1.Clinical Observation of Chronic Hepatitis C Treated by Pegylated Interferon Alpha-2a Plus Ribavirin

Objective To observe the efficacy and safety of pegylated interferon apha-2a combined with ribavirin in the treatment of chronic hepatitis C.Methods One hundred and six patients with chronic hepatitis C were divided into 2 groups randomly.Patients in the observation group were treated with pegylated interferon alpha-2a,and patients in the control group were treated with interferon alpha-1b.All patients were given ribavirin according to the weight,and the treatment course was 48 weeks.HCV-RNA was tested before treatment,4 weeks, 12weeks and 24 weeks after the start of treatment,end of treatment,24 weeks after the end of treatment.The adverse reactions were also observed.Results In the observation group,the rapid virological response (RVR) was 77.4%,the complete early virological response (cEVR) was 83.0%,the end treatment virological response (ETVR) was 90.6%, the sustained virological response (SVR) of 24 weeks after the end of treatment was 84.9%.and these rates were significantly higher than the control group.All patients received the whole course of treatment.Condusion Treatment of chronic hepatitis C with pegylated interferon apha-2a combined with ribavirin is effective and safe.
4.Application of superimposed flaps in hand surgery

Objective To probe the application of superimposed flaps in hand surgery. Methods Form March 1998 to December 2003, we repaired 35 cases of skin defects with non trunk blood flaps and neurocutaneous vascular flaps. The area of flaps ranged from 1cm?1cm to 6cm?8cm. Results After operation, the flaps in 33 cases survived, but died in 2 cases due to unsuitable design. The follow ups ranging from 6 to 12 months showed that the quality and color of the survived flaps were similar to those of the normal tissue around. No swelling was found. Conclusion The superimposed flaps have reliable blood supply so that they are an ideal means to repair skin defects of hand.
5.Study on clinical and pathology of children with gastrointestinal bleeding

Objective To evaluate the etiologies of children with gastrointestinal bleeding(GIB), and the relationship between the pathological findings and clinical.Methods Gastroscopy or proctoscope was performed in 153 children with GIB. Pathological studies and tests as for helicobactor pylori (Hp) were undertaken.Results Among 153 children,140 cases(91.5 %) had definite diagnosis,including 74 children with upper gastrointestinal bleeding(UGIB) and 66 cases with lower gastrointestinal bleeding(LGIB). Gastric pathologic study was conducted in 56 cases. All patients had chronic superficial gastritis(CSFG). Hp test was positive in 33 patients. 6.ANALYSIS OF THE NUTRITIONAL COMPONENTS AND FLAVOROUS SUBSTANCES OF WHITE YAK'S MILK

Objective: To analyse the nutritional components and flavorous substance of the white yak,s milk. Method: In collecting the raw milk of eighteen white yaks,dry substance,protein,fat and ash were detected by routine methods;mineral elements by ICPV-1000S inductively coupled plasma atomic emission spectroscopy,amino acids by 835-Shimadzu amino acid analyzer,volatile substances by GC-MS. Results: The milk of white yak contained dried substance (18.38%),protein (6.53%),fat (5.64%),minerals (0.87%), TAA(6.36%), EAA(2.56%),two limiting amino acids (Met and Trp), EAA / TAA (40.25%), EAA/ NEAA (67.37%); seven flavorous substances: esters, alcohols,ketones and aldehydes,etc. Conclusion: The milk of white yak has distinct propertis: high protein,high fat,high energy,abundant minerals,agreeable flavor,abundant amino acid. So the milk of white yak is an excellent nutritional resource.
7.Antivirus Treatment for Patients with Hepatitis C Cirrhosis at Decompensated Stage after Elimination of Complication

Objective To observe the efficacy of antiviral therapy of pegylated interferon (Peg-IFN) α-2a combined with ribavirin for patients with hepatitis C cirrhosis and hypersplenism underwent splenectomy or partial splenic embolization. Methods Thirty-eight patients with hepatitis C cirrhosis (genotype Ⅰ HCV infection) hypersplenism failed to the anti-viral therapy were performed splenectomy or partial splenic embolization to improving hypersplenism. After 3 months,Peg-IFNα-2a 90μg or 135 μg was given subcutaneously once weekly, plus ribavirin 600-1 000 mg/d orally for 1 year of the treatment. During the treatment, patients were followed at four-week intervals, and then followed-up every month until the 24th week after stopping. Liver function, blood routine, renal function, HCV RNA, and adverse reaction of medication were observed during treatment and the follow-up period. Results Splenic function of patients with hepatitis C cirrhosis and hypersplenism was improved after hypersplenism splenectomy or partial splenic embolization. The sustained virologic response (SVR) rate was 63.88%after giving Peg-IFNα-2a combined with ribavirin anti-viral treatment. Conclusion After splenectomy or partial splenic embolization, patients with hepatitis C cirrhosis and hypersplenism showed the better SVR at the treatment of Peg-IFNα-2a combined with ribavirin. The treatment could delay the progress of the hepatitis C cirrhosis and reduce the incidence of liver failure and liver cancer.
8.Perimenopausal clinical performance as well as cognition and demand for knowledge of perimenopause in community-dwelling women

Objective To explore the clinical performance and related factors of perimenopausal syndrome,cognition and demand for perimenopausal knowledge in community-dwelling women.Methods During June 2009 to December 2011,a cross-section survey of perimenopausal symptoms as well as cognition and demand for perimenopausal knowledge was conducted in 1546 women aged 45 to 65 years in Shanghai Changfeng Community.Results The average menopausal age of this population was (49.5 ±3.7) years.The total prevalence of perimenopausal syndrome was 75.7% (n =1094) and 81.4%,76.5%,65.4% in women with primary school,middle school and college education background respectively (P=0.007).Sexual disorders (80.8%),hot flushes & sweating (64.7%) and fatigue (55.8%) were the most common symptoms of perimenopause.The most serious symptoms affecting daily lives included urinary symptoms,unsatisfactory sexual life and hot flushes & sweating.And 391 (27.1%),238 (16.5%),285 (19.8%) and 296 (20.5%) women did not know the concept of perimenopausal syndrome,possible symptoms,health measures or symptoms requiring medical attention.The common methods of acquiring the knowledge of perimenopause were communicating with peers (458,31.7%),consulting professionals (364,25.2%) and searching health care information (225,17.6%).Conclusions In women,the prevalence of perimenopausal syndrome remains quite high.And a majority of them lack a sufficient knowledge of perimenopause and need professional guidance.

10.Dynamical changes of lung function and immunologic markers in asthmatic children receiving specific immunotherapy with standardized house dust mite extract.

OBJECTIVETo study the effects of specific immunotherapy with standardized house dust mite extract on the lung function and immunologic markers in children with asthma.
METHODSThirty-two children with mild to moderate asthma who were allergic to house dust mite were assigned to two groups: an immunotherapy group that received subcutaneous specific immunotherapy with standardized house dust mite extract beside glucocorticoids treatment (n=15) and a control group that received glucocorticoids treatment alone (n=17). Lung function was determined before treatment and 3, 6, 12, 18 and 24 months after treatment. The frequency of asthma attacks was observed before treatment and 12 and 24 months after treatment. Serum immunological markers, including total IgE, specific IgE and specific IgG4 against house dust mite (Dermatophagoides pteronyssinus, Der P), ECP, IL-10, IL-4, and IFN-γ, were measured before treatment and 12 and 24 months after treatment.
RESULTSThe parameters of lung function were kept at the normal level or near normal level after treatment in the immunotherapy group. Compared with before treatment, the percentage of patients without asthma attacks increased significantly after treatment in the immunotherapy group (P<0.05) and increased more significantly with a prolonged treatment time (P<0.05). The level of serum sIgG4 against Der P increased significantly throughout the period of the immunotherapy treatment compared with before treatment (P<0.01). There were no significant differences in the levels of serum total IgE, sIgE against Der P, ECP, IL-10, IL-4 and IFN-γ before and after treatment in the immunotherapy group. There were no significant differences in all the parameters measured before and after treatment in the control group.
CONCLUSIONSSubcutaneous specific immunotherapy with standardized house dust mite extract for two years decreases the frequency of asthma attacks and the lung functions are kept in a normal level in children with asthma. The role of the specific immunotherapy may attribute to the increased level of serum sIgG4 against Der P.
