Recently, the incidence of both diabetes mellitus and diabetic foot has been increasing.Ischemia, neuropathy, and infection are major causes of diabetic foot ulcer. In addition to other conventional treatments, hyperbaric oxygen is an effective adjunctive therapy with less side-effects. Hyperbaric oxygen may play a role through different mechanisms in improving the prognosis of diabetic foot.
2.Comparison of apparatus of inspired oxygen in hyperbaric oxygen chamber

Objective To discuss the apparatus of inspired oxygen in hyperbaric oxygen(HBO)chamber for patients after tracheotomy.Methods Many kinds of apparatus of inspired oxygen were analyzed,including composition,usage and characteristics.Results The apparatus of inspired oxygen play an active role in treating patients.But in the above apparatus,there are great differences in serving range,efficiency of inspired oxygen,curative effect,reliability and so on.Conclusions When serious patients after tracheotomy are treated in HBO chamber earlier,it is very important to select reasonable apparatus.

This paper introduces the current situation of hyperbaric oxygen (HBO) therapy, the environment characteristics in HBO chamber and the qualification of sputum aspirators. What's more, this paper also describes the construction, principles and characteristics of all kinds of sputum aspirators in HBO chamber. When critical patients are treated in HBO chamber, it is very important to select reasonable sputum aspirators.
4.Effects of Timosaponin AⅢ on Cell Growth of Two Kinds of Melanoma and Macrophage Activation

Objective:To observe the effect of timosaponin AⅢon the proliferation of cultured melanoma B16 and A375 cells and nitrite produced from macrophage activation. Methods:MTT assay was adopted to detect the cell growth inhibition. The morphological changes of B16 and A375 cells were observed under an inverted microscope. Nitrite production of activated mouse macrophages induced by lipopolysaccharide ( LPS) plus IFN-γ was measured by Griess assay. Results: Timosaponin AⅢ could significantly inhibit the growth of B16 cells at the concentration of 16 μmol·L-1 after the 48- and 72-hour treatment, and significantly inhibit the growth of A375 cells at the concentration ranged from 4 to 16 μmol·L-1 after the 48-and 72-hour treatment. Shrinkage, vacuoles and necrosis of B16 and A375 cells were observed after the 48-and 72-hour treatment by 16μmol·L-1 timosaponin AⅢ,the other concentration of timosaponin AⅢ showed no notable effect on B16 cells and vacuoles of A375 cells appeared at the concentration from 4 to 16 μmol· L-1. Compared with RAW 264. 7 stimulated LPS plus IFN-γ,timosaponin AⅢ could significant inhibit nitrite production of macro-phage inflammatory cell model at the concentration of 10μmol·L-1(P<0. 01). Conclusion:Timosaponin AⅢcan inhibit the prolif-eration of melanoma B16 and A375 cells and macrophage inflammation,suggesting it has anti-tumor and anti-inflammatory effects . The anti-tumor effect of timosaponin AⅢ may be related to the inhibition of tumor inflammation.
5.Research Situation of Hyperbaric Oxygen(HBO) Therapeutic Facilities for the Tracheotomy and Tracheal Intubation Patients

When the tracheotomy and tracheal intubation patients were treated in hyperbaric oxygen chamber,not ordi-nary facemask but specially made therapeutic facilities should be used.To select a more reasonable therapeutic facility for patients treatment in HBO chamber,all kinds of hyperbaric oxygen therapeutic facilities for the tracheotomy and tracheal intubation patients of our country in recent years are introduced from the aspects of material,facility,impermeability,ther-apeutic efficiency,influences on oxygen concentration in chamber,the advantages and disadvantages of each kind were demonstrated.
6.Effects of Ellagic Acid on Melanogenesis and Melansome Transfer

Objective To investigate the effect of ellagic acid on human epidermal melanocyte melanogenesis and melanin transfer, and the mechanism thereof. Methods The human melanocytes and and keratinocytes were co-cultured and purified. After passing the second generation, cells of 1∶10 ratio were inoculated into the small dish (3 cm × 3 cm). The changes of melanin content and tyrosinase activity in melanocytes were detected before and after intervention with ellagic acid (100, 10 and 1 mg/L) for 48 h. The melanin transfer in cultured cells was detected by flow cytometry method. The 10 nmol/L arbutin was used as the positive control. Results The tyrosinase activity was down-regulated by ellagic acid in a dose-dependent manner. The ellagic acid can reduce the melanin content except for the 1 mg/L of ellagic acid. The melanin transfer was also inhibited by ellagic acid in a dose-dependence manner. Conclusion Ellagic acid can be used for skin-whitening cosmetic and the depigmenting effect might be due to the down-regulation of melanogenesis and melanin transfer.
7.Investigation of application and administration of hyperbaric oxygen chambers in Beijing

Objective To investigate the application and administration of hyperbaric oxygen chambers in Beijing.Methods Forty-eight hospitals with hyperbaric oxygen chambers in Beijing were investigated by field investigation,letter investigation or telephone investigation.The investigation includes grades of hospital,personnel,equipment,therapy of severe cases,time of therapy and treatment plans in questionnaire.Results Hyperbaric oxygen was popular in Beijing,which ensured emergency treatments.But the scale was little relatively and the number of full-time technicians was less,besides,the therapeutic system and plans were diversified,which should be standardized.Conclusions Something must be done to improve the conditions of hyperbaric oxygen medicine in Beijing,such as cultivating personnel,standardizing therapeutic system,standardizing treatment plans,realizing computer control and so on.
8.Analysis of influencing factors and strategies on archiving and sharing of S & T resources in a medical organization in Beijing

In Scientific Research Institutions,it has great significance of the S&T resources archiving and sharing to promote the deepening reform of S&T system and realize the great leap-forward development of S&T innovation.In this paper,we conduct empirical analysis on the conditions of S&T resources archiving and sharing in a Medical Organization in Beijing.The results show that the S&T resources archiving and sharing in Medical Organization have achieved initial success,however,the achievements are not ideal and the waste and shortage of S&T resources still coexisted.In view of the deficiencies of the idea,system,laws and regulations of S&T resources archiving and sharing,this paper puts forward corresponding countermeasures and suggestions.
9.Development and application of simple gas dynamic exposure device

Objective To develop a gas dynamic exposure device with a simple structure.Methods The device was composed of a water tank, an gas chest, an exposure kit and an gas-out bottle. The chest body was made of organic glass pane, drainage method was used to distribute the gas properly, and airflow was regulated with the gravity and valve.Results The device was proved to have simple structure and high effects.Conclusion The device has low cost, simple structure and properly distributed gas, and can be reused for exposure test.
10.Study on Rac1/MAPK/ERK pathway mediated mechanism and role in rats with ventilator induced lung injury

Objective To investigate the role of Ras-related C3 botulinum toxin substrate 1/mitogen-activated protein kinase/extracellular signal-regulated kinase (Rac 1/MAPK/ERK) signal pathway in rats with ventilator induced lung injury (VILI) and its mechanism.Methods Thirty Sprague-Dawley (SD) rats were randomly divided into spontaneous respiration group,normal tidal volume (VT) group and high VT group with 10 rats in each group.The rats in spontaneous respiration group were kept their spontaneous breathing.The rats in normal VT group and high VT group were performed tracheal intubation after tracheostomy,and underwent mechanical ventilation on bilateral lungs with 6 mL/kg and 40 mL/kg VT respectively with maintenance anesthesia.After 4-hour ventilation,heart blood,bronchoalveolar lavage fluid (BALF) and lung tissues were harvested.The levels of interleukins (IL-1β,IL-6),tumor necrosis factor-α (TNF-α),myeloperoxidase (MPO) and macrophage inflammatory protein-2 (MIP-2) in serum and BALF were determined by enzyme linked immunosorbent assay (ELISA).Lung wet/dry radio (W/D) was determined.The lung tissues were stained with hematoxylin and eosin (HE),and pathological changes were observed,and pathological scores were evaluated.The ultra structure changes in type Ⅱ alveolar epithelial cells (AEC Ⅱ)were observed with transmission electron microscope.The positive expressions of phosphorylation of extracellular signal-regulated kinase (p-ERK) were determined by immunohistochemistry,and those of Racl and F-actin were determined by immunofluorescence.The mRNA expressions of ERK and Rac1 were determined by real-time fluorescent quantitation reverse transcription-polymerase chain reaction (RT-qPCR),and protein expressions of Rac-1,p-ERK and F-actin were determined by Western Blot.Results ① Compared with spontaneous breathing group,lung W/D in both mechanical ventilation groups was significantly increased,with more significant increase in the high VT group (6.64 ± 0.88 vs.1.79 ± 0.36,P < 0.01).② There was no obvious pathological changes in the lung tissue and AEC Ⅱ of the spontaneously breathing group.In the normal VT group,there was slight edema and infiltration of inflammatory cells;AEC Ⅱ had less lamellar bodies and uniform distribution of the villi of the alveolar epithelium.In the high VT group,the edema of the lung tissue,the widening of the pulmonary septum,the alveolus congestion,the infiltration of inflammatory cells,and alveolar structure disorder were found;and AEC Ⅱ was irregular,the number of lamellar bodies in the plastids was decreased and was unevenly distributed.The pulmonary histopathological score in the high VT group was significantly higher than that in the spontaneous breathing group and the normal VT group (12.00 ± 2.00 vs.6.00 ± 1.51,8.50 ± 0.53,both P < 0.01).③ Compared with spontaneous breathing group,IL-1β,IL-6,TNF-α,MPO,and MIP-2in serum and BALF in both mechanical ventilation groups were significantly increased,with more siguificant increase in the high VT group [serum IL-1 β (ng/L):104.2 ± 15.1 vs.20.3 ± 8.3,IL-6 (ng/L):46.6 ± 11.5 vs.22.7 ± 7.5,TNF-α (ng/L):39.4±6.5 vs.5.4± 1.9,MPO (ng/L):0.66±0.24 vs.0.06±0.03,MIP-2 (ng/L):109.2±25.8 vs.22.8±8.4;BALF IL-1 β (ng/L):121.5 ± 25.6 vs.24.0 ± 7.5,IL-6 (ng/L):136.7 ± 32.7 vs.31.4 ± 10.5,TNF-α (ng/L):98.0 ± 14.8vs.10.1 ±2.6,MPO (ng/L):0.80±0.31 vs.0.08±0.04,MIP-2 (ng/L):144.4±28.9 vs.41.2±20.7;all P < 0.01].④ There were only a few p-ERK,Rac1 and F-actin positive expressions in the spontaneous breathing group.The positive expressions in normal VT group were increased.In high VT group,the positive expression of p-ERK was significantly increased;Rac1 and F-actin were mainly distributed in the cell membrane and cytoplasm respectively,the positive expressions were further enhanced.⑤ The gene expressions of ERK and Rac1,and protein expressions of p-ERK,Rac1 and F-actin in the high VT group were significantly higher than those in the spontaneous breathing group and normal VT group [ERK mRNA (2-△△Ct):8.23±2.83 vs.1,3.02± 1.38,p-ERK protein (gray value):1.15±0.36 vs.0.61 ±0.23,0.88±0.22;Rac1 mRNA (2-△△Ct):4.45 ±2.26 vs.1,1.22±0.39,Rac1 protein (gray value):0.91 ±0.16 vs.0.48±0.11,0.55 ± 0.10;F-actin protein (gray value):0.70± 0.09 vs.0.49 ± 0.08,0.55 ± 0.04;all P < 0.01].Conclusion F-actin expression in lung tissue was up-regulated in rats with VILI,which resulted in reconstruction of AEC Ⅱ cyto-skeleton,and variation of cell membrane permeability through Rac 1 /MAPK/ERK sigualing pathway during VILI.
