1.Assessment of auditory function in patient with posterior circulation transient ischemic attack.

OBJECTIVE:
To study the hearing characteristics in patient with posterior circulation transient ischemic attack (PCTIA).
METHOD:
Sixty-two patients diagnosed by neurologist were selected as experimental group for testing of pure tone audiometry, tympanometry, otoacoustic emissions (OAE) and auditory brainstem response (ABR). Thirty healthy subjects were chosen as control group. The data were compared between the PCTIA groups and the control group using the SPSS 14.0 software.
RESULT:
(1) The difference in pure tone threshold across 125 to 8000 Hz between the PCTIA group and the control group was not statistically significant (P>0.05); (2) In PCTIA patients, the prevalence of TEOAE was only 42.7%, significantly different from the control group; (3) The patients had longer ABR latency and wave interval than the normal controls (P<0.01).
CONCLUSION
Brain-stem and cochlear damage can occur in patients with PCTIA. These results suggest that OAE and ABR be used as important diagnostic test for patients with PCTIA, and might be helpful in localizing abnormality in PCTIA.

2.Influencial Factors of Activities of Daily Living for Stroke Patients in Community-based Rehabilitation

ObjectiveTo explore the related factors influencing effect of community-based rehabilitation (CBR) on activities of daily living (ADL) in stroke patients in.Methods202 stroke patients were randomly divided into the CBR group (n=103) and control group (n=99). The patients of the CBR group treated with rehabilitation and following-up including risk factors controlled with drug, rehabilitation, health education and mental treatment, those of the control group only treated by following-up. Before and five months after treatment, patients of two groups were assessed with Barthel Index (BI), clinical never function limitation score, cognitive item of Functional Comprehensive Assessment (FCA). Multielement regression analysis was applied, the ADL score of last assessment was taken as dependent variable, group information, location, smoking, sex, age, course of diseases, education, drinking, sleeping, the first scores of FCA and BI were taken as independent variable.ResultsThe improvement of ADL of stroke patients was related with group information, drinking, course of diseases, the first scores of FCA, BI, and etc.ConclusionEarly CBR can significantly improve the ADL of the stroke patients. Cognitive deficit also has influence on ADL.
3.Allergic fungal rhinosinusitis: one case and literature review.

OBJECTIVE:
To investigate the presentation and the management of allergic fungal rhinosinusitis (AFRS).
METHOD:
One case of AFRS was reported and the relevant literatures were reviewed. CT scan showed high-attenuation like "cloud and mist", Charcot-Leyden crystals and fungal hyphae were found in nasal smear.
RESULT:
The patient removed after a combined therapy of endoscopic sinus surgery, steroids and local antifungal drugs.
CONCLUSION
The diagnosis of AFRS depends on history, the characteristics of CT scanning, pathology, mycologic and immunologic test. The management of surgery, systemic immunotherapy, local antifungal drugs and longer follow up is very important.

4.Study on the practicality of the semi‐automatic measurement for the urogenital hiatus

Objective To determine the consistency of urogenital hiatus ( U H ) data between the semi‐automatic measurement and manual measurement using transperineal pelvic floor ultrasonography . Methods Total of 286 three‐dimensional images of minimal U H dimension were obtained . And they were divided into study group ( 100 images) and test group ( 186 images) randomly . T hree experts traced and created the w hole profile of the U H of those images in the study group by M AT LAB . T hen the semi‐automatic software was obtained through machine learning algorithms . In the test group , 6 parameters of U H ( including anterioposterior diameter , transverse diameter ,circumference , area ,left and right levator urethral gap distance) were measured by two experts ( D 1 and D2 ) both manually and semi‐automatically . T he time experts spent on measuring was also recorded and compared . Results T he time used for semi‐automatic measurement was significantly shorter than that for manual measurement [ ( 7 .49 ± 1 .51 ) s vs ( 42 .42 ± 11 .08) s ,( 7 .52 ± 1 .37) s vs ( 43 .45 ± 9 .09) s for D1 and D2 , t = -12 .09 ,-13 .64 ,all P=0 .00] . T he Pearson correlation coefficients between semi‐automatic and manual measurements of 6 parameters were 0 .857 -0 .985 ( P < 0 .01) ,0 .853 -0 .979 ( P < 0 .01 ) in D1 and D2 ,respectively . T he interclass correlation coefficients ( ICC) of six parameters were ranged from 0 .846 -0 .985 for D1 and 0 .843~0 .979 for D2 ( all P < 0 .01 ) . T he Bland Altman plot also showed good agreement between two methods . Conclusions Intellectual recognition and semi‐automatic measurement has simplified the process for U H measurement ,and it is proved to be a reliable and timesaving method that is practical for clinical use .
5.A feasibility study of the automatic cystocele severity grading software for quantitative evaluation of prolapse of bladder posterior wall by transperineal ultrasound

Objective To investigate the feasibility of the automatic cystocele severity grading software for quantitative evaluation of prolapse of bladder posterior wall by transperineal ultrasound . Methods One hundred and seventy transperineal ultrasound video clips were recorded when the female patients performing the Valsalva maneuver and those clips were divided into training group ( 85 cases) and test group ( 85 cases) randomly ,then the ralated structures of the images from the training group offline were marked . Through machine learning algorithm ,the computer had learned and was able to analyzed the marking information ,then the automatic cystocele severity grading software was obtained . And later the software was ran to mark the structures and get the cystocele severity grading in the images from the test group . Meanwhile , the same structures of the same images manually were marked and after an interval of more than two weeks the process were repeated by 3 doctors . Finally the grading results obtained from the software and the measurers of the 3 doctors were compared . Results The intelligent identification and automatic measurement software obtained from the machine learning algorithm was able to identify the related structures . The grading results of each measurer were of good consistency ( κ :0 .72 -0 .78 ;ICC :0 .980-0 .990) . The grading results between different measurers were of good consistency ( κ :0 .65-0 .75 ;ICC :0 .985-0 .992) . The grading results between automatic software and three different measurers were of good consistency ( κ :0 .63-0 .67 ;ICC :0 .967-0 .969 ; r =0 .936 ,0 .943 ,0 .936 ,all P <0 .01) . Conclusions The automatic cystocele severity grading software is able to identify the related structures in the images and reliable to apply the software in pelvic floor ultrasound .
6.Study on the status quo of KAP towards vision care among parents of preschool children and its ;influencing factors

Objective To investigate the status and the influencing factors of knowledge, attitude and practice (KAP) towards vision care among parents of preschool children, and provide scientific references for vision care intervention.Methods Totally 821 parents of preschool children aged 5 to 6 from 7 kindergartens in Harbin were selected as the research object by convenience sample method, and they were investigated by self-designed KAP questionnaire towards vision care. Results The total score of parents′ KPA towards vision care was (113.18±9.07) points;the score of knowledge dimension was (12.07±2.03) points; the score of attitude dimension was (47.21±4.24) points; the score of practice dimension was (53.90±6.12) points. The single factor analysis showed that, educational level and family monthly income were influencing factors of vision care knowledge (t=14.366,11.349;P<0.05); family monthly income was the influencing factor of vision care attitude ( t=6.586,P<0.05);educational level, family monthly income and visual impairment of children were influencing factors of vision care practice (t=5.903,8.705,8.264;P<0.05).Conclusions The KAP level towards vision care among parents of preschool children are uneven, and the vision care practice is not ideal. Nursing staff in communities should take targeted measures in vision care education according to different people in different groups, to improve the level of KAP towards vision care.
7.Research progress and countermeasures on low vision of community preschool children

It introduced diagnostic criteria and epidemic situation of low vision, and analyzed the influencing factors including genetic factors, birth history, eye habit, sleep, outdoor activity, diet and lighting. And it put forward intervention strategies for primary prevention of low vision of preschool children, and called for relevant departments and professionals in the future research and health work to strengthen the concern on visual health of preschool children.
8.Effect of rural family rehabilitation nursing on psychological status of patients with cerebral stroke

Objective To explore the effects of rural family nursing intervention on psychological status of patients with cerebral apoplexy hemiplegia.Methods This community area was randomly divided into the intervention group and the control group, and 42 stroke patients with hemiplegia were randomly selected in each group. Patients in the intervention group were given family rehabilitation nursing intervention for 12 months by community nurses who received training and evaluation, while patients in the control group received original care mode, regular blood pressure measure, and routine follow up. Patients' psychological status before and after intervention were investigated.Results After 12 months' family rehabilitation nursing intervention, the psychological status of patients in the intervention group was significantly improved, and the differences between two groups were statistically significant (P<0.01).Conclusions Rural family rehabilitation nursing intervention can significantly improve the psychological status of patients with stroke, promote patients' recovery, and improve the quality of life of patients.
9.Correlation between lipoprotein-associated phospholipase A2 and albuminuria in patients with type 2 diabetes mellitus

Objective:
To explore the correlation between lipoprotein-associated phospholipase A2 (Lp-PLA2) level and albuminuria in patients with type 2 diabetes mellitus (T2DM) .
Methods :
200 T2DM patients were chosen to collect general data and relevant laboratory indicators. According to the urinary albumin / creatinine ratio ( UACR) ,they were divided into normal group (UACR<30 mg / g,n = 66) ,microalbuminuria group (30 mg / g≤ UACR<300 mg / g,n = 64) and macroalbuminuria group (UACR≥300 mg / g,n = 70) .On the basis of Lp-PLA2 tertile,they were divided into low Lp-PLA2 group (Lp-PLA2 <104 ng / ml,n = 66) ,medium Lp-PLA2 group ( 104 ng / ml≤Lp-PLA2 <161 ng / ml,n = 67) and high Lp-PLA2 group (Lp-PLA2 ≥161 ng / ml,n = 67) .Group differences were compared by analysis of variance and nonparametric test.Associations between Lp-PLA2 and other indicators were performed by Pearson correlation and Spearman correlation. Related factors of albuminuria in T2DM patients were explored by multivariate Logistic regression analysis. In addition ,receiver operating characteristic (ROC) curve analysis was applied to evaluate the predictive value of Lp-PLA2 for albuminuria in T2DM patients.
Results:
Lp-PLA2 was significantly higher in the macroalbuminuria group than that in both the normal group and the microalbuminuria group,and the differences were statistically significant (P<0. 05) .Compared with the normal group,Lp-PLA2 in the microalbuminuria group increased(P<0. 05) .With the increase in Lp-PLA2 tertile, there was gradual increase in UACR , and the difference was statistically significant (P<0. 05) .Correlation analysis showed that Lp-PLA2 was positively correlated with duration of DM,systolic blood pressure (SBP) ,glycosylated hemoglobin (HbA1c) ,fasting blood glucose (FBG) ,blood urea nitrogen (BUN) ,serum creatinine (Scr) ,UACR and negatively correlated with estimated glomerular filtration rate ( eGFR) (P <0. 05 ) . Multivariate Logistic regression analysis showed that Lp-PLA2 [OR = 1. 046,95% CI( 1. 031,1. 060) ]was an independent risk factor for albuminuria (P<0. 05) .The AUC of Lp-PLA2 for predicting albuminuria was 0. 902 [95% CI(0. 862,0. 942) ]. The cut-off value of Lp-PLA2was 148 ng / ml,the sensitivity was 65. 7% and specificity was 98. 5%.
Conclusion
Lp-PLA2 is closely related to the albuminuria in T2DM patients,which provides a new method for the diagnosis and treatment of diabetic kidney disease (DKD) .
