2.Xenotransplantation of agarose-microcapsulated porcine islets for the treatment of diabetes mellitus

BACKGROUND: Previous studies have shown that agarose-microcapsulated adult porcine islets had some effective biology activities in vitro. However, the biologic effects of such microcapsule materials in transplanted animal need to be further evaluated. OBJECTIVE: To observe the effect of intraperitoneal transplantation of agarose-microcapsulated adult porcine islets in experimental diabetic rats. DESIGN, TIME AND SETTING:A controlled observational experiment was performed at the Central Laboratory of Zhengzhou University from December 2007 to December 2008. MATERIALS: Agarose was used to make microencapsules which were used to coat adult porcine islets by phase-separation method. METHODS: The islets with or without agarose were cultured in RPMI1640, and the culture liquid was changed every two days. The supernatant was collected on the first and fifth days to determine insulin content. The islets placed in different solutions, including 5.6 mmol/L glucose, 16.6 mmol/L glucose, and 10.0 mmol/L alkali, were used for insulin stimulated test by radioimmunoassay. A total of 27 diabetic rats were randomly divided into three groups: control group, unencapsulated islets transplantation group and encapsulated islets transplantation group, injecting saline, (0.9-1.6)?103 unencapsulated islets, and (0.8-1.7)?103 encapsulated islets, respectively. MAIN OUTCOME MEASURES: the difference of basic insulin of microencapsulated islets with agarose; the activity of microencapsulated islets responded to glucose and alkali; the subsistence time of rats accepted microencapsulated islets transplantation. RESULTS: The islets could secrete insulin consecutively for a month, and the cells in capsules survived very well without autolysis and breakage. At day 2 after culture, the islets had marked reactions to the stimulation from glucose in high concentration and theophylline. The released amount of insulin was 1.96 times and 2.58 times as much as that of glucose at a low concentration respectively (P

4.The application of serum procalcition in early diagnosis of nosocomial infection in severe cerebral injury

Objective To investigate clinical value of serum procalcition(PCT) in early diagnosis of nosocomial infection in severe cerebral injury.Methods 58 patients with severe cerebral injury needing ICU monitoring were enrolled.Levels of serum PCT and C-reactive protein(CRP) and white blood cell(WBC) and nosocomial infection when entering ICU or on days 3 were observed.Results There were not difference in level Serum PCT when entering ICU in infection group compared with non-infection group(P > 0.05).Serum PCT increeassd remarkably when entering ICU on days 3 in infection group as compared with non-infection group,the difference was significant (t =5.44,P < 0.01).There were not difference in CRP and WBC level when entering ICU or on days 3 (all P > 0.05).Conclusion Detection of serum PCT helps diagnosis early nosocomial infection in patients with severe cerebral injury.It is suggested that antibiotics should be chosen early in patients with obviously increased PCT,control abuse of antibiotics contributes to reduce incidence of drug resistance and mortality.
5.Clinical Observation of Zhenqi Fuzheng Granules in the Adjunctive Treatment of Cervical Cancer

OBJECTIVE:To observe therapeutic efficacy and safety of Zhenqi fuzheng granules in the adjunctive treatment of cervical cancer. METHODS:86 patients with cervical cancer were randomly divided into control group(43 cases)and observation group (43 cases). Control group was given intravenous dripping of Diphenhydramine hydrochloride injection 20 mg,Granisetron hydrochloride and glucose injection 5 mg,Cimetidine injection 20 mg at 0.5 h before chemotherapy. After 0.5 h of intravenous drip-ping,the patients received chemotherapy:Docetaxel injection 40 mg/m2,d1,d8,intravenous dripping;Cisplatin for injection 25 mg/m2,d1-3,intravenous dripping;21 d as a cycle,a total of 4-6 cycles. On the basis of control group,observation group was addi-tionally given Zhenqi fuzheng granules 5 g,twice 2 day. Clinical efficacies of 2 groups were observed as well as T cell subsets in-dexes [CD3+,CD4+,CD8+,natural killer cell(NK)],KPS scores,termination,terminal time,the times of chemotherapy and the oc-currence of toxic reaction before and after treatment. RESULTS:The total remission rate of observation group was significantly higher than that of control group;the proportion of termination of chemotherapy in observation group was significantly lower than control group;terminal time of chemotherapy was significantly shorter than control group;the times of chemotherapy was signifi-cantly more than control group;the incidence of gastrointestinal reaction,anemia,thrombocytopenia and leukopenia were signifi-cantly lower than control group,with statistical significance(P<0.05). There was no statistical significance in liver function dam-age,renal function injury and stomatitis between 2 groups (P>0.05). Before treatment,there was no statistical significance in T cell subsets indexes and KPS scores between 2 groups(P>0.05). After treatment,CD3+,CD4+ and NK cells in observation group were significantly higher than before treatment and control group,while CD8+ was significantly lower than before treatment and control group;KPS scores of 2 groups were significantly higher than before,and the observation group was significantly higher than the control group,with statistical significance(P<0.05). There was no statistical significance in T cell subsets indexes of con-trol group before and after treatment(P>0.05). CONCLUSIONS:On the basis of routine treatment,Zhenqi fuzheng granules can improve therapeutic efficacy,immune function and the quality of life,moreover,reduce toxic reaction in patients with cervical can-cer.
6.Association of serum visceral adipose tissue-derived serine protease inhibitor levels with carotid atherosclerosis in elderly patients with type 2 diabetes

The serum levels of visceral adipose tissue-derived serine protease inhibitor (vaspin)were measured in 65 elderly type 2 diabetic patients with carotid atherosclerosis( T2DM + CA group), 63 elderly type 2 diabetic patients without carotid atherosclerosis (T2DM group ) and 55 normal elderly individuals( NC group)by enzyme-linked immunosorbent assay. The results showed that the serum vaspin levels in T2DM + CA group were significantly lower than those in T2DM group[(0. 23 ± 0. 13 ) μg/L vs.(0. 41±0. 16)μg/L, P <0. 01]. The serum vaspin levels in T2DM + CA group and T2DM group were significantly lower than those in NC group respectively[(0. 63 ± 0. 11 ) μg/L, P < 0. 01 and P < 0. 05].Logistic regression analysis showed that high sensitivety C-reactive protein and Homeostasis Model Assessment insulin resistance index (HOMA-IR) were negatively correlated with serum vaspin levels,which indicates that lower serum vaspin levels might be associated with the pathogenesis of carotid atherosclerosis in elderly type 2 diabetic patients.

8.Early effect of lumbar dynamic fixation for treatment of lumbar disc herniation

BACKGROUND:Currently, one of common methods is discectomy, nerve root decompression and fusion rigid fixation from the midline approach for disc herniation which is inefficient by conservative treatments. Thus, it is causing degeneration and limiting lumbar physiological activity of adjacent segments. The treatment of non-fusion lumbar disc herniation with the traditional posterior midline incision approach has some disadvantages such as big incision, wide peeling, and back muscle denervation.
OBJECTIVE:To observe therapeutic effects of dynamic stabilization system through Wiltse approach on lumbar disc herniation, and to compare the outcomes with traditional posterior approach.
METHODA total of 46 patients, who had undergone discectomy and internal fixation using dynamic stabilization systems for lumbar disc herniation at the Renmin Hospital of Wuhan University from January 2011 to January 2013, were enrol ed in this study. The operation was performed through the traditional posterior approach in 25 patients and Wiltse approach in 21 patients.
RESULTS AND CONCLUSION:Al 46 patients were fol owed up for 7 to 31 months (averagely, 13.8±2.4 months). The length of incision, intraoperative blood loss, and postoperative drainage amount were less in the Wiltse approach group than in the traditional posterior approach group (P<0.05). No significant difference in visual analog scale scores and operative time was detected between the two groups at 1 week and 6 months after fixation (P>0.05). Radiographs revealed that the position of implants was good in al patients, no loosing or breakage. These data verified that the early effect of dynamic stabilization system through Wiltse approach for lumbar disc herniation is similar to that of traditional posterior approach.
9.Sextant percutaneous pedicle screw fixation for correcting single-segment thoracolumbar fractures

BACKGROUND:Though the effects of conservative or traditional open reduction and internal fixation in the treatment of thoracolumbar fractures are reliable and satisfactory for most cases, two methods also have shortages. Minimal-invasive percutaneous pedicle screw system provides a new available method. OBJECTIVE:To investigate the clinical effect of Sextant percutaneous pedicle screw system in the treatment of thoracolumbar fractures. METHODS:A total of 55 patients, who had undergone percutaneous pedicle screw fixation using Sextant system (25 patients) or traditional open internal fixation (30 patients) for single-level vertebral body compression fractures in Department of Orthopedics of Renmin Hospital of Wuhan University from February 2011 to January 2013, were enrol ed in this study. RESULTS AND CONCLUSION:Except two patients in traditional open internal fixation group were lost after discharge, al other patients were fol owed up for 8-14 months. Operative time, intraoperative blood loss, postoperative drainage amount and hospital day were better in percutaneous pedicle screw fixation group than in the traditional open internal fixation group (P<0.05). Compared with preoperation, the sagittal Cobb angle, visual analogue scale and Oswestry disability index after operation were significantly lower (P<0.05), while anterior vertebral body height ratio was significantly higher in each group (P<0.05). No significant difference was found in terms of correction loss and Oswestry disability index between two groups at 8 months after operation (P>0.05). The results show that percutaneous pedicle screw fixation using Sextant system has a satisfactory outcome in the treatment of thoracolumbar fractures. However, obeying indication strictly is very important for clinical application.
10.Percutaneous pedicle screw-rod fixation using Sextant system in the treatment of thoracolumbar fractures:follow-up evaluation

BACKGROUND:The effects of both conservative and traditional open reduction and internal fixation are dissatisfactory in the treatment of thoracolumbar fractures, however, minimal-invasive percutaneous pedicle screw-rod systems provide a new available method.
OBJECTIVE:To investigate the clinical effect of percutaneous pedicle screw fixation using Sextant minimal-invasive system in the treatment of thoracolumbar fractures.
METHODS:A total of 55 patients, who had undergone percutaneous pedicle screw fixation using Sextant system (25 patients) or open pedicle screw fixation (30 patients) for single-level vertebral body compression fractures in Department of Orthopedics, Renmin Hospital of Wuhan University from February 2011 to January 2013, were enrol ed in this study. They showed no neurological signs and symptoms. The operative time, intraoperablood loss, postoperative drainage amount, length of hospital stay, pre-and post-operative Cobb angle and anterior vertebral body height ratio were recorded and compared between two groups.
RESULTS AND CONCLUSION:Except two patients in open pedicle screw fixation group were lost after discharge, al other patients were fol owed up for 8-14 months. There were significant differences in operative time, intraoperative blood loss, postoperative drainage amount and length of hospital stay between two groups (P=0.000 0). The post-operative Cobb angle was significantly lower while anterior vertebral body height ratio was significantly higher at one week than those before fixation in two groups (P=0.000 0). No significant difference was found in correction loss between two groups at 8 months after operation. Experimental findings indicate that, percutaneous pedicle screw fixation using Sextant system has a satisfactory outcome in the treatment of thoracolumbar fractures. However, understanding the correct indications is very important for clinical application.
