1.Effect of budesonide combined with montelukast on lung function and serum levels of IL-6,IL-8 and TNF-αin patients with chronic obstructive pulmonary disease

Objective To explore the effect and lung function of the budesonide combined with montelukast for treatment of chronic obstructive pulmonary disease(COPD)and the effect on serum IL-6,IL-8 and TNF-α.Methods 120 patients with COPD in our hospital from April 2012 to December 2013 were randomly divided into observation group and control group (n =60 ).Observation group was given budesonide combined with montelukast on the basis of general routine treatment and control group was given budesonide on the basis of general routine treatment.The lung function and the level of IL-6、IL-8 and TNF-αin serum were detected berore and after treatment.Results The efficacy in observation group was significantly better than that in control group(P<0.05).The forced vital capacity(FVC),forced expiratory volume in one second(FEV1),maximal mid expiratory flow(MMEF),and peak expiratory flow(PEF)in the observation group were significantly higher than those in the control group(P<0.05 ).In addition, after treatment the levels of IL-6,IL-8 and TNF-αexpression in serum of the observation group and control group were significantly lower than before treatment,but the levels of the observation group after treatment was significantly lower than the control group,the difference were statistically significant (P<0.05).Conclusion The efficacy of the budesonide combined with montelukast for treatment of COPD is exact by reducing cytokine release, controling airway inflammation to improve the lung function.It is worthy to be popularized.
2.Clinical Observation of Levodopa Combined with Comprehensive Therapy for Ametropic Amblyopia Chil-dren and Teenagers

OBJECTIVE:To investigate the clinical efficacy of levodopa combined with comprehensive therapy for ametropic amblyopia children and teenagers and its effeets on pattern visual evoked potential(P-VEP)and visual function. METHODS:70 pa-tients(80 eyes)with ametropic amblyopia were randomly divided into observation group(35 cases,40 eyes)and control group(35 cases,38 eyes). Control group received comprehensive therapy as wearing correctional glasses,occlusion therapy and visual stimu-lation training;observation group additionally received levodopa 0.125(d1-3)-0.25 g/time,bid,on the basis of control group for 6 months. Therapeutic efficacy,P-VEP,Visual sensitivity and visual function were compared between two groups,and the occur-rence of ADR was recorded. RESULTS:The effective rate of observation group(92.50%)was significantly higher than that of con-trol group(76.32%),with statistical significance(P<0.05);the wave amplitude of P-VEP P100 in observation group were signifi-cantly greater than in control group,and the incubation period was significantly shorter than control group,with statistical signifi-cance(P<0.05);visual acuity under different spatial frequency contrast were significantly higher than control group,and correc-tion convergence range and correction separation range of observation group were significantly greater than those of control group, while corrected near stereo vision was significantly less than control group,with statistical significance (P<0.05). There was no statistical significance in ADR incidence between 2 groups (P>0.05). CONCLUSIONS:Levodopa combined with comprehensive therapy can improve the function of visual central neurons,and improve visual acuity and binocular stereo vision functions so as to improve therapy efficacy.
3.Pharmacodynamics Research of Lilium brownii Polysaccharide

Objective To research the pharmacodynamics of lilium brownii Polysaccharide.Methods Five tests,including hypoxia tolerance test,serum content determination of SOD and MDA in mice,carbon clearance and weight determination of immune organ,content determination of serum hemolysin,and delayed-type hypersensitivity test were carried out to evaluate the main pharmacological indexes of lilium brownii Polysaccharide.Results Lilium brownii Polysaccharide can increase the hypoxia tolerance,anti-oxidation action and immuno-enhancing activity in mice.Conclusion Lilium browni Polysaccharide is one of the main active constituents of lilium brownii.
4.Prenatal ultrasound application in diagnosis and treatment of twin reverse arterial perfusion syndrome

Objective To investigate the value of prenatal ultrasound in diagnosis and treatment of twin reverse arterial perfusion (TRAP) syndrome. Methods A retrospective study was performed in 5 TRAP cases, including ultrasound images, clinical data and pregnancy outcomes. The sonographic characteristics were summarized. Results Five TRAP cases were diagnosed during 13 to 28 weeks' gestation and confirmed after birth. Color Doppler unltrasonography revealed retrograde umbilical artery perfusion towards acardiac twin. Two of 5 cases ended up in induced abortion, 1 in spontaneous abortion, 1 was delivered at 37 weeks' gestation after ultrasound-guided feticide of the acardiac twin and 1 was monitored closely with ultrasound and delivered alive at 32~(+4) weeks' gestation. Conclusion Prenatal ultrasonography has great applicative value for TRAP syndrome in early diagnosis, choosing optimal treatment and prognosis assessment.
5.Mutations in E154 of Diphtheria Toxin (DT) and Their Biologic Activity

According to the results of quantum chemistry calculation and the present research status in the relationship between the structures and the functions of DT, the E154 in DT catalyzing domain was mutated to aspartic acid and arginine in order to study the effects of the alteration on the biological activities. By means of gene site-direct mutation, two mutated genes were prepared and the high performance expression was obtained in E.coli system. The results of toxcity studies indicated that the acute toxicity in guinea pig and cytotoxicities of mutant E154D increased slightly in compared with those of recombination wild toxin, and contrarily, those of E154R decreased obviously.

Objective To assess the relationship between gastric carcinoma perfusion imaging parameters with the multislice spi-ral CT (MSCT)and the tumor angiogenesis(MVD,VEGF).Methods (1)33 patients with gastric cancer were carried on perfusion CT scanning in the suspected lesions,and compared with operation and histological result.MSCT perfusion parameters tumor,such as local blood flow (BF),blood volume (BV),mean transit time of contrast agent (MTT),permeability surface (PS),were recor-ded,and compared with clinical pathological data.(2)27 patients of 33 cases which CT perfusion plane matching with operation pa-thology specimens performed with tumor microvessel density (MVD),vascular endothelial growth factor (VEGF)monoclonal anti-body immunohistochemical examination of MVD,the most intensive areas of high power (×200 HP)field counted,and VEGF stai-ning positive judged.Results Achievement ratio of gastric carcinoma MSCT perfusion imaging was 84.85% (28/33).The average value of BF,BV,MTT and PS were 63.658 ± 18.305,7.5 1 1 ± 2.427,1 1.952 ± 4.325 and 31.81 7 ± 13.533,respectively,and MVD was 37.7 ± 11.1/200 HP (range:13-60).VEGF was positive in 16 cases,negative in 11 cases.Gastric carcinoma undifferentiated group perfusion parameter PS value (35.1 5 ± 12.74 )and MVD (40.53 ± 10.66 )were higher than the differentiation group (23.90 ± 12.71 and 31.13 ± 9.82 )(P < 0.05 ),but BF,BV,MTT not statistically significant;Differences of CT perfusion parameters and MVD were not significant statistically between invasive serosa and noninvasive ;PS value (36.65± 12.80)of lymph node metastasis was greater than without metastasis(22.70 ± 1 1.1 5 )(P <0.01 ),the other was no significant difference;TNM staging Ⅲ,Ⅳ phase group of BF value (69.56 ± 1 6.49),PS value (34.90 ± 12.80)and MVD value (40.74 ± 10.53)were higher than Ⅰ,Ⅱ Group (49.63 ± 1 5.04),(24.50 ± 13.13)and (30.63 ± 9.61)(P <0.01).Spearman correla-tion analysis in confidence (two tails)of 0.01 was statistically significant between MVD in tumor tissues and gastric cancer MSCT perfusion parameters of BF (r=0.404)and MTT (r=0.371),whereas BV and PS were no significance.The regression equation of MVD with BF and MTT:MVD =1 6.602+0.1 50XBF +0.967XMTT,model checking of F values was 6.62,P =0.003.Conclusion The gastric carcinoma multi-slice CT perfusion imaging parameters BF,MTT and MVD,VEGF(+)was positive correlation, MSCT perfusion imaging parameters reflects tumor VEGF positive expression of gastric carcinoma.
7.Comparison of MR neurography of the brachial plexus with unidirectionally encoded diffusion-weighted imaging with background signal suppression and enhanced three dimensional short inversion time inversion recovery sampling perfection with application optimized contrasts using different flip angle evolutions

Objective To evaluate two different MRI techniques , enhanced 3D STIR SPACE and unidirectionally-encoded DWIBS MR Neurography in visualizing the brachial plexus.Methods Twenty healthy volunteers underwent MR scanning by the procedures of unidirectionally -encoded DWIBS and enhanced 3D STIR SPACE sequence of the brachial plexus.Original images were reconstructed with coronal maximum intensity projection ( MIP ).The image quality was assessed by comparing the visualization of various parts of the brachial plexus and the suppression ratio of image background.The degree of visualization was compared via χ2-test of paired data , and comparison of background suppression scores was performed using a nonparametric Wilcoxon signed rank sum test.Results The successful rate of visualizing the brachial plexus supraclavicular section ( C5-T1 ) was 84% ( 167/200 ) and 99% ( 198/200 ) , respectively, for the DWIBS and enhanced 3D STIR SPACE, and the rate for the infraclavicular section was 33%( 13/40 ) and 95% ( 38/40 ).The differences between these two techniques were statistically significant (χ2 value was 28.18 and 31.15 respectively, P <0.01).In all images, the scores of grade (Ⅰ-Ⅳ) of background suppression were 0, 0, 4 and 16 for unidirectionally-encoded DWIBS, respectively;while they were 15, 4, 1 and 0 for enhanced 3D STIR SPACE.There was significant difference between the two techniques ( Z =3.96,P <0.01).Conclusions High-quality MR neurography of the brachial plexus can be obtained using the enhanced 3D STIR SPACE sequence.It offers a complete anatomical coverage of the brachial plexus , and thus demonstrates a better capacity in depicting the anatomy of brachial plexus as compared with the unidirectionally-encoded DWIBS.
8.Therapeutic Observation of Dragon-tiger Contending Needling at Xi-cleft Points plus Electroacupuncture for Postherpetic Neuralgia

Objective To evaluate the clinical efficacy of dragon-tiger contending needling at Xi-cleft points plus electroacupuncture in treating postherpetic neuralgia (PHN).Method Seventy PHN patients were randomized into a treatment group of 36 cases and a control group of 34 cases. The treatment group was intervened by dragon-tiger contending needling at Xi-cleft points plus electroacupuncture, while the control group was treated with electroacupuncture alone. In the 4-week treatment, the Visual Analogue Scale (VAS) scores right before and after the 1st, 2nd, 6th, 9th and 12th treatment sessions were recorded, “the maximum pain intensity since the last treatment session” and “real-time analgesic effect” were observed, and the total therapeutic efficacy was also evaluated.Result The real-time analgesic effects right after the 1st, 2nd and 6th treatments in the treatment group were significantly better than those in the control group (P<0.05). The “maximum pain intensities since the last treatment session” prior to the 2nd, 6th, 9th, and 12th treatment sessions were significantly different from the pain intensity before the intervention in the two groups (P<0.05). The “maximum pain intensities since the last treatment session” prior to the 6th, 9th, and 12th treatment sessions were significantly different from those in the control group (P<0.05). The total effective rate and recovery plus markedly-effective rate were respectively 97.2% and 77.8% in the treatment group, versus 91.2% and 59.2% in the control group. There was a statistically significant difference in the recovery plus markedly-effective rate between the two groups (P<0.05).Conclusion Dragon-tiger contending needling at Xi-cleft points plus electro- acupuncture is an effective approach in treating PHN; it acts efficiently and can produce a satisfactory real-time analgesic effect.

Objective To develop a RP-HPLC method for the determination of piperine in the root of Piper nigrum L.Methods RP-HPLC was carried out on Luna C18 column(250 mm? 4.60 mm,5 ? m) with the column temperature of 35 ℃.The mobile phase consisted of a mixture of methanol-water(77 :23) at a flow rate of 1.0 mL? min-1.The determination wavelength was at 343 nm.Results The calibration curve was linear within the concentration range of 0.164 ? g~ 0.984 ? g,r=0.9996,and the average recovery was 98.09 %,RSD=2.67 %(n=9).The average content of piperine in three batches of pepper roots was in the range of 6.67~6.77mg?g-1.Conclusion Pepper root contains piperine,and this method is suitable for the quality control of the root of Piper nigrum L.
