Objective To explore the association between circulating endothelial cells (CECs)and atherosclerosis in maintenance hemodialysis(MHD)patients. Methods A crosssectional study was performed to investigate the association between CECs and carotid atherosclerotic change in 65 MHD patients,25 non-hemodialysis patients with chronic kidney disease(CKD)of stage 4 or 5(CKD-non-HD)and 24 age-and Sex-matched healthy controls. CECs in peripheral blood were determined by multiparametrie flow cytometry(FCM).CECs were labeled with CD3-PerCP and CD146-PE before FCM and identified as CD3dim,CD146bright.Atherosclerosis in both groups Was assessed by the measurement of common carotid arery intimamedia thickness (CCA-IMT)and plaque of the common carotid arteries with ultrasound scanner. Results CECs were significantly higher in pre-dialysis patients[(151.52±98.24) cell/ml]and CKD-non-HD patients[(183.00±81.38)cell/ml ] compared with control group[(106.50± 24.14)cell/ml](P<0.05 and P<0.01,respectively).But the number of CECs was not significantly different between MHD and CKD-non-HD patients.CCA-IMT was also significantly higher in MHD patients[(0.94±0.36)mm]and CKD-non-HD patients [(1.02±0.37)mml compared with control group[(0.75±0.15)mm](P<0.05 and P<0.01,respectively).The number of pre-dialysis CECs was positively correlated with CCA-IMT in MHD patients(r=0.328,P<0.01).Multivariate analysis showed that CEC level was a strong independent risk factor of CCA-IMT. Conclusion InMHD patients, CEC level is associated with carotid atherosclerosis and may be used as a marker to evaluate the endothelial damage.

Objective To explore possible associations between osteopontin(OPN) and intact parathyroid hormone(iPTH),to investigate effects of them on the progression of carotid artery calcification in patients receiving long-term hemodialysis. Methods Forty-eight maintenance hemodialysis (MHD) patients and 28 age-and sex-matched healthy volunteers were recruited. The concentration of OPN in peripheral blood was determined by enzyme linked immunosorbent assay (ELISA).Levels of iPTH and presence of plaques in the common carotid arteries were also measured. The demographics were recorded. Results Compared with controls, levels of OPN[(137.4±80.8) ng/L vs (31.6±6.7) ng/L,P < 0.01] and iPTH[(456.4±326.4) ng/L vs (66.9±19.3) ng/L,P < 0.01] were higher in MHD patients before hemodialysis, the numbers of calcific plaques in the common carotid arteries were increased in MHD patients (P < 0.01).There was a positive correlation between pre-dialysis OPN levels and iPTH levels (r =0.620,P < 0.01) in MHD patients.Higher levels of OPN and iPTH correlated with greater numbers of calcific plaques in the common carotid arteries after division into three subgroups of MHD patients based on calcific plaques. In multiple linear regression analysis, the correlation between the pre-dialysis OPN and iPTH levels remained the same even if adjusting for confounding effects[β =0.468,95%C1 (0.036,0.195),t =2.936,P =0.005].Conclusion OPN level is positively correlated with iPTH level in hemodialysis patients, which suggesting that both of them play important roles in the progression of carotid artery calcification.

Objective To introduce the procedures, preliminary experience, and advantages of operation using combination of hysteroscopy and laparoscopy. Methods Sixty-five women underwent transcervical resection of endometrium (11), myoma (38), polyp (2), septa (9), adhesion (1) or foreign body (4). All had the indications for simultaneous laparoscopic monitoring and operation. They were first diagnosed laparoscopically to decide the type of laparoscopic operation. Then they were treated hysteroscopically and finally received laparoscopic operation. Results Laparoscopic examinations confirmed the original diagnoses, revealed the causes of pelvic pain in 8 patients, and detected ovarian cyst, endometriosis, and pelvic adhesion in 5. Two cases of uterine perforation were found by laparoscopic monitoring. Thirty laparoscopic operations included removal or puncture of ovarian cyst, lysis of adhesion, coagulation of endometriosis, Rubin's test, and partial salpingectomy. The postoperative course was smooth and the operative result was satisfactory. Conclusions Operations using combination of hysteroscopy and laparoscopy are safe and effective. They also reduce the pain, and save the time and money of patients. Laparoscopic monitoring may detect uterine perforation in time.

Journal of Traditional Chinese Medicine 2024;65(18):1934-1937
It is believed that the pathogenesis of white lesions on the vulva lies in liver and kidney dysharmony, and the principle of regulating liver and kidney is recommended, with a combination of internal and external treatment, and using multiple methods. Clinically, it is differentiated into deficiency and excess. For deficient patients, liver-kidney yin deficiency is most common, for which self-prepared Zidi Yanghu Decoction (紫地养户汤)is used for internal treatment to nourish the kidney and liver, invigorate blood and dispel wind; self-made Shougui Xiyang Formula (首归息痒方) for fumigation, washing and sitz bath is used as external treatment to supplement and invigorate blood, dispel wind and relieve itching. Damp-heat in the liver channel is the most common excess syndrome, for which the internal treatment is self-made Xiaoyao Xiegan Decoction (逍遥泻肝汤) to clear the liver and drain dampness, unblock collaterals and relive itching, and the external treatment is self-made Kubai Zhiyang Formula (苦柏止痒方) for fumigation, washing and sitz bath in order to clear heat, drain dampness and relieve itching. If there is accompanied local hypopigmentation, obvious itching, painful ulcers, or scratching due to itching, self-made Zima Yukui Oil (紫麻愈溃油) is often applied topically to eliminate macules and engender flesh.

Objective It conducted a grounded theory on the factors influencing the sense of job significance of hospital administrators to provide suggestions and references for the promotion of high-quality development of hospital management work. Methods From April to September 2023,it interviewed 23 administrators from various departments across Provinces/Cities,including Zhejiang,Heilongjiang,Guangdong,Shanghai,Henan,Jilin,Sichuan and others, using a theoretical sampling method.Employing Grounded Theory,it encoded over transcriptional information at three levels. Results Three-level coding process yielded 66 free nodes,19 initial concepts,7 main categories, and 3 dimensions, leading to the development of a theoretical model encompassing existence, relatedness, and growth. Conclusion The job meaning for hospital administrators is shaped by existence, relatedness, and growth. It recommend revising performance assessments, fostering an empowering culture, enhancing informal communication, providing professional growth pathways, and offering diverse career development options to deepen administrators'job meaning.

Objective To explore the influencing factors of the career plateau among hospital administrators,to provide scientific reference for enhancing the quality of hospital management and operation. Methods The interview data were analyzed by grounded theoretical research methods. It conducted semi-structured interviews with 22 tar-get objects and conducted qualitative research on the influencing factors of hospital administrators'career plateau with the help of NVivo 11.0 software. Results After a three-level coding,114 initial concepts,30 subcategories,10 main categories, and 3 core categories were summarized.A theoretical model was developed, encompassing three dimensions:aggravating factors, dissipating factors, and convertible factors. Conclusion The career plateau of hospi-tal administrators is affected by a complex interplay of aggravating, dissipating and convertible factors.Hospitals need to provide regular vocational training to cultivate organizational loyalty for their employees.Hospital administra-tors should engage in proactive career planning and skill diversification.and the government should coordinate the es-tablishment of links between hospitals and the external system to promote the openness and connection of the healthcare system to the outside world.
