1.Peripherally inserted central catheter site with skin injury:progress of management and research

Peripherally inserted central catheter (PICC) has become internationally widespread in various healthcare settings (hospital, clinics ,homecare, et al), mainly used for administration of chemotherapy drugs, hypertonic parenteral nutrition solution and infusion therapy in two extreme age patients (neonates, especially in premature infants;elderly), which provides necessary vascular access for medium to longer term transfusion therapy. The risks of complications related with PICC are still existed, and PICC site with impaired skin integrity is prevalent, but described by only paucity of literature. International evidence-based guidelines provide little strategy. Our study was to review PICC site with skin injury in order to better understand the etiology, evolution and familiar with the international nursing dynamics, and identify directions for future research in this field, promote to overcome the nursing conundrum, and provide a basis for construction of guidelines for the skin management of PICC.
2.Methylation and aberrant expression of Wnt antagonist SFRP1 in bladder cancer

Objective To measure the expression of SFRP1 in bladder cancer and explore the corresponding mechanism,in order to study the roles of SFRP1 in the pathogenesis of bladder cancer.Methods SFRP1 mRNA was detected by RT-PCR.Methylation status of SFRP1 was detected by methylation specific PCR.SFRP1 protein was determined by Western blotting.Results SFRP1 was methylated in bladder cancer cell lines T24 and 5637,not in SCaBER.SFRP1 mRNA and protein were detected in SCaBER,but not in T24 and 5637.Six hours after treating T24 and 5637 with 5'-aza-deoxycytidine,both T24 and 5637 expressed SFRP1 mRNA and protein.In 45 bladder cancers,methylation of SFRP1 was detected in 28 (62.2%).In matched cancer adjacent tissues,6 (13.3%) were found with SFRP1 methylation.The methylation rate in bladder cancers was significant higher than that in matched cancer adjacent tissues (P < 0.01).Conclusions SFRP1 is downregulated in some bladder cancers due to methylation.SFRP1 methylation and aberrant expression may be involved in the pathogenesis of bladder cancers.
3.Medical adhesive-related skin injury associated factors at PICC insertion site in patients with hematologic malignancies

Objective To measure the prevalence and analyzed associated factors of medical adhesive-related skin injuries (MARSI) at PICC insertion site in patients with hematologic malignancies. Methods A cross- sectional observational study lasting two weeks was carried out in Department of Hematology. Skin assessment data and photographs of skin were collected during PICC maintenances by the same investigator. The related information was got out from the medical record. The observed skin injuries were classified by the dermatologist and PICC specialized nurse together. MARSI prevalence was calculated. The associated factors were analyzed statistically. Results All of the 153 patients with hematologic malignancies were included. The prevalence of total MARSI at PICC insertion site was 33.99%(52/153), including mechanical skin injury (22.87%,35/153), contact dermatitis (CD) ( 9.80%,15/153), moisture-associated skin damage (0.65%,1/153), folliculitis(0.65%,1/153), respectively. Multivariate analysis identified two independent risk factors for mechanical skin injury including age≥50 years old, P=0.016, odds ratio (OR)=5.919, 95%CI 1.389-25.220 and non-specialist nurses in PICC maintenance, P=0.013, OR=8.333,95%CI 1.566-44.360. History of skin allergies (P=0.000,OR=12.333,95%CI 3.669-41.454) and arsenic trioxide infusion through PICC(P=0.003,OR=4.565,95%CI 1.661-12.547) were associated with CD. Conclusions MARSI at PICC insertion site is a prevalent event in the patients with hematologic malignancies. Epidemiological data and independent risk factors are presented in our study, which provide a basis for future study in this area.
4.Current situationanalysis of geriatrics construction in guangxi general hospital

Objective To investigate the current situation of the construction of geriatrics in second and third level gen-eral hospitals in Guangxi,analyze the existing problems,and propose suggestions.Methods A self-developed"Survey on the Current Situation of Geriatrics Construction"was used to conduct a questionnaire survey on 250 second and third level general hospitals in Guangxi from April to September 2021.Results A total of 98 geriatrics were established in 196 general hospitals,and the overall number met national requirements.Among them,36 are independent institutions,including 24 tertiary hospitals and 12 secondary hospitals,with 2 172 beds and 1 764 medical staff.Conclusion In the future,Guangxi should ensure the high-quality construction of geriatrics,reasonably allocate geriatric human resources,enhance disciplinary medical service capa-bilities,strengthen overall two-way health connections,and vigorously promote the construction and development of geriatrics in Guangxi.
5.Effect and Potential Mechanism of Resistance Exercise on the Dissolution of Catheter-associated Thrombus

Objectives:To investigate the effect of resistance exercise(RE)on the dissolution of catheter-associated thrombus(CRT)and its possible mechanism.Methods:A total of 36 male SD rats were randomly divided into control group,CRT group,CRT+ RE group(n=12 in each group).Rats in the CRT group did not receive any intervention,while rats in the CRT+RE group received RE intervention 10 days after modeling for 28 days.The thrombolysis rate was evaluated by Hematoxylin-Eosin staining.The contents of irisin,interleukin(IL)-6 and IL-10 in serum were detected by ELISA.The relative mRNA expression levels of peroxisome proliferator-activated receptor γ coactivator α(PGC-1α)and fibronectin type Ⅲ domain-containing 5(FNDC5)in gastrocnemius were detected by real-time quantitative PCR(qPCR).Results:There was no thrombosis in control group.Compared with control group,the contents of irisin and IL-10 in CRT group were significantly decreased,while the content of IL-6 in CRT rats was significantly increased(all P<0.01).Thrombolysis rate was significantly higher,and the mRNA expressions of PGC-1α and FNDC5 in gastrocnemius were significantly upregulated in CRT+RE group than in CRT group(all P<0.01).Serum irisin,IL-10 contents were significantly increased,IL-6 content was significantly lower in CRT+RE group than in CRT group(all P<0.01).Conclusions:Resistance exercise for 28 days could promote the dissolution of CRT,which may be related to the up-regulation of irisin expression and the inhibition of inflammation.
