1.The clinical efficacy of multiple antibiotic in treating tubal infertility infected by ureaplasma urealyticum and chlamydia trachomatis

Objective To observe the clinical efficacy and adverse reactions of antibiotic in treating tubal infertility,which provide support in the clinical applicaiton.Methods 68 cases of tubal infertility caused by Uu and Ct were randomly divided into the control group and observation group according to random digital table.There were 34 cases in each group.The control group was taken hydrotubation.The observation group was given doxycycline 0.1g orally twice a day for 7 days and azithromycin according the results of drug sensitive test on the basis of the control group.In two groups mycoplasma and chlamydia positive rate,pregnancy,clinical efficacy and adverse reactions were compared.Results After 7 days of treatment,Uu positive rate in the observation group was 14.7%.Ct positive rate was 17.6%.Uu positive rate in the control group was 58.8%.Ct positive rate was 64.7%;Uu positive rate and Ct the positive rate in both group showed significant difference(χ2 =14.23,15.54,all P<0.05).The total effective rate in the observation group was 85.3%.In the control group,the total effective rate was 58.8%.The rate in the two groups showed significant difference(χ2 =5.91,P<0.05).Adverse reactions occurring rate in the control group was 17.6%,and the incidence rate of adverse reactions in the observation group was 26.4%.The rate of adverse reactions had no significant difference between the two groups(χ2 =0.77,P >0.05).Conclusion Antibiotic in treating patients with tubal infertility showed significant clinical efficacy and little side effects.We could draw a conclusion that this program was worthy of further clinical application and study.

Asymmetric dimethylarginine(ADMA) is an endogenous inhibitor of nitric oxide synthases(NOS),which is a risk marker of cardiovascular endothelial dysfunction.Recent studies found that ADMA was a predictor not only for acute cardiovascular disease,but also for critical illness and mortality.Studies have found that exogenous ADMA altered lung function and induced apoptosis in animal experiments and vitro experiments.The levels of plasma ADMA significantly increased in children with hypoxia,PPHN,brain damage,and preterm infants who required mechanical ventilation.ADMA could be a independent risk factor to predict neonatal complications and death.
3.Effects of sleep deprivation at different time during night on serum cortisol and emotion of healthy men

Objective To investigate the effects of short-term sleep deprivation(SD)at different time during night on serum cortisol level and emotion state of healthy men,and to find out the influence of work shift onto human circadian rhythm.Methods Two periods of sleep-time were deprived in the present study:the late-night SD(permitted to sleep from 0:00 to 3:00 each day)and the early-night SD(permitted to sleep from 3:00 to 6:00 each day).Ten healthy male adults were chosen to undergo the late-night SD first,followed by normal sleeping for eight nights for washout,and then underwent early-night SD.Each SD period lasted 6 days,i.e.1 day before SD,4 days of SD and 1 day after SD for recovery.Fasting blood samples,for detection of serum cortisol,were collected at 7:00 am every SD day,and the state anxiety inventory(S-AI)scoring was done at the same time.Results The cortisol levels were lowered after 2 types of SD(P0.05).Dependability analysis showed that the cortisol level during the late-night SD period was negatively related to the SD days(r=-0.954 7,P0.05).Both types of SD could elevate anxiety scores,which positively correlated with the SD days(late-night SD:r=0.990,P

Objective To investigate the role of follicle stimulating hormone receptor (FSHR) expression on granulosa cells in ovarian response of women undergoing gonadotropin stimulation. Methods A total of 60 women were recruited and divided into poor, moderate and high responders according to the number of follicles. FSHR expression was detemined by Western blot on granulosa cells obtained by follicular aspiration. The peak levels of serum estradiol (E_2) were meassured by immunofluorescent assay. 5.Application of hysteroscopy in the diagnosis and treatment of infertility caused by intrauterine factors

Objective To study the value of hysteroscopy in the diagnosis and treatment of infertility caused by intrauterine lesions. Methods Hysteroscopic examinations and operations were performed in 82 cases of infertility caused by intrauterine lesions. Results No abnormal findings were found in 4 cases,while the other 78 cases of intrauterine abnormity comprised 23 cases of endometrial polyp, 29 cases of adhesive uteritis,18 cases of submucous myoma or adenomyosis, and 6 cases of uterine deformity. After hysteroscopic surgery and related treatment, the postoperative pregnancy rate reached 62 32% (43 of 69). Conclusions Hysteroscopy may be effectively used in the diagnosis and treatment of infertility caused by intrauterine factors.
6.Alternation and Significance of P-selectin in Plasma During Rats Traumatic Shock

Objective To investigate the alternation and significance of P-selectin in plasma during traumatic shock.Methods 24 rats were randomly assigned to three groups;control group, shock group and L-Arg group. The rats were subjected to induce traumatic shock ,and to determine the plasma P-selectin with ELISA after shock 30 min and 4 hour.Results P-selectin was expression in plasma of normal rats. 30 min after shock, P-selectin in sera was higher than in the control nontraumatized,but no statistical significance;4h after shock, the P-selectin was significantly increased. The P-selectin of L-Arg group in L-Arg group was obviously decreased than those of shock group.Conclusions Traumatic shock could induce P-selectin hyperexpression.It is related to remarkable microcirculatory perturbation and endothelial dysfunction during shock.L-Arg maybe decrease the expression of P-selectin through promoting the synthesis of the endothelial NO.
7.Modified Prolift procedure without trachelectomy or hysterectomy for the treatment of advanced pelvic organ prolapse complicated with cervical elongation

Objective To evaluate the effect and safety of a modified Prolift procedure, without preceding partial trachelectomy or hysterectomy for pelvic organ prolapse (POP) with coexistent cervical elongation. Methods Clinical data of 72 patients that underwent a modified Prolift procedure for POP with coexistent cervical elongation, between December 2008 and June 2012 in Fuzhou General Hospital of Nanjing Military Command was retrospectively analysed. A comparison was carried out between preoperative and postoperative parameters of pelvic organ prolapse quantitation system (POP-Q), and an objective evaluation was made according to the overall cure rate and recurrence rate. Pelvic floor distress inventory-short form 20 (PFDI-20) was used to investigate the subjective cure rate and improvement of symptoms. Results Patients were followed up at median 52 months (36-78 months). One bladder perforation and one rectum perforation occurred during the procedure. Four patients (6%, 4/72) had uterine prolapse at 9-19 months after the opertaion and had transvaginal hysterectomy laterly. The overall anatomical correction rate was 94%(68/72). Six patients (8%, 6/72) had mesh exposures at 3-9 months after the opertaion. Scores of PFDI-20 decreased sifnificantly after the procedure (118.2±25.2 vs 12.1±8.0 vs 12.5±9.5 vs 13.0±9.9, P<0.05). The patients′ satisfaction rate was 92% (66/72). Conclusion This modified Prolift procedure, without preceding partial trachelectomy or hysterectomy, could effectively and safely correct POP with coexistent cervical elongation.
8.Application value of combined detection of virus nucleic acid and antibody in HFMD

Objective To investigate the application value of combined detection of enterovirus 71 type(EV71) nucleic acid and antibody in diagnosis of hand-foot-mouth disease(HFMD).Methods From Jan.to Nov.2015,a total of 180 children with suspected HFMD were divided into nucleic acid detection group,antibody detection group and combined detection group,with 36 subjects for each group.Viral nucleic acid was detected by using reverse transcription polymerase chain reaction.Antibody was detected by using enzyme linked immunosorbent assay.All detected results were analyzed.Results The positive rate of nucleic acid detection group was the highest(80.55%),while that of antibody detection group was the lowest(47.22%).Among various symptoms,the positive rates of nucleic acid,antibody and combined detection in subjects with skin rash were the highest.Conclusion Combined detection of EV71 nucleic acid and antibody could avoid the false positive result of nucleic acid detection and false negative result of antibody detection,thus could be helpful for increasing diagnostic accuracy rate of HFMD.
9.Nursing of a patient with chronic pyogenic osteomyelitis of the mandible caused by multidrug-resistant bacteria

Objective To conclude the nursing experience in a patient with chronic pyogenic osteomyelitis of the mandible caused by multidrug-resistant bacteria. Methods Through the nursing process of patients, detailing the points of disinfection and isolation management, vacuum sealing drainage (VSD) care, drug therapy, hyperbaric oxygen therapy, strengthening primary care, psychological care of the patients with chronic pyogenic osteomyelitis of the mandible caused by multidrug-resistant bacteria. Results Illness was cured after 24 days in the hospital. Conclusions Through the nursing of a patient with chronic pyogenic osteomyelitis of the mandible caused by multidrug-resistant bacteria, reinforces the basic knowledge of nurses, improve nursing skill, which increases the theoretical basis and practical experience in clinical work.
10.Research on the protocol of limited ovarian stimulation to prevent ovarian hyperstimulation syndrome

Objective To investigate whether the limited ovarian stimulation (LOS) could avoid the severe ovarian hyperstimulation syndrome (OHSS) in patients with high risk of OHSS. Methods Ten infertile patients were diagnosed as polycystic ovarian syndrome(PCOS) with high risk of OHSS. All of them were arranged LOS cycles, a long-induction protocol using gonadotropin releasing hormone agonist (GnRHa) for controlled ovarian hyperstimulation. A full dose of human chorionic gonadotropin (10 000 U) was administered when the leading follicle reached a mean diameter of 12.0-14.5 mm. Oocytes were retrieved 36 hours later, followed by conventional in vitro fertilization (IVF) or intracytoplasmic sperm injection (ICSI) and embryo transfer. Results Ten patients produced 11-35 oocytes, including 4-28 oocytes of metaphase Ⅱ. Embryos were transferred in all of ten patients and eight clinical pregnancies were diagnosed. Unfortunately, ectopic pregnancy occurred in one of the patients. None of them experienced the symptoms of OHSS. Conclusions LOS may play a valuable role in preventing OHSS and achieving enough mature oocytes for the patients of PCOS. Furthermore, this protocol would not reduce the pregnancy rate of the patients.
