1.Effect of fuzheng paidu pian on the changes of CD4+Tlymphocytes in early infective phase of asymptomatic human immunodeficiency virus

BACKGROUND: Anti-virus therapy is not suitable for the patients who have asymptomatic human immunodeficiency virus (HIV); otherwise, it is significant for early intervention of Chinese herbs to cure asymptomatic HIV.OBJECTIVE: To observe the interventional effects of fuzheng paidu pian on level of CD4+T lymphocytes and changes of plasma viral vector expression in early phase of asymptomatic HIV infection.DESIGN : Observational contrast study.SETTING: Institute of Acquired Immunodeficiency Syndrome (AIDS), Henan College of Traditional Chinese Medicine.PARTICIPANTS: A total of 69 asymptomatic HIV patients, who were infected by paid blood, were randomly selected from some countryside in November 2005. There were 39 males and 30 females. Their ages ranged from 30 to 61 years and the mean age was (43±8) years, and the mean course was (13±3) years. Inclusion criteria: ① coincident with diagnostic criteria of asymptomatic HIV infection published by National AIDS committee (modified in 2005); ②from 18 to 65 years old; ③ 200/mm3 ≤CD4+T lymphocyte count < 600/mm3; ④ Patients did not treat with antiviral and other drugs; ⑤ All patients provided the confirmed consent. Exclusion criteria: ① severe liver and renal malfunction,combining with cardio-cerebral vascular disease and severe primary diseases of lung and hematopoietic system,mental diseases; ② primary immunodeficiency, secondary immunodeficiency induced by hormone and radiation therapy, hematologic disease; ③ pregnant or lactational women. Another 25 asymptomatic HIV patients were randomly selected.and regarded as the blank group. Inclusion and exclusion criteria were as the same as those in the treatment group.METHODS: Patients in the treatment group were treated with fuzheng paidu pian (consisting of huangqi, raw baizhu,fangfeng, baihuashe shecao, ganca o, etc.; Aolinte Pharmaceutical Factory of Henan Institute of Traditional Chinese Medicine; batch number: 050601) by using warm boiled water, 5 tablets once (eqUal to 6.6 g raw materials) for three times a day. Three months were regarded as a course and the treatment was performed four courses in total. Patients in the blank group were not treated with any drugs.MAIN OUTCOME MEASURES: ① Values of CD4+, CD8+ and CD4+/CD8+ in the treatment group before and after treatment;CD4+T lymphocyte count in the blank group after 6 months; ② Viral vector was detected by using reverse transcription polymerase chain reaction (RT-PCT) in the treatment group.RESULTS: All 69 patients in the treatment group and 25 patients in the blank group were involved in the final analysis. ①CD4+ value was decreased in the blank group after 6 months, and there was significant difference before and after 6 months based on α =0.05 level (P < 0.05).However, CD4+ value was increased in the treatment group. ② Viral vector was randomly detected from 23 patients in the treatment group. As compared with before treatment, plasma viral vector was decreased in 9 patients at 6 months after treatment and in 12 patients at 1 year after treatment. The effective rate was 91.30%.CONCLUSION: Fuzheng paidu pian can improve immunological function of asymptomatic HIV patients and decrease viral vector in some cases during early interventional period.
2.Analysis of serum expression of cystatin C,macrophage migration inhibitory factor and matrix metalloproteinase-1 in elderly patients with coronary heart disease

Objective To explore the differences of serum expression of cystatin C,macrophage migration inhibitory factor (MIF)and matrix metalloproteinase-1 (MMP-1)in elderly patients with coronary artery disease.Methods 120 cases of senile cor-onary heart disease patients in the department of cardiovascular medicine from July 2010 to August 2012 were selected and divided into the control group (n=30),stable angina pectoris group(SAP group,n= 34),unstable angina group (UA group,n=28)and a-cute myocardial infarction group (AMI group,n=28)according to the results of coronary arteriography combined with the electro-cardiogram and myocardial enzyme spectrum test.The serum levels of cystatin C,MIF,MMP-1 and C reactive protein (hs-CRP) were compared among the four groups,and correlations of the four indicators were analysed as well.Results There were significant differences in the serum levels of cystatin C,MIF,MMP-1 and hs-CRP among the four groups(P <0.05).The serum levels of cys-tatin C,MIF and MMP-1 were gradually increased with the progression of disease,and significant differences were found between any two groups(P <0.05).There were significant differences in the levels of hs-CRP between the AIM group and the other three groups(P <0.05).While among the control group,SPA group and UA group,no significant differences were found in the levels of hs-CRP between any two groups(P >0.05).The significant positive correlations were observed among cystatin C,MIF and MMP-1.Conclusion The serum levels of cystatin C,MIF and MMP-1 have a gradually increasing trend with disease progression in pa-tients with coronary heart disease,which could be important indicators of disease evaluation.
3.Detection of mutation in mitochondrial 12S and 16S ribosomal RNA genes in aminoglycoside-induced deafness

Objective To identify the possible mutations other than A1555G in mitochondrial 12S and 16S rRNA genes that responsible for aminoglycoside-induced deafness, and to provide the basis for genetic diagnosis for this disease. Methods A total of twenty-seven blood samples were obtained from five families with aminoglycoside-induced deafness for screening the A1555G mutation and other possible mutations by PCR- Alw26 I digestion and sequence analysis. Results All samples examined in four families (A, C, D and E) carried the same homoplasmic A1555G mutation, but no A1555G mutation was found in family B. Sequencing of the DNA samples from this family displayed a rare insertion of "AA" at nucleotide 2227 in 16S rRNA gene. Conclusions Our findings suggested that the 1555 G mutation was not the only genetic defect associated with aminoglycoside-induced deafness since we did not find the A1555G mutation in one family, in which the typical maternal inheritance pattern of the aminoglycoside-induced deafness was seen. It is not enough to identify prospectively patients who are likely to be hypersensitive to aminoglycoside ototoxicity by screening A1555G mutation only. Other possible mutations in mitochondrial DNA that associated with aminoglycoside -induced deafness should be tested also.

5.Isolation and Identification of a Subgroup A Avian Leukosis Virus from Imported Meat-type Grand-parent Chickens

An exogenous avian leukosis virus (ALV) strain SDAU09C1 was isolated in DF-1 cells from one of 240 imported 1-day-old white meat-type grand parent breeder chicks. Inoculation of SDAU09C1 in ALV-free chickens induced antibody reactions specific to subgroup A or B. But gp85 amino acid sequence comparisons indicated that SDAU09C1 fell into subgroup A; it had homology of 88.8% - 90.3% to 6 reference strains of subgroup A, much higher compared to other subgroups including subgroup B. This is the first report for ALV of subgroup A isolated from imported breeders.
6.Advances in serum-free culture systems of human embryonic stem cells

BACKGROUND:Human embryonic stem cels are able to self-renew indefinitely and have the capacity to differentiate into al three germ layers (ectoderm, endoderm and mesoderm). These properties imply great potential in the basic research and clinical application, including regenerative medicine, drug screening and toxins, early human embryo, cel transplantation, gene therapy,etc. However, it is a substantial chalenge to develop efficient techniques for their large-scale culture under defined conditions, and for controling and directing their differentiation. For therapeutic purposes, many scholars are trying to establish methods for maintaining pluripotency in defined xeno-free conditions and scalable culture systems. OBJECTIVE:To discuss the progress of serum-free culture systems in human embryonic stem cel research reported in recent years and to highlight the chalenges and advances being made towards the development of serum-free and xeno-free culture systems suitable for therapeutic applications. METHODS:A computer-based search of CNKI and PubMed academic database was performed for articles addressing serum-free culture systems of human embryonic stem cels published from 2008 to 2015. Repetitive and old articles were excluded. Finaly, 58 articles were summarized. RESULTS AND CONCLUSION:Several groups have attempted to exclude individual animal components by using feeder-free matrices, feeder cels of human origin, or defined xeno-free media, aiming to select a suitable matrix and medium that can minimize or not use heterologous components, in order to obtain cel lines at clinical level. However, the current cel products are far from clinical application. There are stil many problems to be solved, such as standardization, normalization and individualization of cel products. With the normative development of stem cel research and industry, human embryonic stem cel products are expected to be widely used in clinic.
7.Data Mining Analysis of Medical Records on AIDS Fever by Professor Li Fazhi

This study was aimed to analyze the medication rules of AIDS fever by Professor Li Fazhi, in order to pro-vide the corresponding medication reference basis for Chinese medicine treatment of AIDS fever. The complex net-work analysis method was used. This study was conducted among AIDS fever patients treated by Professor Li Fazhi from September 2007 to June 2012 in Weishi County, Henan Province. The analysis was made on the etiology and pathogenesis of AIDS fever, syndrome differentiation and treatment as well as medication of senior famous Chinese medicine doctors. The results showed that through a multidimensional query analysis, the core medication in the treatment of AIDS fever included harmonization and heat-clearing herbs such as Bupleurum, Licorice, Scutellaria, as well as qi-supplementing and healthy qi reinforcing herbs such as Codonopsis and Astragalus. The core prescriptions were modified Xiaochaihu Decoction and Buzhong Y iqi Decoction. It was concluded that the treatment rules of AIDS fever by Professor Li Fazhi was to clear heat in combination with supplementing the center and boost qi.
9.Value of plasma brain natriuretic peptide levels on the cardiac function evaluation in children with left-right shunt congenital heart disease

Objective To study the value of plasma brain natriuretic peptide (BNP) levels on the cardiac function evaluation in children with left-right shunt congenital heart disease (CHD).Methods Thirtytwo children with left-right shunt CHD were selected and divided into right ventricular group (14 cases) and left ventricular group (18 cases) according to the heart load capacity types.Twenty healthy children were selected as control group.Then plasma BNP levels were determined by enzyme-linked immunosorbent assay method and the left ventricular ejection fraction (LVEF),left ventricular end-diastolic diameter (LVEDD),right ventricular end-diastolic diameter (RVEDD),pulmonary blood flow (Qp)/systemic blood flow ratio(Qs) and left heart Tei index were determined by echocardiography and compared.Results The plasma BNP levels,LVEDD,RVEDD,Qp/Qs and left heart Tei index were (60.21 ± 26.78) rng/L,(35.71 ± 6.98) mm,(25.04 ± 5.52) mm,1.74 ± 0.24,0.34 ± 0.12 in right ventricular group.The plasma BNP levels,LVEDD,RVEDD,Qp/Qs and left heart Tei index were (64.57 ± 25.18) ng/L,(45.27 ± 7.26) mm,(12.34 ± 2.18) mm,1.78 ± 0.19,0.36 ± 0.11 in left ventricular group.The plasma BNP levels,LVEDD,RVEDD,Qp/Qs and left heart Tei index were (33.42 ± 9.46) ng/L,(32.31 ± 4.87) mm,(10.98 ± 1.60) mm,0.92 ± 0.11,0.28 ± 0.08 in control group.The plasma BNP levels,LVEDD,RVEDD,Qp/Qs and left heart Tei index in right ventricular group and left ventricular group were higher than those in control group,and there were significant differences (P < 0.05).There was no significant difference in LVEF among three groups (P > 0.05).The plasma BNP levels in right ventricular group had positive correlation with RVEDD (r =0.634,P < 0.05),Qp/Qs (r =0.721,P < 0.05) and left heart Tei index (r =0.647,P < 0.05).The plasma BNP levels in left ventricular group had postive correlation with LVEDD (r =0.547,P < 0.05),Qp/Qs(r =0.794,P < 0.05) and left heart Tei index (r =0.745,P < 0.05).There was no correlation between the plasma BNP levels and LVEF in right ventricular group and left ventricular group.Conclusion The plasma BNP levels determination helps the early cardiac function evaluation of left-right shunt CHD,and combined with echocardiography can accurately reflect the early cardiac function of the left-right shunt CHD,which can provide objective basis for the clinical diagnosis and treatment.
10.Application of Cluster Analysis Method to the Establishment of Rapid Dispensing Area in Automated Phar-macy

OBJECTIVE:To optimize the placement of extra-machine medicines in the automated pharmacy and establish a ra-pid dispensing area to shorten medicine dispensing time. METHODS:12 588 prescriptions made within one week were collected from the outpatient pharmacy of our hospital,in which 29 kinds of extra-machine medicines used frequently were clustered on the basis of Pearson correlation coefficients. The extra-machine medicines used at a higher frequency were arranged in the storage posi-tions near to the medicine dispensing window,and those related to the above-mentioned medicines were stored in the positions adja-cent to them,thereby a rapid dispensing area was established. The effect of the above-said method was evaluated by comparing the time it took for the pharmacist to dispense medicines one week before and after the establishment of the rapid dispensing area. RE-SULTS:29 kinds of extra-machine medicines were clustered into 4 major categories,and stored in 4 positions in the rapid med-icine dispensing area respectively,and then the position codes were assigned. After the rapid dispensing area was established,the average single prescription dispensing time for the pharmacist decreased by about 9 s (40.43 s vs. 31.43 s,P<0.01). CONCLU-SIONS:The establishment of the rapid dispensing area based on cluster analysis method has increased the efficiency of dispensing medicines in the outpatient pharmacy of our hospital.
