1.Efficacy Analysis of Edaravone Combined with Nimodipine in the Treatment of Acute Large Area Cerebral Infarction

OBJECTIVE:To investigate therapeutic efficacy and safety of edaravone and nimodipine in the treatment of acute large area cerebral infarction. METHODS:101 patients with acute large area cerebral infarction were analyzed retrospectively and divided into control group (53 cases) and observation group (48 cases) according to drug use. After admission,control group re-ceived routine treatment as relieving cerebral edema,anticoagulant and antiplatelet agglutination. Observation group was additional-ly given Edaravone injection 30 mg added into 0.9%Sodium chloride injection 100 mL intravenously,within 30 min,twice a day+Nimodipine injection 12 mg added into 0.9%Sodium chloride injection 500 mL intravenously,once a day,at dripping speed of 0.5μg/(kg·min),for 10 days,and then given Nimodipine tablet 60 mg orally instead,3 times a day,for 5 d,on the basis of control group. Treatment course of 2 groups lasted for 15 d. Clinical efficacies of 2 groups were observed as well as SpO2,hemoglobin (Hb),total hemoglobin (HBT),reduced hemoglobin (MHb),NIHSS score and the occurrence of ADR before and after treat-ment. RESULTS:Total response rate of observation group was significantly higher than that of control group,with statistical signifi-cance(P<0.05). Before treatment,there was no statistical significance in the levels of SpO2,Hb,HBT,MHb and NIHSS score between 2 groups (P>0.05). After treatment,the levels of SpO2,HBT and MHb in 2 groups were significantly higher than be-fore,and the observation group was significantly higher than the control group;NIHSS score of 2 groups were significantly lower than before,and the observation group was significantly lower than the control group;Hb level of 2 groups were significantly low-er than before,with statistical significance(P<0.05),but there was no statistical significance in Hb level between 2 groups(P>0.05). No obvious ADR was found in 2 groups during treatment. CONCLUSIONS:On the basis of conventional therapy,edara-vone combined with nimodipine in the treatment of acute large area cerebral infarction has significant therapeutic efficacy,can im-prove neurological impairment,but doesn't increase the occurrence of ADR.
2.Quality Standard Study on Danqi Tablets

[Objective] To set up standard method of quality to Danqi Tablets.[Method] Take HPLC to make nature authentication,take reverse HPLC to test the content of salvianolc acid B in the preparation.[Result] The authentication is strong in attribute and has good repetition,the measurement has good linear relation,the recycle rate is 101.7%,RSD 1.77%.[Conclusion] The quality standard can effectively control the quality of Danqi Tablets.
3.CSK-conjugated PLGA nanoparticles enhance oral delivery of insulin

Objective To investigate the effects of CSK-conjugated PLGA nanoparticles on oral delivery of insulin in vitro and in vivo.Method CSK-INS-NPs were prepared by double-emulsion. Nanoparticle size、zeta potential and entrapment efficiency were measured. The efficiency of cellular uptake on Caco-2 cells in vitro was evaluated. The hypoglycemic effects were evaluated by monitoring the glucose levels in diabetic rats. Results The average sizes was(134. 4 ± 15)nm and their PDI values were less than 0.3.The insulin entrapment efifciency was around 71%. The cellular uptake of CSK-INS-NPs by Caco-2 cells was 2.8 times higher than INS-NPs. The CSK-INS-NPs transferred more insulin across the Caco-2 cell monolayer than INS-NPs and insulin solution did. In vivo experiments,the CSK-INS-NPs could reduce the blood glucose level of diabetic rats after oral administration in 10 h.Conclusion Compared with insulin solution,CSK-INS-NPs enhanced the insulin through Caco-2 cell monolayer by transcellular pathway and may be a potential delivery system for Oral Delivery of Insulin.
4.Biocompatibility of a new ureteral stent material

BACKGROUND:Studies have shown that polyethylene polymer-synthesized ureteral stents have good biocompatibility in vivo, but the side effects are found to induce the body to produce inflammatory, pyrogenic, and al ergic reactions. Therefore, studies have mainly aimed to explore the biocompatibility, toxic and side effects, inflammation of newly developed ureteral stents synthesized by L-lactic acid, glycolide, and barium sulfate. OBJECTIVE:Based on the biocompatibility comparisons on cel toxicity test, inflammatory reaction, al ergic reaction, pyrogen reaction and mortality after implantation between the usage of polyethylene material and L-lactic acid, glycolide, barium sulphate syntheses, to provide a better reference to introduce a new clinical ureteral stent material. METHODS:There were three groups in the experiment:blank control group, new material group, polyethylene group. Cel s were cultured in normal medium, leach liquid of L-lactic acid, glycolide, barium sulphate syntheses and leach liquid of polyethylene, respectively. RESULTS AND CONCLUSION:The toxic effects were significantly enhanced in the urethral epithelial cel s that grew in the leach liquids of two materials compared with the blank control group (P<0.05), but the two kinds of materials had no significant statistical difference in the toxicity test (P>0.05). Hematoxylin-eosin staining results showed that there were obvious inflammatory infiltrations in muscle tissue and the number of neutrophils and eosinophils were increased when the two kinds of materials were implanted in the rats for 2 and 6 weeks (P<0.05), but the inflammatory infiltration and tissue damage in the new material group was less than that in the polyethylene group (P<0.05). ELISA results showed that the serum concentrations of interleukin-10 and interleukin-23 were significantly increased when the two kinds of materials were implanted in the rats for 6 weeks (P<0.05), but the content of inflammatory cytokines in the new material group was less than that in the polyethylene group (P<0.05). In the new material group, there was only one rat that developed al ergic reaction and there was no pyrogen reaction or death;however, in the polyethylene group, there were three rats with presence of al ergic reaction and one rat with pyrogen reaction but no death. These findings indicate that the new ureteral stent material synthesized with L-lactic acid, glycolide and barium sulphate can reduce inflammation reactions, al ergic reactions and pyrogen reactions to the body, and has better biocompatibility than polyethylene composite material.
5.Nursing progress of fecal bacteria transplantation

Since the fecal bacteria transplantation (FMT) had been exist for a long time, it attracted more attention in recent years because this therapy could adjust the bacteria disturbance rapidly in clinical settings. Recent studies demonstrated that fecal microbiota transplantation (FMT) has great clinical value in the treatment of Clostridium difficile infection, inflammatory bowel disease, Intractable functional constipation and other diseases. But the FMT is not as perfect as we expect, so we did this article to review the history and terminology of FMT, and summarized the progress in nursing methods of FMT.
6.Clinical value of microvascular anastomotic devices in repairing oral carcinoma defects by anterolateral thigh flap

Objective To evaluate the clinical value of microvascular anastomotic devices in free anterolateral thigh flap in the recon-struction of oral carcinoma defects.Methods From September 2013 to September 2014,52 patients with oral cancer were treated by func-tional neck cleaning,primary tumors resection,and free anterolateral thigh flap in the reconstruction.Of which 27 patients received free of femoral anterolateral flap to repair veins by using microsurgical line end to end anastomosis.And 25 pieces of microvascular anastomotic ad-vices were applied in 25 patients with oral carcinoma defects.The length of anastomosis time,flap survival rate,and complications were recor-ded.Results In 52 patients,microvascular anastomotic devices were applied in 25 veins with shorter anastomosis time,(6.3 ±1.9)min vs. (12.3 ±1.4)min,and the difference was significant(P =0.001).The flap survival rate and vascular anastomosis patency rate was 100%.No intra-and post-operative complications such as blood leak,stapling,excessive tension and tear,thrombosis in flap vein crisis associated with microvascular anastomotic devices were observed.Twenty-seven patients with 40 root vein received manual microsurgical anastomosis,2 of them showed vein crisis.Conclusion The microvascular anastomotic devices used in repairing the soft tissue defects by anterolateral thigh flap with venous anastomosis for patients of oral cancer after surgery has the advantage of higher quality,shorter time and less complications.
7.Inhibited autophagy enhance metformin induced apoptosis in oral squamous cell carcinoma

Objective To study the effects of metformin on proliferation and apoptosis of oral squamous cells so as to create a new path for the treatment of oral carcinoma.Methods CAL27 cells were treated with different dosage of metformin(5,10,20 mmol/L)for 24,48, and 72 hours.The number of apoptosis was detected by flow cytometry and TUNEL.The autophagic vacuole was detected by immunofluores-cence.The expression of hallmark of apoptosis and autophagy was detected by Western blot.Results Metformin could induce apoptosis in CAL27 cells.The CL-PARP and Bcl-2 expression significantly increased.In line with the apoptosis,metformin can trigger autophagy in CAL27 cells.The expression of LC3,Beclin-1 and GFP-LC3 positive autophagosomes were increased by metformin.Metformin inhibit the expression of STAT3 and mTOR signaling pathways at the same time might be the possible reasons of the autophagy.And inhibited autophagy could en-hance metformin induced Caspase-3 activity in CAL27 cells.Conclusion Inactivation of STAT3 and mTOR pathway contributes to metform-in-induced autophagy.Inhibited autophagy could enhance metformin induced apoptosis in oral squamous cell carcinoma.
8.Effects of recombinant human epidermal growth factor on wound healing after endoscopic sinus surgery

OBJECTIVE To investigate the effects of recombinant human epidermal growth factor (rhEGF) on wound healing of chronic sinusitis patients after functional endoscopic sinus surgery. METHODS A total of 110 patients with chronic sinusitis underwent functional endoscopic sinus surgery with both sides of their nose. The right side of their nasal cavity (control group) treated by nasal irrigation and steroid nasal spray, the left side of their nasal cavity (study group) treated by nasal irrigation and the nasal spray of steroid hormones and rhEGF. Surgical effect was evaluated under nasal endoscope at 2, 4, 12 and 24 weeks after surgery, while subjective symptom was evaluated depending on visual analogue scale (VAS). Both the score of endoscopic examination and VAS, also the rates of epithelization of the two groups were compared. RESULTS The score of endoscopic examination of the study group at 2 weeks after surgery was better than that of the control group, which had statistical significance, while the score of VAS had no statistical difference. The score of endoscopic examination and VAS, the rate of epithelization in study group at 4 and 12 weeks after surgery were better than those in control group, and the difference was statistically significant, but they were no statistical difference at 24 weeks after surgery. CONCLUSION The using of rhEGF nasal spray on chronic sinusitis patients with functional endoscopic sinus surgery can shorten the time of epithelization, improve the situation of nasal mucosa and patients' subjective symptoms, it is worthy of clinical application.
9.Effect of diosmin on immune mediated factors in serum and prostatic fluid and clinical effect in patients with chronic prostatitis

Objective To explore effect of diosmin on serum and prostatic fluid immune mediated factors and clinical effect in patients with chronic prostatitis.Methods 89 cases of old male patients with chronic non bacterial prostatitis were selected, and divided into two groups.The control group were treated with universal tablets, the experiment group were treated on the base of the control group with diosmin.12 weeks as a course.Clinic effect, adverse reaction rate, prostatic fluid MIP-2 and MIP-1αwere compared after treatment.Results Compared with control group, MIP-1αand MIP-2 in serum and prostatic fluid were lower(P<0.05),NIH-CPSI score and QOL scores were lower(P<0.05), total effective rate was higher than control group(P<0.05),the incidence of adverse reactions was lower(P<0.05).Conclusion Diosmin can reduce MIP-1α, MIP-2 in serum and prostate fluid of patients with chronic none bacterial prostatitis, and improve the pelvic pain, dysuria, sexual dysfunction and other symptoms, and has less adverse reaction.
10.Construction, expression and in vitro biological effects of idiotype Ig Fab fragment of B-chronic lymphocytic leukemia.

The purpose of this study was to construct expression vectors of idiotype (Id) SmIg in patients with B-chronic lymphocytic leukemia and to express them in E.coli to obtain recombinant Id, and to investigate the effect of the protein on the proliferation and secretion of IL-2 and IFN-gamma of stimulated peripheral blood mononuclear cells (PBMC) in vitro. Light chain gene and Fd fragment of heavy chain gene were inserted into fd-tet-DOG2 vector to construct fd-tet-DOG2-Fab. Fab gene was further cloned into expression vector pHEN2 to construct the soluble expression vector pHEN2-Fab. After induction by IPTG, Fab protein was purified by Ni-NTA-chromatography. MTT was used to determine the effects of purified protein on the proliferation of stimulated PBMC in vitro and the concentrations of IL-2 and IFN-gamma in the culture supernatants were detected by ELISA. The results showed that recombinant pHEN2-Fab expression vector was constructed successfully. Fab protein was expressed in positive clone after induced by IPTG and two specific bands at 24-25 kD position were observed by SDS-PAGE electrophoresis. Proliferation of PBMC could be induced by purified Fab and the concentrations of IL-2 and IFN-gamma in culture supernatants were increased significantly after induction. It was suggested that the expression vector of SmIg Fab fragment was constructed successfully, and expressed and secreted from E. coli. The Fab protein could induce proliferation of PBMC and promote secretion of IL-2 and IFN-gamma.
