3.Post-Operative Treatment and Intensive Care of Infants with Congenital Heart Disease

Objective To explore post-operative treatment and intensive care of infants with congenital heart disease(CHD).Methods Clinical data of post-operative treatment and intensive care in 264 infants with CHD were analyzed retrospectively from May.1998 to Apr.2006.Among them,256 cases underwent radical operation,and 8 cases with palliative operation.Results Sixteen cases died early.Among them,after operation,there were 8 cases with Fallot′s syndrome,2 cases with complete atrioventricular canal anomalous,3 cases with ventricular septal defect(VSD)and pulmonary hypoplasia(PH),1 case with tricuspid atresia,1 case with single ventricle,and 1 case with persistent truncus arteriosus.Causes of death include low cardiac output,acute renal failure,degree of Ⅲ atrioventricular block(AVB),lung infection,respiratory failure and arrhythmia.One case with complete pulmonary vein ectopia was discharged due to post-opertative refractory hypercapnia;1 case with atrial septal defect(ASD)and patent ductus arteriosus(PDA) died of refractory heart failure due to high pulmonary arterial pressure 1 year after operation;1 case with tetralogy of Fallot died of lung hemorrhage after radical correct operation followed by palliative outflow tract deo-ppilant operation.Of them 245 cases alive,there were 233 cases with heart function grade Ⅰ,12 cases with grade Ⅱ.Conclusion Correct management of respiratory system,systemic electrocardial care,handling with arrhthymia on time,low cardiac output and nutritional support are the important measures in improving survival rate of infants with CHD.
4.Research progress of bone marrow-derived mesenchymal stem cells in blood diseases

Bone marrow mesenchymal stem cells (MSC) are the non-hematopoietic cellular component in the bone marrow that have multiple differentiation potency. MSC play a key role in the regulation of bone marrow hematopoietic niche and modulation of immune function through various mechanisms. They are currently recognized as a promising cell source in tissue engineering, a vehicle in gene therapy and a powerful tool in the management of graft-versus-host disease after allogeneic stem cell transplantation. In this review some momentous aspects regarding the current status and potential clinical applications of MSC in hematopoietic stem cell transplantation, aplastic anemia and multiple myeloma were summarized.

6.Preliminary study on the correlation between regulation of Cox-2 gene expression and radiosensitivity of esophageal cancer

Objective To investigate the mechanism of the radiosensitivity effect of Cox-2 gene in esophageal cancer.Methods Cox-2 specific siRNA was constructed and transfected to EC9706 cells to downregulate intracellular Cox-2 expression.The expressions of MMP-2,Bcl-2 mRNA,AKT and phosphorylated AKT proteins were assayed after radiation.Colony formation,cell proliferation,apoptosis and cell invasion in vitro were examined as well.One-way ANOVA method was used to analyze the data.Results Affter 2 and 4 Gy irradiation,a significant increase in the mRNA expression of Bcl-2 was observed in the Cox-2 up-regulation group (F =3.36,4.32,P < 0.05).In the group of Cox-2 downregulation,the expression of MMP-2 mRNA was significantly reduced(F =3.86,8.09,P < 0.05).Affter irradiation,a significant decreaseof Bcl-2 mRNA (F =3.73,5.64,P < 0.05) as well as an increase of Bax(F =7.03,7.42,P < 0.05) was detected,and the levels of total and phosphorylated AKT proteins had the highest level in the Cox-2 upregulation group and had the lowest level in the Cox-2 downregulation group.In the Cox-2 downregulation group,the apoptosis induction obviously increased with dose (F =317.40,P < 0.05),and the proportion of cells in Go-G1 phase gradually increased but the proportion of cells in S and G2-M phases decreased,concomitant with the obvious suppression of cell proliferation,in addition,cell invasion was decreased.Conclusions Downregulation of intracellular Cox-2 mRNA expression,concomitant with subsequent downregulation of MMP-2 and Bcl-2 and upregulation of Bax,resulted in reduction of the invasion and metastatic capabilities of tumor cells,and induction of Go-G1 phase arrest and apoptosis.Downregulation of AKT and phosphorylated AKT (pAKT) protein expression might also interfere with the capability of the PI3K/Akt signal transduction pathway to resist radiotherapy.
7.Effects of TGF-β3 combined with dental pulp stem cells on the repair of rabbit facial nerve injury

ObjectiveTo investigate the effects of using transforming growth factor-β3 (TGF-β3) combined with dental pulp stem cells (DPSCs) on the regeneration of rabbit facial nerve.MethodsSixteen New Zealand adult rabbits were selected randomly.These rabbits were divided into three groups (TGF-β3 and DPSCs treated group,TGF-β3 control group and PBS control group.treatment group (silicone guidance channel with collagen protein sponge were filled with TGF-β3 and DPSCs),TGF-β3 control group (silicone guidance channel with collagen protein sponge were filled with TGF-β3) and PBS control group(silicone guidance channel with collagen protein sponge were filled with PBS).After 3 months,a series of examinations were performed,including gross morphology,histological staining,neuroelectrophysiological tset.ResultsIn the experimental group,the diameter of the regenerating nerve and near distal nerve stem were almost the same.There were no formation of neuroma.The adventitial angiogenesis was rich and with tough texture.3 months after operation,in the experimental group,the facial nerve membrane integrity,nerve fibers arranged in neat rows,form a more complete,myelin swelling.The total number of regeneration of nerve fibers in the experimental group were more than of control groups,statistical analysis was significant (P<0.05).Diameter of regenerating nerve fibers in the experimental group were greater than that of control groups,statistical analysis was significant (P<0.05).The ultra-thin section showed that the regenerated fibers in the treatment group were mainly myelinated never fiber.The layer structure of myelin sheath was clear,and there were rich organells axoplasma.The neuroelectrophysiological examinations revealed that the latency of nerve and muscle action conduction in the treatment group was shorter than that of the control groups,the treatment group:(1.96±0.32) ms,the TGF-β3 control group:(2.35±0.41) ms,the PBS control group:(3.42±0.55) ms.The wave amplitude of nerve and muscle action conduction in the treatment group was obviously higher than that of control groups,the treatment group:(11.06±3.25) mV,the TGF-β3 control group:(8.40± 1.68) mV,the PBS control group:(4.62±0.77) mV.ConclusionThe combination of TGF-β3 and DPSCs can improve the effects on the repair of facial nerve injury.
8.Relationship between platelet distribution width, fibrinogen and severity of diabetic retinopathy

AIM: To define the relationship between platelet distribution width ( PDW) , fibrinogen ( FIB) and severity of diabetic retinopathy ( DR) .
METHODS: The survey included 99 patients with DR (48 with non-proliferative and 51 with proliferative DR) in our hospital during June 2012 and May 2014. Another 50 diabetic patients without DR and 50 healthy volunteers were matched as controls. Demographic data and disease history were gained. Fasting blood sample were collected to measure PDW, FIB, platelet count, fasting blood glucose and HbA1c.
RESULTS: Compared with healthy controls ( 16. 6%±1.2%) , a significant difference was found in PDW values among diabetic patients ( all P < 0. 05 ). The higher development of DR corresponded with a significantly higher level of PDW 17. 6%±1. 8%, 19. 1%±2. 1%, and 20%±1. 9% for patients without DR, non-proliferative DR and proliferative DR, respectively, the difference had statistical significance (P<0. 05). A significant difference was also found in FIB values among diabetic patients and healthy controls (P<0. 05). After correction for age,
gender, disease duration and HbA1c, multi factor Logistic analysis showed that there were significant increased risks in the prevalence of non-proliferative ( OR: 1. 464, PDW) ( OR: 2. 199, FIB) and proliferative DR ( OR: 1. 652, PDW ) ( OR: 2. 691, FIB ) with the increased PDW and FIB value (all P<0. 05).
CONCLUSION: The PDW and FIB value are parallel with the severity of DR, and there is increased risk of non-proliferative and proliferative DR with the PDW and FIB value increases.
9.Screening of KIF21 A gene mutation in a Han family with concomitant exotropia

AIM:To study the KIF21A gene mutation in a Han family with concomitant exotropia. ·METHODS: The genomic DNA of five family members was extracted from peripheral blood leukocytes and amplified with PCR. The PCR products were purified for DNA sequencing. DNA sequences were aligned with the human KIF21A gene sequences registered in GenBank. · RESULTS: Mutation analysis of all exons of the pedigree's KIF21A gene reveals no gene mutation in any of the families. ·CONCLUSION: Our study demonstrates that the KIF21A gene maybe is not virulence gene in this pedigree.
10.Risk factors and predictive biomarkers of immune checkpoint inhibitor-associated pneumonia in non-small cell lung cancer

The use of immune checkpoint inhibitors (ICIs) has changed the clinical outcome of non-small cell lung cancer (NSCLC), with the widespread application of ICIs, immune-related adverse events (irAEs) have also appeared. Immune checkpoint inhibitor pneumonitis (CIP) is a serious adverse event of ICIs treatment that needs attention. Therefore, early identification of high-risk groups of CIPs and early intervention can reduce the occurrence of permanent drug withdrawal and severe CIPs, thereby improving patients′ prognosis.
