1.Pure tone threshoIds and Otoacoustic Emissions in PCI Patients without Hearing Loss CompIaints

Objective To study the audiometry and otoacoustic emissions in patients with posterior circulation ischemia(PCI) .Methods Forty patients treated by neurologists were selected as the experimental group who re_ceived pure tone audiometry ,tympanometry ,otoacoustic emissions (OAE) and distortion products otoacoustic emis_sions (DPOAE) examinations .Thirty healthy objects were chosen as the control group .The data from the PCI groups and the control group were compared using the SPSS 14 .0 software .ResuIts The difference in pure tone thresholds across 125 to 8 000 Hz between the PCI group and the control group was not statistically significant (P>0 .05) .In the PCI patients ,the prevalence of TEOAE was only 47 .5% ,significantly different from that of in the control group .The prevalence of DPOAE at 0 .5~8 kHz were between 57 .9% ~77 .6% in PCI patients ,and the amplitudes of DPOAE were reduced significantly (P<0 .05) .ConcIusion Cochlear damages can occur to patients with PCI ,especially in the high frequency range .These results suggest that OAE can be used as an important diag_nostic test for patients with PCI ,and might be helpful for the location diagnosis of PCI .
2.The comparative analysis for two kinds of serum procalcitonin quantitative detection and prediction of blood culture

Objective The detection results of two methods of quantitative methods and blood culture method were compared to explore the value of quantitative detection of serum procalcitonin.Methods From February 2014 to January 2015,the clinical data of 192 patients who were tested for two quantitative detection and blood culture were collected at the same time,and the results were eligible analyzed.One of the two quantitative detection methods was the electrochemical luminescence,and the other was the up-converting phosphor method.Results Compared with the result of blood culture,the positive rate was significantly higher in electrochemical luminescence and the up-con-verting phosphor method (χ2 =70.531,43.671,all P<0.05).The positive rates of up-converting phosphor and electrochemical luminescence were 66.1% and 75.5% respectively, and the difference between two quantitative methods was also statistically significant (χ2 =5.297,P<0.05).Method of electrochemical luminescence tested on higher sensitivity.When the level of PCT was less than threshold,for method of the electrochemical luminescence,the sensitivity on the blood culture was 93.7%,the specificity was 33.3%,the positive predictive value was 40.7%,the negative predictive value was 91.5%,the area under ROC curve was 0.628.For method of up-converting phosphor, the sensitivity on the blood culture was 90.5%,the specificity was 46.5%,the positive predictive value was 45.7%, the negative predictive value was 90.7%,the area under ROC curve was 0.554.Conclusion The electrochemical luminescence detection method of serum procalcitonin was better than up -converting phosphor method and blood culture.The electrochemical luminescence method which makes rapidly qualitative and accurately quantitative detec-tion,can give early diagnosis,medication guide in a short time,and predict the prognosis of disease.
3.Determination of Schizandrin A and Tanshinone Ⅱ_A in Wulingwan with TLC-Scanner Method

TLC-scanner method was used to determine the content of schizandrin A and tanshinone ⅡA in wulingwan. The average recovery of both schizandrin A and tanshinone ⅡA are 98.31% (CV = 2.1% ) and 99.15% (CV = 1.1% ) respectively.This method is simple and rapid.Its reproducibility is satisfactory.
4.Expression and Clinical Significance of Macrophage Migration Inhibitory Factor in the Serum and Lung Tissues in Patients with Non-Small Cell Lung Cancer

[Objective]To investigate the expression and the clinical significance of macrophage migration inhibitory factor (MIF)in the serum and lung tissues of the patients with non-small cell lung cancer(NSCLC).[Methods]Eighty-eight eligible inpatients were confirmed by pathology of lung tissues ,including 66 patients with NSCLC and 22 patients with benign lung lesions. ELISA was performed to measure serum concentration of MIF of these patients ,which was compared with 30 healthy individuals. Meanwhile,immunohistochemistry(IHC)was performed to examine the expression of MIF in the lung tissues of the two groups. MIF expression level was compared between two groups and among different subgroups of NSCLC. The correlationship between serum MIF level and high expression rate in lung tissues was also analyzed. All the data were analyzed by SPSS17.0.[Results]The serum concentration of MIF in NSCLC group was significantly higher than that in healthy control(14.79 ng/mL vs 10.69 ng/mL,P=0.001), and was slightly higher but not significantly different from benign lung lesions group(14.79 ng/mL vs 13.68 ng/mL,P=0.580). The comparison among subgroups of NSCLC showed that the serum MIF level was not only correlated with the histological grade and clini?cal stage of the cancer,but also correlated with the gender and smoking history of the host. Immunohistochemistry showed that the MIF high expression rate in the lung tissues of NSCLC was markedly higher than that of benign lung lesions group(30.3%vs 4.5%, P=0.014). Among the subgroups of NSCLC,IHC showed MIF high expression rate was correlated with the histopathologic types and clinical stage of the cancer. Simultaneously ,the serum MIF level showed a positive correlation with MIF expression rate in the lung tissues in all patients and NSCLC group(P<0.05).[Conclusion]MIF was strongly related to the clinicopathological characteristics of NSCLC. It could be helpful for the diagnosis and clinical evaluation of NSCLC.
5.Analyzing Complications of 73 Cases of Laparoscopic Gynecologic Surgery

Objective:The causes, management, and prevention of complications in laparoscopic gyneco-logic surgery were investigated. Methods:A retrospective analysis was done based on clinical data of 8279 cases of laparoscopic gynecologic surgery from January 1996 to December 2008 in this hospital. Results:The complication rate was 0.88% (73/8279). These included five cases intraperitoneal bleeding, five cases injury of ureter, 20 cases abdominal vessel damage, 10 cases vulva emphysema, 25 cases wound healing delay and each one case respectively for stomach injury, bladder injury, secondary abdominal cavity preg-nancy,infection and incision hernia. The occurrence rates of complications between hysteroscopy combined with laparoscopic surgery and pure adnexal surgery were no difference( P >0.05). Compared to adnexal op-erations, there was a significantly higher complication rate in myomectomy and laparoscopic assistant vaginal hysterectomy (P < 0.01 ). Conclusions:Complications of laparoscopic gynecologic surgery are correlated with the difficulty of operation and the skillfulness of operators. In order to reduce complications, careful oper-ation and avoiding over self -confidence are essential.

8.Assessment of auditory function in patient with posterior circulation transient ischemic attack.

OBJECTIVE:
To study the hearing characteristics in patient with posterior circulation transient ischemic attack (PCTIA).
METHOD:
Sixty-two patients diagnosed by neurologist were selected as experimental group for testing of pure tone audiometry, tympanometry, otoacoustic emissions (OAE) and auditory brainstem response (ABR). Thirty healthy subjects were chosen as control group. The data were compared between the PCTIA groups and the control group using the SPSS 14.0 software.
RESULT:
(1) The difference in pure tone threshold across 125 to 8000 Hz between the PCTIA group and the control group was not statistically significant (P>0.05); (2) In PCTIA patients, the prevalence of TEOAE was only 42.7%, significantly different from the control group; (3) The patients had longer ABR latency and wave interval than the normal controls (P<0.01).
CONCLUSION
Brain-stem and cochlear damage can occur in patients with PCTIA. These results suggest that OAE and ABR be used as important diagnostic test for patients with PCTIA, and might be helpful in localizing abnormality in PCTIA.

9.Evaluate the Significance of Th1 7 Cells in Systemic Lupus Erythematosus with Cardiovascular Disease

Objective To explore the role of Th17 cells in systemic lupus erythematosus (SLE)with cardiovascular disease (CVD).Methods 61 patients of SLE were collected from September 2011 to March 2013 in the Second Hospital of Hebei Medical University by revised SLE classification standards of ACR in 1997.These patients were divided two groups:22patients of SLE with CVD and 39 patients of SLE without CVD;the control group include 20 healthy.Th17 cells were measured by flow cytometry,IL-1 7 was detected by enzyme-linked immunosorbent assay.The correlation among them and the disease active index were analyzed.Results ①The percent of Th1 7 cells in the group of SLE with CVD,that in the group of SLE without CVD and that in control group were (2.09±0.98)%,(1.75±0.75)% and (0.89±0.44)%,respec-tively.The percent of Th1 7 cells in healthy group were lower than that in SLE with CVD and SLE without CVD group (t=4.717~5.030,P<0.001).The level of IL-17 in the group of SLE with CVD,that in the group of SLE without CVD and that in control group were 85.64±20.76 pg/ml,75.25±28.14 pg/ml and 35.06±6.58 pg/ml respectively,and the serum of IL-17 in healthy group were lower than in SLE with CVD and SLE without CVD group (t=6.275~9.954,P<0.001). There were no significant difference of Th1 7 cells and IL-1 7 between SLE with CVD and SLE without CVD groups (t=1.520,P>0.05;t=1.513,P>0.05).②The level of IL-17 were correlated positively with SLEDAI and the anti-double strand DNA (r=0.393,P=0.008;r=0.558,P<0.001),were correlated negatively with complement C3 (r=-0.423,P=0.005).The percent of Th17 cells in CD4+T cells were correlated positively with SLEDAIand the anti-double strand DNA (r=0.681,P<0.001;r=0.492,P=0.015)were correlated negatively with complement C3 (r=-0.534,P=0.027).Con-clusion The level of Th1 7 cells and IL-1 7 were high in SLE,and they were related with the disease activity.The cardiovas-cular factor had not affect the expression of Th1 7 cells and IL-1 7 in SLE.
10.Effects of recombinant human erythropoietin on expressions of tumor necrosis factor-alpha and inter ieukin-6 in rats with acute cerebral ischemia

Objective To investigate the effects of recombinant human erythropoietin(rhEPO)on expressions of tumon necrosis factor-alpha(TNF-α) and inter leukin-6(IL-6) in rats after focal cerebral ischemia and to explore its neuroprotective mechanism.Methods A total of 36 healthy male SD rats were randomly divided into sham-operated group (n=12),model group (n=12) and rhEPO treatment group (n=12).The suture method to make permanent middle cerebral artery occlusion model was adopted.rhEPO treatment group was injected with rhEPO 5000 U/kg intraperitoneally after 2 h of ischemia,whereas model group and sham-operated group were given identical saline at the same time.All rats were decapitated after 24 h of ischemia.6 rats were randomly selected in each group and the infarct volume of groups were measured by Triphenyl tetrazolium chloride (TTC)staining method.The expressions of TNF-α,IL-6 in other rats were detected by immunohistochemistry.Results No infarction was found in sham-operated group.Percentage of infarct volume in model group and rhEPO group were (36.672.40)% and (27.49± 1.47)%,respectively.Compared with the model group,the volume of infarction in rhEPO group was significantly reduced.Cells stained by immunohistochemistry showed that The numbers of TNF-α-positive cells in the 3 groups were 9.001.41,27.83±2.48,17.50±1.87 and IL 6 positive cells were 8.94±2.31,20.33±3.53,14.83±1.70,respectively.Compared with sham operated group,the expressions of TNF-α and IL 6 in model group were significantly increased (q=16.1,19.6,P<0.01).Compared with the model group,the expressions of TNF α and IL-6 in rhEPO group were significantly decreased (q=8.19,3.44,all P<0.01).Conclusions rhEPO can decrease the infarct volume in SD rats after acute focal cerebral ischemic injure.rhEPO might exert its neuroprotective effect by reducing the expressions of TNF α and IL-6.
