1.SCREENING OF PROBIOTIC LACTOBACILLUS AND THEIR CHARACTERISTIC

According to the requirements of ptobiotics, we have separated 13 strains of Lactobacillus from the content in gut of lactating piglet. Among them 3strains are L. acidophilus, one is L. casei, one is L. atenaforme and 8 strains are L. fermentum. They can resist 1.0% bile salt and the acidity of pH3.0, can inhibit pathogenic Escherichia coli and Staphylococcus aureus. They are safe and non-toxic to mice.
2.Expression of interphotoreceptor retinoid-binding protein in lens-induced myopia in guinea pig

Background The pathogenesis of myopia is currently unclear. Studies showed that retinoic acid regulates the development of myopia. Whether IRBP, a retinoid transport proteins, participates in the development of myopia or not is unknown. Objective The aim of this study was to investigate the expression of interphotoreceptor retinoid-binding protein (IRBP) in the retina of guinea pig with lens-induced myopia and study the relationship between IRBP expression and experimental myopia. Methods Fifty guinea pigs were randomized into the normal controls(n=10) ,defocus Ⅰ group (n=20) and defocus Ⅱ group (n=20). Guinea pigs in the defocus Ⅰ groups wore -10. 00 D lenses on the right eyes for 14 days and the defocus Ⅱ group for 28 days. The left eyes of both defocus groups did not wear any lens served as self-control. The refraction diopter and axial length were measured before and after wearing lenses. The protein expression of IRBP in the retina was detected by immunohistochemisty and Western blot. RT-PCR was used to observe the level of IRBP mRNA in the retina. Result Compared with the control eyes,the right eyes in the defocus Ⅰ and Ⅱ groups developed to ( -5. 53±1.93) D and (-8.69±2.46) D, and the ocular axial length elongated to (0. 31±0. 15 ) mm and (0.41 ±0. 13 ) mm, showing statistically significant differences between them ( group Ⅰ: t= 12. 57,29.63 ,P<0.05 ;group Ⅱ :t= 15.82,44.35 ,P<0.05 ) ,and the degree of myopia was lower and axial length was shorter in group Ⅰ than the eyes of group Ⅱ ( P<0.05 ). There was no statistically significant differences found between the left eyes of defocus group Ⅰ and defocus group Ⅱ or eyes of the control group in diopter and axial length (P>0.05). Immunochemistry revealed that the mean gray value of IRBP protein expression was 165.62t4. 93 and 171.00±4. 25 in defocus Ⅰ group and defocus Ⅱ group and was significantly higher than that of the self-control eyes and the normal control group ( 156. 31±4.00,155.26 ±3.49 and 158.61 ±4. 58 ) ( P<0.05 ). Western blot demonstrated that the expressios of IRBP protein were down-regulated and RT-PCR showed the IRBP mRNA level of IRBP was significantly lower in defocusIgroup and defocusⅡgroup,compared with the self-control eyes and the normal control group (P<0. 05). Conclusion Expression of IRBP in guinea pig may play a role in lens-induced myopia.
3.Early Exercises of Affected Arm for Postoperative Breast Cancer Patients

Objective To explore the effect of early systemic phased exercise post mastectomy on the recovery of activities of daily living. Methods 100 patients who received modified radical mastectomy were assigned to the experimental group and control group with 50 patients in each group. They all accepted routine nursing, and the experimental group accepted early systemic phased exercise in addition. They were assessed with range of motion (ROM) of shoulder and modified Barthel Index (MBI). Results ROM of shoulder improved in 90% of the experimental group and 70% of the control group (P<0.05). MBI was (84.5±18.2) in the experimental group and (51.8±16.6) in control group (P<0.05) . Conclusion Early systemic phased exercises may promote the recovery of the affected arm for the patients post mastectomy.
4.Evaluation of community health service institutions responsiveness in Henan Province:Key in-formant survey

Objective:To understand the responsiveness of community health service institutions in Henan Province. Methods:This research collected information through questionnaire investigations. The object of the investigations is key informants familiar with community health service institutions. Various dimensions of responsiveness within Henan province community health service institutions are determined through a multiple-input pecking order graph. The fuzzy comprehen-sive evaluation method is used to evaluate the level of responsiveness. Responsiveness inequality index is used to measure the distribution of reactions. Results:A total of 431 valid key informants were surveyed. The various dimensions of the questionnaire of Cronbach alpha coefficient was greater than 0. 623;the total scale of the Cronbach alpha coefficient was 0. 850;the correlation coefficient of each dimension of test-retest reliability were greater than 0. 907. The levels of responsive-ness and distribution index were 8. 33 and 0. 2744, respectively;each dimension by order of importance:timely attention>dignity>infrastructure>communication>confidentiality >social support >selective >autonomy. Approximately 50% of key informants considered that vulnerable groups in community health service institutions were sometimes or often treated unfairly. Conclusion:Key informants considered the responsiveness level of overall higher community health service institu-tions in Henan Province was high, and the distribution of responsiveness was relative equilibrium. However, there is a large shortage of infrastructure, privacy protection, doctor-patient communication, etc.
8.Sputum analysis in patients with "disharmony of stomach"

OBJECTIVE: To analyze the amount and inflammatory components of the sputum in patients with "disharmony of stomach", including gastroesophageal reflux disease, peptic ulcer and chronic gastritis. METHODS: Sputum production was induced by inspiration of aerosol of 3.5% hypertonic saline with ultrasonic nebulizer, and the total cell count in the sputum was examined. Sputum smear stained with Gimsa was used in differential counting under light microscope. Bacterial culture in the sputum was performed in the usual way. Peak expiratory flow (PEF) was tested to study the airway responsiveness. RESULTS: The amount of sputum, the total cells and eosinophils in the sputum of the patients with "disharmony of stomach" were higher than those of the control. PEF decreased following sputum induction. These parameters returned to control level after treatment with Chinese herbs. Bacterial culture was negative. CONCLUSION: "Disharmony of stomach" can cause the bronchial inflammation (non-infection) characterized by eosinophil increase.
9.A survey on chronic heart failure management in community physicians in Chengdu

A questionnaire survey on chronic heart failure management was conducted in 110 physicians from Xindu,Supo,Jiajiang and Xinhua community hospital in Chengdu from January 2007 to June 2010.Results showed that 77.3% (85/110) of community physicians lacked knowledge about prevention,diagnosis and treatment of chronic heart failure,and the diagnostic accuracy rate was only 51.3% (40/78) in these community hospitals.There was lack of awareness of the guideline of chronic heart failure:the rate of β blockers use was 15.0% (6/40) and only 5.0% (2/40) used the target dose; the rate of angiotensin converting enzyme inhibitor use was 17.5% (7/40) and only 7.50% (3/40) used the target dose.In addition,90% (99/110) of community doctors lacked the education and management for patients with heart failure.The survey suggests that the current situation of chronic heart failure management in community physicians in Chengdu is unsatisfactory.It is necessary to strengthen the training of community physicians.
10.The clinical phenotype and MRI manifestations of neurosyphilis

Objective To analyze the clinical and MRI manifestations of neurosyphilis in order to improve the recognition and di‐agnosis of the disease .Methods The MRI and clinical data of 11 patients with neurosyphilis confirmed in clinic were collected .The patients were categorized into different phenotypic types according to the clinical manifestations ,and the MRI manifestations were al‐so analyzed .Results 5 patients with parenchymal types showed cerebral atrophy .3 patients with meningovascular types showed cer‐ebral infarction .1 patient with encephalitis showed brain signal changes and meningeal enhancement .1 patient without symptom showed signal changes in brain parenchyma and mild enhancement .1 patient of myelopathic type showed signal changes of thoracic spinal cord without enhancement .Conclusion There is an overlap of the MRI findings of neurosyphilis in different clinical pheno‐types ,exhibiting no specifity .Neurosyphilis can be diagnosed when the young patient is manifested as cerebral atrophy ,infarction , MR signal intensity abnormality of cerebral parenchyma and spinal cord ,as well as meningeal enhancement .
