1.Efficacy and safety of Sanhuang gancao mixture combined with antibiotics in purulent meningitis

Objective To observe the efficacy and safety of Sanhuang gancao mixture combined with antibiotics in the treatment of children with purulent meningitis.Methods 52 cases with purulent meningitis were randomly and double-blindly divided into observation group and control group of 26 cases in each group.The control group received ceftriaxone sodium treatment, observation group received Sanhuang gancao mixture combined with ceftriaxone sodium therapy.The tumor necrosis factor (TNF-α), high-sensitivity C-reactive protein (hs-CRP) and white blood cell (WBC) levels in cerebrospinal fluid and dispearance time of related symptoms, therapeutic effect and adverse reactions in two groups were observed before and after treatment.Results After treatment, the TNF-α, hs-CRP and WBC levels in cerebrospinal fluid of observation group were lower than the control group (P<0.05).The time returning to normal temperature, time of consciousness recovery, time of cerebrospinal fluid returning to normal, dispearance time of intracranial hypertension were shorter than control group (P<0.05).The total efficacy in observation group was higher than control group(P<0.05). There existed adverse reactions in two groups but had no significant difference.Conclusion Sanhuang gancao mixture combined with antibiotics could reduce the inflammatory, return white cells to normal and accelerate the recovery process of sign whose efficacy is better than antibiotics alone without increasing the adverse reactions in the treatment of children with purulent meningitis.

3.Dual-energy CT in the evaluation of solitary pulmonary nodules by virtual non-enhanced images:initial experience

Objective To determine the accuracy and radiation dose of dual-energy computed tomography (CT) in evaluating solitary pulmonary nodules (SPNs) by virtual non-enhanced images.MethodsSixty-one patients with SPNs including 39 malignant and 18 benign nodules proved by pathology underwent DECT scans. The CT values of SPNs on enhanced weighted average and virtual non-enhanced images were compared by using Liver VNC processing unit in terms of their diagnostic accuracy for distinguishing malignant and benign nodules with a 20 HU threshold. Diagnostic accuracy was compared. In 28 patients of all patients, image noise and quality score of the non-enhanced and virtual non-enhanced images were compared, and radiation doses of each patient were recorded. The paired t test was used to analyze the noise difference between the plain scan and virtual non-enhanced scan. The Mann-Whitney U test was used to analyze statistically significant differences of the image quality score and radiation dose between the non-enhanced scan and virtual non-enhanced scan. ResultsThe sensitivity, specificity and accuracy for distinguishing benign and malignant nodules by using the virtual non-enhanced image of were 89. 7%(35/39), 72.2% (13/18), 84.2% (48/57)respectively.The noise of common CT and virtual nonenhanced images were(8. 49 ± 1. 94) HU and( 10. 14 ± 2. 18 ) HU, and there were statistically difference (t=9.45,P<0. 01 ). The quality scores of common CT and virtual non-enhanced images were (4.71 ±0. 46 ) and ( 4.42 ± 0. 57 ), and there were no statistical difference ( U = 290. 00, P > 0. 05 ). The radiation doses of common CT and DECT were ( 3. 72 ± 0. 48 ) mSv and ( 3.78 ± 0. 45 ) mSv, and there were no statistical difference ( U = 350. 50,P > 0. 05 ). ConclusionDECT by using virtual non-enhanced images is useful tool to distinguish the benign and malignant SPN without additional radiation dose.
4.Application value of combined detection of virus nucleic acid and antibody in HFMD

Objective To investigate the application value of combined detection of enterovirus 71 type(EV71) nucleic acid and antibody in diagnosis of hand-foot-mouth disease(HFMD).Methods From Jan.to Nov.2015,a total of 180 children with suspected HFMD were divided into nucleic acid detection group,antibody detection group and combined detection group,with 36 subjects for each group.Viral nucleic acid was detected by using reverse transcription polymerase chain reaction.Antibody was detected by using enzyme linked immunosorbent assay.All detected results were analyzed.Results The positive rate of nucleic acid detection group was the highest(80.55%),while that of antibody detection group was the lowest(47.22%).Among various symptoms,the positive rates of nucleic acid,antibody and combined detection in subjects with skin rash were the highest.Conclusion Combined detection of EV71 nucleic acid and antibody could avoid the false positive result of nucleic acid detection and false negative result of antibody detection,thus could be helpful for increasing diagnostic accuracy rate of HFMD.

OBJECTIVE To learn the situation of cephalosporins used in our hospital,and offer the scientific guidline base for their clinical use.METHODS Statistics methods were used to analyze dosage,sales volume and DDDS of cephalosporins during 2003-2007.RESULTS The usage of the first generation cephalosporins was reduced year by year,but the third generation of compoands cephalosporins and their oxef cephalosporins occupied the main places.The DDDs of cefoperazone/tazobactam usage ranked second for three years(2005-2007),and at the same period their DDDs consumption got top.CONCLUSIONS The situation of the usage of cephalosporins in our hospital is reasonable basically,but more attention should be payed to the usage of the third cephalosporins and their compounds and oxefs.

8.Utilization of Traditional Chinese Medicine Injections in Our Hospital During 2001~2006

OBJECTIVE:To analyze the drug utilization of traditional Chinese medicine injections in our hospital so as to provide scientific basis for clinical rational use of traditional Chinese medicine injections.METHODS:The data of annual consumption and total consumption of traditional Chinese medicine injections in our hospital during 2001~2006 were calculated,and the DDDs and the daily average costs of traditional Chinese medicine injections were analyzed statistically.RESULTS: The annual consumption of traditional Chinese medicine injections showed an increasing trend in our hospital during 2001~2006,with the consumption sum of the top 10 traditional Chinese medicine injections showing a higher proportion,which had a great impact on the total consumption of traditional Chinese medicine injections.Moreover,the rank ratios were less than one for both consumption sum and DDDs of those drugs with high average daily costs.CONCLUSION: Great importance should be attached to the DDDs of traditional Chinese medicine injections and the post-marketing reevaluation for traditional Chinese medicine injections should be intensified.
10.Investigation and its clinical significance of the relative activity of multidrug resistance P-glycoprotein in the plasma of breast cancer patients

Objective To investigate the relative activity of multidrug resistance P-glycoprotein in the plasma of breast cancer patients, analyze the expression of P-glycoprotein before and after chemotherapy and evaluate its clinical value. Methods The plasma of patients and chemical reagents were mixed and distributed into the 96-well plate. The plate was read at 340 nm absorbaace before and after reaction. The variation of absorbance for each well was calculated, and then relative activity of each sample was compared to the referent compound activity. The relative activity of over 30 % was taken as positive expression of the P-gp for each sample. Results The frequency of P-glycoprotein expression was significantly increased from 16.2 % before chemotherapy to 43.2 % after chemotherapy (P<0.05). The positive expression of P-glycoprotein before and after chemotherapy were 5/6 and 9/16, respectively, in patients with positive lymph node metastasis, and 1/6 and 7/16, respectively, in patients with negative lymph node metastasis. Conclusion The results of this preliminary study suggest in P-glycoprotein expression in plasma of patients play an important role in the selection of drugs, which may be useful to evaluate the prognosis for treatment of breast cancer patients.
