Objective To verify the genes screened by the polymerase chain reaction (PCR) chip of cell cycle. Methods The colon cancer cells SW480 were randomized into two groups, the test group (with gastrin stimulation) and con-trol group (without gastrin stimulation). The method of Western blot was used to detect the expression of calcylin binding pro-tein/Siah-1 interacting protein (Cacybp/SIP) before and after gastrin stimulation. The differential expression genes, cyclin de-pendent kinase 8 (CDK8) and cyclin dependent kinase subunit (CKS2), were verified by using real-time quantitative PCR (qRT-PCR). Results It was found that before the stimulation, CacyBP/SIP was located and expressed in cytoplasm, and then in both cytoplasm and nucleus after gastrin stimulation. The qRT-PCR results of CDK8 and CKS2 genes were consis-tent with those of microarray detection. The expressions of CDK8 and CKS2 were up-regulated (P < 0.05). Conclusion The stimulation of human gastrin can lead to the nuclear translocation of CacyBP/SIP. The results of microarray are reliable, and the differentially expressed genes screened through gene chip deserve further study.
3.Evaluation of dynamic morphology of atrial septal defect by real-time three-dimensional transesophageal echocardiography

Objective To determine whether real-time three-dimensional transesophageal echocardiography(RT-3D TEE) is an accurate non-invasive technique for defining the morphology of atrial septal defects(ASD). Methods In 20 patients with secundum ASD, mean age 42.2 years (7 male, 13 female) ,live three dimensional surgical views of ASD were acquired,qualitative morphology of ASD such as the shape, presence of fenestrations and the defect margins were noted during the cardiac cycle. The measurements obtained from 2-dimensional transesophageal echocardiography(2D-TEE) and RT-3D TEE were compared to those obtained from stretched balloon diameter(SBD) or surgery. Measurements of the size and area change of ASD were validated during the cardiac cycle by RT-3D TEE,stepwise multiple linear regression analysis was performed to test the correlation between the maximum change ratios of area and diameter of ASD and age, atrial septal length, ASD shunting velocity, the maximum diameter of ASD and right ventricular systolic pressure. Results ① The morphology of ASD was circle-like or elliptical in systole,irregular or elliptical in diastole. ②The area and diameter of ASD measured by RT-3D TEE was minimal in isovolumetric contraction phase, maximal in isovolumetric diastole phase. The correlation coefficient of maximum diameter measured by 3D with SBD was 0.962 , greater than that by 2D and SBD (0.820). ③The change ratio of area and maximum diameter of ASD acquired by RT-3D-TEE was 11.48%-71.12% and 2.80%-43.87% respectively,and the correlation coefficient of them was 0.921. Conclusions RT-3D TEE using live 3D-Zoom mode accurately displayed the varying morphology,dimensions and spatial relations of ASD. RT-3D TEE can offer visualization morphological changing of ASD in different periods of cardiac cycle,providing more information for percutanous catheter intervention and open heart surgery.

AIM: To investigate the effects of in situ transplantation of mobilized autologous bone marrow stem cells on infarction size and cardiac function in patients with acute myocardial infarction. METHODS:25 patients with first acute myocardial infarction were randomly divided into stem cells in situ transplantation group and control group, 12 patients in stem cells in situ transplantation group were injected subcutaneously with 300 ?g granulocyte colony-stimulating factor(G-CSF) daily for four days in addition to standard therapy. 13 patients in control group were treated with standard therapy alone. The conventional 12 leads electrocardiogram were recorded on 1, 28 days after admission and the cardiac function was scored by the QRS scoring system proposed by Wagner. Furthermore, the infarction size was assessed by radionuclide myocardial perfusion imaging 7, 28 days after admission. OBJECTIVETo clone the full-length cDNA of a gene responsible for vascular smooth muscle cell (v-SMC) proliferation in atherogenesis, and study its function.
METHODSOxidized low density lipoprotein (ox-LDL) at optimal concentration was used as the stimulant to induce v-SMC proliferation in culture medium. A cDNA subtractive library of v-SMC proliferation specific to ox-LDL stimulation was established using subtractive hybridization technique. Methods, including blotting, Northern hybridization and gene sequencing, were used to clone new gene fragments. By using full-length cDNA screening and protein expression techniques, one full-length cDNA was cloned and its function was studied.
RESULTSOne full-length cDNA was cloned. The new gene (Genbank AF 174647) expressed a 44 kDa protein, which might be associated with the activity of ox-LDL.
CONCLUSIONThe new gene cloned may be associated with SMC proliferation in atherogenesis.

6.Optimum compatibility of nabupine mixed with ropivacaine for patient-controlled epidural analgesia after cesarean section

Objective To evaluate the optimum compatibility of nalbufine combined with ropivacaine for patient-controlled epidural analgesia (PCEA) after cesarean section.Methods A total of 100 parturients who were at full term with a singleton fetus,aged 24-35 yr,with body mass index of 29-33 kg/m2,of American society of Anesthesiologists physical status Ⅱ,scheduled for elective cesarean section under combined spinal-epidural anesthesia,were divided into 4 groups (n =25 each) using a random number table method:sufentanil 0.5 μg/ml plus 0.15% ropivacaine group (SR group),nalbufine at final concentration of 0.2 mg/ml plus 0.15% ropivacaine group (N1 R group),nalbufine at final concentration of 0.4 mg/ml plus 0.15% ropivacaine group (N2R group) and nalbufine at final concentration of 0.4 mg/ml plus 0.1% ropivacaine group (N3Rgroup).PCEA solution was prepared correspondingly after surgery,and all the drugs were diluted to 100 ml in normal saline in each group.The PCA pump was set up to deliver a 0.5 ml bolus dose with a 15-min lockout interval and background infusion at 2 ml/h.Visual analog scale scores of incisional pain and anduterine contraction pain were maintained<4.Ramsay sedation scores were recorded at 8,12,24 and 48 h after surgery.The total pressing times of PCEA were recorded within 48 h after surgery.The development of adverse reactions such as nausea,vomiting,skin itching,numbness of lower extremity,urinary retention and respiratory depression was recorded in the analgesia period.Venous blood samples were collected before surgery and at 24 and 48 h after surgery for determination of plasma prolactin concentrations,and the time of colostrum was recorded.Neonatal nerve and adaptive capacity was assessed and scored.Results Compared with group SR,the total pressing times of PCEA were significantly reduced in N2R and N3R groups (P<0.05),and no significant change was found in the total pressing times of PCEA in group N1R (P>0.05).Compared with group N1R,the total pressing times of PCEA were significantly reduced in N2R and N3 R groups (P<0.05).There was no significant difference in the total pressing times of PCEA between group N2R and group N3R (P>0.05).The Ramsay sedation score was 2 in four groups.There was no significant difference in numbness of lower extremity,plasma prolactin concentrations or neonatal nerve and adaptive capacity scores among four groups (P>0.05).Conclusion Nalbufine at final concentraction of 0.4 mg/ml mixed with 0.1% ropivacaine is the optimum compatibility when used for PCEA after cesarean section.
7.Antimicrobial resistance profile and genotypes of Haemophilus influenzae isolates from respiratory tract in children

Objective To investigate the antibiotic resistance profile and ftsⅠ genotypes of Haemophilus influenzae isolates from respiratory tract in children.Methods A total of 141 consecutive nonduplicate clinical strains of H.influenzae were collected from January to March 2016.Antimicrobial susceptibility was determined using Kirby-Bauer method.Beta-lactamase production was detected by nitrocefin disk test.PCR technique was employed for ftsⅠ genotyping.Antimicrobial resistance was compared between differentfisⅠ genotypes.Results The prevalence of β-lactamases was 40.4% (57/141) in H.influenzae isolates.More than half (53.2%,75/141) of the strains were resistant to ampicillin.Mutation offtsI gene was positive in 72.3% (102/141) of the isolates.The dominant genotype of genomic beta-lactamase-negative ampicillin-resistant (gBLNAR) strains was type Ⅲ (72/102,70.6%).The gBLNAR strains showed higher resistance rate to ampicillin and cefuroxime than the genomic beta-lactamase-negative ampicillin susceptible (gBLNAS) strains (P<0.05).Conclusions High prevalence offisⅠgene mutation is found in the strains ofH.influenzae isolated from respiratory tract in children.The dominant genotype of gBLNAR strains was type Ⅲ.Mutation offtsⅠ gene in H.influenzae is associated with higher resistance rate to ampicillin and cefuroxime.
8.Pathogen spectrum of febrile respiratory syndrome among hospitalized children in Shanghai from 2019 to 2020

Objective:To understand the pathogenic spectrum and clinical characteristics of respiratory syndrome among hospitalized children in Shanghai, to provide reference for disease prevention and clinical diagnosis.Methods:Twenty-two pathogens were detected in 634 lower respiratory tract aspirates by using fluorescence quantitative PCR combined with the determination method of dissolution curve, and chi-square test was used for statistical analysis of different detection rates.Results:Among the 634 inpatient specimens, 555 were positive for pathogen nucleic acid, with a total positive rate of 87.54%. The detected pathogens were mainly Mycoplasma pneumoniae (MP), adenovirus (ADV), respiratory syncytial virus (RSV), influenza A virus (FluA) and Boca virus (BoV), and the ratio of multiple pathogen infection was greater than single pathogen infection, the pathogen type of multiple infection was ADV, RSV, FluA or BoV co-infection with MP; in the age distribution, FluA has a higher detection rate in the " <2 year old" and " 2-5 years old" age groups, and RSV had a significantly higher detection rate in the " <2 year old" age group compared with the other two age groups, the detection rate of coronavirus (CoV) in " 2-5 years old" and " >5 years old" is higher than that in " <2 year old" age group; combined with the analysis of clinical symptoms, RSV was found in non-febrile patients. The detection rate of RSV and PIV in cough patients was higher than that in non-cough patients, and the detection rate of RSV in wheezing patients was higher than that in non-wheezing patients. H3N2 in RSV and FluA had a high probability of causing pneumonia; FLuA, FluB and RSV had obvious epidemic peaks in winter and spring.Conclusions:We should strengthen the continuous monitoring of respiratory pathogens such as MP, ADV, RSV, FluA and BoV for the purpose of better understanding the changes in their epidemiological characteristics and pathogenicity, and provide the scientific data for the prevention and control of respiratory diseases in children.
9.Hand-brain perception and movement training based on mirror neuron theory promote the recovery of upper limb function after a stroke

Objective:To explore the effect of combining hand-brain perception training with hand-brain motor training based on mirror neuron theory on the recovery of upper limb function after a stroke.Methods:A group of 105 stroke survivors with upper limb dysfunction were randomly divided into a hand-brain perception (HP) group, a hand-brain motor (HM) group, and a combination (C) group, each of 35. In addition to conventional rehabilitation treatment (including exercise therapy, occupational therapy and physical factor therapy), the HP and HM groups were given hand-brain perception training and hand-brain motor training respectively, while group C was provided with both. Before the intervention and after 4 weeks, the upper limb motor functioning of all of the participants was assessed using the simplified version of the Fugl-Meyer upper limb motor function scale (FMA-UE). Sensory functioning was quantified using the tactile Semmes Weinstein monofilament examination (SWME), and the modified Barthel index (MBI) was used to quantify the participants′ ability in the activities of daily living.Results:After the intervention the average FMA-UE, MBI and SWME scores of all three groups had improved significantly, with group C′s average FMA-UE and MBI scores significantly better than the other two groups′ averages. The average SWME score of group C was then significantly better than that of group HM.Conclusions:Hand-brain perception combined with hand-brain motor training based on mirror neuron theory can further promote the recovery of upper limb sensory and motor functioning of stroke survivors., Such therapy is worthy of clinical promotion and application.

Objective:To assess the effectiveness of a 6-month Ba Duan Jin exercise program in improving the balance of community-dwelling older adults.Methods:A two arms, parallel-group, cluster randomized controlled trial was conducted in 1 028 community residents aged 60-80 years in 40 communities in 5 provinces of China. Participants in the intervention group (20 communities, 523 people) received Ba Duan Jin exercise 5 days/week, 1 hour/day for 6 months, and three times of falls prevention health education, and the control group (20 communities, 505 people) received falls prevention health education same as the intervention group. The Berg balance scale (BBS) score was the leading outcome indicator, and the secondary outcome indicators included the length of time of standing on one foot (with eyes open and closed), standing in a tandem stance (with eyes open and closed), the closed circle test, and the timed up to test.Results:A total of 1 028 participants were included in the final analysis, including 731 women (71.11%) and 297 men (28.89%), and the age was (69.87±5.67) years. After the 3-month intervention, compared with the baseline data, the BBS score of the intervention group was significantly higher than the control group by 3.05 (95% CI: 2.23-3.88) points ( P<0.001). After the 6-month intervention, compared with the baseline data, the BBS score of the intervention group was significantly higher than the control group by 4.70 (95% CI: 4.03-5.37) points ( P<0.001). Ba Duan Jin showed significant improvement ( P<0.05) in all secondary outcomes after 6 months of exercise in the intervention group compared with the control group. Conclusions:This study showed that Ba Duan Jin exercise can improve balance in community-dwelling older adults aged 60-80. The longer the exercise time, the better the improvement.
