1.Observation of arterial compliance in patients with abnormal blood glucose metabolism

In this study, the effects of blood glucose and blood pressure on the compliance of large and small arteries were investigated. The results showed that arterial compliances of both large and small arteries were decreased in patients with diabetes mellitus. In non-diabetic patients with well controlled blood pressure, the compliance of small arteries was markedly improved. These results suggested that both blood glucose and blood pressure affected arterial compliance.
2.The impact of procedural sequence on the quality of bowel preparation in patients with propofol sedation for same-day bidirectional endoscopy

Objective To evaluate the impact of sequence on the quality of bowel preparation in patients with anesthesia for same-day sequential bidirectional endoscopy and propose the optimal procedural sequence.Methods Single center blinded randomized observational study.Sixty-five patients were randomized to either the gastroscopy-first group or the colonoscopy-first group.Bowel cleanliness according to Boston bowel preparation scale (BBPS) scores were evaluated,also done the propofol dosage,caecal intubation time,procedure duration and complications.Results The BBPS score of entire colon showed no difference (6.72 ± 1.34 vs.6.89 ± 1.50,P =0.638),but the BBPS of ileal-cecum portion was higher in the colonoscopy-first group (1.21 ±0.54 vs.1.55 ±0.73,P =0.035).The total procedure time,propofol dosage and complications were similar between the two groups.Conclusion The bowel cleanliness of ileal-cecum portion in colonoscopy-first group is better than that of gastroscopy-first group during sequential bidirectional endoscopy in patients with propofol sedation.We propose colonoscopy first in patients with suspicious ileal-cecum lesion.
4.A retrospective case-control study of immunoglobulin G4-related disease combined with malignancy

IgG4-related disease (IgG4-RD) has been proved to be associated with malignancy.The incidence and risk factors of malignancy development in IgG4-RD were not clear.Nine IgG4-RD patients with malignancies and 27 IgG4-RD control cases were analyzed for risk factors and clinical features.The incidence of malignancy in IgG4-RD was 3.3%,higher than age-controlled general population.Smoking history was significantly more common in patients with malignancies than in the control group (9/9 vs 16/27,P < 0.05).A total of 6/9 malignancies occurred within the first year after the diagnosis of IgG4-RD.Colorectal,biliary and thyroid cancers were the leading types.Smoking history is a risk factor for IgG4-RD associated malignancy.Careful vigilance to monitor malignancy needs to be paid during follow-up.
5.Analysis of bone strength and influencing factors in adults

Objective To investigate bone strength in adults,and explore its influencing factors. Methods One hundred and ninety-one adults undergoing health examinations were selected.The questionnaires about milk intake and exercise conditions were performed,the body weight,fat mass,body mass index(BMI)and calcaneal bone strength were measured,and the relationship between each parameter and bone strength was analysed. Results Bone suength decreased with age increase,and was higher in males than that in females.Bone strength of overweight to obese adults was higher than that with normal body weight(P<0.05).Bone strength of adults with high milk intake was higher than that of those with low milk intake(P<0.05).Multivariate analysis revealed that age was negatively related to bone strength,females were more susceptible to bone strength decrease,and BMI and milk intake were positively related to bone strength. Conclusion Gender,age,BMI and milk intake are main influencing factors for bone strength.Bone strength decreases with age increase,and increase of BMI and milk intake will benefit bone strength,especially for females.
6.Analysis of 32 cases of acute biliary pancreatitis with pancreaticobiliary maljunction

Objective To investigate the relationship between acute biliary pancreatitis and pancreaticobiliary maljunction and the role of magnetic resonance cholangiopancreatography(MRCP) in evaluation of pancreatico biliary maljunction.Methods To compare the liver function indicators of different groups of acute biliary pancreatitis patients(153 cases) associated with pancreatico biliary maljunction and without pancreatico biliary maljunction before and after the conservative treatment.Results The 32 acute biliary pancreatitis patients with pancreatico biliary maljunction detected by MRCP were compared with the 121 cases without pancreatico biliary maljunction.The ALT,AST,GGT after conservative treatment in both group of pancreatico biliary maljunction and Npancreatico biliary maljunction were significantly decreased (P < 0.05).ALT,AST and GGT of pancreatico biliary maljunction group were higher than that of Npancreatico biliary maljunction group with statistical significance (P < 0.05).Conclusions MRCP as a noninvasive cholangiopancreatography study of pancreatico biliary maljunction is a safe and reliable examination method,pancreatico biliary maljunction is one of the important causes of acute biliary pancreatitis.
7.Endoscopic balloon dilatation of small bowel stricture in Crohn′s disease with single balloon enteroscopy

Objective To explore the efficacy and safety of endoscopic balloon dilatation of small bowel stricture in Crohn′s disease with single balloon enteroscopy (SBE).Methods Seven Crohn′s disease patients (four males and three females with a mean age of 37 years) were underwent 10 times dilatation.Before performance,Crohn′s disease with varying degrees of intestinal obstruction was confirmed by image and pathological examinations.There were 8 severe strictures,6 strictures in jejunum.The dilatation was performed step by step with controlled radial expansiondilator (CRE) balloon.After dilatation,the scope could reach the further small bowel was taken as a standard.Results The overall suecess rate was 8 of 10.The average diameter of intestinal stricture was expanded from (4.1 ± 2.5) mm before therapy to (10.2 ±1.5) mm (P<0.01),and the average distance of the further small bowel that the scope reached was 62 cm.Two patients underwent twice and three times of dilatation.After the performance,all patients′ symptoms of abdominal pain and obstruction relieved obviously and there were no severe complication of bleeding or perforation.After the performance,the average follow-up time was 14 months,two patients were treated by surgery and the other two underwent dilatation treatment again because of the recurrence of symptoms.The median symptom-free time was 15 months and 70% of patients were without symptom recurrence in one year.Conclusion Endoscopic balloon dilatation of small bowel stricture with SBE is a new technology of endoscopic treatment,which is safe and effective in the treatment of Crohn's diseasecomplicated with intestinal obstruction.
8.Diagnostic value of double-balloon enteroscopy for small bowel Crohn disease

ObjectiveTo investigate the value of double-balloon endoscopy for the diagnosis of small bowel Crohn disease(CD).MethodsData of 141 patients with suspected CD undergoing double-balloon endoscopy were reviewed.Diagnosis was made based on pathological,endoscopic findings and clinic follow-up results.Detection rates and diagnosis rates of small bowel CD were compared by double-balloon enteroscopy,CT enterography and capsule endoscopy.ResultsThe detection and diagnosis rates of small bowel CD by double-balloon endoscopy were 90.8% ( 128/141 ) and 98.4% ( 126/128),respectively.These two variables by CT enterography were 76.0% (19/25) and 89.5% (17/19),and those by capsule endoscopy were 60.0% (15/25) and 93.3% (14/15).ConclusionDouble balloon enteroscopy has high application value for the diagnosis of small bowel CD.For those contraindicated with endoscopy,CT enterography can be considered as a preferred auxiliary diagnostic modality.
9.Correlation study of aspirin resistance and inflammatory factors in patients with coronary heart disease

Objective To investigate correlation between aspirin resistance(AR) and inflammatory factors. Methods One hundred and ten patients with coronary heart disease took aspirin 0.1 mg/d for 14 days.It was detected platelet aggregation function induced with adenosine disphosphate (ADP) and arachidonic acid (AA), and investigated correlation between AR and inflammatory factors. Interleukin-1? (IL-1?),interleukin-6 (IL-6) and high sensitive C-reaction protein (hs-CRP) levels. Results IL-6 level of patients with AR was significantly higher than that of aspirin sensitive (AS) patients. The other two index were not different between the two groups. Conclusion IL-6 levels could be used as predictor.
10.Investigation of the nursing interns'knowledge about emergency medications

Objective To check the nursing interns′ knowledge about emergency medications. Methods A self-designed questionnaire was used to investigate the knowledge of emergency medications (including dosage, indications and usage) and learning demands in 258 nursing interns. Results The mean score of all students was 18.06 ± 3.35,accounting for 82.2%(212/258)nursing interns expected to obtain the knowledge by the clinical teachers.Both different education backgrounds and training periods didn′t have significant influence on the scores of emergency medications. There were significant differences in total scores among practice nurses from different areas (F=2.87, P<0.05). Conclusion Generally,the nursing interns are not qualified enough to know emergency medications.We should focus on training nursing interns in pharmacological knowledge and the ability of their knowledge management cognition.
