1. Reserved sutures for incision closure of transvesical natural orifice transluminal endoscopic surgery in pigs
Academic Journal of Second Military Medical University 2011;32(5):490-493
Objective: To close the incision of transvesical natural orifice transluminal endoscopic surgery (NOTES) with reserved sutures in pigs, and to assess the feasibility and safety of the technique, so as to lay a foundation for future clinical application. Methods: Five female pigs were inserted with self-made Trocar into the bladder under general anesthesia. Guided by ureteroscope, the needle passed the top wall of the anterior bladder and reached the abdominal wall, with two suture ends reserved for closure, one at the abdominal wall and the other in the urethra (out of Trocar). After finishing the transvesical NOTES, we pulled the suture end at the abdominal wall to the bladder and knotted outside the Trocar; then we dragged the urethra suture end to make the knot on the abdominal wall locating on the lateral wall of bladder, and then the urethral suture was knotted in Trocar and the knot was pushed to the bladder wall using a knot pushing device; finally the bladder incision was closed. The closure of the bladder incision was observed and tested with water filing method. Results: The first two cases failed due to the overweight and large size of the pigs, or because the bladder incision was to close to the bladder neck, or due to the poor quality of the suture. The procedures were successful in the following 3 pigs, with the operation time periods being 45 min, 30 min, and 25 min, and with satisfactory closure. Conclusion: Reserved suture for incision closure of transvesical NOTES procedure is safe and feasible; the technique is easy to perform and needs no special surgery devices; and it deserves further study.

Aim To study the effects of a novel psychoactive drug, modafinil,on the GABAA receptor-induced current in rat hippocampus pyramidal neurons. Methods Whole-cell patch-clamp recordings were applied in cultured hippocampus pyramidal neurons. Results The GABA-activated currents were blocked by bicuculline,a competitive antagonist of GABAA receptor.Modafinil could partly inhibit IGABA.Pre-treating hippocampus neurons with glibenclamide for 30 min,modafinil-induced inhibition on IGABA was removed. Conclusion Modafinil is different from other psychostimulants. The protection of modafinil may be via modulating ATP sensitive potassium channel activation.

Objective Adenosine receptor agonist NECA has a certain myocardial protection, but the specific mechanism is not clear. This paper aimed to study the effect and mechanism of NECA inhibiting endoplasmic reticulum stress to against myocardial ischemia reperfusion injury in rats. Methods 56 Wistar rats of SPF grade were selected and divided into Sham group, I/R group, NECA group and TUDCA group through random number table method. We established the isolated rat heart ischemia reperfusion model by using the Langendorff device.Sham group: heart threaded but not ligated, Kerb-Henseleit buffer continuous infusion 170min;I/R group: heart stability 20min, ischemia 30min, reperfusion 2h;NECA group and TUDCA group: heart stability 20min, ischemia 30min, reperfusion 2h, perfusion solutions containing 0.1μmol/L NECA and 30μmol/L TUDCA were respectively given at 5min before reperfusion, and ended at 30min after reperfusion.Transmission electron microscope was used to evaluate alterations of the myocardial ultrastructures.Western blot analysis was used to detected the expression levels of endoplasmic reticulum stress IREl-XBPl signaling pathway marker protein IRE1α, XBP1s.Immunohistochemical staining was used to detect the expression of IRE1α.Results The results of transmission electron microscopy showed that most of the myofilament ruptured, sarcomere contracture deformation, visible mitochondrial vacuoles degeneration in I/R group, and injury in NECA group and TUDCA group were less than the I/R group, appeared as the filaments arranged more neat, sarcoma only had mild contracture.Immunohistochemical results showed that IRE1αpositive staining was not found in the sham group, and the area of positive staining of IRE1α in I/R group was significantly increased, while the NECA group and TUDCA group were significantly decreased.Compared to the Sham group, the expression level of IRE1α and XBP1s was significantly increased in I/R group(P<0.05);but compared with the expression level of IREα and XBP1s in I/R group(1.72±0.27, 0.97±0.19), the NECA group(1.14±0.16, 0.6±0.13) and the TUDCA(1.07±0.27, 0.58±0.15) group were significantly decreased(P<0.05).Conclusion NECA can reduce endoplasmic reticulum stress through inhibiting IREl-XBPl pathway to protect the myocardium.

Aim To observe the neuroprotective effect of platelet activating factor receptor antagonist Kadsurenone on rat brain with cerebral ischemia/reperfusion.Methods Animal model was established through middle cerebral artery occlusion(MCAO).we studied the changes of infarct volume,NAA and Lactate in Kadsurenone-treated group with DWI and()~1H-MRS examination after cerebral ischemia 60 min and reperfusion 1,3,6 h.Results Significant reductions in infarct volume were found in the Kadsurenone-treated group as well as in the Ginkgolides-treated group compared with the control group. And Kadsurenone decreased the concentration of lactate and prevented the decline of the concentration of NAA after cerebral ischemia/reperfusion.Conclusion As platelet-activating factor receptor antagonists, both Kadsurenone and Ginkgolides show remarkable neuroprotective effect. And MRI offers exact neuroimaging information for studying the neuroprotective meachnism of Chinese traditional medicine after cerebral ischemia and reperfusion.

Objective To investigate the effects of Kinesio taping on function of knees in girls. Methods 20 female students were tested the peak torque of centripedal and eccentric contraction of knee flexion and extension with Biodex isokinetic testing at 60°/s angular velocity, with Kinesio taping, placebo taping and without taping. Their amplitude root of mean square (RMS) of surface electromyography of vastus medialis (VM), rectus femoris (RM) and vastus lateralis (VL) were also recorded. Results For centripedal isokinetic contraction, the relative extensors peak torque was the most as Kinesio taping (P<0.01). There was no significant difference in relative flexor peak torque (P> 0.05). Standardized RMS of VM and VL were the least as Kinesio taping (P<0.01), but not significantly different of RF among all the conditions (P>0.05). For eccentric isokinetic contraction, the relative extensor peak torque of both extensors and flexors were the most as Kinesio taping (P<0.01), while the Standardized RMS of VM and VL were the least (P<0.01), but no significant difference was observed in RF (P> 0.05). Conclusion Kinesio taping may enhance the strength of centripedal, eccentric contractions of quadriceps and eccentric contractions of hamstring, and increase the muscle fibers recruitment of VM and VL.
