This article discusses the three-dimensional ambulance under the future war conditions.The unexpectedness of battlefields and complexity of the wounded in the future warfare necessitate the establishment of a three-dimensional system of medical rescue that can be organized by land,water and air.With an analysis of the features of modern war and based on the theory of modern treatment,the authors have proposed the requirements for designing the mode of three-dimensional rescue in the war zone and its typical procedures,set forth the view that rapid reaction and three-dimensional rescue are the most effective medical guarantee of combat strength,and provided a scientific basis for building a medical rescue system in the future warfare.

Objective To compare the clinical efficacy of esomeprazole or omeprazole triple therapies on gastric ulcer patients complicated with type 2 diabetes mellitus.Methods Seventy-five patients with Hprelated gastric ulcer and type 2 diabetes mellitus were enrolled and were randomly divided into group A(n=39)and group B (n =36).Patients in group A were treated with esomeprazole,amoxicillin and clavulanate potassium dispersible tablets,Clindamycin.Patients in group B were treated with omeprasole,amoxicillin and clavulanate potassium dispersible tablets,Clindamycin.Patients in both groups were treated with hypoglycemic therapy.The efficacy and the Hp eradicate rate of the two groups were observed and compared.Results The cure effect in group A was 97.44％(38/39),much better than that of group B(80.56％(29/36);x2=5.598,P=0.025).The Hp eradicate rate in group A was 92.31％,also much better than that of group B(75.00％;x2 =4.172,P =0.041).Conclusion The clinical egicacy and the Hp eradicate rate of esomeprazole triple therapies on gastric ulcer patients complicated with type 2 diabetes mellitus is better than that of omeprazole triple therapies.Esomeprazole triple therapies and it is worthy of being clinically promoted.

Objective To investigate the relationship between serum interleukin 6( IL-6 ),tumor necrosis factor α( TNF-α)and C reaction protein( CRP)levels and smoking and body mass index( BMI)in the elderly patients with stable chronic obstructive pulmonary disease( COPD ). Methods Elderly participants including 50 cases smokers with stable COPD,45 cases ex-smokers with stable COPD and 40 cases healthy ex-smokers were recruited in this study,who were hospitalized in the People's Hospital of Gansu Province from Dec. 2012 to Feb. 2014. Serum IL-6,TNF-α and CRP levels were detected. Correlation analysis was performed between serum IL-6,TNF-α,CRP levels and smoking index( SI),BMI in COPD smokers. Results The levels of serum IL-6,TNF-α and CRP in smoking group were(45. 9 ± 12. 1)mg/L,(58. 2 ± 15. 8)ng/L,(12. 2 ± 4. 1) mg/L,significantly higher than those in stop-smoking group((38. 1 ± 9. 6)mg/L,(45. 9 ± 11. 2)ng/L,(8. 6 ±3. 2)mg/L respectively),and healthy controls group((17. 0 ± 9. 9)mg/L,(27. 3 ± 13. 2)ng/L,(6. 3 ±5. 2)mg/L),and the differences were significant(F=84. 934,57. 224,23. 023;P﹤0. 01). In patients with smoking index≥400,the levels of serum IL-6,TNF-α and CRP were(50. 1 ± 12. 1)mg/L,(64. 2 ± 12. 6) ng/L,(13. 4 ± 3. 7)mg/L,significantly higher than those in patients with SI ﹤400 group((41. 0 ± 10. 2) mg/L,(47. 8 ± 14. 0)ng/L,(10. 8 ± 4. 2)mg/L respectively),and the differences were significant( t=2. 845, 4. 343,2. 347;P ﹤0. 01 or P ﹤0. 05 ). The levels of serum IL-6,TNF-α and CRP in smoking group were positively correlated with SI(r=0. 458,0. 438,0. 313;P﹤0. 01 or P﹤0. 05). The levels of serum IL-6,TNF-αwere negatively correlated with BMI,and the correlation coefficient were - 0. 358,- 0. 319( P ﹤0. 05). Conclusion The increase of serum IL-6,TNF-αand CRP caused by smoking may play an important role in the pathogenesis of COPD. And chronic smoking is the reason of the decline of BMI in COPD patients,and the decline of BMI is related to the high levels of serum IL-6 and TNF-α.

Objective To study the cytoprotective effect of prostaglandin E1(PGE1)in the imme- diate period after orthotopic liver transplantation (OLT).Methods Ten patients undergoing OLT were divided into 2 groups: group P (n=6) was administered with PGE1 and group C (n=4) served as control group. Serum ALT, AST, TBIL, DBIL, bile output and the period of intensive care unit (ICU) in both groups were observed in the first 3 weeks after OLT. Objective To investigate the effect of hypertonic sodium chloride hydroxyethyl 40 injection (HSH)on craniocerebral injury accompanied with hypovolemic shock and ICP.Method Sixty patients suffering from craniecerebral injury accompanied with the hypovolemic shock and admitted to ICU and Neurosurgical Department of Guang dong General Hospital from September 2007 to June 2008,were chosen into the study prospectively.Those who was younger than 18years or older than 70 years,and those who were pregnant or meustruous women,or serious hepatic and renal insufficiency,hypertension,coronaxy artery disease,diabets mellitue,uncontrolled bleeding,brain death,erc were excluded.The patients were randomly divided into 3 groups.Each group included 20 patients and they were resuscitated with LR(500 ml),HIS(4 ml/kg)and HSH(4 ml/kg)respectively.The changos of blood pressure,laboratory examination indices and ICP Were detected before and at 30,60,120 minutes after resuscitation.Results MAP raised over 60 mmHg and ICP declined 10%within 30 min in HIS and HSH groups.The effect of HSH on the improving of blood pressure and the reduction of ICP wag more obvious than that of LR and HIS at 120 minutes(F=18.43,8.99,P<0.05)after resuscitation.There were no obvious oboes of the laboratory examination indices after resuscitation.Conclusions The themputic effect of HSH on ICP and MAP in craniocerebral injury accompanied with hypovolemic shock is obvious and lasting,which would be beneficial to the protection of cerebral function.
