1.Analgesic Effect of Propofol Combine with Dezocine after Painless Abortion

To explore the analgesic effect of propofol combine with dezocine after painless abortion and the influence on stress hormone and hemodynamics of the patients. Methods:Totally 126 cases of patients with painless abortion in our hospital from March 2013 to March 2014 were randomly divided into the observation group and the control group with 63 ones in each. The observa-tion group was anesthetized with propofol combine with dezocine, while the control group was anesthetized only by propofol. The dosage of propofol, anesthesia effect and postoperative recovery of the patients in the two groups were compared, and the level of stress hor-mones and hemodynamic parameters before and after the surgery were also compared. Results: Compared with that in the control group, the dosage of dezocine and the score of VAS after the surgery was significantly reduced in the observation group with obviously better anesthesia effect, and the differences had statistical significance (P<0. 05). The level of Cor, NE and ACTH was significantly improved, and the level of A-I and A-II was reduced significantly in the observation group, and compared with that in the control group, the differences showed statistical significance(P<0. 05). Compared with those that in the control group, the levels of SBP, DBP and SpO2 were improved significantly and the level of HR was reduced significantly in the observation group during and after the surgery, and the differences showed statistical significance (P<0. 05). The incidence of adverse drug reactions in the observation group was significantly lower than that in the control group, and the difference had statistical significance (P<0. 05). Conclusion:Propofol combine with dezocine for the surgery of painless abortion can reduce the dosage of dezocine, improve the anesthesia effect and reduce the harem pain with lower stress hormone fluctuation, hemodynamic effect and incidence of adverse reactions.
2.Mycoplasma infection and drug sensitivity analysis in 3026 patients with urogenital tract infections

Objecfive To investigate the prevalence of Ureaplasma urealyticum(UU)and Mycoplasma hominis(MH)infections in urogenital tract and their sensitivities to drugs in Qinhuangdao area from 2006 to 2007.Methods UU and MH were detected by Mycoplasma IST kits in urine samples from 3062 patients suspected as urogenital tract infection and the sensitivity tests for 8 antimicrobial agents were performed.Results Mycolasmas were detected in 1262 patients from 3062 patients suspected as urogenital tract infection,in which 1037 were of UU infection,41 were of MH infection and 184 were of UU+MH infection.Both strains were sensitive to josamycin,doxycycline and tetracycline,but were less sensitive to ofloxacin and ciprofloxacin.Conclusions Drug resistance is increasing for Mycoplasma and sensitivity test is important in treatment of urogenital tract infection by Mycoplasma.
3.Associaiton of irisin and vaspin with clinical presentations of metabolic syndrome in patients with type 2 diabetes mellitus

Objective To explore the changes in serum concentrations of irisin,vaspin and reactive oxygen species (ROS) in patients with type 2 diabetes mellitus (T2DM) and metabolic syndrome (MS),and to investigated the correlation of irisin and vaspin with clinical parameters of MS.Methods A total of 260 T2DM patients were enrolled.Age and gender were recorded,anthropometrics,biochemical parameters,and levels of irisin,vaspin and ROS in fasting serum were measured,and homeostatic model assessment of insulin resistance (HOMA-IR) calculated.Wilcoxon rank sum test,correlation analysis,Logistic regression analysis,multiple linear regression analysis and receiver operating characteristic (ROC) curve analysis were performed.Results Compared to T2DM patients without MS,T2DM patients with MS had lower serum level ofirisin [male:112.81 (86.96-191.84) μg/Lvs.156.23 (110.61-225.97) μg/L,female:141.09 (77.52-175.55) μg/L vs.172.15 (95.69-240.37) μg/L,P <0.01],higher levels of vaspin and ROS [male:1.13 (0.95-1.38) μg/Lvs.0.36 (0.21-0.82) μg/L,1 540 (1 250-1 860) kU/Lvs.1 020 (920-1 350) kU/L;female:1.52 (1.13-1.80) μg/Lvs.0.51 (0.47-1.08) μg/L,1 650 (1 320-1 940) kU/Lvs.1 120 (980-1 420) kU/L,P <0.01].In the T2DM patients,serum irisin level was negatively correlated with vaspin (r =-0.382,P < 0.01) and ROS (r =-0.410,P < 0.01),while vaspin was positively correlated with ROS (r =0.400,P < 0.01).Multiple linear regression analyses showed that irisin was significantly correlated with body mass index (BMI),waist circumference and triglyceride,while vaspin was correlated with gender,BMI,and waist circumference (all P < 0.05).Logistic regression analysis revealed that irisin,vaspin and ROS were all associated with MS (OR =0.77,95 % CI 0.608-0.978;OR=1.39,95% CI 1.252-1.539;OR=1.38,95% CI1.112-1.718,all P<0.05).ROC analysis demonstrated that irisin and vaspin had significant area under the curve (AUC =0.931,P <0.01;AUC =0.777,P < 0.01) for the prediction of MS.Conclusions Serum irisin level was significantly decreased,while vaspin and ROS were significantly increased in T2DM patients with MS.Irisin and vaspin were associated with clinical presentations of MS,suggesting that irisin and vaspin might be valuable predictors of MS.
4.Lymphoscintigraphy for lateral lymph node drainage in low rectal cancer

ObjectiveTo assess the diagnostic value of preoperative lymphoscintigraphy for lateral lymph node metastasis in low rectal cancer. MethodsFrom May 1999 to March 2001, pelvic and lower abdominal lymphoscintigraphy with 99mTc sulfur colloid was performed on 32 patients with proven colorectal cancer one day prior to operation. Among these patients were 27 rectal cancers, 3 sigmoid cancers and 2 colon cancers. Fifteen cases of rectal cancer with the tumor located at or below the peritoneal reflection received extended lymphadenectomy with lateral dissection (D3 lymphadenectomy). The result of lymphoscintigrams was correlated with histologic lateral node examination. Results The image ratio of pararectal nodes, obturator nodes, and internal iliac aorti lymph trains was 69%, 91%, and 100% respectively. In 15 patients receiving D3 lymphadenectomy, 10 had negative lymphoscintigrams whereas 5 had positive image, and the lateral pathology positive rate was 13%(2/15). Analysis of these results revealed the sensitivity (100%), specificity (77%) and accuracy (80%) of this diagnostic method. ConclusionPelvic and lower abdominal lymphoscintigraphy with 99mTc sulfur colloid could demonstrate the lateral lymph node drainage of low rectal cancer, and the correlation with postoperative histologic lateral node examination was high enough to enable surgeons to adopt an individualized surgical approach.
5.Potential predicting function of betatrophin in patients with polycystic ovary syndrome and it's relationship with 25-hydroxy vitamin D3

Objective Through the detection of the levels of serum betatrophin,anthropometric and biochemical indices,to determine the alteration of betatrophin levels in patients with polycystic ovary syndrome (PCOS) and the relationship between betatrophin levels and metabolic indexes such as 25-hydroxy vitamin D3 [25 (OH) D3].Methods The study group was composed of 24 women with PCOS and 20 age-natched healthy women as controls.Following general physical examination of the subjects,anthropometrie measurements were performed (height,weight,waist circumference,hip circumference).Body mass index (BMI) and waist-hip ratio were calculated;25 (OH) D3,Ca2+,fasting blood glucose,fasting insulin,triglycerides,high-density lipoprotein cholesterol (HDL-C),low-density lipoprotein cholesterol (LDL-C) were also measured and homeostasis model assessment of insulin resistance (HOMA-IR) were evaluated in all subjects.Serum betatrophin levels were examined with an enzyme-linked immunosorbent assay (ELISA).Using independent sample T test to compare the differences between groups,the linear correlation analysis was performed to study the correlation of betatrophin with anthropometric and biochemical indices,the influencing factors of betatrophin were analyzed by multiple linear regression.Results Circulating betatrophin levels [(0.341±0.034) ng/ml vs.(0.810±0.162) ng/ml,t=4.271,P<0.001] and HDL-C [(1.014±0.321) mmol/L vs.(1.419±0.287) mmol/L,t=2.218,P<0.05] significantly decreased in women with PCOS compared with controls,and BMI [(27.691±4.392) kg/m2 vs.(23.310±4.781) kg/m2,t=-2.073,P<0.05],fasting blood glucose [(5.950±0.411) mmol/L vs.(4.883±0.314) mmol/L,t=-2.142,P< 0.05],HOMA-IR [(4.946 ± 0.741) vs.(3.387± 0.397),t=-2.493,P<0.05],triglycerides [(1.510± 0.848) mmol/L vs.(1.037±0.402) mmol/L,t=-2.223,P<0.05],LDL-C [(3.431±0.479) mmol/L vs.(2.396±0.435) mmol/L,t=-2.433,P<0.05] were obviously increased.Moreover,there was a postive correlation between betatrophin and HOMA-IR (r=0.425,P<0.05) as well as 25(OH)D3 (r=0.577,P<0.05) and Ca2+ (r=0.448,P<0.05),while the betatrophin was negatively related to BMI (r=-0.451,P<0.05),triglycerides (r=-0.454,P<0.05),LDL-C (r=-0.551,P<0.05).Circulating betatrophin levels were higher when 25 (OH) D3 levels were beyond 35 nmol/L in PCOS patients [(0.539±0.092) ng/ml vs.(0.199±0.031) ng/ml,t=3.072,P<0.001],and the multiple linear regression analysis showed that the main factors affecting the levels of betatrophin were BMI (OR=-0.260),HOMA-IR (OR=0.218),25(OH)D3 (OR=0.238),and glycerol (OR=-0.162).Conclusion It is speculated that betatrophin may be a valuable predictive factor of PCOS because betatrophin is closely associated with insulin resistance,lipid metabolism disorders and the lack of 25-hydroxy vitamin D3 in PCOS patients.
6.Expression of CUL7 and CCDC8 proteins in normal mouse tibial growth plate

To investigate the changes of expression location and amount of CUL7 and CCDC8 proteins in the growth plate of normal mice aged 0-4 weeks, and to clarify the roles of CUL7 and CCDC8 proteins in the proliferation and differentiation of tibia growth plate of mice. The expression location and levels of CUL7 and CCDC8 proteins in the tibial growth plate of normal mice aged 0-4 weeks were observed with immunohistochemical staining. CUL7 protein was expressed in the cell cytoplasm and membrane in three zones of tibial growth plate three bands at 0-4 weeks. The expression level and total expression level of CUL7 protein in each zone of growth plate decreased gradually with the increase of week of age( F=369.61, P=0.001). The expression of CUL7 protein decreased most significantly in the proliferative zone, followed by the stationary zone and the hypertrophic zone. CCDC8 protein was mainly expressed in proliferation zone and hypertrophic zone cell membrane and nuclear membrane of growth plate at 0-2 weeks, and mainly expressed in hypertrophic zone cell membrane and nuclear membrane at 3-4 weeks. The expression of CCDC8 protein in the proliferating zone changed inversely with week of age, and the expression of CCDC8 protein in the hypertrophic zone increased over 4 weeks( F=453.67, P<0.001). The total expression of CCDC8 protein in growth plates decreased with the increase of week of age.The expression levels of CUL7 and CCDC8 decreased with the increase of week of age, suggesting that CUL7 and CCDC8 may promote the proliferation and differentiation of growth plate chondrocytes.

Methods We searched the literatures about the relation between PNI and the prognosis of patients treated with radical cystectomy published from the inception to January 30, 2021 in PubMed, Embase, Web of Science, CNKI, Wanfang, VIP and Chinese Medical Journal Database, and used RevMan5.3 software for Meta analysis. 8.Lymphoscintigraphy in sentinel lymph node biopsy of breast cancer.

OBJECTIVETo evaluate the feasibility of lymphoscintigraphy in sentinel lymph node biopsy of breast cancer.
METHODLymphoscintigraphy was performed after peritumoral or subdermal injection of radioactive colloid. Then, sentinel lymph node (SLN) biopsy guided by gamma detector probe was performed. Factors correlated with identification-detection rate were assessed.
RESULTSLymphatic drainage was present in preoperative lymphoscintigraphy in 88(93%) out of 95 patients, with 39 (44.3%) positive for lymphatic drainage other than in the axilla. A total of 91 (95.8%) patients had their SLN identified in the intraoperative procedure. The quality of lymphoscintigraphic image was closely related to SLN identification-detection rate in the intraoperative procedure (P = 0.025).
CONCLUSIONSentinel lymph node outside the axilla can be detected by lymphoscintigraphy. The combination of lymphoscintigraphy and gamma detector probe for sentinel lymph node biopsy of breast cancer not only is acceptable but promising.
