3.New development of analgesics brought about by studies on pain

OBJECTIVE:To review the recent advances in analgesics study and summarize the drugs for central sensitization and analgesia model by relating to the latest means and methods of research.DATA SOURCES:The literature on analgesics was retrieved in NCBI database for the related papers published between January 1996 and December 2001, with the key words of "analgesia", "analgesic", and "pain"and language restricted to English. At the same time, papers published in the past two years were retrieved in http:∥www.wanfangdata.com.cn, with the key words of "analgesia", "analgesic" and "ganglioplegic" in Chinese and the language restricted to Chinese.STUDY SELECTION:Totally more than 50 000 research references repeated contents.DATA EXTRACTION: Altogether 100 basic research articles were collected. We selected 15 articles relevant to the research development in analgesic drugs, and excluded 82 review papers with repeated content.expressed by proto-oncogene c-Fos located in the nucleus is a kind of DNA conjugated protein. It can regulate the expression of some pain-related genes. Some studies found that substance P synthesis is likely to play a research: Developing the central sensitization drugs is the orientation of sal root ganglion neurons and model of swallowing response were established.CONCLUSION:By investigating the effects of various new research methods and models on pain, we intended to look for more new drugs, to further study neurotransmitters and the clone of transmitter receptor at the cellular and molecular level so as to find the new effect target of the drugs, and to have a better understanding of the integrity of chronic pain in humans and achieve more breakthroughs in the development of neuroscience research.
4.Different patterns of titanium surface nanocrystallization and drug load aimed at establishing orthopedic implants with antimicrobial property

Titanium and titanium alloys,which are bio-inert materials and have excellent mechanical properties,have broad applications in clinic.On the one hand,TiO2 nanotube can effectively enhance the osteogenic differentiation of bone marrow mesenchymal stem cells and have antimicrobial abilities in some extent;On the other hand,TiO2 nanotube,an outstanding drug-carrier,could effectively prevent and treat bone implant-related infection by loading antimicrobial agents.By means of modifying the nanotube coating,improving the efficiency of drug loading and ameliorating release profile,the ideal antimicrobial property could be achieved.Strategies for drug release can be divided into two approaches,namely mechanical release and intellectual release.Mechanical release could fortify the antibacterial ability of coating,but the unicity and uncontrollability of agents diluting need to be resolved.By contrast,intellectual agents release has the advantages of multiple drug species,controllable release volume and programmed trigger condition.This article reviews the current and potential methods of antibacterial substances loading and release from TiO2 nanotube,and expects to provide the orientation for future direction of controllable and intellectual nanotube drug release.
5.Study of positive results of electrocardiogram treadmill exercise test for patients with chest pain

Objective To correctly analyze the positive results of electrocardiogram treadmill exercise test (TET) and improve the level of diagnosis of CHD Methods Two hundred and fifty eight patients with chest pain underwent TET and CAG; the case history and different clinical parameters were collected (1) Clinical data of the true positive group and the pseudo positive group were compared and underwent a statistic analysis (2) Clinical data of the female positive group were compared and underwent a statistic analysis Results The number of females in the pseudo positive group was larger than in the true positive group;The number of patients with typical chest pain, hyperlipidemic history, history of smoking, family history of CHD was smaller than in the true positive group;In the pseudo positive group,the changes of ST segment were mainly in Ⅱ, Ⅲ and aVF The number of extent of changes of more than 0 2 mV and with angina induced during exertion in pseudo positive group was smaller than in true positive group In females, under 60 years, the number of ST segment changes of resting ECG and left ventricular hypertrophy was larger in pseudo positive group than in true positive group The difference was significant Conclusion The factors including sex, typical chest pain,risk factors of CHD, the location and extent of ECG changes and angina induced following exertion should be combined to analyze the positive results of TET
6.Comparison of the characteristics of several polymer materials used in hydrophilic matrix tablets.

Pure and drug hydrophilic matrix tablets were prepared by direct compression method with theophylline as a model drug. The characteristics of four hydrophilic matrix polymers, hydroxypropylmethylcellulose (HPMC), polyethylene oxide (PEO), sodium alginate (NaAlg) and xanthan gum (XG), were compared by investigating the water absorption, swelling, erosion and gel layer strength. The sequence of water absorption rate was XG >> NaAlg (H) > PEO > NaAlg (L) >> HPMC; The sequence of swelling index was XG >> PEO >> HPMC >> NaAlg; The sequence of erosion rate was NaAlg (L) > NaAlg (H) >> PEO80 > PEO200 > PEO300 > XG approximately PEO400 approximately K4M > K15M > PEO600 approximately K100M; The sequence of the gel layer strength was PEO > HPMC > XG >> NaAlg. For the PEO and HPMC matrix tablets, with the polymer molecular weight increased, the drug release mechanism was gradually transferred from mainly depending on the erosion to the diffusion; for SAL matrix tablets, the drug release mainly depends on erosion mechanism; and for XG matrix tablets, the drug release mainly depends on non-Fick diffusion mechanism. Comparison of the performance difference between the polymer materials will contribute to rational design and prediction of drug release behaviors from matrix tables and ultimately to achieve clinical needs.
8.The treatment analysis of 128 cases of nonpenetrated cornea trauma caused by crops

Objective To investigate the clinical treatment of nonpenetrated cornea trauma caused by crops. Methods Clinical data of 128 cases of nonpenetrated cornea trauma caused by crops were retrospectively analyzed. According to the interval time between occurrence of trauma and clinic visiting, the patients were divided into 3 groups:group A (33 cases,<24 h), group B (72 cases, 24 h≤interval time<1 week) and group C (23 cases, ≥ 1 week). The therapeutic effects and prognosis were analyzed. Results There was statistical difference in the incidence of corneal ulcer among group A, group B and group C: 6.1% (2/33), 62.5% (45/72) and 100.0% (23/23), χ2= 52.32, P<0.01. In group B, 12 cases were treated with conjunctival flap covering, 2 cases received keratoplasty and 2 cases undertook enucleation. In group C, 10 cases were treated with conjunctival flap covering, 4 cases received keratoplasty and 2 patients undertook enucleation finally. All the other patients were cured with local debridement and medical treatment. Conclusions Patients with nonpenetrated cornea trauma caused by crops may develop infectious keratitis, and prompt and proper treatment can avoid the secondary infection and improve the outcome. Local debridement in combination with iodophors disinfection can prevent the incidence of infectious keratitis. Conjunctival flap covering is an effective technique in the treatment of corneal ulcer caused by nonpenetrated cornea trauma.
9.Clinical evaluation of peripheral vascular disease withwall thickness, stiffness and hemodynamics of femoral artery in type 2 diabetes mellitus

Objective To evaluate the relationship between femoral artery intima-media thickness (FA-IMT), stiffness, hemodynamics factors of femoral artery and peripheral vascular disease (PVD) in type 2 diabetes mellitus. Methods According to the presence of PVD, 151 patients with type 2 diabetes were divided into PVD group and non-PVD group. FA-IMT was tested with 2D ultrasound. Ds, Dd, PSV, EDV were observed with color flow rate and volume quantitative technology. FA-IMT, FA-stiffness β, peak systolic velocity (PSV) and end diastolic velocity (EDV) were compared between the two groups. Results Compared with non-PVD group, FA-IMT and FA-stiffness β increased in PVD group (P<0.01), whereas PSV and EDV decreased in PVD group (P<0.01). There was no relationship between FA-stiffness β and FA-IMT in PVD group (P=0.01). Multiple Logistic regression indicated the presence of PVD symptoms associated closely with FA-stiffness β, PSV, EDV and duration of diabetes. Conclusion Stiffness β plays an important role in the clinical evaluation of type 2 diabetes mellitus.
10.Feasibility of real-time triplane strain rate imaging for quantitative assessment of left atrial function in normal subjects

Objective To evaluate the feasibility of real-time triplane strain rate imaging for the quantitative assessment of left atrial function in normal subjects. Methods Totally 40 healthy volunteers were enrolled. Left atrial expansion index (LAEI), left atrial passive emptying fraction (LAPEF) and left atrial active emptying fraction (LAAEF) were measured using real-time triplane volume analysis. Mean systolic peak strain rate (MSRs), mean early diastolic peak strain rate (MSRe) and mean late diastolic peak strain rate (MSRa) were measured using real-time triplane strain rate imaging. The results were compared respectively. Results MSRs correlated positively with LAEI (r=0.61, P<0.01). MSRe correlated inversely with LAPEF (r=-0.67, P<0.01), so did MSRa with LAAEF (r=-0.78, P<0.01). Conclusion Real-time triplane strain rate imaging is feasible for the quantification of left atrial function in normal subjects.
