results The general data showed significant difference in admission blood glucose,Killip grade,preinfarction angina incidence and balloon opening time,peak value of creatine kinase isoenzyme (CK-MB) and creatine kinase (CK) (P<0.05);In addition,there was significant difference in high thrombus load rate (P<0.05);Logistic regression analysis showed that the intravenous infusion of tirofiban,pre infarction angina,admission blood glucose,high thrombus load and onset to balloon time were factors affecting myocardial perfusion for PCI patients (P<0.05).Conclusion Blood glucose,intravenous infusion of tirofiban,preinfarction angina,high thrombus load and the onset to balloon time on admission are influencing factors for elderly STEMI patients after undergoing PCI myocardial perfusion.

8.The ultrasonographic features and pathological analysis of adrenal gan-glioneuroma

Objective To explore the ultrasonography findings and pathological features of adrenal ganglioneuroma and to improve diagnostic level. Methods Ultrasonography appearance and clinical pathological data of 12 cases of primary adrenal ganglioneuroma were retrospectively analyzed. The location, shape, size, echo characteristics, with or without calcification, the color signal of blood flow and its neighboring structure relationship on ultrasound were analyzed. Re-sults In 12 cases, 9 cases were found in the right adrenal gland and 3 cases in the left. All the tumors were solitary and the diameter was between 1.8 and 12 cm. The tumors were round or oval, some irregular or"embedded" growth. E-chogenic focus were found in 5 cases which was pathologically confirmed calcification, 3 cases of tumor showed a small dot color flow signal, all tumors with clear boundaries around the surrounding tissue. Pathology of tumor was mainly composed of ganglion cells and Schwann cells. Conclusion Ultrasonography is of great value in the diagnosis of adrenal ganglioneuroma owing to some specific sonograms.
9.Inhibition of glial activation in rostral ventromedial medulla attenuates mechanical allodynia in a rat model of cancer-induced bone pain.

Descending nociceptive modulation from the supraspinal structures plays an important role in cancer-induced bone pain (CIBP). Rostral ventromedial medulla (RVM) is a critical component of descending nociceptive facilitation circuitry, but so far the mechanisms are poorly known. In this study, we investigated the role of RVM glial activation in the descending nociceptive facilitation circuitry in a CIBP rat model. CIBP rats showed significant activation of microglia and astrocytes, and also up-regulation of phosphorylated p38 mitogen-activated protein kinase (p38 MAPK) and pro-inflammatory mediators released by glial cells (IL-1β, IL-6, TNF-α and brain-derived neurotrophic factor) in the RVM. Stereotaxic microinjection of the glial inhibitors (minocycline and fluorocitrate) into CIBP rats' RVM could reverse the glial activation and significantly attenuate mechanical allodynia in a time-dependent manner. RVM microinjection of p38 MAPK inhibitor (SB203580) abolished the activation of microglia, reversed the associated up-regulation of pro-inflammatory mediators and significantly attenuated mechanical allodynia. Taken together, these results suggest that RVM glial activation is involved in the pathogenesis of CIBP. RVM microglial p38 MAPK signaling pathway is activated and leads to the release of downstream pro-inflammatory mediators, which contribute to the descending facilitation of CIBP.
