1.Microsurgical management of urethral fistula occurred after urethroplasty in hypospadias

Objective To evaluate the role of mierosurgery in surgical treatment to urethrocutaneous fistula after urethroplasty in hypospadias and improve surgical results.Methods From 1999 to 2006,44 urethrocutaneous fistulae (more than 3mm in diameter) after urethroplasty for hypospadias in 33 patients were repaired with different skin flaps.For example,Thiersch technique,urthroplasty,etc.Microsurgical tech- nique was employed in every case.Results The success rates of different procedure were 84.8% (28/33) for Thiersch technique,100% (11/11) for urethroplasty respectively.The total success rate was 88.6%(39/ 44).Conclusion It's just application of skin flap for repairing of big or complex urinary fistula after hypos- padias surgery.The application of microsurgical technique can increase success rate.It is necessary to excise scar and partial urethra for hypospadias fistula combined with urethral structures,cicatricial eontracture and in- curvation of penis.Rich blood-supply,low tension and atraumatic technique are all very important to improve surgery success rates of urinary,fistula after hypospadias repair.
2.Effect of recombinant human erythropoietin on expressions of matrix metalloproteinase-9 and BCL-2 in the cerebal cortex after occlusion/reperfusion in rats

Objective To investigate the possible mechanism of recombinant human erythropoietin (rhEPO) neuroprotection by studying the effect of rhEPO on expressions of matrix metalloproteinase-9 (MMP-9) and BCL-2 following focal cerebral ischemia-reperfusion in rats. Methods A rat middle cerebral artery occlusion/reperfusion (MCAO/R) model was induced by the intraluminal filament method, and intraperitoneal injection of rhEPO was used for intervention. Histopathological changes were observed by HE staining, and the expressions of MMP-9 and BCL-2 in the cerebral cortex of ischemic side were detected with immunohisto-chemistry. Results HE staining: At all time points, the numbers of surviving nerve cells were significantly higher in the rhEPO group, and their injury degree was significantly lower. MMP-9 immunohistochemistry staining: The positive cells were observed occasionally in the normal control group and the sham-operation group; the MMP-9 positive cells at the ischemic side of brain tissue in a normal saline control group began to appear at 6 hours after reperfusion, it reached the peak at 24 hours and began to decrease at 72 hours; the change trend of MMP-9 positive cells in the rhEPO group was similar to that in the normal saline control group, but it was significantly lower than that in the normal saline control group at the same time points (t were 12. 023 6, 12. 635 0, 12. 779 6, respectively, all P <0. 01). BCL-2 immunohistochemistry staining: No positive cells were found in the normal control group and sham-operation group. The numbers of BCL-2 positive cells reached the peak at the ischemic side of brain tissue in the normal saline control group at 6 hours after reperfusion, it reached the peak at 24 hours and further decreased at 72 hours; the change trend of BCL-2 positive cells in the rhEPO group was similar to that in the normal saline control group, but it was significantly higher than that in the normal saline control group at the same time points (t were 5. 763 1,8. 110 1, and 5. 798 7, respectively, all P <0. 01). Conclusions rhEPO may inhibit cortical neuronal apop-tosis at the ischemic side by inhibiting MMP-9 expression and up-regulating BCL-2 expression so as to play a neuroprotective effect.

Choroidal metastasis is one of the most common malignant tumors inside the eyes. It causes pain, hypopsia and some other related symptoms. It reduces the quality of the patients' life. It's significant for the patients to be detected and treated early, therefore they will have better vision and longer life. The treatments of choroidal metastasis are developing quickly. Both the vitreous cavity injection of targeted drug and gene therapy are hot topics of research. This paper summarizes the etiology, development, diagnosis and treatment of choroidal metastasis nowadays.
4.Effect of Erythropoietin on Inflammation of Cerebral Ischemia-reperfusion Injury in Rats

Objective To explore the protective effect of erythropoietin(EPO)on inflammatory injury induced by focal cerebral ischemia/reperfusion in rats.Methods 54 male Sprague-Dawley rats were randomly divided into 3 groups:sham operation group,normal saline control group and EPO group.The focal cerebral ischemia-reperfusion injury model was made by suture-occluded method.The content of interleukin-1 β(IL-1β)and the activity of myeloperoxidease(MPO)were measured by radioimmunoassay and chromatoptometry at 6 h,24 h and 48 h reperfusion following ischemia 2 h.The scores of behavior obstacle in rats were assessed at 24 h.Results Compared with the normal saline control group,EPO group got better score of behavior obstacle(all P<0.05).The content of IL-1β and the activity of MPO in brain in the EPO group were significantly lower than those in the normal saline control group(all P<0.01).Compared with two reperfusion groups,the sham operation group had no obvious abnormal change in each measurement item.Conclusion EPO can reduce the content of IL-1β and inhibit the infiltration of leukoeyte,which can provide protective effect on cerebral ischemic-repefusion injury in rats.

Objective To explore the relationship between children obesity and adiponectin(ADPN),leptin(LP),and high-sensitive C-reactive protein(hsCRP).Methods Seventy-one obese volunteers as obesity group and 30 normal volunteers who matched obese volunteers in age and sex as control group were selected from 13 702 children and adolescents of 2 to 18 years old in Xiangtan city by sampling survey.The body mass index(BMI),ADPN,LP,hsCRP,fasting blood-glucose(FBG),and fasting pure insulin(INS) were checked respectively.The homeostasis model assessment for insulin resistance(HOMA-IR) and the quantitative insulin-sensitivity check index(QUICKI) were calculated.The difference between the two groups was compared and the relationship of all indexes was analyzed.Results The values of ADPN and QUICKI in obesity group were significantly lower than those of control group(P
6.Correlation between five RNA markers of rat's skin and PMI at different temperatures.

OBJECTIVE:
To explore the correlation between postmortem interval (PMI) and five RNA markers of rat's skin--β-actin, glyceraldehyde-3-phosphate dehydrogenase (GAPDH), 18S ribosomal RNA(18S rRNA), 5S ribosomal RNA (5S rRNA), and microRNA-203 (miR-203), at different temperatures.
METHODS:
Eighteen SD rats were randomly divided into three environmental temperature groups: 4 °C, 15 °C and 35 °C, respectively. Skin samples were taken at 11 time points from 0 h to 120 h post-mortem. The total RNA was extracted from the skin samples and the five RNA levels were detected by real-time fluorescent quantitative PCR. Proper internal reference was selected by geNorm software. Regression analysis of the RNA markers was conducted by GraphPad software.
RESULTS:
5S rRNA and miR-203 were most suitable internal references. A good linear relationship between PMI and RNA levels (β-actin and GAPDH) was observed in two groups (4 °C and 15 °C), whereas the S type curve relationship between the expression levels of the two markers (β-actin and GAPDH) and PMI was observed in the 35 °C group. The partial linear relationship between 18S rRNA and PMI was observed in the groups (15 °C and 35 °C).
CONCLUSION
Skin could be a suitable material for extracting RNA. The RNA expression levels of β-actin and GAPDH correlate well with PMI, and these RNA markers of skin tissue could be additional indice for the estimation of PMI.

7.Tat-NBD peptide inhibits inflammation effects related to NF-kB on AR42J cell

Objective To investigate the effects of Tat-NBD(NEMO-binding domain,NBD)peptide on LPS-stimulated AR42J acinus cells inflammatory response.Method Lipopolysaccharide(LPS)was added to culture media at doses of 10 mg/kg for 24 hours to stimulate the AR42J cells.For pretreatment.cells were incubated with different peptides for 2 hours before LPS stimulation.The expression of TNF-α mRNA WaS conducted using a semi-quantitative RT-PCR method.TNF-α protein in culture medium were detected by enzyme linked immunosorbent assay(ELISA).The expression and translocation of the NF-kB-p65 protein of AR42J was determined by Strept Actividin-Biotin Complex(SABC)method.Results LPS(10 mg/L)resulted in an increase of TNF-αmRNA and TNF-αprotein,whereas significant inhibitory effects were observed when cells were incubated with Tat-NBD(WT)just on dose of 0.1 me/L(P<0.05).The Tat-NBD(WT)peptide decreased inflammatory cytokine expression by a dose-dependent manner and its peak role was on dose of 100 mg/L.Consisting with TNF-α expression decrease,NF-kB-p65 expression signitieantly decreased in Tat-NBD(WT)pretreatment group,especially on the largest dose.NO significant changes in the control peptide group.Conclusions Tat-NBD(WT)peptide can inhibit the inflammation of acinus simulated by LPS.
8.Analysis of polymethylmethacrylate leading to canine acute pulmonary embolism in vertebroplasty

Objective To study whether the leakage of polymethylmethacrylate (PMMA) can lead to pulmonary embolism in vertebroplasty (VP), and to observe the correlation of embolism degree with the doses of PMMA. Methods Fifteen canines were randomly divided into 3 groups: group A, B, C, and were injected 0.5, 1.0, 2.0 ml PMMA into the femoral veins, respectively. Before and after the injection, chest CT and angiography of pulmonary artery were performed, the pressure of pulmonary artery was measured, and the blood gas analysis of femoral artery was done. At last, the canines were euthanized and examined and histologically after HE-stained. Results In group C, the pressure of O_2 decreased obviously, and significant difference was found between before and 10 min after injection (P<0.05). The pressure of CO_2 increased quickly in group C, significant difference was detected between before and 1 min after injection (P<0.05). Pressure of pulmonary artery in both group B and C were significantly different between before and after injection (P<0.05). Conclusion The leakage of PMMA can lead to pulmonary embolism. The degree of pulmonary embolism is correlated to the doses of PMMA. The large dose of bone cement leakage can lead to disorder of respiratory function.
9.Perspectives on Pharmacy Intravenous Admixture Services

METHODS: The construction of PIAS,the request on staff and the way to facilitate hospital pharmacy development and so on were analyzed and reflected,and some suggestions were brought forward. RESULTS & CONCLUSIONS: To do well in promoting PIAS and facilitate the development of hospital pharmacy calls for the implementation of related policies in the concerned department as well as the cooperation of medical staff.
10.Activation of NF-?B and anti-inflammation mechanism of pyrrolidine dithiocarb amate in rats with acute colitis

AIM: To investigate DNA-binding activity of nuclear factor ka ppa B (NF-?B) and pyrrolidine dithiocarbamate (PDTC) anti-inflammation effect and mechanism in trinitrobenzene sulfonic acid (TNBS)-induced acute colitis in rats. METHODS: Acute colitis was induced by instilling TNBS/ ethanol into the lumen of the rat colon. Rats were randomized into PDTC-treat g roup (including group P10,P25,P50,P100), TNBS/ethanol group and normal control g roup. The rats in PDTC-treated group were injected intraperitoneally with PDTC at the dosages of 10, 25, 50, and 100 mg?kg -1 , respectively. Rats wer e killed at 4 h after enema. Colonic inflammation, the ratio of wet/dry weig ht and MPO, SOD, and MDA activity were detected. DNA-binding activity of NF-? B was assessed by EMSA. RESULTS: NF-?B activity in colon homog enate was increased markedly at acute colitis in rats. PDTC attenuates the devel opment of rat colonic inflammation but not by reducing the NF-?B activity. CONCLUSION: NF-?B is activated in rats with acute colitis, which may be a mechanism of self-protection. PDTC can attenuate the development of in flammation.
