1.Identification of high-risk groups of charcot foot in the people with diabetic foot neuropathic ulcerations

Objective To identify high?risk groups of Charcot foot( CN) in the people with diabetic foot neuropathic ulcerations( NU) . Methods Twenty cases patients with CN who were diagnosed in General Hospital of the Chinese People Air Force from June 2008 to June 2013 and 58 patients with diabetic neuropathic ulcer who were hospitalized from January 2010 to December 2011 and followed up until June 2014 without foot deform?ity were retrospectively analyzed. All patient's general condition, examination and laboratory results, diabetic chronic complications,complication,diabetes distribution of foot ulcers,and plain features. Results There were no statistically significant differences in terms of patients' average age, sex ratio, proportion of smokers, BMI, HbA1c,blood lipid,dorsalis pedis artery diameter and diabetic nephropathy (Ⅲ?Ⅳperiod) ,chronic kidney dis?ease stage 3 above,proliferation diabetic retinal pathological changes,the prevalence of coronary heart disease between the two groups(P>0. 05). Compared with NU group,patients with single high proportion(40. 00%(8/20) vs. 10. 34%(6/58)),Short duration of diabetes((12. 37±5. 64) years vs. (14. 27±8. 04) years),Feet long numbness(6(5,9) years vs. 4(2,20) years),low rate of hardening of the arteries narrow(ABI<0. 9)( 0 ( 0/20) vs. 39. 66%( 13/58) ) ,high recurrent diabetic foot ulcer prevalence( 70. 00%( 14/20) vs. 25. 86%( 15/58)),more patients with diabetes mellitus autonomic neuropathy(75. 00%(15/20) vs. 39. 66%(23/58)),less combined with hypertension ( 25. 00%( 5/20 ) vs. 58. 62%( 34/58 ) ) , the differences were significant ( t orχ2=6. 981,2. 259,4. 068,3. 887,12. 405,7. 436,6. 724;P<0. 05) . Diabetic foot wound distribution on mesopodi?um of CN group and NU group was 36. 84%(7/19),6. 90%(4/58) respectively,the difference was significant (χ2=11. 443,P=0. 003) . Diabetic foot amputation rate( Wanger 4,5 grade) of CN group and NU group was 44. 44%(4/9),6. 90%(2/29) respectively,the difference was significant(χ2=4. 732,P=0. 020). Conclusion The characteristics of high?risk groups of diabetics Charcot foot in the people with diabetic foot neuropathic ulcerations are middle aged,no foot of ischemia,combine the diabetic autonomic neuropathy and the feet always with recurrent ulcers.
2.p73 gene protein expression in lung cancer as related to clinicopathological characteristics and prognosis in the elderly

Objective To study the expression of p73 gene in lung cancer and tumorigenesis, progression of lung cancer in the elderly. Methods The expression of p73 protein in 65 cases of lung cancer, adjacent tissues of cancer and normal tissues was determined by immunohistochemical SABC method. The results were analysed by combining the clinicopathological data and the prognosis. 3.Effect of total parenteral nutrition with glutamine on the plasma endotoxin in the MODS patients

Objective:To determine the effect of total parenteral nutrition(TPN) supplemented with glutamine(Gln) on the concentration of plasma lipopolysaccharide(LPS)of the multiple organ dysfunction syndrome(MODS) patients.Methods:40 MODS patients in the ICU were randomly divided into routine group(Group A,n=20),the Gln+TPN treatment Group (Group B,n=20).The two groups of patients received treatment of parenteral nutrition with equal nitrogen and equal calorie.Group A were given routine TPN,and Group B were given routine TPN and Gln 0.27 g/(kg?d)[i.e.dipeptiven 0.4 g/(kg?d)] for seven days.The concentration of plasma Gln and LPS were checked before the treatment of TPN and on the first,the third and the seventh day after the treatment of TPN.At the same time 20 healthy blood donators were chosen as the healthy control group(Group C).Results:Before treatment the concentration of plasma Gln of the two patient groups were both lower than that of Group C(P0.05).On the first,the third and the seventh day of the treatment of TPN,the concentration of plasma Gln of Group A was lower than that of Group B(P0.05).After the treatment of Gln plus TPN,the concentration of plasma LPS of Group B was lower than that of Group A(P0.05).Conclusion:The Gln(dipeptiven) can help to decrease the concentration of plasma LPS and the TPN treatment time of the MODS patients.
4.Effect of ketamine on spatial learning and memory in a rat model of incisional pain

Objective To investigate the effect of ketamine on spatial learning and memory in a rat model of acute pain produced by an incision in plantar area. Methods Seventy-two 3-month old male SD rats weighing 200-250g were randomly divided into 3 groups: (1) control group (C) received no plantar incision (n = 24); (2) acute pain group (M) received an incision in the sole of the hindpaw according to the method described by Brennan (n = 24) and (3) ketamine group (K) received plantar incision and intraperitoneal ketamine 10 mg?kg-1 every day for 7 days starting from the day when plantar incision was made ( n = 24). In group C and M normal saline (NS) was given i.p. instead of ketamine. The three groups were further divided into 2 subgroups according to the time when Morris water maze (MWM) testing was started-1 week (C1, M1,K1) or 3 weeks (C3, M3, K3) after plantar incision. MWM test was performed 4 times a day for 6 consecutive days. The latent periods and swimming distances were recorded automatically by MWM monitoring system. Six days after the last MWM testing the animals were anesthetized and killed. The hippocampus was removed for microscopic examination. 5.Effects of profound hemodilution with 6% HES on coagulation and platelet function curve

Objective To study the effects of profound hemodilution with 6% HES (200/0.5) on coagulation.Methods Ten male new Zealand long-ear rabbits weighing (2.43 ?0.19) kg were anesthetized with intravenous thiopentone 8 mg-1 and tracheotomized and mechanically ventilated (VT = 15 ml-1 , RR = 24 bpm) . Anesthesia was maintained with intravenous infusion of thiopentone and succinylcholine. Femoral artery and vein were cannulated for BP and CVP monitoring. Hemodilution was performed in 6 steps at 30 min intervals. Blood was withdrawn from artery and simultaneously replaced by intravenous infusion of equal volume of 6% HES until Hct was 5 % -8 % . Blood samples were taken before hemodilution and 30 min after each step of hemodilution for determination of coagulation function curve using Sonoclot coagulation and platelet function analyzer. Results When Hct was 25%-30% there was no significant difference in coagulation function before and after hemodilution. 7.Construction of recombinant adeno-associated virus vector with human preproenkephalin gene

Objective To construct the recombinant adeno-associated virus vector with human preproenkephalin gene (rAAV-hPPE). Methods The human preproenkephalin (hPPE) gene was cloned into the adeno-associated virus (AAV) vector plasmid pSNAV which contained neo expression box. The recombinant pSNAV-hPPE was then transfected into BHK cells using lipofectamine?2000. The G418-resistant cells, BHK / SH1, were obtained. The BHK / SH1 cells were infected with HSV1-rc /△UL2 which has the function of packaging the recombinant AAV(rAAV) . After purification, the construction of rAAV-hPPE was achieved.Results The construction of pSNAV-hPPE was confirmed by digestion with restriction enzyme. Southeon bolting was used to detect the virus liters (2.5 ? 1212 v.g /ml) .Conclusion This rAAV-hPPE virus vector with high liter and strong infectivity can be used in transgenic analgesic research.
8.Effect of sevoflurane postconditioning on expression of Pim-1 kinase during myocardial ischemia-reperfusion in rats: an in vitro experiment

Objective To evaluate the effect of sevoflurane postconditioning on the expression of Pim-1 kinase during myocardial ischemia-reperfusion (I/R) in rats.Methods Male Sprague-Dawley rats,weighing 250-300 g,were used in this study.After the animals were anesthetized,their hearts were immediately removed and retrogradely perfused with an oxygenated K-H solution at 37 ℃ in a Langendorff apparatus.Thirty-six isolated rat hearts were assigned into 3 groups (n=12 each) using a random number table:control group (group C),group I/R and sevoflurane postconditioning group (group SP).The hearts were subjected to ischemia for 30 min followed by 120 min of reperfusion to establish the model of myocardial I/R injury.In group SP,the hearts were perfused with K-H solution saturated with 3% sevoflurane for 15 min starting from the beginning of reperfusion.Heart rate (HR),left ventricular end-diastolic pressure (LVEDP),left ventricular developed pressure (LVDP) and the maximum rate of increase and decrease of left ventricular pressure (±dp/dtmax)were recorded at the end of equilibration and 30,60,90and 120 min of reperfusion.Myocardial tissues were obtained at T2 for determination of the expression of Pim-1 kinase,Bcl-2 and cytochrome c (Cyt c) in cytoplasm and mitochondria by Western blot.The hearts were selected at T4 for determination of myocardial infarct size (by TTC staining) and for examination of mitochondrial ultrastructure (with transmission electron microscope).Results Compared with group C,HR,LVDP and ±dp/dtmax were significantly decreased,and LVEDP was increased at T1-4,the myocardial infarct size was enlarged,the expression of Pim-1 and Bcl-2 in cytoplasm and mitochondria was down-regulated,the expression of Cyt c in cytoplasm was up-regulated,and the expression of Cyt c in mitochondria was down-regulated in group I/R (P<0.05).Compared with group I/R,HR,LVDP and ±dp/ dtmax were significantly increased,and LVEDP was decreased at T1-4,the myocardial infarct size was decreased,the expression of Pim-1 kinase and Bcl-2 in cytoplasm and mitochondria was up-regulated,the expression of Cyt c in cytoplasm was down-regulated,and the expression of Cyt c in mitochondria was upregulated in group SP (P<0.05).The damage to mitochondria was significantly attenuated in group SP as compared with group I/R,and was aggravated as compared with group C.Conclusion The mechanism by which sevoflurane postconditioning attenuates myocardial I/R may be related to up-regulation of Pim-1 kinase expression in rats.
9.Effect of chronic intermittent hypoxia on the activities of apoptosis regulating factor cysteine-containing aspartate-specific protease-3 and oxidative stress marker myeloperoxidase in cardiomyocyte in rats fed a high-fat diet

Objective To investigate the effect of chronic intermittent hypoxia (CIH) on myocardial tissue pathology,oxidative stress and apoptosis in rat fed a high-fat diet,and to explore the possible mechanism of CIH induced cardiomyocyte injury.Methods A total of 24 male Wistar rats were randomly divided into 3 groups (n=8 each).The control group was fed common rat forage,the high-fat group was fed high-fat forage,and the high-fat plus intermittent hypoxia group was fed high-fat forage combined with a 7h/d intermittent hypoxia treatment.The changes of myocardial tissue pathology and ultrastructure of cardiomyocyte,and the activities of apoptosis regulating factor cysteine-containing aspartate-specific proteases-3 (caspase-3) and oxidative stress marker myeloperoxidase (MPO) were observed in the 3 groups after 4 weeks of treatment.Results There were significant differences in the activities of caspase-3 and MPO among the three group (F=89.94,71.24,both P=0.001).The activities of caspase-3 and MPO were lower in the control group than in the high-fat group and in high fat plus intermittent hypoxia group [(0.21±0.06) vs.(0.80±0.11),(1.15±0.21),(3.20±0.58) vs.(10.87±1.96),(13.17±2.22),P<0.01].The activities of caspase-3 and MPO were higher in the high-fat plus intermittent hypoxia group than in the high fat group[(1.15±0.21) vs.(0.80±0.11),(13.17±2.22) vs.(10.87±1.96),P<0.01].No abnormal findings in the structure of cardiomyocyte were observed in the control group,while multiple pathologic damages in cardiomyocyte were detected in the high-fat group,and more obvious injuries in the high-fat plus intermittent hypoxia group.Conclusions The pathologic damages to cardiomyocyte are more serious in high fat and intermittent hypoxia group than in the high-fat group.Apoptosis induced by oxidative stress may play an important role in the pathogenesis of these injuries.
10.Analysis of serum lipid levels in elderly female cadre population in Tianjin

Objective To investigate the trend of blood lipid in female elderly with aging, and further discuss the strate?gies of blood lipid regulation therapy and prevent and control cardiovascular disease during senile period. Methods Physi?cal examination data of 847 elderly female cadres (in 1 837 females) were analyzed to investigate the general level and abnor?mality features of blood lipid in this elderly female group. Results The levels of total cholesterol (TC), triacylglycerol (TG), low density lipoprotein-cholesterol (LDL-C) and body mass index (BMI) were significantly higher in elderly group than those of non-elderly group (P<0.01). The levels of TC, TG, LDL-C, LDL-C/HDL-C, TG/HDL-C, and non-HDL-C increased with aging, and then decreased at the same age group, and reached the peak value at 60 to 69 years old. The level of HDL-C showed a reverse pattern, decreased with aging first and then increased at the same group, and reached the bottom at 60 to 69 years old. Conclusion In this elderly female cadre population, the level of blood lipid changes regularly with aging, indicat?ing the high risk of cardiovascular disease.
