1.Clinical observation of the tension-free vaginal tape-obturator treatment for 130 cases of female stress urinary incontinence

Objective To observe the clinical short-and long-term curative effect of the surgical treatment of female stress urinary incontinence (SUI) with tension-free vaginal tape-obturator (TVT-O) treatment,and discuss the safety of the operation and postoperative quality of life.Methods The data were collected from 130 patients with SUI who were underwent TVT-O treatment.The patients'perioperative period,follow-up of postoperative complications,and comparison of the quality of life before and after surgery were analyzed retrospectively.Results A total of 130 patients was successfully completed their surgeries with a mean operative time (47.01 ± 18.82)min,average blood loss (64.38 ±99.62)ml,mean catheterization (2.67 ±0.90)d,and the average length of stay (4.73 ±2.14)d.A total of 101 cases was completed the postoperative follow-up with (a) preoperative quality of life in patients with ⅡQ-7 score 6 to 21 points and an average of (15.74 ± 3.87) min,(b) symptoms of lower urinary tract UDI-6 score of 3 to 22 minutes and the average (10.51 ± 3.70) min,(c) postoperative quality of life improved significantly ⅡQ-7 score from 0 to 21 points and an average of (1.59 ±4.37)points,and (e) UDI-6 score 0 to 14 points and an average of (1.63 ± 2.66)points.A total of 73 patients (72.3%) had the postoperative urinary incontinence,which subjective symptom were completely cured,significant improvement was 11 cases (10.9%),ineffectiveness was 6 cases (5.9%),pad using was 13 patients (12.9%),vaginal mesh exposure and erosion was 2 patients(2.0%),the sexual life postoperatively was affected in 8 patients (7.9%).Conclusions TVT-O treatment of SUI is not only easy to operate but also has clinically high security,few complications,low recurrence rate,and significantly improved patients' living quantities.However,patch compli cations can not be ignored and need further discussion.
2.The value of two-dimensional ultrasound combined with three-dimensional spatio-temporal image correlation (STIC)in the diagnosis of fetal heterotaxy syndrome

Objective To evaluate the diagnostic accuracy and integrity of two-dimension ultrasound (2DUS)assisted with spatio-temporal image correlation (STIC)for cardiac and vascular abnormalities in fetal heterotaxy syndrome (HS).Methods The retrospective study was conducted from Nov 2007 to Feb 2013.Forty-five fetuses with suspected heterotaxy syndrome on routine prenatal ultrasonic screening underwent STIC volume sweep.The diagnosis was confirmed by pathological or echocardiographic examination.STIC volume data sets of fetal hearts were off-line analyzed blindly by one doctor who had practiced fetal echocardiography for more than five years.The concordance and integrity levels of diagnosis with 2DUS or assisted with STIC were compared according with pathological results.Results There were 397 cardiac and vascular defects in 37 cases of right isomerism.The overall concordance of various defects in right isomerism diagnosed assisted with STIC (96.5%)was significantly higher than that of 2DUS (60.2%) (P <0.05).The concordance rates of right atria and ventricle isomerism,anomalies of ventriculo-arterial junction,arterial arch,systematic and pulmonary veins diagnosed assisted with STIC (78.9%,96.7%, 100%,100%,100%)were higher(P <0.05)than those of 2DUS (49.1 %,60%,34.7%,23.2%,62.2%). Percentages of diagnostic integrity levels (0 ~3)on 2DUS+STIC (8% on level 2,92% on level 3 )were significantly different(P <0.05)with those on 2DUS (1 1 % on level 0,21 % on level 1 ,57% on level 2, 1 1 % on level 3).There were 59 cardiac and vascular defects in 8 cases of left isomerism with the overall concordance of defects diagnosed assisted with STIC (89.8%)and 2DUS (81 .4%)being no difference (P >0.05).There was no significant difference in concordance rates of various defects of left isomerism diagnosed between two methods,except that the concordance rate of left atria isomerism diagnosed assisted with STIC was higher(P <0.05).There was no difference in percentages of diagnostic integrity levels (0-3)in left isomerism between two methods.Conclusions STIC technology may provide considerable diagnostic information for ultrasonic diagnosis of fetal heterotaxy syndrome,especially in abnormalities of atria and ventricle isomerism,ventriculo-arterial junction,arterial arches,systematic and pulmonary veins in right isomerism.
3.Assessment of financial risk exposure of county-level public hospitals reform pilots in Inner Mongolia Autonomous Region

Objective To establish an assessment model of financial risk exposure for the county-levelpublic hospitals in Inner Mongolia Autonomous Region,which can be used to assess the risk exposure of the hospital in question,and as decision making reference for their financial management and risk prevention and control.Methods Using indicators standardized methods and entropy method to process 1 5 financial indicators (quantitative indicators and qualitative indicators )for the 20 public hospitals,and using the gray clustering method to assess financial risk exposure.Results 70% of the county-level public hospitals are faced with less financial risks,while four of them need to pay close attention,and two have large loopholes pending solution.Conclusions Entropy-Gray clustering methods can complement each other,as found in the study.This study proves its significance,and health authorities should establish their long-term financial risk control mechanisms.
4.The effects ofXuling-Jianguformula on bone mineral density and the bone biomechanic in osteoporosis model rats

Objective To study the effects ofXuling-Jiangu formula on bone mineral density and the bone biomechanic in the osteoporosis model rats.Methods According to the random number table method, 40 SD female rats were randomly divided into the caltrate D group, theXuling-Jianguformula group, the model group and the sham group,10 rats each group. In addition to the sham group, the other groups were osteoporosis model. After 30 days, the Caltrate D group received intragastric caltrate D mixed suspension 0.126 g/kg; the Xuling-Jiangu formula group receivedXuling-Jianguformula solution 15 g/kg, and the sham group and the model group received normal saline 10 ml/kg. After 12 weeks treatment, detection of left tibia bone mineral density andthree-point bending method was used for biomechanical testing.Results The mineral density of the Xuling-Jiangu formula group (0.244 ± 0.022 g/cm2,0.195 ± 0.017 g/cm2vs. 0.223 ± 0.013 g/cm2) were significantly higher than the model group and caltrate D group (P<0.01); compared with the model group, the bone biomechanictests in theXuling-Jiangu formula group (0.072 ± 0.036 kN vs.0.041 ± 0.015 kN; 843.754 ± 428.722 N/mm2vs. 482.084 ± 176.646 N/mm2) were significantly higher (P<0.05).ConclusionXuling-Jiangu formula canimprove the bone mineral density and the bone lbiomechanic of osteoporosis rats.
5.The role of liver-targeted CD4+ CD25+ Foxp3+ regulatory T cells in the patients with chronic hepatitis B

Objective To study the correlation between the percentage of intrahepatic regulatory T cell (Treg) and liver inflammatory activity in chronic hepatitis B (CHB) patients.Methods Twenty-six cases of CHB patients admitted to First Department of Internal Medicine,Nagasaki University School of Medicine,Sakamoto,Nagasaki,Japan were enrolled and performed liver biopsy in this study.CD3+cell and Foxp3+ cell in liver were detected by immunohistochemistry.the percentage of Foxp3+/CD3+cell was determined.Clinical data including alanine aminotransferase (ALT).aspartate aminotransferase(AST), hepatitis B virus (HBV) DNA level and the inflammatory activity of histological activity index score (HAD of liver pathology using Knodell evaluating system were collected.The data was analyzed by SPSS 13.0 software.Results Foxp3+Tregs in serial sections of CHB accumulated mainly in the portal area.There Was a significance correlation between the percentage of Foxp3+/CD3+ cell and liver parenchyma inflammation (P=0.007 6).Moreover,Foxp3+Treg in CHB patients with high serum ALT or AsT level presented in a higher frequency than in patients with low AIT or AST level.The difference between these tWO groups was statistically significant (rALT=O.438,PALT=0.025; fAST=O.436,Past=O.026).There Was a tendency between the percentage of Foxp3+/CD3+ cell in liver and HBV DNA level.however the correlation was not statistically significant.Conclusion It is suggested that CD4+ CD25+ Foxp3+Treg may play a major role in the pathogenesis of liver injury in the CHB patients.
6.Effect of sodium ferulate on transdifferentiation and extracellular matrix of rat renal tubular epithelial cells induced by transforming growth factor beta 1

BACKGROUND: Renal tubulointerstitial fibrosis is mainly featured as the accumulation of extracellular matrix (ECM) in renal interstitium. The tubular epithelial-myofibroblast transdifferentiation (TEMT) is important to the pathogenesy of renal tubulointerstitial fibrosis. OBJECTIVE: To examine the effects of sodium ferulate (SF) on TEMT, and ECM main components such as collagen Ⅰ, collagen Ⅲ and fibronectin, in rat renal tubular epithelial cellsinduced by transforming growth factor-beta 1 (TGF- β1)- DESIGN: Randomized and controlled experimental study based on cells. SETTING: Department of Kidney in West China Hospital of Sichuan University. MATERIALS: Rat renal tubular epithelial cells (NRK-52E) originated from American Type Culture Collection (ATCC), were offered by the laboratory of Department of Nephrology in Australian Monash Medical Center. Cell strain used in this study was cultured at the 36th passage. SF white crystal with water solubility and more than 98.0% purify, was from Chengdu Hengda Pharmaceutical Co., Ltd. Different concentrations of SF (125,250, 500μreel/L) were designed in this study. Rabbit anti-rat α-smooth muscle actin (α -SMA) was produced by Wuhan Boster Company. Enzyme-linked immunosorbent assay (ELISA) kit was the produced of Shanghai Senxiong Science and Technology Co.,Ltd. Human recombinant TGF- β1 was produced by R&D Company. DNA Engine OpticonTM real-time fluorescence quantitative polymerase chain reaction apparatus was the product of MJ Research Company. METHODS: Rat renal tubular epithelial cells (NRK-52E) cultured in vitro were divided into five groups. Control group was added with serum-contained DMEM; TGF-β1-induced group was added with TGF-β1 at final concentration of 5 ng/L; SF at different concentrations groups were added with 125, 250, 500 μ mol/L SF and TGF- β1 at final concentration of 5 ng/L,respectively. MAIN OUTCOME MEASURES: The contrast phase microscope, real-time fluorescence quantitative polymerase chain reaction and ELISA method were used to detect TEMT of NRK52E cells induced by TGF-β1 and levels of collagen Ⅰ, collagen Ⅲ and fibronectin in the supernatant. RESULTS: Morphology of NRK52E cells: Compared with control group, TGF-β1 could induce the transdifferentiation of NRK52E cells, showing fibroblast-like in morphology after 3 days, which were previously the typical road stone-like epithelial cells. In three different concentration SF groups, the morphologic transformation stimulated by TGF-β1 could be partly ameliorated in a dose-dependent manner. Expression of α-SMA mRNA: Compared with control group, 5 ng/L TGF- β1 enhanced expression of α-SMA at 6 hours, and reached a peak at 72 hours; SF depressed the expression in a dose-dependent manner at 72 hours (P < 0.05). Changes of ECM: After induced by 5 ng/L TGF- β1 for 72 hours, the levels of collagen Ⅰ, collagen Ⅲ and fibronectin in the supernatant increased significantly (P < 0.05), whereas SF decreased these levels in a dose-dependent manner (P < 0.05). CONCLUSION: TGF- β1 induces the TEMT, and promotes the secretion of collagen Ⅰ, collagen Ⅲ and fibronectin. SF can inhibit TGF- β1-induced TEMT In a dose-dependent manner.
7.Preparation and properties for magnetic/pH double sensitive hydrogel beads carrying carbamazepine

BACKGROUND:The faults or defects in pharmaceutical dosage form designed for carbamazepine may lead to irregular drug absorption, great individual differences between the pharmacokinetics, narrow therapeutic concentration range, and the therapeutic drug monitoring for this drug. OBJECTIVE:To prepare a magnetic/pH double sensitive hydrogel beads carrying carbamazepine and to evaluate its properties. METHODS:Using chitosan, alginate and Fe3O4 nanoparticle as carrier materials, the magnetic/pH double sensitive hydrogel beads carrying anti-epileptic carbamazepine were prepared, and the composition and preparation technology were optimized by orthogonal test of L9(34). The surface morphology and structure of the hydrogel beads were characterized by scanning electron microscopy and Fourier transform infrared spectroscopy respectively. Also, the superparamagnetism, swel ing and release in vitro of hydrogel beads were determined. RESULTS AND CONCLUSION:The optimized preparation technology were described as:0.5%(w/v) chitosan, 1.5%(w/v) alginate, 2.0%(w/v) calcium chloride, and 1:2 ratio for magnetic versus carrier materials. The hydrogel beads under the optimal preparation conditions showed a round shape and smooth surface, and average encapsulation efficiency, loading efficiency and hydrogel beads diameter were 94.36%, 25.05%and 1-2 mm respectively. The hydrogel beads appeared to have superparamagnetism, the swel ing degrees were associated with pH value of medium, and the sequential release amount of carbamazepine from the hydrogel beads in simulated gastric fluid was 22.77%for 2 hours. Then, the beads were moved to the simulated intestinal fluid, and this value approached 91.63%for 24 hours. Experimental findings indicate that, the composition and preparation technology of magnetic/pH double sensitive hydrogel beads carrying carbamazepine was rational and feasible, and hydrogel beads show obvious pH sensitivity and magnetic sensitivety. The control ed-release effect of hydrogel beads in vitro is also good.
8.Effects of iptakalim on the functions of endothelin system in human pulmonary artery endothelial cells

Aim To investigate the effects of iptakalim(IPT),a novel K_(ATP) opener,on the functions of endothelin system in human pulmonary artery endothelial cells.Methods Primary cultured human pulmonary artery endothelial cells were incubated with different concentrations iptakalim for 24 h.The levels of ET-1 in medium were observed by radioimmunoassay.Reverse transcription polymerase chain reaction(RT-PCR)was performed to analyze the expression of ET-1 and ECE.Results When endothelial cells were incubated with IPT at concentrations above 10 μmol·L~(-1),the levels of ET-1 release in medium and the levels of ET-1 mRNA were significantly inhibited.When endothelial cells were incubated with IPT at concentrations above 1 μmol·L~(-1),the levels of ECE mRNA were significantly inhibited.Conclusions IPT can inhibit the expression of ET-1 and ECE mRNA from human pulmonary artery endothelial cells, thus it inhibits the secretion of ET-1 from endothelial cells. Iptakalim may serve as a promising candidate drug to treat pulmonary hypertension.
9.Study on curative effect of Yishen recipe combined with progesterone in the treatment of recurrent spontaneous abortion

Objective To analyze and research the Chinese medicine of tonifying kidney recipe on the mechanism of recurrent spontaneous abortion IL-4, IFN-gamma cytokine.Methods60 patients in accordance with the inclusion criteria with recurrent miscarriage from January 2013 to December 2015 were selected, according to the random number table method, the patients were randomly divided into Chinese medicine group, Western medicine group,Combine traditional Chinese and western medicine group, Chinese medicine group treated with traditional Chinese medicine of tonifying kidney recipe, Western Medicine group were given intramuscular injection of progesterone injection,combining traditional Chinese and Western medicine group was treated with acupuncture combined with progesterone Chinese medicine.The course of treatment was 15 days, before and after treatment in patients with pregnancy outcome and IL-4, IFN-gamma levels.ResultsThere was no significant difference between the three groups before treatment with IL-4 and IFN-.After treatment,compared with the traditional Chinese and Western medicine group, western medicine group, Chinese medicine group of IL-4, IFN-, respectively, showed a rise and fall, the difference was statistically significant(P<0.05).After treatment, compared with the traditional Chinese medicine group, the IL-4 and IFN-of the western medicine group were increased and decreased, respectively, and the difference was statistically significant (P<0.05).Combination of traditional Chinese and Western medicine to obtain a higher clinical cure rate.ConclusionThe combination of TCM and Western medicine for improving the efficacy of pregnancy outcome of recurrent spontaneous abortion and exact expression may obviously improve the IL-4 and IFN-gamma, help to improve the IL-4, reduce the expression of IFN-gamma, improve the outcome of pregnancy.
10.Nursing care of patients with Epstein-Barr virus related post-transplantation lymphoproliferative disorder after allogeneic hematopoietic stem cell transplantation

Objective To summarize the nursing care measures about patients with Epstein-Barr virus related post-transplantation lymphoproliferative disorder (EBV-PTLD) after allogeneic hematopoietic stem cell transplantation. Methods The nursing care measures about five patients with EBV-PTLD after allogeneic hematopoietic stem cell transplantation from October 2014 to May 2015 were summarized and analyzed. Results After carefully treating and nursing care, three patients were discharged. One patient with exacerbations, respiratory, epilepsy, liver failure, and disseminated intravascular coagulation was automatically discharged and then died. And the other patient died of other diseases. Conclusions Epstein-Barr virus infection is a complication after allogeneic hematopoietic stem cell transplantation. The key points to prevention and treatment of EBV infection included monitoring regularly, diagnosing and early intervention, which was proved to be effective, timely and active.
