1.The significance of urodynamic analysis in evaluating neurogenic vesicoureteral reflux in patients with thoracolumbar spinal cord injury

Objective To investigate the utility of urodynamic testing in the evaluation of neurogenic vesicoureteral reflux in patients with thoracolumbar spinal cord injury at the T10 to L2 level.Methods Twenty-six patients with thoracolumbar spinal cord injury at the T10 to L2 level and who displayed vesicoureteral reflux were enrolled and divided into a detrusor hyper-reflexia group (n =21) and a detrusor a-reflexia group (n =5)according to their cystourethrograms and urodynamic test results.Their bladder volume,detrusor pressure (Pdet) and compliance were observed and analyzed at the vesicoureteral reflux point and the leak point.Results The bladder volume and compliance of the detrusor hyper-reflexia group at the vesicoureteral reflux point were (122.46 ± 87.89) ml and (5.94 ± 4.96) ml/H2 O respectively,significantly different from the detrusor a-reflexia group (P < 0.01).At the leak point,the average bladder volume of the detrusor hyper-reflexia group was (210.81 ± 69.72)ml,while the average bladder compliance was (9.53 ± 5.43)ml/H2O and the average detrusor pressure was (42.29 ± 9.57)cm H2O.All were significantly different from the detrusor a-reflexia group's averages.Moreover,there was a significant difference in the Pdet between the leak point and the vesicoureteral reflux points in the detrusor hyper-reflexia group.Conclusions Patients with thoracolumbar spinal cord injuries at the T10 to L2 level with neurogenic bladder and detrusor hyper-reflexia have low bladder volumes and compliance,and vesicoureteral reflux tends to occur at an early stage after injury with a destrusor pressure lower than 40 cm H2 O.For patients with detrusor a-reflexia,vesicoureteral reflux and bladder leakage may occur when detrusor pressure is low.
3.Investigation on Toxoplasma gondii infection and awareness of toxoplasmosis-related knowledge in women with poor pregnant outcomes in Wuxi City

Objective To investigate the prevalence of Toxoplasma gondii infection and awareness of toxoplasmosis?related knowledge among women with poor pregnant outcomes in Wuxi City. Methods A total of 217 women with poor pregnant out?comes from Wuxi City during the period of January 2011 to December 2015 were randomly selected as the study subjects( a study group),while 250 women with normal pregnancy were served as controls(a control group). The sero?prevalence of T. gon?dii infection was detected by using ELISA and compared between the study and control groups. The awareness of toxoplasmosis?related knowledge was investigated by using a self?designed questionnaire and compared between the study and control groups. Results The positive rate of anti?Toxoplasma antibody was 30.88%in the study group,which was significantly higher than that (8.8%)in the control group(χ2 =36.7,P<0.01). The positive rates of anti?Toxoplasma IgG and IgM antibodies were 20.74%and 10.14% in the study group respectively,which were significantly higher than those(6% and 2.8%)in the control group (χ2=22.53 and 10.74,both P values<0.01). In addition,the positive rates of anti?Toxoplasma,anti?Toxoplasma IgG and IgM antibodies were significantly higher in the women with missed miscarriage,natural abortion,stillbirth and birth defect than those in the women with normal pregnancy(all P values<0.05). The awareness rates of“Do you hear about Toxoplasma or toxo?plasmosis?”(P<0.01),“Do you know that breeding pet cats or dogs may cause Toxoplasma infection?”(P<0.05)and“Do you know that pregnancy women require the detection of Toxoplasma infection?”(P<0.01)were significantly lower in the study group than those in the control group,while no significant differences were seen in the awareness rates of“Do you know that eat?ing hot pot may cause Toxoplasma infection?”,“Do you know that the use of chopping block in regardless of cooked and un?cooked food may cause Toxoplasma infection?”,“Do you know that Toxoplasma infection may transfer from mother to fetus?”,“Do you know that Toxoplasma infection may cause adverse pregnant outcomes like abortion,stillbirth or fetal abnormalities ?”, and“Do you know that the women infected with T. gondii during pregnancy require treatment?”between the two groups(all P values>0.05). Conclusions This study demonstrates higher prevalence of T. gondii infection in women with poor pregnant out?comes than in those with normal pregnancy in Wuxi City. Considering the harm of T. gondii infection during pregnancy and the low awareness of toxoplasmosis?related knowledge in pregnant women,the health education of toxoplasmosis?related knowledge should be strengthened,especially for pregnant women,so as to reduce the prevalence of T. gondii infection among pregnant women to improve the better child?bearing and rearing level.
4.Management of Medical Refuse Is Important to Control Hospital Infection

OBJECTIVE To strengthen the management of medical refuse in order to control the onset of hospital(infection.) METHODS It has been done including setting up special tissue,formulating supervising system,strengthening training,carrying out supervising measures and strengthening examination and inspection.(RESULTS) It could prevent hospital infection effectively to regulate the whole course of the medical refuses(collection,) storage,dealing with and registration.CONCLUSIONS It is very important to strengthen the management of medical refuse in order to prevent hospital infection.
5.Effects of Psychological Intervention and Sedative on Stress Response to Gastroscopy

Objective:To study effects of psychological intervention and sedativn on stress response to gastroscopy.Methods:86 patients for gastroscopy were randomly divided into three groups:29 patients in group of psychological intervention watched video materials about psychological intervention before gastroscopy;29 patients in sedative group were given intravenous midazolam and propofol.; 28 patients in control group were no psychological intervention or sedation. Patient's discomfort and changes of blood pressure(BP),heart rate(HR) , saturation of pulse oxygen(SpO 2) were recorded. The level of norepinephrine(NE), adnephrin(E), dopamine(DA) and cortisol(CS)in serum were detected.Results:The incidence of cough,nausea and vomiting, and restlessness in group of psychological intervention and in sedative group was lower as compared with the control group. BP,HR,NE,E and CS in control group rose significantly during the procedure while that of patients in group of psychological intervention and in sedative group was no significant change.Conclusion:The psychological intervention or sedation with midazolam and propofol can reduce stress response to gastroscopy.
6.Clinical Curative Effect Observation of Bumetanide on Elderly Type 2 Diabetic Patients with Middle and Advanced Stages Nephropathy

Objective The purpose of this study was to observe the effects of bumetanide on elderly type 2 diabetic patients with middle and advanced stages nephropathy. Methods Forty cases with diabetic nephropathy (DN) were divided into two groups:control group (20 cases) and treatment group (20 cases) . The control group accepted furosemide (20 mg, once a day) and the bumetanide was orally administrated at the dose of 1.0 mg, twice a day to the treatment group for 3 months. The assessment of transferrin (TRF) and urine microalbumin (UALB) was performed at the time points at the end of 1, 4, 8, 12 weeks after treatment. Results (1) The levels of BUN, SCr and UA after treatment were lower than before treatment, but there were no significant differences. (2) In the treatment group, the levels of TRF and UALB have decreased after treatment for 1 week, with no statistically significant. But at the end of 4, 8 and 12 weeks,the differences were statistically significant ( <0.05) . Especially, during the three months follow-up,the levels of TRF and UALB at 12 weeks have decreased obviously. Conclusion The therapeutic effect of bumetanide on DN might be better than that of furosemide.
7.Effect of Protein Kinase C Inhibitor Enzastaurin on Growth of Gefitinib-resistant Human Non-small Cell Lung Cancer Cell Lines

Objective: To observe the effect of a novel targeted agent enzastaurin alone or in combination with gefitinib on ge-fitinib-resistant human non-small cell lung cancer cells to explore the rational drug combination. Methods: CCK-8 assay was used to measure the cell proliferation. Combination index ( CI) was calculated by Chou-Talalay method to assess the efficacy of the combination therapy. The flow cytometry (FCM) was used to analyze the change in the cell cycle. Results:In 72h, the IC50 value of gefitinib and enzastaurin for the lung cancer NCI-H460 cells was 6. 99μmol·L-1 (95%CI:3. 55-13. 79μmol·L-1 ) and 7. 25μmol·L-1 (95%CI:4. 77-1. 02 μmol·L-1), respectively. The inhibition effect on the cell proliferation was stronger in the combination treatment than that in the monotherapy (P<0. 05), and the simultaneous treatment showed the most significant inhibition effect (P<0. 01). The IC50 value of gefitinib for H460 cells in the simultaneous administration group, the sequential administration group with gefitinib used first and the sequential administration group with enzastaurin used first was 0.006 μmol·L-1(95%CI:0.002-0.020μmol·L-1), 0.02μmol·L-1(95%CI:0.011-0.037 μmol·L-1) and 0.085 μmol·L-1(95% CI:0.042-0.170μmol·L-1, respectively. The CI of the simultaneous administration group was lower than one when the gefitinib concentration was above 0. 05μmol·L-1 . The cell cycle distribution result indicated that the simultaneous administration group had significantly increased G0/G1 proportion (P<0. 05) and induced cell cycle arrest at G1 phase. Conclusion:Protein kinase C inhibitor enzastaurin combined with EGFR inhibitor gefitinib shows a synergistic effect, suggesting that the combination treatment of the two drugs might be a new strategy for the follow-up therapy of gefitinib-resistant non-small cell lung cancer.

Objective To investigate the effects of Tong Xin-Luo on cultured human umbilical vein endothelial cells (HUVECs) impaired by Lysophosphatidylcholine.Methods The herbage-contained serum of TXL was prepared,HUVECs were cultured in vitro.The study was designated to 4 group:normal control,LPC group,TXL group,and TXL + LPC intervened group.The cell function was determined by cell morphology and MTT colorimetric assay.Results Compaired with normal control group (0.380 ±0.023 ),LPC ( 0.320 ± 0.024 ) could significantly decrease the cells activity,promote cells death ( P <0.05 ).After TXL intervened(0.424 ±0.034),cells activity was significantly increased,cells death was d significantly decreased( P <0.05 ).Conclusions Tong Xin-Luo could protect human umbilical vein endothelial cells function by against the LPC-induced damage.
9.Clinical Observation of Fuyankang Combined with Azithromycin in the Treatment of Pelvic Inflammatory Disease

OBJECTIVE:To observe the clinical efficacy and safety of Fuyankang combined with azithromycin in the treat-ment of pelvic inflammatory disease. METHODS:90 patients with pelvic inflammatory disease were selected and randomly divided into observation group and control group,with 45 cases in each group. Control group was given Lactose acid azithromycin for injec-tion 500 mg,ivgtt,qd,and then Azithromycin capsule 250 mg,po,qd;observation group was additionally given Fuyankang Tab-let 1.56 g,tid,on the basis of control group. A treatment course of 2 groups lasted for 7 d,and both received 3 courses of treat-ment. Clinical efficacies of 2 groups were observed as well as the level of CRP,IL-2 and life quality score before and after treat-ment,and the occurrence of ADR was compared between 2 groups. RESULTS:Total effective rate of observation group was 97.78%,which was significantly higher than that of control group (86.67%),with statistical significance (P<0.05). The level of CRP,IL-2 and life quality score of 2 groups were improved significantly after treatment,and the observation group was more better than the control group,with statistical significance (P<0.05). No obvious ADR was found in 2 groups during treatment. CONCLUSIONS:Fuyankang combined with Azithromycin is effective for pelvic inflammatory disease,and can improve the quali-ty of life,reduce inflammation cytokines infiltration and the generation of inflammatory medium with good safety.
