BACKGROUND:Currently, there is stil no clear conclusion on the pros and cons of arthroscopic anterior cruciate ligament reconstruction with or without stump preservation residues as wel as the impact of reconstruction with stump preservation on proprioceptive sensation. OBJECTIVE:To compare the curative effect of arthroscopic anterior cruciate ligament reconstruction with or without stump preservation on the function and stability of the knee joint during a 1-year folow-up. METHODS: A retrospective analysis of 75 cases of arthroscopic anterior cruciate ligament reconstruction was done, including 37 cases of anterior cruciate ligament reconstruction without stump preservation and 38 cases of anterior cruciate ligament reconstruction with stump preservation. Autologous hamstring tendons were selected as implants. Evaluation of preoperative (6 months before operation) and postoperative (6 months and 1 year after operation) knee function and stability was implemented on the basis of Lysholm scores, IKDC Scores and KT2000. The comparative analysis was carried out as wel on repeated passive angle for proprioception testing. RESULTS AND CONCLUSION:Al of the patients were folowed up for 12 to 20 months. Range of motion of the knee joint was basicaly restored in the two groups 6 months after operation. The results showed the Lysholm scores, IKDC scores and KT-2000 scores at 6 months and 1 year after operation were significantly improved in the two groups as compared with the preoperative data (P < 0.05), and there was no statisticaly difference between the two groups. The proprioception difference between the healthy and affected sides was lower in the patients with stump preservation than those without stump preservation at 6 months and 1 year after operation (P< 0.05). There were two cases in each group whose bone tunnel of the tibia was found deviating from the ideal position through postoperative X-ray examination, but cyclopia was unseen. Anterior cruciate ligament reconstruction with stump preservation is beneficial to the recovery of postoperative proprioception without increasing of postoperative complications.

Objective To evaluate the clinical effect and complications of total hip replacement (THR) in novel femoral neck fracture, old femoral neck fracture, aseptic necrosis of femoral head and coxa degenerative osteoarthropathy. To provide instructions to surgical indications and treatment effects analysis. Methods One hundrde and four patients were divided into 4 groups by disease type: novel femoral neck fracture group (n = 32 ), old femoral neck fracture group (n = 22) ,aseptic necrosis of femoral head group (n =34) and coxa degenerative osteoarthropathy group (n = 16). These patients were followed-up for 12 - 144 months after THR, their Harris standard score and complications data, before and after operation, were analyzed retrospectively. Results After operation, the Harris standard scores were 92. 6 ± 5.8,90. 1 ± 5. 2,86. 3 ± 4. 6,81.9 ±4. 1 in novel femoral neck fracture, old femoral neck fracture, aseptic necrosis of femoral head and coxa degenerative osteoarthropathy groups respectively, which were significantly higher than the scores before operation (25.6±1.8,36.7±2.6,52.9±4.3,42. 1 ±3.8,Ps <0.05). Conclusion THR has good effects in the four types of diseases. Short length of stay and high healing rate are marked characteristics of THR. More attention shoud be paid to the complications of THR.

Nanotechnology-based photothermal therapy has attracted great attention in the past decade. Nevertheless, photothermal therapy has some inherent drawbacks, such as the uneven heat production and limited laser penetration, often leading to insufficient treatment outcomes. Here, we developed a combination strategy to improve cancer therapy. The biomimetic albumin-modified gold nanorods (AuNRs) were prepared with incorporation of paclitaxel (PTX). This therapeutic system was characterized by several features. First, the albumin modification enhanced the biocompatibility and colloidal stability. Second, the surface-coated albumin promoted cellular uptake the albumin-binding protein pathway. Third, PTX was incorporated hydrophobic interaction between PTX and the albumin lipophilic domain. Fourth, the system can be used for combined photothermo-chemotherapy for yielding synergistic effects. The antitumor activity of the system was evaluated both and using the HCT116 colon cancer cell and tumor model. The combination therapy was found with an enhanced treatment efficiency and no obvious side effect. Most importantly, the thermal effect was also discovered with the ability to modulate the tumor microenvironments and suppress the macrophages polarization towards the M2 pro-tumor phenotype. It could be a mechanism for photothermal immunotherapy. The combination strategy and the system provide a potential method for cancer therapy.
