1.Biocompatibility of a new ureteral stent material

BACKGROUND:Studies have shown that polyethylene polymer-synthesized ureteral stents have good biocompatibility in vivo, but the side effects are found to induce the body to produce inflammatory, pyrogenic, and al ergic reactions. Therefore, studies have mainly aimed to explore the biocompatibility, toxic and side effects, inflammation of newly developed ureteral stents synthesized by L-lactic acid, glycolide, and barium sulfate. OBJECTIVE:Based on the biocompatibility comparisons on cel toxicity test, inflammatory reaction, al ergic reaction, pyrogen reaction and mortality after implantation between the usage of polyethylene material and L-lactic acid, glycolide, barium sulphate syntheses, to provide a better reference to introduce a new clinical ureteral stent material. METHODS:There were three groups in the experiment:blank control group, new material group, polyethylene group. Cel s were cultured in normal medium, leach liquid of L-lactic acid, glycolide, barium sulphate syntheses and leach liquid of polyethylene, respectively. RESULTS AND CONCLUSION:The toxic effects were significantly enhanced in the urethral epithelial cel s that grew in the leach liquids of two materials compared with the blank control group (P<0.05), but the two kinds of materials had no significant statistical difference in the toxicity test (P>0.05). Hematoxylin-eosin staining results showed that there were obvious inflammatory infiltrations in muscle tissue and the number of neutrophils and eosinophils were increased when the two kinds of materials were implanted in the rats for 2 and 6 weeks (P<0.05), but the inflammatory infiltration and tissue damage in the new material group was less than that in the polyethylene group (P<0.05). ELISA results showed that the serum concentrations of interleukin-10 and interleukin-23 were significantly increased when the two kinds of materials were implanted in the rats for 6 weeks (P<0.05), but the content of inflammatory cytokines in the new material group was less than that in the polyethylene group (P<0.05). In the new material group, there was only one rat that developed al ergic reaction and there was no pyrogen reaction or death;however, in the polyethylene group, there were three rats with presence of al ergic reaction and one rat with pyrogen reaction but no death. These findings indicate that the new ureteral stent material synthesized with L-lactic acid, glycolide and barium sulphate can reduce inflammation reactions, al ergic reactions and pyrogen reactions to the body, and has better biocompatibility than polyethylene composite material.

Since the fecal bacteria transplantation (FMT) had been exist for a long time, it attracted more attention in recent years because this therapy could adjust the bacteria disturbance rapidly in clinical settings. Recent studies demonstrated that fecal microbiota transplantation (FMT) has great clinical value in the treatment of Clostridium difficile infection, inflammatory bowel disease, Intractable functional constipation and other diseases. But the FMT is not as perfect as we expect, so we did this article to review the history and terminology of FMT, and summarized the progress in nursing methods of FMT.

4.Effect of diosmin on immune mediated factors in serum and prostatic fluid and clinical effect in patients with chronic prostatitis

Objective To explore effect of diosmin on serum and prostatic fluid immune mediated factors and clinical effect in patients with chronic prostatitis.Methods 89 cases of old male patients with chronic non bacterial prostatitis were selected, and divided into two groups.The control group were treated with universal tablets, the experiment group were treated on the base of the control group with diosmin.12 weeks as a course.Clinic effect, adverse reaction rate, prostatic fluid MIP-2 and MIP-1αwere compared after treatment.Results Compared with control group, MIP-1αand MIP-2 in serum and prostatic fluid were lower(P<0.05),NIH-CPSI score and QOL scores were lower(P<0.05), total effective rate was higher than control group(P<0.05),the incidence of adverse reactions was lower(P<0.05).Conclusion Diosmin can reduce MIP-1α, MIP-2 in serum and prostate fluid of patients with chronic none bacterial prostatitis, and improve the pelvic pain, dysuria, sexual dysfunction and other symptoms, and has less adverse reaction.
5.Surgical treatment for infra-cardiac total anomalous pulmonary venous drainage and early to mid follow up results In 17 cases

Objective To evaluate the effectiveness of urgent surgical correction for infra-cardiac total anomalous pulmonary venous drainage (TAPVD) in infants and children. Methods From July 2000 to April 2009, seventeen patients with infra-cardiac type of total anomalous pulmonary venous connection received surgical correction in our department. The age of patients ranged from 15 days to 3.7 months at admission, 9 patients ( 52.9 % ) were younger than 1 month of age. The mean body weight was (4.10 ±0.95) kg and was less than 5 kg in 15 patients (88.2%). Deep hypothermic circulatory arrest (DHCA) without cerebral perfusion was used in 13 patients (76%), and continuous hypothermic low-flow perfusion was used in4 (24%). Incisions on the left atrium and pulmonary veins were sutured without tension, and the width of the anastomosis was 2.5 to 4.0 cm. Results Urgent operations were performed in all patients and no early death occurred during surgery.Mean bypass time was 121.9 minutes ( ranging from 70 to 277 minutes) and mean aortic cross-clamp time was 44 minutes ( ranging from 30 to 74 minutes). 6 neonates had delayed wound closure after surgery and 4 had obvious pulmonary edema. Inhalation of nitric oxide was given to 5 cases for the management of significant postoperative pulmonary hypertension, while peritoneal dialysis was performed in 2 cases with temporary renal failure. Transient atrioventricular block of grade 3 occurred in 2patients and acute renal failure occurred in 2 cases. All patients were uneventfully discharged. The mean ventilation time was 45 h (ranging from 12 to 430 h). Mean duration in ICUs and hospitals were 15 days ( ranging from 4 to 40 days) and 22. 5days ( ranging from 10 to 42 days), respectively. During the period of follow up, one patient presented with pulmonary venous obstruction and recurrent pulmonary infection with increased flow speed at the site of anastomosis, pulmonary hypertension and tricuspid regurgitation on echocardiogram. No improvement was observed in the patient spite of medical treatment and he was lost during follow up one year after operation. 16 patients received postoperative follow up for 1 month to 9 years, mean (51.8 ± 35.0) months. Echocardiography, chest radiography, and ECG were performed during this period. As a result, most children had good cardiac function, with sinus rhythm on ECG and apparently reduced pulmonary congestion on radiography.No obstructive pulmonary venous return was observed on echocardiography. Most patients had good cardiac functions. Conclusion Corrective operation for infra-cardiac TAPVC on urgent basis may provide favorite outcomes. The prognosis is associated with the size of anastomosis between the pulmonary vein and left atrium, as large anastomosis may prevent the obstruction of pulmonary venous retum.
6.A novel experimental model of New Zealand rabbit for the biocompatible assessment of 0.1 mm PTFE

Objective To establish a novel experimental model of New Zealand rabbit to assess the biocompatibility of 0.1 mm polytetrafluoroethylene (PTFE),a novel material of pulmonary valve.Methods Forty-two adult New Zealand rabbits about 3 kg were selected to give 35 mg/kg ketamine and 0.25 mg/kg dexmedetomidine intramuscularly for the anesthesia.The chest was open to expose the upper segment.The surface of right ventricular outflow tract (RVOT) was exposed after the pericardium was opened partially.The valve material (0.1 mm PTFE) was inserted into the right ventricle via the central mini-incision.Then the skin was closed and the rabbits were recoved with the oxygen inhalation.Results Forty-two adult New Zealand rabbits accepted the operation.Six rabbits died during the early period due to the inappropriate anesthesia drug (n =2),pneumothorax (n =1) and thrombosis (n =3),which occurred the 5th,7th and 8th postoperative day.The anticoagulation treatment was adjusted to solve the thrombosis problem.Thirty-six rabbits survived for several months with weight increase until the experiment was finished.Conclusions The experimental model of New Zealand rabbit is appropriate for the biocompatible assessment of 0.1 mm PTFE.The advantage is to avoid intubation and cardiopulmonary bypass (CPB) and to decrease the pneumothorax.
7.Arterial switch operation for Taussig-Bing anomaly

Objective To report the experience of the arterial switch operation(ASO) for Taussig-Bing anomaly and late outcomes.Methods From January 2001 to December 2015,57 patients were underwent arterial switch operation for Taussig-Bing anomaly in Fudan university affiliated children's hospital cardiac center,Median age and weight at operation was 63(37.5-88.5)days, 4.1(3.4-5.0)kg, respectively.29 patients with Arch anomalies(50.9%), 23 patients with unusual coronaries(40.3%),according to have arch anomaly or not and surgery time, dividing the patients into two groups, group A(have, n=29)and group B(not have, n=28), earlier experience into group 1(2001-2008, n=27), later experience into group 2(2009-2015, n=30), respectively.Results The Mortality was12.3%, the mortality of group A and group B was 13.8%, 10.7%(P>0.05),group 1 and group 2 was 22.2%, 3.3%(P<0.05) respectively, follow up was complete in 47 patients with a mean follow-up of(6.2±3.5) years , three patients lost, there was no late mortality, the actual survival at 1, 5year was 87%, 87%, respectively.Reintervention was required in 10 patients(21.3%), the aorta-PA valve diameter ratio was a risk factor for reintervention(group A P=0.02, group B P=0.04) ,and 1,2,5year free of reintervention was 95.6%, 86.6%, 77.2%, respectively.Conclusion The ASO approach can be applied to Taussig-Bing anomaly with acceptable mortality , and it is the procedure of choice at our institution.One stage to repair TBA with aortic arch abnormalities did not influence outcomes.The aorta-PA valve diameter ratio<0.5 was a risk factor for reintervention.
8.Combined Detection of Serum MiR-221-3p and MiR-122-5p Expression in Diagnosis and Prognosis of Gastric Cancer

PURPOSE: To investigate the clinical value of serum miR-221-3p and miR-122-5p expression levels in the diagnosis and prognosis of gastric cancer. MATERIALS AND METHODS: Serum samples from 141 gastric cancer cases (gastric cancer group), 110 gastric polyps (gastric polyp group), and 75 healthy people (healthy control) were used to detect miR-221-3p and miR-122-5p expression using real-time reverse transcription polymerase chain reaction. RESULTS: Serum miR-221-3p expression was significantly higher in the gastric cancer group than in the gastric polyp group, and it was significantly lower than that before operation. The miR-221-3p expression was significantly higher in the death group than in the survival group. The proliferation and migration ability significantly increased and the apoptosis rate significantly decreased by miR-221-3p transfection in gastric cancer cells. In contrast, the function of miR-122-5p in gastric cancer cells was opposite of miR-221-3p. Serum miR-221-3p expression was negatively correlated with that of miR-122-5p in gastric cancer. Serum miR-221-3p and miR-122-5p expressions were significantly correlated with the degree of differentiation, tumor, node, metastasis stage, lymph node metastasis, and invasion depth. miR-221-3p and miR-122-5p expression levels were independent prognostic factors for postoperative gastric cancer. In the diagnosis and predicting prognosis of gastric cancer, receiver operating characteristic analysis revealed that the area under curve of combined detection of serum miR-221-3p and miR-122-5p expression had a greater diagnostic effect than either single maker. CONCLUSIONS: The miR-221-3p and miR-122-5p are involved in the development of gastric cancer, and they have important clinical values in gastric cancer diagnosis and prognosis.

9. Comparison of risk rating for chemical hazards in a petrochemical enterprise with two risk assessment methods

OBJECTIVE: To compare and analyze the risk rating for chemical hazards of aromatics complex in a petrochemical enterprise using two risk assessment methods. METHODS: Occupational hygiene investigation and chemical detection for workplaces of the construction project were made. The risk ratings were made in five workplaces including that of the aromatics extraction,disproportionation,isomerization,xylene fractionation and adsorptive fractionation. Two risk assessment methods were employed. They were the optimal indicator system of risk assessment on occupational chemical hazards( optimal indicator system) and the semi quantitative risk assessment model of Singapore. The results of risk rating generated by the two risk assessment methods were then compared and analyzed. RESULTS: Using the optimal indicator system,the risk rating of benzene was mild in the five workplaces; and the risk rating of methylbenzene and xylene was slight. However,using the semi quantitative risk assessment model of Singapore,the risk rating of benzene was extremely high in both the aromatics extraction and disproportionation workplaces; the risk rating was high in xylene fractionation,and was medium in the isomerization and adsorptive fractionation workplaces. The risk rating of methylbenzene was high in the aromatics extraction and disproportionation workplaces,and was low in the other three workplaces. The risk rating of xylene was low in the five workplaces. CONCLUSION: The optimal indicator system is more feasible,practical and objective for the assessment of chemical hazards. It is more suitable for risk assessment of occupational chemical hazards in our country.
10.Clinical features and genetic analysis of seven patients with congenital hyperinsulinism.

OBJECTIVETo analyze clinical characteristics, genetic mutation and therapeutic effect of seven patients diagnosed with congenital hyperinsulinism(CHI).
METHODSClinical data for the patients was retrospectively analyzed.
RESULTSAll patients presented with hyperinsulinism(serum insulin:2.0-58.4 mU/L),even after hypoglycemia (blood glucose: 0.7-2.39 mmol/L) has developed. Mutations were identified in 4 patients (57.1%), which included a heterozygous c.262C to T(p.R88C) mutation in exon 4 of the UCP2 gene, a heterozygous c.1495C to A(p.G499C) mutation in exon 12 of the GLUD1 gene, a heterozygous c.1493C to T(p.S498L) mutation in exon 1 of the GLUD1 gene, and a heterozygous c.4432G to A(p.G1478R) mutation in exon 37 of the ABCC8 gene. The patient carrying a maternally inherited ABCC8 mutation was treated with cornstarch and had his blood glucose kept normal. All other patients responded well to diazoxide.
CONCLUSIONA genetic diagnosis was attained for 51.7% of patients in this study. Mild CHI patients can have their blood glucose controlled by giving cornstarch. Diazoxide is safe and effective for most CHI patients.
