1.Relation of Toll-like Receptor 4 and Monocyte Chemoattractant Protein-1 with Acute Coronary Syndrome

Objective To investigate the relation of Toll-like receptor 4 (TLR4)and monocyte chemoattractant protein-1 (MCP-1)with acute coronary syndrome.Methods 60 patients with coronary heart disease patients including 39 cases of a-cute coronary syndrome (ACS group),21 cases of stable angina (SA group)from January 2015-March 2016 in Xi’an First Hospital were in this observation.ACS group was divided into acute myocardial infarction (AMI,n=21)and unstable angina pectoris (UAP,n=18).Another 30 cases with angiographically normal was included in control group.Enzyme-linked immu-nosorbent assay (ELISA)detection was used to detect the serum content of MCP-1,TLR4 expression was tested by flow cy-tometry.Triglyceride (TG),total cholesterol (TC),low-density lipoprotein cholesterol (LDL-C)and high-density lipopro-tein cholesterol (HDL-C)were detected.Results TLR4 levels of ACS group was significantly higher than SA group (t=4.455,P=0.021).Serum MCP-1 levels of ACS group were significantly higher than SA group (t=3.220,P=0.002)and control group (t=6.197,P=0.000).MCP-1 levels of ACS group were higher than control group (t=2.306,P=0.025). The Spearman correlation test TLR4 was positively with MCP-1 (r=2.389,P=0.025).Conclusion MCP-1 and TLR4 may determine the severity of coronary heart disease,combined detection of MCP-1 and TRL4 can provide the clinical value for early diagnosis and treatment of ACS.
2.Influence of Urapidil at Different Doses on Blood Pressure and Prognosis after Thrombolsis with Urokinase Patients with Acute Ischemic Stroke

Objective: To compare the effect of urapidil at different doses on blood pressure and prognosis of patients with acute ischemic stroke (AIS) so as to provide data evidence for its clinical use.Methods: Totally 62 patients with AIS were divided into 3 groups in a prospective study: 20 cases in high dosage group, 21 cases in middle dosage group and 21 cases in low dosage group.All the patients were treated with urapidil and urokinase, and the initial dose of urapidil was 25 mg, 15 mg and 5 mg, respectively.Systolic blood pressure (SBP) and diastolic blood pressure (DBP) of the patients before and after the treatment were observed, and the US national institutes of health stroke scale (NIHSS), modified Rankin scale (mRS) and activity of daily living scale (ADL) were also evaluated in the follow-ups.Results: The time of blood pressure up to standard in the middle dosage group and low dosage group was longer than that in the high dosage group.At the same time, the time of blood pressure up to standard in the low dosage group was longer than that in the middle dosage group, and the difference had statistical significance (P<0.05).After the treatment, SBP and DBP at T1 in all the groups were notable lower than those at T0,and those at T2 and T3 were lower than those at T1, and the differences had statistical significance (P<0.05).After the treatment, the SBP and DBP levels in the high dosage group were lower than those in the middle dosage group and low dosage group, and those in the middle dosage group were lower than those in the low dosage group, and the differences had statistical significance (P<0.05).The NIHSS, mRS and ADL scores of the middle dosage group were lower than those of the low dosage and the high dosage group (P<0.05), and there was no significant difference between the latter two groups (P>0.05).Conclusion: With the initial dose of 15 mg, urapidil can ensure good prognosis on the premise of effective decompression.
5.How to identify the increase of peripheral white blood cells caused by glucocorticoid and infection

Objective To distinguish the increase of white blood cells after glucocorticoid is the adverse reaction of glucocor‐ticoid or infection aggravated .If we can analyse it properly ,the abuse of antimicrobials and waste of medical could be avoid re‐source .Methods 74 patients which were in our hospital from 2011 .1 to 2012 .12 were recruited into this study .They were divided into three groups ,including non‐infection with glucocorticoid(n=15) ,infection without glucocorticoid(n=29) and infection with glucocorticoid(n=30) according to disease and therapies .After being treated 0 day ,3 days ,7 days ,venous blood of patients were collected to detect white blood count ,CRP and morphology of leukocyte .Results The group of non‐infection with glucocorticoid applied glucocorticoid after 3 days and 7days ,WBC was increased significantly(P<0 .01 ,respectively) .And compare with the group of infection without glucocorticoid ,more band neutrophils could be seen in peripheral blood smear of the other two(P< 0 .05) . While there was no difference on level of CRP between the groups of infection without glucocorticoid and infection with glucocorti‐coid(P>0 .05) .Conclusion Glucocorticoid rise the count of WBC in peripheral blood especially the neutrophil .We can evaluate in‐fectious status by the level of CRP and the count of band neutrophils in peripheral blood smear .
6.Epidemiology and clinicopathological characteristics analysis of 279 neuroendocrine neoplasms cases in Dalian

Background and purpose:Neuroendocrine neoplasm is one kind of infrequent neoplasms from neuroendocrine cell and peptidergic neurons. This study aimed to investigate the epidemiology and clinicopathological characteristics of neuroendocrine neoplasms (NEN) in Dalian. Methods: We retrospectively analyzed all neuroendocrine neoplasms patients in First Afifliated Hospital of Dalian Medical University from 2000 to 2013. The epidemiology characteristics, pathogenic sites and pathological types were reviewed, and the differences between gender and age were also analyzed. The statistics analysis such as independent-samples t test and one-way ANOVA and chi-square test were performed. Results:There were 279 NEN cases, including 166 male and 113 female patients (male:female=1.14:1), detection rate was 0.258%. The mean age of all cases was 59.4±17.1 (20-83), for male 58.9±19.4 (20-81) and female 61.7±15.0 (29-83). There was no statistical signiifcance of detection rate and mean age between male and female patients. Digestive system was the most common occurred site of NEN (71.68%), then was the respiratory system (20.79%). There was no statistical significance of pathogenic sites between male and female.In all case, 132 with neuroendocrine tumor (47.31%), 140 with neuroendocrine cancer (50.18%), 7 with mixed adenoneuroendocrine carcinoma (2.51%). There was no statistical signiifcance of pathological types between male and female (P>0.05).Conclusion:Recently, the detection rate of neuroendocrine neoplasms was raised in Dalian, but there was no signiifcance differences in detection rate,pathogenic sites and pathological types between male and female.
7.The evaluation of traditional laboratory examination in identifying the rise of peripheral blood leukocytes

Objective To evaluate the role of several traditional laboratory examinations in identifying the cause of WBC count increase,which might be the adverse reaction of glucocorticoid or infection.Methods 74 patients treated in the hospital from Jan. 2011 to Dec.2012 were recruited in the study.Glucocorticoid without Infection(n =15 ),Infection without Glucocorticoid(n =29) and Infection with Glucocorticoid(n=30)Group were devided according to the type of disease and therapies.After being treated for 0,3,7 d,respectively venous blood samples of patients were collected to determine the concentration of CRP,WBC count,leukocyte classification and NAP score.CRP was used as the Judgment index for the degree of infection,the changes of other indexes were compared.Results Compared with the Infection without Glucocorticoid Group,the CRP concentration of the Infection with Glu-cocorcoid Group was becoming lower(P >0.05).The Non-infection with Glucocorticoid Group applied glucocorticoid for 3 days and 7 days respectively,the count of neutrophilic granulocyte and NAP score increased significantly compared with that before glucocor-ticoid application(P <0.01)while the count of lymphocyte reduced(P <0.05)and no significant change in the count of eosnophils (P >0.05).Conclusion The increase of WBC after glucocorticoid can not be identified whether it is the adverse reaction of glu-cocorticoid or aggravated infection by using classification of leukocyte,the increasing amount of WBC and NAP score.
8.Investigation in evaluation of various capacity requirement of probationer nursing students by nurses in operation room

Objective To investigate in evaluation of various capacity requirement of probationer nurs-ing students by nurses in operation room in order to improve clinical teaching work. Methods A self-de-signed questionnaire was applied, 191 qualified teaching nurses from nine grade three A general hospitals in Beijing were invited to fdl the questionaires. The results underwent subsequent analysis. Results The teach-ers had high expectations for various capacity of the probationer nursing students. Conclusions Clinical teachers should establish feasible teaching scheme according to the existing teaching syllabus in order to in-crease teaching quality, make students gain new insights through reviewing old materials, so that they can mas-ter various technique in operation room in short time.
9.Effect of pristimerin on experimental inflammation in mice and rats

AIM To study the anti-inflammatory effects and mechanisms of pristimerin. METHODS Several inflammatory models were established, such as ear edema induced by croton oil, hind paw swelling by carrageenan, elevation of capillary permeability by acetic acid in mice and a-cute peritonitis induced by carrageenan in rats. Protein content was measured by Coomassie brilliant blue method, nitric oxide (NO) content by Griess reaction assay, N-acetyl-?-D-glucosamini-dase (NAG) activity by colorimetry, superoxide dismutase (SOD) activity by hydroxylamine method, catalase (CAT) activity by ultraviolet spectro-photometry, and malondialdehyde (MDA) content by fluorescence method in peritoneal exudate in rats. RESULTS Pristimerin ip 0. 156 - 0. 625 mg ? kg-1 or im 1-4 mg - kg-1 inhibited ear edema, hind paw swelling, and elevation of capillary permeability in mice. In the rat peritonitis induced by carrageenan, pristimerin im 1 - 2 mg ?kg-1 reduced neutrophil counts, lessened protein and NO content, inhibited the production of MDA and decreased NAG activity, while augmented the SOD and CAT activity in exudate. CONCLUSION Pristimerin has a significant anti-inflammatory effect which may be related to the inhibition of NO production, scavenging oxygen free radicals, anti-lipoperoxidation and stabilizing lysosome membrane.
10.Expression and clinical significance of bcl-2 gene in breast cancer

Purpose:To study the expression level of bcl 2 gene in breast cancer tissues, paracancer tissues, metastatic lymph nodes and normal tissues. To study the relation of the gene expression and tumor formulation, development and clinical significance. Methods:Semi fixed quantity RT PCR was performed in 16 patients to detect the level of bcl 2 gene expression in cancer tissues, paracancer tissues, metastatic lymph nodes and normal tissues.
