1.Effect of Qingyi Granule on HMGB1 Expression in Liver and Renal Tissues of Severe Acute Pancreatitis Rats.

OBJECTIVETo explore the effect of Qingyi Granule (QYG) on high mobility group box-1 (HMGB1) expressions in liver and renal tissues of severe acute pancreatitis (SAP) rats.
METHODSFifty-four Sprague-Dawley (SD) rats were divided into the sham-operation (SO) group, the SAP group, and the QYG group according to random digits table. Rats in the SAP group were induced by injecting 5% sodium taurocholate (STC). Liver and renal pathological changes were observed by HE staining. Serum contents of amylase (AMS), MDA, IL-1, and HMGB1 were detected by ELISA. HMGB1 protein expressions in liver and renal tissues were tested by immunohistochemistry. HMGB1 mRNA expressions in liver and renal tissues were detected by reversed transcription PCR.
RESULTSThe pathological scores, serum levels of AMS, MDA, IL-1 and HMGB1, and protein and mRNA HMGB1 expressions in liver and renal tissues were increased more obviously in the SAP group than in the SO group (P < 0.05, P < 0.01). All of them could be down-regulated by QYG intervention, with the most significant effect seen at 72 h (P < 0.05, P < 0.01) in a time-effect relationship.
CONCLUSIONSHMGB1 participated in SAP complicated liver and renal injuries. QYG could effectively inhibit HMGB1 expressions, thereby attenuating SAP complicated liver and renal injuries.

Long-term potentiation(LTP) of synaptic activity in the hippocampal is the most widely researched model of synaptic plasticity,which is believed to underlie the brain function of learning and memory.Mitogen-activated protein kinases(MAPK) respond to a variety of cellular and extracellular stimuli, such as growth factors,cytokines,extracellular mitogen and stresses.MAPK are involved in complex processes such as in cell differentiation,proliferation and programmed cell death.It has been reported that the upstream regulators and downstream substrats of MAPKs still widely exist in the mature neuron.MAPKs cascade induces phosphorylation of many functional protein including some receptors and kinases that is associated with induction and maintain of the LTP in the neuron,indicating that MAPKs do play a crucial role in the synaptic plasticity.Extracellular signal-regulated kinase(ERK),c-Jun N-terminal kinase(JNK) and p38 activity and its relationship with LTP are reviewed.
4.Treatment of Gustilo III distal tibiofibula fractures through trans-fibular anterior-lateral approach.

OBJECTIVETo explore the methods and therapeutic effects of trans-fibular anterior-lateral approach combined with external fixation in the treatment of Gustilo III distal tibiofibula fractures.
METHODSFrom 2007 to 2010,9 patients including 7 males and 2 females with the mean age of 40 years(ranging from 29 to 51 years). All patients received internal fixation of fibula after debridement on the first phase, external fixator were used to fix tibia across ankle joint, and removed after successful skin graft; The second phase tibia was used to fix through the lateral incision used in phase I. Early functional exercise was encouraged ,the union condition and functional results of the ankle joint was evealuated. The criteria of the AOFAS Foot and Ankle Surgery was used to evaluate the effects.
RESULTSAll patients were followed up,and the duration ranged for 8 to 37 months(averaged 21 months). Nine patients were achieved bony union, the average healing time was 24 weeks. No plate rupture or screw loosening was found. According to the AOFAS Foot and Ankle Surgery evaluation system, 3 cases got excellent results, 4 good cases and 2 fair.
CONCLUSIONTrans-fibular anterior-lateral approach combined with external fixation for Gustilo III distal tibiofibula fractures can receive satisfactory reset, debond ankle joint eralier and imporove the clinical effects.

7.Characterization of capsid protein VP3-VP1 genes of hepatitis A virus prevalent strains circulated in China.

OBJECTIVETo analyse the genetic characteristics of the capsid protein VP3-VP1 region of hepatitis A virus strains circulated in China.
METHODSThe nucleotide sequences of VP3-VP1-2A region of 42 HAV IgM positive serum samples were sequenced and analysed for nucleotide and amino acid identities and genetic characteristics of the VP3-VP1 region.
RESULTSThe nucleotide and amino acid identities in the VP1-2A junction region among the 42 strains were 89.1% - 100% and 97.3% - 100%; while in the complete VP3-VP1 region, the identities were 87.6%-100% and 98.8%-100%. Strains with identical nucleotide sequences in the VP1-2A junction region had 98.4%-100% nucleotide identity in the complete VP3-VP1 region and 0-2 amino acid differences in this region. There were no amino acid changes at neutralizing antigenetic sites of VP3-VP1 region within the 42 HAV strains.
CONCLUSIONAll the 42 HAV strains belonged to genotype I, with 40 strains clustering to sub-genotype IA and 2 to sub-genotype IB. Different HAV strains analysed in this paper differed in the nucleotide sequences of the VP3-VP1 region, but the amino acid sequences were highly conserved with no changes at neutralizing antigenetic sites. Both the nucleotide and amino acid sequences of the strains with the same VP1-2A junction region were identical or closely related when compared in the complete VP3-VP1 region.

8.Seroepidemiological survey of sheep hepatitis E virus infection in Aksu region of Xinjiang Autonomous.

Four hundreds and ninety sheep sera from seven breeds raised at eight counties and one city of Aksu region in Xinjiang were tested by ELISA for the presence of antibodies against hepatitis E virus (HEV), and the positive rate differences were compared based on administrative areas, breeds and age by Chi-square test. The result showed that the general positive rate was 28.98% (142/490), the positive rate were 35.44% (28/79), 29.67% (27/91), 20% (4/20), 40% (12/30), 32.5% (26/80), 38% (19/50), 22.5% (9/40), 8% (4/50) and 26% (13/50) respectively in eight counties and one city, there was a significant difference between Xayar and other administrative areas (P<0.01); there was also a significant difference among age ranges (P<0.01), being 38.75% (31/80) over 2 years old, 15.45% (17/110) below 1 year old; The seroprevalence was still related to breeds, i. e. there was a significant difference between Mongolia sheep and other breeds (P<0.01). From these data, it is confirmed that there is a possibility of previous and potential infection of sheep HEV in Aksu region of Xinjiang Autonomous.

Background Endophthalmitis is a serious,sight-threatening condition.Identifying the causative microorganisms is very important for available treatment of endophthalmitis.Objective This survey was to analyze the spectrum of organisms causing culture-proven endophthalmitis and their sensitivities to commonly antimicrobial agents.Methods Medical data of patients with culture-proven endophthalmitis at Zhongshan Ophthalmic Center from January 2003 through December 2010 were respectively reviewed.The outcomes included intravitreal isolates and antibiotic sensitivities were analyzed.Results Four hundred and sixty-nine strains of organisms were isolated from 447 eyes of 447 patients with infective endophthalmitis,including 22 eyes of polymicrobial infection.In the organisms,gram-positive organisms were 241 (51.4%),fungi were 125 (26.7%) and gram-negative organisms were 103 (22.0%).The most common organisms were Staphylococcus epidermidis in 29.4%,Aspergillus in 7.7% and Pseudomonas aeruginosa in 5.3%.In this group of infective patients,the most common clinic settings were posttraumatic endophthalmitis (72.7%),and then were postoperative endophthalmitis (10.5%),endogenous endophthalmitis (9.8%) and keratitis (6.9%).Most gram-positive organism and gram-negative organism were sensitive to levofloxacin and cefoperazone.The susceptibility rate of gram-positive organism to chloromycetin was increased in 2007-2010 years compared with 2003-2006 years (x2=5.398,P<0.05).The susceptibility rate to ciprofloxacin of gram-negative organisms declined (x2 =5.398,P < 0.05),but that to rifampicin increased in the duration of 2007-2010 compared with 2003-2006 (x2 =4.500,P < 0.05).Conclusions Gram-positive organisms are the most commonly causative organisms of endophthalmitis.Most bacterial organisms are sensitive to levofloxacin and cefoperazone.Local data of culturing and susceptibility test offers a guideline for the treatment of infectious endophthalmitis.
