1.Diagnosis and treatment of neonatal hypoglycemia

Neonatal hypoglycemia is a common metabolic problem. The definition and clinical management of neonatal hypoglycemia remains controversial. The concerned reasons are mainly as follows:the course of a physiological decrease and recovery in the level of blood glucose is usually existed in newborn infants after birth;asymptomatic hypoglycemia(many newborn infants' blood glucose level is very low but without any clinical signs or symptoms);and the lack of the complete comprehension for the relationship between neonatal hypoglycemia and the long-term neurologic prognosis. The review describes the etiology,definition and clinical management of hypoglycemia in newborn infants.
2.Correlation between HDL-C/TG ratio and cardiovascular risk in elderly patients with hypertension

Objective To investigate the correlation between HDL-C/TG ratio and cardiovascular risk in elderly patients with hypertension, in order to set up an evaluation criterion which may objectively assess the cardiovascular risk in elderly population. Methods A survey was carried out among 2011 subjects aged 60 or over, who received health examination in the General Hospital of PLA in May 2008. Hypertension was diagnosed by using the definition recommended by WHO/ISH. The subjects were divided into 4 groups (male hypertesion group, male control group, female hypertension group and female control group) according to their gender and with or without hypertension. The parameters of hematology (systolic pressure, diastolic pressure, pulse pressure and mean arterial pressure) and the indicators of lipidemia [total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C) and low-density lipoprotein cholesterol (LDL-C)] were measured, and HDL-C/TG ratio was calculated. Logistic regression analysis was used to analyze the correlation between the indicators of lipidemia and cardiovascular risk. Results In both male and female hypertension groups, systolic pressure, diastolic pressure, pulse pressure and mean arterial pressure showed significant differences compared with those of the control groups gender-wise (P0.05). Other indicators of lipidemia showed no significant differences between hypertension group and control group. 3.Relationship between Self-concept and Mental Health in Middle School Students

To explore the relationship between self-concept and mental health. Methods:398 middle school students were tested by SCL-90 and Song-Hattie Self-concept Scale. Results: The correlation coefficients between the total score of SCL-90 and academic self-concept, non-academic self concept and total self concept were -0.22, -0.21 and -0.23 respectively. Conclusion: Self concept was significantly correlated with mental health in middle school students.
4.Therapy of multiple myeloma

Allogeneic hematopoietic stem cell transplantation is the only measure which can cure multiple myeloma,but the high mortality rate related with it limited its use.Autologous hematopoietic stem cell transplantation(Auto-HSCT) has made a great contribution at improving the patients' quality of life and prolonging the survival.In recent years,the application of new drugs and new chemotherapy regimen consisted of traditional chemotherapy and new drugs further enhance the curative effect.
5.Comparative analysis on CTG repeat numbers with BAEP,SEP and VEP in myotonic dystrophy

Objective To study the relationship between the changes of cytosine,thymine,guanine(CTG)repeat numbers and brain stem BAEP,SEP and VEP in patients with myotonic dystrophy(DM) and their family members.Methods The repeat numbers of CTG,BAEP, SEP and VEP of DM gene were determined by PCR amplification and DNA hybridization in 5 patients with DM and 16 members from 3 families.Results The repeat numbers of CTG in 10 normal persons were 30,BAEP,SEP and VEP were normal.The repeat numbers of CTG in 5 patients with DM were over 85,two cases of them were over 1605,they were significantly higher than the normal persons.4 cases in 16 family members were normal,CTG repeat numbers of 12 cases were over normal genes,CTG repeat numbers were related to clinical symptom, BAEP,SEP and VEP.Conclusion The gene diagnosis of DM was found to be consistent with its clinical symptom,the changes of BAEP,SEP and VEP.
6.Expression levels of 3 cytokines in judging the injury time in the healing process of mouse skin wounds

BACKGROUND: Wound healing is a complex biological process regulated temporally and spatially by many repair factors. Studies have showed the predominant expression of cytokines in wound healing process, which is a kind of molecular vital reaction, and may be a new reference index in wound age estimation. But the precise relation of the expressions of cytokines and distribution to wound age is not clear.OBJECTIVE: To probe into the relation of the expressions of cytokines and distribution to wound age in the healing process of the mouse skin wounds.DESIGN: A randomized controlled confirmation study based on the experimental animalsSETTING: Department of Pathology of the First Military University of Chinese PLA, which is a key laboratory of molecule oncology of Chinese PLA.MATERIALS: The study was completed in the Key Laboratory of Molecule Oncdogy of Chinese PLA from July to September in 2003. Sixty-five Kunming mice of either gender, weighing 25 - 28 g, were involved.INTERVENTIONS: The mice were randomly divided into the experimental and the control group. The mice in the experimental group were divided into 12 subgroups according to the different times of trauma. The mice were killed at the following time intervals: 15 minutes, 30 minutes, 1 hour, 3 hours, 6hours, 12 hours, 24 hours, 48 hours, 72 hours, 96 hours, 1 week or 2 weeks after wound and the wounded skin was excised( n = 5 each). As a control, 5mice without wounding were examined.MAIN OUTCOME MEASURES: The amount and distribution of staining positive inflammation and repair cells in skin wound tissues.RESULTS: Persistent positive expression of vascular endothelial growth factor(VEGF) was seen in the epidermal cells and sebaceous gland cells. The expressions of transforming growth factor β1(TGF-β1) and basic fibroblast growth factor (bFGF) in the epidermal cells were increased at 3 hours after incision and lasted till 12 hours and 48 hours respectively. There was a similar distribution in the positive amount of inflammation and repair cells among VEGF, TGF-β1 and bFGF. Six hours after injury, some of neutrophils showed positive reactions. After 12 hours, macrophages and fibroblasts were mainly positive cells. The number of the positive cells was increased with wound age,and the peak occurred at 96 hours. There were still some positive macrophages and fibroblasts at week 1 and week 2 after the incision.CONCLUSION: There was a time-related expression of TGF-β1 and bFGF in the epidermal cells in mouse skin wounds. The amount and distribution of inflammation and repair cells that express VEGF, TGF-β1 and bFGF were changed in a time-related manner in the healing process of mouse skin wounds.
7.Effect of leukocyte-depleted intraoperative salvaged blood on erythrocyte immunity and systemic inflammatory response during perioperative period

Objective To investigate the effect of leukocyte-depleted intraoperative salvaged blood on the erythrocyte immunity and systemic inflammatory response during the perioperative period in patients.Methods Twenty ASA Ⅰ or Ⅱ patients (aged 20-44 years and weighing 40-75 kg) requiring blood salvage during operation (with the estimated intraoperative blood loss of 15-20 ml/kg) were randomly divided into two groups (n =10 each):control group (group C) and leukocyte depletion group (group D).Anesthesia was induced with midazolam,fentanyl,propofol and vecuronium and maintained with isoflurane inhalation and intravenous infusion of propofol and remifentanil.The patients were mechanically ventilated after endotracheal intubation.Intraoperative blood salvage and reinfusion were performed in all the patients.In group D,the salvaged blood was filtered by a leukocyte depletion filter placed in the line of the reinfusion circuit.Blood samples were collected from the central vein before anesthesia (T1),at the end of surgery (T2) and 12 hours (T3) and 36 hours (T4) after operation in all the patients.The rosette rates of red blood cell-C3b receptors (RBC-C3bRR) and RBC-immune complex (RBC-ICR) were determined.The number of leukocytes and polymorphonuclear neutrophils (PMNs) were counted.The plasma levels of interieukin-6 (IL-6),tumor necrosis factor-alpha (TNF-α) and malondialdehyde (MDA) were measured,too.Results Compared with group C,the plasma levels of IL-6,TNF-α and MDA were significantly lower at T2-T4,the RBC-C3bRR was significantly higher at T3-T4 (P < 0.01),and the number of PMNs was significantly lower at T4 in group D(P<0.05).Conclusion Leukocyte-depleted intraoperative salvaged blood is helpful to improve the erythrocyte immunity during the perioperative period in patients,and the decrease of systemic inflammatory response may be involved in the mechanism.

9.Reliability and validity of the Chinese revised version of the intolerance of uncertainty scale

Objective To explore the reliability and validity of the Chinese revised version of the Intolerance of uncertainty scale(IUS).Methods A total of 930 college students completed the IUS.45 college students participated in the retest 3 weeks later.The self-rating anxiety scale (SAS) and the self-rating depression scale (SDS) were used as criterions for assessment of the validity of the Chinese revised version of IUS.Results The Chinese revised version of IUS contained 21 items.Exploratory factor analysis found a two-factor model (negative belief and emotional experience,behavioral dysfunction and negative self-reference) with a cumulative variance contribution rate of 42.09%.And the results of Confirmatory factor analysis (x 2/df =2.82,GFI =0.90,TLI =0.90,CFI =0.91,RMSEA =0.065) indicated that this model provided a reasonable good fit for Chinese college students.The Cronbach α of internal consistency reliability were good for IUS and two subscales,which were 0.929,0.885 and 0.864,respectively.The test-retest reliabilities for IUS and two subscales were 0.782,0.766 and 0.714 respectively.The IUS scores were positively correlated with the scores of SAS and SDS (r =0.489 ~0.575,P < 0.001).Conclusion It suggests that the Chinese revised version of IUS has good reliability and validity,which is qualified enough for relevant research and applications for Chinese adults.
10.Effect of fuzheng paidu pian on the changes of CD4+Tlymphocytes in early infective phase of asymptomatic human immunodeficiency virus

BACKGROUND: Anti-virus therapy is not suitable for the patients who have asymptomatic human immunodeficiency virus (HIV); otherwise, it is significant for early intervention of Chinese herbs to cure asymptomatic HIV.OBJECTIVE: To observe the interventional effects of fuzheng paidu pian on level of CD4+T lymphocytes and changes of plasma viral vector expression in early phase of asymptomatic HIV infection.DESIGN : Observational contrast study.SETTING: Institute of Acquired Immunodeficiency Syndrome (AIDS), Henan College of Traditional Chinese Medicine.PARTICIPANTS: A total of 69 asymptomatic HIV patients, who were infected by paid blood, were randomly selected from some countryside in November 2005. There were 39 males and 30 females. Their ages ranged from 30 to 61 years and the mean age was (43±8) years, and the mean course was (13±3) years. Inclusion criteria: ① coincident with diagnostic criteria of asymptomatic HIV infection published by National AIDS committee (modified in 2005); ②from 18 to 65 years old; ③ 200/mm3 ≤CD4+T lymphocyte count < 600/mm3; ④ Patients did not treat with antiviral and other drugs; ⑤ All patients provided the confirmed consent. Exclusion criteria: ① severe liver and renal malfunction,combining with cardio-cerebral vascular disease and severe primary diseases of lung and hematopoietic system,mental diseases; ② primary immunodeficiency, secondary immunodeficiency induced by hormone and radiation therapy, hematologic disease; ③ pregnant or lactational women. Another 25 asymptomatic HIV patients were randomly selected.and regarded as the blank group. Inclusion and exclusion criteria were as the same as those in the treatment group.METHODS: Patients in the treatment group were treated with fuzheng paidu pian (consisting of huangqi, raw baizhu,fangfeng, baihuashe shecao, ganca o, etc.; Aolinte Pharmaceutical Factory of Henan Institute of Traditional Chinese Medicine; batch number: 050601) by using warm boiled water, 5 tablets once (eqUal to 6.6 g raw materials) for three times a day. Three months were regarded as a course and the treatment was performed four courses in total. Patients in the blank group were not treated with any drugs.MAIN OUTCOME MEASURES: ① Values of CD4+, CD8+ and CD4+/CD8+ in the treatment group before and after treatment;CD4+T lymphocyte count in the blank group after 6 months; ② Viral vector was detected by using reverse transcription polymerase chain reaction (RT-PCT) in the treatment group.RESULTS: All 69 patients in the treatment group and 25 patients in the blank group were involved in the final analysis. ①CD4+ value was decreased in the blank group after 6 months, and there was significant difference before and after 6 months based on α =0.05 level (P < 0.05).However, CD4+ value was increased in the treatment group. ② Viral vector was randomly detected from 23 patients in the treatment group. As compared with before treatment, plasma viral vector was decreased in 9 patients at 6 months after treatment and in 12 patients at 1 year after treatment. The effective rate was 91.30%.CONCLUSION: Fuzheng paidu pian can improve immunological function of asymptomatic HIV patients and decrease viral vector in some cases during early interventional period.
