1.Efficacy Analysis of Edaravone Combined with Nimodipine in the Treatment of Acute Large Area Cerebral Infarction

OBJECTIVE:To investigate therapeutic efficacy and safety of edaravone and nimodipine in the treatment of acute large area cerebral infarction. METHODS:101 patients with acute large area cerebral infarction were analyzed retrospectively and divided into control group (53 cases) and observation group (48 cases) according to drug use. After admission,control group re-ceived routine treatment as relieving cerebral edema,anticoagulant and antiplatelet agglutination. Observation group was additional-ly given Edaravone injection 30 mg added into 0.9%Sodium chloride injection 100 mL intravenously,within 30 min,twice a day+Nimodipine injection 12 mg added into 0.9%Sodium chloride injection 500 mL intravenously,once a day,at dripping speed of 0.5μg/(kg·min),for 10 days,and then given Nimodipine tablet 60 mg orally instead,3 times a day,for 5 d,on the basis of control group. Treatment course of 2 groups lasted for 15 d. Clinical efficacies of 2 groups were observed as well as SpO2,hemoglobin (Hb),total hemoglobin (HBT),reduced hemoglobin (MHb),NIHSS score and the occurrence of ADR before and after treat-ment. RESULTS:Total response rate of observation group was significantly higher than that of control group,with statistical signifi-cance(P<0.05). Before treatment,there was no statistical significance in the levels of SpO2,Hb,HBT,MHb and NIHSS score between 2 groups (P>0.05). After treatment,the levels of SpO2,HBT and MHb in 2 groups were significantly higher than be-fore,and the observation group was significantly higher than the control group;NIHSS score of 2 groups were significantly lower than before,and the observation group was significantly lower than the control group;Hb level of 2 groups were significantly low-er than before,with statistical significance(P<0.05),but there was no statistical significance in Hb level between 2 groups(P>0.05). No obvious ADR was found in 2 groups during treatment. CONCLUSIONS:On the basis of conventional therapy,edara-vone combined with nimodipine in the treatment of acute large area cerebral infarction has significant therapeutic efficacy,can im-prove neurological impairment,but doesn't increase the occurrence of ADR.
2.Effect of hyperthermic intraperitoneal perfusion on blood coagulation in patients undergoing laparoscope-assisted radical surgery for gastric cancer

Objective To evaluate the effect of hyperthermic intraperitoneal perfusion on blood coagulation in patients undergoing laparoscope-assisted radical surgery for gastric cancer.Methods Forty patients of both sexes, aged 40-60 yr, weighing 50-80 kg, of American Society of Anesthesiologists physical status Ⅰ or Ⅱ , undergoing elective radical surgery for gastric cancer, were equally randomized into control group (group C) and hyperthermic intraperitoneal perfusion group (group HIP).The patients were treated with hyperthermic intraperitoneal perfusion for 1 h after the end of the radical surgery in group R.Before induction of anesthesia (T0) , immediately before hyperthermic intraperitoneal perfusion (T1), and at 1 h of hyperthermic intraperitoneal perfusion (T2) , venous blood samples were collected, blood coagulation was measured using thromboelastography, and the reaction time, coagulation time, α angle and maximal amplitude were recorded.Results Compared with group C, no significant change was found in blood coagulation parameters at T0 and T1 (P>0.05), the reaction time and coagulation time were significantly increased, and α angle and maximal amplitude were decreased at T2 in group HIP (P<0.05).Conclusion Hyperthermic intraperitoneal perfusion can improve blood coagulation in the patients undergoing laparoscope-assisted radical surgery for gastric cancer.
3.Performance of early-warning of compartmentalized anaerobic reactor.

Early-warning of compartmentalized anaerobic reactor (CAR) was investigated in lab-scale. The performance stability of CAR at high loading rate was worse than that at common loading rate. At high loading rate, the fluctuation of effluent chemical oxygen demand (COD) concentration and volatile fatty acids (VFA) concentration was larger than that of influent COD concentration. The average relative standard deviation of effluent COD concentration and VFA concentration was 32.95% and 40.46% respectively, while that of influent COD concentration was 8.08%. The saturation of volumetric loading rate (S(VLR)) and VFA (S(VFA)) could be used to alarm the performance of anaerobic reactors. The working performance was good when the CAR was operated at normal organic loading rate (OLR), in which S(VLR) and S(VFA) were below 0.89 and 0.40 respectively. The fluctuation of performance became larger when the CAR was operated at OLR near saturation, in which S(VLR) and S(VFA) were close to 1. The performance of CAR was deteriorated when the S(VLR) and S(VFA) were more than 1.

4.Start-up of a full-scale system for short-cut nitrification and Anammox in treatment of pharmaceutical wastewater.

In order to broaden the application area of the new nitrogen removal technology, a full-scale system for short-cut nitrification and anaerobic ammonium oxidation (Anammox) was investigated in the nitrogen removal from a strong-ammonium pharmaceutical wastewater. When the influent ammonium concentration was (430.40 ± 55.43) mg/L, ammonia removal efficiency was (81.75 ± 9.10)%. The short-cut nitrification and Anammox system could successfully remove nitrogen from the pharmaceutical wastewater. The start-up of short-cut nitrification system took about 74 d and the nitrite accumulation efficiency was (52.11 ± 9.13)%, the two-step mode using synthetic wastewater and actual wastewater was suitable for the start-up of short-cut nitrification system. The start-up of Anammox system took about 145 d and the maximum volumetric nitrogen removal rate was 6.35 kg N/(m3·d), dozens of times higher than those for the conventional nitrification-denitrification process. The strategy achieving Anammox sludge by self-growth and biocatalyst addition was suitable for the start-up of Anammox system.

5.Guiding of serum procalcitonin-guided antibiotic in elderly early-onset stroke-associated pneumonia

Objective To evaluate the effects of serum procalcitonin (PCT)-guided antibiotic therapy in elderly patients with early-onset stroke-associated pneumonia (EOP).Methods Totally 179 eligible elderly patients with EOP were randomly devided into 2 groups:standard therapy group (standard group,n=88) and PCT-guided group (PCT group,n=91).Patients in standard group received antibiotics according to antibiotics guidelines in China by the treating physicians.Patients in PCT group were treated with antibiotics for 5 days,then the antibiotic treatment was based on serum PCT levels as follows:discouraged if PCT<0.25 μg/L and encouraged if PCT≥0.25 μg/L.Length of hospitalization,duration of antibiotics,costs of hospitalization and antibiotics,clinical efficacy,andmortality,National Institutes of Health Stroke Scale (NIHSS) score and Barthel index (BI) on the 28th day were observed.Results There were no significant differences in clinical efficacy,mortality,NIHSS score and BI between the two groups on the 28th day [(85.7% vs.86.3%),(8.8% vs.7.9%),10.1 (7.8,16.2) vs.9.8 (6.0,15.5),60.1(42.5,82.3) vs.57.9 (39.2,84.8),respectively,all P> 0.05].The length of hospitalization,antibiotic duration,costs of hospitalization and antibiotics were lower in PCT group than in standard group [19 (10,38) d vs.26(17,42) d,10 (7,14) dvs.15 (6,21) d,3350 (2052,6163) yuanvs.10355 (6877,15421) yuan,7532 (4810,12116) yuan vs.5358 (3089,8144) yuan,respectively,all P<0.05].Conclusions PCT guidance of antibiotic therapy is effective and safe for the treatment of early-onset stroke associated pneumonia in elderly patients.It can reduce the antibiotic duration and costs of hospitalization.
6.Clinical and anatomic analysis of children with congenital bridging bronchus malformation

Objective To analyze the clinical and anatomic characteristics of bridging bronchus (BB) malformation in children,and to explore its diagnostic strategy,treatment and risk factors.Methods A retrospective study was performed in 23 pediatric patients with BB who were admitted to Guangzhou Women and Children's Medical Center form May 2010 to October 2016.All their clinical features,imaging examination,treatment and prognosis were reviewed and analyzed.Results (1) Among the 23 patients with BB,15 were males and 8 were females.The age range of onset was 0 to 4 years old with a median age of 3.17 months.(2)The main clinical manifestations were cough (23/23 cases,100.0%),recurrent wheezing (20/23 cases,87.0%),cyanosis (8/23 cases,34.8%) and feeding diffficulty/slow body weight growth (6/23 cases,24.6%).(3) There were 17 cases of type Ⅰ and 4 cases of type Ⅱ according to Wells,and the other 2 cases were anterior BB.There were 2 cases with atypical anatomical morphology in type Ⅰ and type Ⅱ respectively.(4) Twenty-one cases were associated with bronchial stenosis in which the origin of left main bronchus complicated with BB stenosis was the most common(5 cases).Bronchoscopy revealed bronchomalacia in 5 of the 16 patients.Sixteen patients associated with cardiac or vascular anomalies,11 of whom were compound anomalies,and the most common type was sling left pulmonary artery (SLPA) (12 cases).(5) Nine of 12 patients with SLPA received surgical management,ages ranging from 1 month to 8 years old.The respiratory symptoms remitted gradually or disappeared from 4 to 17 months after surgical management.Two died and one has been lost to follow-up among the remaining 3 patients with SLPA.Conclusions BB is a rare tracheobronchial malformation that is often associated with stenosis or bronchomalacia,and cardiac or vascular anomalies.The severity of the disease mainly depends on whether there is airway stenosis or malacia and its extent.The definitive diagnosis of BB requires the combination of different imaging modalities.The combination with simple SLPA could get better curative effect.
7.Influencing factors of fetal growth restriction in patients with preeclampsia and the establishment of a Nomogram prediction model

Objective:To explore the influencing factors of fetal growth restriction (FGR) in patients with preeclampsia (PE) and construct a Nomogram prediction model.Methods:From Aug. 2021 to May. 2023, 273 PE patients admitted to our hospital were regarded as the study subjects, and grouped into a modeling group (n=191) and a validation group (n=82). Multivariate logistic regression analysis was applied to determine the influencing factors of FGR in PE patients. R4.3.1 was applied to construct a Nomogram model for predicting FGR in PE patients. Receiver operating characteristic (ROC) curve and the Hosmer Lemeshoe (H-L) goodness of fit test were applied to evaluate the discrimination and consistency of the Nomogram model in predicting FGR in PE patients.Results:There was no statistically obvious difference in gestational age, blood pressure, hemoglobin, urinary protein (UP), uric acid, umbilical artery systolic/diastolic blood pressure (S/D), D-dimer (D-D), or birth frequency between the modeling group and the validation group ( P>0.05). Compared with no concurrent FGR group, the onset of pregnancy in the concurrent FGR group was earlier, the levels of UP, S/D, and D-D, and the proportion of oligohydramnios were obviously higher, and the platelet count (PLT) was obviously lower ( t/χ 2=2.588, 1.437, 6.262, 5.464, 9.881, 3.326, P<0.05). Multivariate Logistic regression analysis showed that UP, S/D, D-D, and oligohydramnios were risk factors for FGR in PE patients ( OR=1.004, 3.807, 1.006, 4.348, P<0.05), while PLT was a protective factor ( OR=0.980, P<0.05). Nomogram model showed that when the total score of the above 5 influencing factors in PE patients was 149, the probability of concurrent FGR was 60%; when the total score was 167, the probability of concurrent FGR was 90%, and the probability of exceeding 167 was over 90%. Modeling group H-L test χ 2=6.736, P=0.565, validation group χ 2=5.812, P=0.668. The area under the ROC curve (AUC) of the modeling group and the validation group was 0.924 (95% CI: 0.883-0.965) and 0.932 (95% CI: 0.880-0.984), respectively. The sensitivity was 83.93% and 90.48%, and the specificity was 89.63% and 81.97%, respectively. Decision curve analysis (DCA) was applied to evaluate the clinical application value of the Nomogram model in predicting FGR in PE patients. Conclusion:The Nomogram model constructed based on the five indicators of UP, S/D, D-D, PLT, and oligohydramnios for predicting the risk of FGR in PE patients has high discrimination and consistency.
8.Micro-particles of bioceramics could cause cell and tissue damage.

We conducted studies to confirm the hypothesis that the cellular damage occurring around implanted biphasic bioceramics could be related to a micro-particles release because of an insufficient sintering. An in vitro cytotoxicity study was performed on four biphasic ceramic (BCP) samples. Without the treatment of extraction medium, a cytotoxicity was observed, although after centrifugation this cytotoxicity disappeared in all samples. (2) Micro-particles of HA, beta-TCP and 40%beta-TCP/60%HA mixture were used for a cell inhibition study. A decrease of cell viability was observed with the increase in particles concentration. At 10000 particles/ cell, the viability and proliferation were completely inhibited. (3) HA, beta-TCP and BCP ceramic granules were implanted in rabbit femoral cavities for 12 weeks. No degradation of HA granules was observed. The degradation was higher for beta-TCP (40%) than for BCP (5%). On the other hand, new bone formation was significantly higher for beta-TCP (21%) and HA (18%) than for BCP (12%). Much more micro-particles were formed around BCP granules than around beta-TCP, and were phagocytosed by macrophages. The release of ceramic micro-particles could be related to the sintering process. BCP ceramics have to be sintered at only 1160 degrees C. Consequently, HA microparticles of BCP ceramic are incompletely sintered and easily released after immersion or implantation. The microparticles could be at the origin of local inflammation and cell damage and could perhaps modify osteogenesis. Particular attention must be paid to this problem with regard to BCP ceramics because of the sintering difficulties of this bioceramic.

Objective To understand the potential risk for schistosomiasis transmission caused by introduction of infection source from mobile population in Shanghai. Methods Field investigation was conducted in the suburb of Shanghai City by screening the mobile population living in Shanghai for more than 1 month and over 1 years old in a procedure of interviewing, serum indirect hemagglutination (IHA) test, and then fecal examination to detect the eggs with nylon sedimentation approach for those IHA positives. Human adenoviruses (HAdVs) are the common infectious pathogens in children, which mainly infect the respiratory tract, digestive tract and eyes.Severe HAdVs may even be life-threatening.HAdVs enter the host and cause infection by binding to the host epithelial cells.At the same time, they are recognized by various immune cells and then activate the immune defense response.This study aims to review the interaction between HAdVs and the host, and the immune defense mechanism, thus improving the understanding of the immune response to HAdVs.
