1.Trans-umbilical single site double-ports laparoscopy hernia sac high ligation in children (102 cases)

Objective To explore the reliability, advantages and precautions of trans-umbilical single site double-ports laparoscopy hernia sac high ligation in children. Methods Retrospective analysis of the clinical data of 102 patients of trans-umbilical single site double-ports laparoscopy hernia sac high ligation from April 2015 to June 2016, then summarize the treatment process, method and experience. Results All 102 cases of children with inguinal hernia, 84 cases of unilateral, bilateral in 18 cases. Unilateral cases, found in 24 cases of lateral occult hernia. Operation time: one side (16.17 ± 8.23) min, bilateral (23.68 ± 9.72) min. All the patients were discharged after 1 d, and the incision was beautiful. There was no swelling in the scrotum or labia, no hematoma, no adhesive intestinal obstruction, and followed up from 6~20 months without recurrence. Conclusion It is safe, reliable and effective to treat inguinal hernia in children with trans-umbilical single site double-ports laparoscopy hernia sac high ligation. It has advantages such as beautiful incision, small trauma, quick recovery, low requirements on technology and equipment, easy to grasp.
2.Construction of Evaluation Model of Rational Use of Drugs of Medical Institutions

Objective To study the evaluation of rational use level of drugs in medical institutions.Methods According to the specific requirements of rational use of drugs in medical institutions,a criteria system is established.And a rational use of drugs evaluation model of medical institutions is to be built on the theory of fuzzy evaluation,group decision making and the analysis method of multi-index evaluation.Results Based on the fuzzy evaluation and group decision making,a rational use of drugs evaluation model of medical institutions is presented in the paper and one example is used to show the feasibility and the effectiveness of the proposed model.Conclusion The rational use of drugs evaluation model based on fuzzy evaluation and group decision making can effectively accomplish the evaluation of rational use of drugs of medical institutions,and build the foundation for improving the rational use of drugs of medical institutions and improving the level of universal health care.
3.The Regulation Factors of the Osteogenic Differentiation of Bone Marrow Mesenchymal Stem Cells

Disuse-related osteopenia is mainly due to decrease of bone formation which relates closely to the changes of osteoblast function and differentiation. Bone marrow mesenchymal stem cells which are multipotent cells present in bone marrow can differentiate into osteoblast. The factors which regulate the osteogenic differentiation of bone marrow mesenchymal stem cells were introduced. This will increase the understanding and aid in the prevention and treatments of bone loss to create a healthier life for astronauts and patients after disuse.
5.Relationship of subclinical hypothyroidism with gestational diabetes mellitus during early pregnancy

A screening of thyroid function was performed in 2 320 pregnant women whose gestational age was less than 12 weeks.The incidence of subclinical thyroid disease and its relationship with gestational diabetes mellitus were analyzed.The results showed that there were 8 patients (0.34%) with clinical hypothyroidism,226 (9.74%) with subclinical hypothyroidism,and 14 patients (0.6%) with hyperthyroidism among 2 320 pregnant women.The prevalence of gestational diabetes mellitus in pregnant women with subclinical hypothyroidism was apparently higher than that in normal pregnant women (33.3% vs 12.9%,P<0.05),suggesting that the increased incidence of gestational diabetes mellitus seems to be related to subclinical hypothyroidism during early pregnancy.Therefore,it is important to screen the function of thyroid during early pregnancy
6.Effects of Acidic Fibroblast Growth Factor on Hippocampal Astrocytes Injury Induced by Gentamicin:Role of Mitogen-Activated Protein Kinase Signal Pathway

Objective To explore the mechanism of the protection of acidic fibroblast growth factor (aFGF) for hippocampal astrocytes from injury induced by gentamicin. Methods Hippocampal astrocytes were isolated from newborn (24 hours) Sprague-Dawley rats, puri-fied, and identified with glial fibrillary acidic protein (GFAP) immunofluorescence. The third generations were cultured for 3 days and divid-ed into 3 groups:control group was cultured routinely, injury group was cultured with 2.0 g/L gentamicin for 24 hours, and protection group was cultured with 4.25μg/L aFGF for 24 hours and then cultured with 2.0 g/L gentamicin for 24 hours. Western blotting was adopted to de-tect the expressions of P38, extracellular signal-regulated kinase (ERK)1/2 and c-Jun N-terminal kinase (JNK)1/2. Results Hippocampal as-trocytes were culturated successfully with the purity above 95%. The ERK1 increased in the injury group compared with the control group (P<0.05). Compared with the injury group, the p38 increased (P<0.05) and the ERK1 decreased (P<0.05) in the protection group. There was no significant difference among others (P>0.05). Conclusion The mitogen-activated protein kinase signal pathway, especially P38 and ERK1, may associate with the protection of aFGF for hippocampal astrocytes from injury induced by gentamicin.
8.Perioperative protective procedures of myocardium in patients undergoing heart-valve replacement

The objective of this retrospective study was to evaluate the myocardial protection of different periop erative procedures in 100 cases with heart-valve replacement (HVR) between 1980 and 1992. Before Feburary. 1989. HVRs of 48 cases (group Ⅰ) were undertaken with hypothermia (28C=30C) and cold cardioplegic solution (4C. 500ml). under morphine-diazepam-pancuronium anesthesia: following cardiac resuscitation, the procedures were performed including blood transfusion, diuresis, supplemental KCI. inotropic support with dopomine, norepincphrine and aramine. and continuous mechnical ventilation. Since 1989, the procedures were increased to be conducted in 52 cases (group Ⅱ) as followed: A. preoperative managements for achiving the optimum status of cardiac function: 1. oxygen therapy by mask. 2. antirhenmatic treatment for patients aged less than 35 years. 3. oral isosorbide and captopril for pulmonary hypertension, 4, infusion with fructose 1.6-diphusphatc and glucose-insulin-KC1 mixture. 5. clinical dose of digitora and diuretics for cardiac failure; B. methods during surgery: 1. anesthesia was induced with fentanyl, midazolam and pancuronium. and maintained with inhalation of nitrous oxide and infusion of fentanyl at 30-35?g/kg/min, 2, hy pothermia (24C-27C) and cold cardioplegic solution(600-1000ml) 3. immediately before the blockage of aorta was overcome. the artificial perfusion blood pressure was reduced; afterwards, it was gradually recovered, in order to prevent myocardium from injury of reperfusion. 4. intra-and extracoporal circulations worked together over half of blokage time of arota, to improve myoardial oxygen delivery; C. treatments in early postoperative stage: 1. continuous positive end-expiratory pressure ventilation, 2. infusion with magnesium-potassium aspartyl: and the others were similar to those before 1989. 9.Effects of TanshinoneⅡA Sulfonic Acid Sodium on Blood Supply and Hemorheology of Patients with Central Retinal Vein Occlusion

OBJECTIVE:To observe the effects of Tanshinone ⅡA sulfonic acid sodium injection on blood supply and blood rheology of patients with central retinal vein occlusion )CRVO). METHODS:92 CRVO patients were selected and randomly divided into control group and trial group with 46 cases in each group. Control group was given routine treatment of laser photocoagulation;trial group was additionally given Tanshinone ⅡA sulfonic acid sodium injection 80 mg added into 0.9% Sodium chloride injection 150 ml,ivgtt,qd. A treatment course of 2 groups lasted for 7 days,and both received 4 courses of treatment. The hemodynamics and hemorheology of central retinal artery (CRA) were recorded in 2 groups before and after treatment. The fluorescence angiogra-phy (FFA) and optical coherence tomography (OCT) of 2 groups were also observed. RESULTS:Before treatment,there was no statistically significant difference in Vmax,Vmin and RI of CRA between 2 groups (P>0.05);after treatment,Vmax,Vmin and RI of CRA in 2 groups were improved significantly,the trial group was better than control group,with statistical significance(P<0.05). After treatment,whole blood high shear viscosity,whole blood low shear viscosity,plasma specific viscosity and fibrinogen of trial group were significantly lower than those of control group,and FFA and OCT of trial group were better than those of control group, with statistical significance(P<0.05). CONCLUSIONS:TanshinoneⅡA sulfonic acid sodium injection can obviously improve hemo-dynamics and hemorheology of CRVO and blood supply and vision of lateral eye.
10.Construction and Empirical Study of Evaluation Model for Essential Medicine Bidding and Procurement Based on Fuzzy Group Decision Making

OBJECTIVE:To establish the essential medicine bidding evaluation model and improve the scientificity and effec-tiveness of essential medicine bidding and procurement. METHODS:Combined with the main objective of essential medicine cen-tralized bidding and procurement,a criteria bidding evaluation index system for essential medicine was established,and a bidding evaluation model for essential medicine was built based on fuzzy group decision making and the feasibility was verified by exam-ples. RESULTS:The essential medicine evaluation model based on fuzzy evaluation and group decision making was established, and empirical analysis for Atorvastatin Calcium tablet was used to show the feasibility of evaluation model. CONCLUSIONS:The essential medicine evaluation model based on fuzzy group decision making can scientifically and effectively accomplish a compre-hensive evaluation of pharmaceutical enterprises,which builds the foundation for promoting the transparency of essential medicine bidding and procurement work and reducing the economic burden for the patients.
