1.Restoration and reconstruction of the knee stability for floating knee injury

Objective To investigate the methods and therapeutic effects in reconstructing and restoring the knee stability of floating knee injury. Methods A total of 38 patients with floating knee injury combined with acute knee instability treated from January 2000 to January 2006 were retrospectively studied to evaluate the efficacy of different procedures. Results All patients were followed up for 1-4 years (mean 1.5 years), which showed fracture healing in all patients. There occurred postoperative wound infection in three, internal fixator loosening in two, anterior cruciate ligament laxity in one and osteearthritis in two. According to Score of Hospital for Special Surgery (HSS) for knee function,the knee function was excellent in 32 patients, good in five and fair in one, with excellence rate of 84%. Conclnsions During the treatment of floating knee injury, attention should not only be paid to the reconstruction of the bone continaity, but also to the restoration of ligaments, meniscus, joint capsule and muscles.
2.Surgical treatment of acute biliary pancreatitis

Objective To investigate the timing of operation and the selection of surgical procedure for acute biliary pancreatitis.Methods The clinical data of 247 cases of acute biliary pancreatitis were retrospectively analysed.Results Among 10 non-operation patients,4 died;12 patients underwent emergency operation,5 had postoperative complications and 2 patients died after operation.169 patients underwent delayed operation after effective conservative therapy and 1 patient had postoperative complication;56 patients underwent(selective) operation without any postoperative complication or death.Conclusions Early emergency operation should be performed for relief of biliary obstruction in acute biliary obstructive pancreatitis.The patients with acute non-obstructive biliary pancreatitis should undergo conservative therapy at first and then,after(pancreatitis) has been controlled,delayed operation or selective operation to treat the biliary pathologic condition should be performed.
3.The effect of electroacupuncture pretreatment on the functioning of microvascular endothelial cells in the cerebrum after middle cerebral artery obstruction

Objective:To observe any effect of electroacupuncture pretreatment (EP) on the function of cerebral microvascular endothelial cells in mice with middle cerebral artery obstruction (MCAO), and to explore the role of miRNA-155 RNA and the p53 pathway in determining it.Methods:Forty-two mice were randomly divided into a sham operation group, a model group, an EP group, an miR-155 inhibitor (MI) group, an miR-155 agonist (MA) group, an miR-155 inhibitor negative control (MIN) group and an miR-155 agonist negative control (MAN) group. Each group had 6 rats. All except the sham-operated and model groups received EP, followed by injecting into the corresponding lateral ventricles inhibitor and agonist in groups MI and MA, and placebos in group MIN and MAN. All 7 groups were subjected to MCAO modeling 48 hours after the EP. Twenty-four and 72 hours after the modeling, neurological deficits were quantified using modified neurological severity scoring (mNSS). All of the mice were then sacrificed and any brain tissue damage was observed using hematoxylin-eosin (HE) staining. Terminal deoxynucleotidyl transferase mediated dUTP nick-end labeling (TUNEL) was used to observe any apoptosis of endothelial cells, and enzyme-linked immunosorbent assays (ELISA) were employed to detect the level of intercellular cell adhesion molecule-1 (ICAM-1) in the serum. Apoptotic endothelial cells in the cerebral microbleed and ICAM-1, Bcl-2 and Bax positive cells were detected using immunofluorescence. Quantitative real-time polymerase chain reactions (qRT-PCR) were carried to observe the expressions of miRNA-155 RNA and YY1 and p53 proteins in the ischemia/reperfusion (I/R) region.Results:Twenty-four and 72 hours after the MCAO the average mNSS score of the EP group was significantly lower than the model group′s average. HE staining showed that the cell morphology of the EP group was more regular. The number of apoptotic cells, the expression of ICAM-1, the relative expression of ICAM-1, the Bax/Bcl-2 ratio, as well as the relative expression of miRNA-155 RNA and YY1 and p53 proteins were all then significantly lower in the EP group than in the model group, on average.The expression of miRNA-155 RNA, p53 protein and ICAM-1, as well as the Bax to Bcl-2 ratio in the MI group was then significantly lower than in the MIN group, on average.Conclusion:Electroacupuncture pretreatment improves microvascular endothelial cell functioning in the cerebrum after ischemia and reperfusion (at least in mice) and may attenuate cerebral cell injury by down-regulating the miRNA-155-mediated YY1/p53 pathway.
4.Effect of transversus abdominis plane block combined with diclofenac sodium suppositories on postopera-tive early recovery after laparoscopic hysterectomy

Objective To observe the effect of ultrasound-guided transversus abdominis plane block combined with diclofenac sodium suppository in anus on postoperative analgesia and early recov-ery after laparoscopic hysterectomy.Methods Sixty-six patients of laparoscopic hysterectomy,aged 40-60 years,were randomly divided into group T and group D,33 cases in each group.Group T used ultrasound-guided bilateral transversus abdominis plane (TAP)block after surgery.Group D used TAP block and diclofenac sodium suppositories in anus.All patients received PCIA.Postoperative 24 h sufentanil dosage out of analgesia pump and the effective press times were recorded.The incidence of dysphoria,nausea and vomit within postoperative 24 h,the time of first out-of-bed activity,the time of first anal exhaust and the time of removing urine tube were measured.The plasma concentra-tion of interleukin-6(IL-6),C reactive protein (CRP)and cortisol (Cor)were recorded at the follow-ing four time points:before anesthesia,just postoperative moment,8 and 12 h after surgery. Results Compared with group T,the sufentanil dosage [(18.6±3.2)μg vs (40.8±5.1)μg]and the effective press times [(9.3±2.2)times vs (20.4±3.5)times]were fewer in group D (P<0.05).The incidence of dysphoria,and nausea and vomit were less in group D compared with group T (P<0.05).The IL-6,CRP and Cor of group D at 12 h were lower than those of group T (P<0.05). But the times of first out-of-bed activity,anal exhaust and removing urine tube in two groups were not significantly different.Conclusion Transversus abdominis plane block combined with diclofenac sodium suppositories in anus can provide safe and effective postoperative analgesia for laparoscopic hysterectomy,and promote the early fast recovery.
5.SWOT PEST analysis and optimization strategy for the development of tertiary public pediatric spe-cialty hospitals based on stakeholder theory

SWOT-PEST model was used to analyze the internal advantages and disadvantages,external,opportunities and challenges of tertiary public pediatric hospitals.Based on stakeholder theory,through the literature review method,policy analysis method,combined with the actual work,and summarize its main role and demands.Put forward the development strategy of three-level public pediatric specialty hospital.Through the analysis of interest demands of stakeholders of tertiary public pediat-ric specialty hospitals,the overall development is good at present,but there are still problems that need to be solved urgently.From the perspective of internal and external stakeholders,the optimization strategy involving multiple levels,multiple angles and multiple subjects is proposed to promote the comprehensive development of tertiary public pediatric specialty hospitals.
6.Well-designed board sanding can improve the upper extremity motor functioning of hemiplegic stroke patients

Objective:To observe the effect of well-designed board sanding training on the upper extremity motor functioning of hemiplegic stroke survivors.Methods:Sixty stroke survivors with hemiplegia were randomly divided into an observation group (30 cases) and a control group (30 cases). All received conventional rehabilitation. The observation group′s training involved intelligent board sanding, while the control group′s training involved traditional sanding.Results:After the treatment, significant improvement was observed in the Fugl-Meyer upper extremity scores, modified Barthel index scores and reported shoulder pain in both groups, with the observation group′s averages significantly better than those of the control group. After the intervention, the average scores of both groups on the modified Ashforth scale had also improved significantly.Conclusions:Supplementing conventional rehabilitation treatment with intelligent board sanding can significantly improve upper extremity motor function and ability in the activities of daily living of stroke survivors with hemiplegia while somewhat relieving shoulder pain. The effect is better than with traditional board sanding.
7.Sexual orientation and characteristics of sexual behaviors among 200 HIV positive male college students in Beijing

Objective:
To understand sexual orientation and characteristics of sexual behaviors related with HIV infection among HIV positive male college students in Beijing and to provide suggestions for development of sexual health education and HIV prevention strategy for student population.
Methods:
HIV positive male college students diagnosed from 2016 to June 30, 2019 in Beijing were recruited. Questionnaire was used to retrospectively investigate sexual orientation and characteristics of sexual behaviors before HIV infection. Behaviors related to HIV infection were compared between absolutely homosexual and others sexual orientation.
Results:
The average age of male students firstly identified to be HIV positive was (22.18±2.70) years old, 61.5%(123) of them were undergraduate, 69.5%(139) were not registered permanent residency in Beijing, 38.5%(77) of their interviewed sexual partners were from online chat and 83.0%(166) had homosexual behaviors. Sexual orientation score analysis showed that 50.0% of the participants self identified as exclusively homosexual. Compared with other sexual oriental group, exclusively homosexual group had lower mean age of their HIV infection firstly identified( t =2.77, P =0.01), higher rate of Rush use, firstly insertive sexual behavior with male, firstly insertive sexual behavior before 18 years old, sexual partners more than three persons, having regular partners, nonpersistent use of condom, being diagnosed of sexual transmitted disease and the frequency of homosexual behaviors more than 1 time per month ( χ 2=5.15,28.06,4.16,5.34,5.89,7.39,6.68, P <0.05). Rush users had higher rate of STD diagnosis than non users in exclusively homosexual group ( χ 2=6.26, P =0.01).
Conclusion
Risky sexual behaviors associated with HIV infection were higher in exclusively homosexual group then other sexual orientation groups among HIV positive male college students. Family and school should concern with sexual health education byreinforcing health education via network media to improve college students’ awarenees on HIV/AIDS.
8.Analysis of child mortality under 5 years old in Liuyang City from 2013 to 2020.

OBJECTIVES:
To provide reference basis for reducing the mortality for children under 5 years old and promote the healthy development, the mortality for children under 5 years old and the main causes for death in Liuyang City from 2013 to 2020 are analyzed.
METHODS:
The data of 725 cases of death for children under 5 years old in Liuyang City from 2013 to 2020 were collected.The causes and difference of death among the children were analyzed retrospectively by descriptive statistic methods.
RESULTS:
There were a total of 144 516 live births in Liuyang City from 2013 to 2020. The mortality for children under 5 years old was 5.01‰, for infants was 3.39‰, and for newborns was 1.63‰. The male child mortality was 5.28‰, and the female child mortality rate was 4.72‰, with significant difference (P>0.05). The mortality for children under 5 years old was seasonal fluctuation, without significant difference among seasons (P>0.05). For the past 5 years, the top 3 causes for death among children under 5 years old were preterm birth and low birth weight, congenital heart disease, and pneumonia. Before death, 341 cases (47.04%) were treated in provincial hospitals, 198 cases (27.31%) in county-level hospitals, 56 cases (7.72%) in village-level hospitals, and 130 cases (17.93%) were not treated.
CONCLUSIONS
The mortality for children under 5 years old in Liuyang City is gradually reduced in the past 5 years. The main causes for death are premature birth and low birth weight, congenital heart disease and pneumonia. We should develop healthy education, improve the rate of prenatal diagnosis, promote the construction of obstetrics and paediatrics, and fundamentally reduce the mortality for children under 5 years old.
