Objective To select optimum water extraction process of SuXingErChen Syrup by orthogonal test and multivariate nonlinear regression analysis.Methods Naringin was extracted by water, Used the naringin content and extraction rate of extractum as indexes.Based on the orthogonal test and multivariate nonlinear regression analysis to make sure the best way for extracting naringin from SuXingErChen Syrup.Results The optimal conditions of water extraction process was that the herbs were added seven times the amount of water without infiltration,extracted two times with 80 min each time,which was selected by multivariate nonlinear regression analysis.Conclusion The process of water extraction for the preferred Su XingErChen Syrup is exact, high effective and suitable for production needs.

Objective To compare the therapeutic effect of auricular acupuncture with that of hormone substitution on perimenopause syndrome patients to study the mechanism of auricular therapy.Methods Totally 68 female cases were randomized into the auricular acupuncture group and hormone group,34 cases in each.The auricular acupuncture group was given electro-acupuncture on Neifenmi(CD18),Ovarium,and Uterus;and the hormone group was given Tibolone.Before and after treatment,record Kupperman score and determine serum estradiol(E2),follicle-stimulating hormone(FSH)and luteotropic hormone(LH)respectively.Results After treatment,the Kupperman scores of both groups were decreased,the level of E2 increased obviously,FSH and LH decreased,the difference was significant(P0.05).Conclusion The auricular therapy can effectively relieve the perimenopause syndrome and improve the reproductive endocrine function of perimenopause females.

Objective To summarize the experience of surgical treatment and to explore the oper-ation method choice of ascending aortic aneurysm caused by Marfan Syndrome. Methods The clinical da-ta of 16 patients from 2005 January to 2011 November were retrospectively analyzed. Results Among all, there was no operative mortality,but 6 cases of early postoperative complications(37. 5%),including 2 ca-ses of arrhythmia,2 cases of pulmonary infection,1 case of renal insufficiency and 1 case of pneumothorax. The follow-up time was 12~63(25 ± 9)months. During follow-up,2 patients died(12. 5%),1 due to rup-tured abdominal aortic aneurysms and 1 due to renal failure. Besides there were 2 cases of anticoagulation complications and 1 case of coronary heart disease. Conclusion Although sparing aortic root replacement of aortic valve has similar curative effect as Bentall operation and avoids the mechanical valve-related com-plications,the indications are relatively strict. Bentall operation is still the first choice for severe patients.

Objective To evaluate the pain control, general condition and well-being of the patients with advanced cancer treated with transdermal fentanyl as well as the effects of transdermal fentanyl on heart rate variability (HRV) and endocrine function. Methods This open prospective study included 40 patients with advanced cancer. The direct conversion ratio from oral morphine to transdermal fentanyl system (TFS) was 100:1 daily. In patients who had never received any opioid before the initial dose of TFS was 25?g/h. The transdermal system was changed every 72h and the dosage was adjusted to the need of the patients according to the VAS scores and the requirement of morphine. 8 hospitalized patients were monitored by 24h Holter for changes in HRV. Blood samples were taken before transdermal fentanyl patch when pain was severe and 24h after TFS for determination of ACTH,?-endorphin, growth hormone(GH) and insulin levels.Results Pain relief during treatment with transdermal fentanyl was significant accoring to VAS scores. Well-being scores showed that patients' quality of life including appetite, sleep, mental state and daily activities was significantly improved after two weeks treatment. Objective To analyze implementation outcomes of the new health reform policy at a provincial hospital for the past six months.Methods Trend analysis,factor analysis and ratio analysis approaches were used in monthly surveillance,along with comparative study of the data before and after the reform,to probe into impacts of the new policy on hospital operations.Results Medical income increased by 22.09％,mainly contributed by volume growth (22.28％) of medical services; Medical income dropped by 0.16％ due to pricing policy factors; medicine proportion deceased by 4.84％; following the reform,the income per outpatient and inpatient increased by 12.32％ and 5.35％ respectively.Conclusion Following the drug zero profit policy and rise of the charging standards for medical treatment service at the hospital,the growth of total income mainly comes from growing volume of medical services instead of pricing policy factors.The new health reform is conducive to optimizing the hospital's income structure and raising value to doctors' technical service.

OBJECTIVE:To observe the clinical efficacy and safety of Fuyankang combined with azithromycin in the treat-ment of pelvic inflammatory disease. METHODS:90 patients with pelvic inflammatory disease were selected and randomly divided into observation group and control group,with 45 cases in each group. Control group was given Lactose acid azithromycin for injec-tion 500 mg,ivgtt,qd,and then Azithromycin capsule 250 mg,po,qd;observation group was additionally given Fuyankang Tab-let 1.56 g,tid,on the basis of control group. A treatment course of 2 groups lasted for 7 d,and both received 3 courses of treat-ment. Clinical efficacies of 2 groups were observed as well as the level of CRP,IL-2 and life quality score before and after treat-ment,and the occurrence of ADR was compared between 2 groups. RESULTS:Total effective rate of observation group was 97.78%,which was significantly higher than that of control group (86.67%),with statistical significance (P<0.05). The level of CRP,IL-2 and life quality score of 2 groups were improved significantly after treatment,and the observation group was more better than the control group,with statistical significance (P<0.05). No obvious ADR was found in 2 groups during treatment. CONCLUSIONS:Fuyankang combined with Azithromycin is effective for pelvic inflammatory disease,and can improve the quali-ty of life,reduce inflammation cytokines infiltration and the generation of inflammatory medium with good safety.

Objective To explore the role of Th17 cells in systemic lupus erythematosus (SLE)with cardiovascular disease (CVD).Methods 61 patients of SLE were collected from September 2011 to March 2013 in the Second Hospital of Hebei Medical University by revised SLE classification standards of ACR in 1997.These patients were divided two groups:22patients of SLE with CVD and 39 patients of SLE without CVD;the control group include 20 healthy.Th17 cells were measured by flow cytometry,IL-1 7 was detected by enzyme-linked immunosorbent assay.The correlation among them and the disease active index were analyzed.Results ①The percent of Th1 7 cells in the group of SLE with CVD,that in the group of SLE without CVD and that in control group were (2.09±0.98)%,(1.75±0.75)% and (0.89±0.44)%,respec-tively.The percent of Th1 7 cells in healthy group were lower than that in SLE with CVD and SLE without CVD group (t=4.717~5.030,P<0.001).The level of IL-17 in the group of SLE with CVD,that in the group of SLE without CVD and that in control group were 85.64±20.76 pg/ml,75.25±28.14 pg/ml and 35.06±6.58 pg/ml respectively,and the serum of IL-17 in healthy group were lower than in SLE with CVD and SLE without CVD group (t=6.275~9.954,P<0.001). There were no significant difference of Th1 7 cells and IL-1 7 between SLE with CVD and SLE without CVD groups (t=1.520,P>0.05;t=1.513,P>0.05).②The level of IL-17 were correlated positively with SLEDAI and the anti-double strand DNA (r=0.393,P=0.008;r=0.558,P<0.001),were correlated negatively with complement C3 (r=-0.423,P=0.005).The percent of Th17 cells in CD4+T cells were correlated positively with SLEDAIand the anti-double strand DNA (r=0.681,P<0.001;r=0.492,P=0.015)were correlated negatively with complement C3 (r=-0.534,P=0.027).Con-clusion The level of Th1 7 cells and IL-1 7 were high in SLE,and they were related with the disease activity.The cardiovas-cular factor had not affect the expression of Th1 7 cells and IL-1 7 in SLE.

18 years of age were enrolled. The blood pressure measurements at the wrist were compared with the results obtained by auscultatory measurement on left upper arm. Both measurements were taken 3 times in each person. A total 273 measurements were obtained in 91 participants. The evaluation of the accuracy was assessed by 2 statistical methods. (1)The differences between the 2 devices for a given measurement. (2) The differences between the 2 devices for a given person. Results Method 1 showed the sample mean errors and standard deviation of errors were 0.5?7.1 mm Hg for systolic blood pressure(SBP) and -1.6?6.1 mm Hg for diastolic blood pressure(DBP). Method 2 showed they were 0.5?6.1 mm Hg for SBP and -1.6?5.3 mm Hg for DBP. Blood pressure level and the wrist circumferences had no influence on the measurement errors. Conclusion The HEM-6000 wrist blood pressure device apparently met the AAMI criteria and is valid for patient self-measurement.

Objective To investigate the effects of recombinant human erythropoietin(rhEPO)on expressions of tumon necrosis factor-alpha(TNF-α) and inter leukin-6(IL-6) in rats after focal cerebral ischemia and to explore its neuroprotective mechanism.Methods A total of 36 healthy male SD rats were randomly divided into sham-operated group (n=12),model group (n=12) and rhEPO treatment group (n=12).The suture method to make permanent middle cerebral artery occlusion model was adopted.rhEPO treatment group was injected with rhEPO 5000 U/kg intraperitoneally after 2 h of ischemia,whereas model group and sham-operated group were given identical saline at the same time.All rats were decapitated after 24 h of ischemia.6 rats were randomly selected in each group and the infarct volume of groups were measured by Triphenyl tetrazolium chloride (TTC)staining method.The expressions of TNF-α,IL-6 in other rats were detected by immunohistochemistry.Results No infarction was found in sham-operated group.Percentage of infarct volume in model group and rhEPO group were (36.672.40)％ and (27.49± 1.47)％,respectively.Compared with the model group,the volume of infarction in rhEPO group was significantly reduced.Cells stained by immunohistochemistry showed that The numbers of TNF-α-positive cells in the 3 groups were 9.001.41,27.83±2.48,17.50±1.87 and IL 6 positive cells were 8.94±2.31,20.33±3.53,14.83±1.70,respectively.Compared with sham operated group,the expressions of TNF-α and IL 6 in model group were significantly increased (q=16.1,19.6,P＜0.01).Compared with the model group,the expressions of TNF α and IL-6 in rhEPO group were significantly decreased (q=8.19,3.44,all P＜0.01).Conclusions rhEPO can decrease the infarct volume in SD rats after acute focal cerebral ischemic injure.rhEPO might exert its neuroprotective effect by reducing the expressions of TNF α and IL-6.

Objective To observe the efficacy of liposomal doxorubicin combined with cyclophosphamide, vincristine and prednisone in the treatment of refractory Non-Hodgkin’s lymphoma.Methods Liposomal doxorubicin (40mg/m2) was given by intravenous drip in the 1st day. Cyclophosphamide (750mg/m2) was given by intravenous injection in the 1st day. Vincristine (2mg) was given by intravenous injection in the 1st day. Prednisone (100mg/m2) was given orally from the 1st to the 5th day. A cycle was repeated every 3 to 4 weeks. Every patient took at least 2 cycles of the regimen.Results A total of 13 patients were assessed in the group. Among them, 7 were completely release (53.8), 4 were partially release (30.67) and 2 remained the same (15.35). The B symptom of 7 patients in the 9 with that disappeared, and that of the other 2 patients was improved obviously. The most common adverse effects were slight gastrointestinal reactions and the grade Ⅲ bone marrow suppression in a few patients. Conclusion The regimen of liposomal doxorubicin combined with cyclophosphamide, vincristine and prednisone is effective in the treatment of refractory Non-Hodgkin’s lymphoma with tolerable toxicity. It may be a salvage chemotherapeutic regimen deserving further study.
