1.Clinical study of femtosecond laser in vitrectomy combined with phacoemulsification and intraocular lens implantation

AIM: To analyze the clinical value of femtosecond laser in vitrectomy combined with phacoemulsification and intraocular lens (IOL) implantation.
METHODS: We selected 100 patients ( 102 eyes ) undergoing vitrectomy combined with phacoemulsification and IOL implantation in our hospital from October 31 2013 to October 31 2015. They were divided into routine operation group, 50 cases (52 eyes), and femtosecond laser group, 50 cases (50 eyes). The parameters were compared between the two groups, including total operation time, postoperative complications ( elevated intraocular pressure, uveitis, vitreous hemorrhage, and retinal detachment ), postoperative intraocular pressure and postoperative half of the visual acuity ( near visual acuity, visual acuity, uncorrected visual acuity, corrected visual acuity).
RESULTS: The difference on total operation time of the two groups was statistically significant ( P < 0. 05). The differences between the two groups on postoperative intraocular pressure, uveitis, vitreous hemorrhage, retinal detachment were not statistically significant ( P >0. 05). There were no significant differences on internal pressure, near visual acuity, far visual acuity, uncorrected visual acuity, corrected visual acuity at 6mo after surgery (P>0. 05).
CONCLUSION: Femtosecond laser in vitrectomy combined with phacoemulsification and IOL implantation has a high safety and efficacy with the advantages as accurate, efficient.
2.Establishment and identification of a ClC-3/MMTV-PyMT hybrid mouse strain

Objective To establish a CLCN3/MMTV-PyMT double transgenic mouse model of spontaneous breast tumor with simultaneously overexpressing ClC-3.Method CLCN3 transgenic mice were crossed with MMTV-PyMT spon-taneous mammary tumor model mice.The genotype was determined by PCR.The expression of ClC-3 in tissues was detec-ted by immunofluorescence and Western blot.Results CLCN3 and MMTV-PyMT transgenic mice were bred and CLCN3/MMTV-PyMT hybrid mouse model was successfully established.The ClC-3 expression in CLCN3/MMTV-PyMT hybrid mice was higher than that in the MMTV-PyMT mice, assessed by immunofluorescence and Western blot analysis.Conclu-sions Transgenic mouse models of spontaneous breast cancer with simultaneously overexpressing ClC-3 are successfully es-tablished.The double transgenic mice provide a good animal model for further research of ClC-3 in tumor growth and metas-tasis.

4.Survey and analysis of the results of creatinine measurement by 31 detection systems of 13 hospitals in Beijing

Objective Understand the variation of serum creatinine measurement in clinical laboratories of some hospitals in Beijing.Methods 8 samples of mixed frozen human serum added different creatinine concentration standard materials(the creatinine concentration were80-1 000 ?mol/L)and 8 samples of mixed frozen serum of patients(contained different creatinine concentration)were distributed to 13 clinical laboratories(31 series of detection systems)with the way of spot investigation.Every clinical laboratories measured the samples followed the standard operating procedure.Results As to the mixed frozen human serum added different creatinine standard materials,the CV of different detection systems results were 5.74%-9.68%;as to the mixed frozen patients' serum,the CV was 5.90%-11.69%. Compared with Beckman closed detection systems,the results of Dade systems(which used the kinetic alkaline pieric acid method)showed the bias were-5.99%-0.35%,and as to the other systems which measured by alkaline picric acid method,when creatinine concentrations were 200 ?mol/L,the results showed negative bias,and the greatest bias was-8.45%.The bias plots revealed negative for all of the detection systems with enzymatic method over the whole concentration range,and the greatest bias was -8.88%.Conclusions The creatinine determination results of Beckman and Dade closed detection systems were consistent.The results of detection systems which used enzymatic method were generally lower than Beckman detection systems.What's more,the creatinine measurement variations of clinical laboratories were very large,especially for the results of unclosed detection systems,so it was urgent need to solve the standardization of creatinine measurement.
5.Total flavonoids of litsea coreana decreases the cytotoxicity of oxaliplatin in TM3 Leydig cells via enhancing the function of gap junction.

OBJECTIVETo investigate the effects of total flavonoids of Litsea Coreana (TFLC) on the gap junction (GJ) intercellular communication in TM3 testicular Leydig cells and whether TFLC can reduce the cytotoxicity of oxaliplatin (OHP) in vitro.
METHODSWe detected the effect of TFLC on the dye spread of the in vitro cultured TM3 cells by parachute assay, observed changes in the expression of connexin 43 (Cx43) total protein in the TFLC-treated TM3 cells by Western blot, and determined the effects of TFLC on the expression of Cx43 on the membrane of the TM3 cells by immunofluorescence assay and on the cytotoxicity of OHP by MTT assay.
RESULTSTFLC obviously enhanced the GJ function with the increasing of the TFLC concentration in the TM3 cells. Western blot and immunofluorescence assay confirmed that TFLC significantly enhanced the expression of Cx43 total protein and Cx43 expression on the membrane of the TM3 cells. MTT assay showed that at a high cell density (confluent with GJ formation), 20 microg/ml TFLC enhanced the GJ function of the TM3 cells and reduced the cytotoxicity of OHP (P < 0.05), while at a low density (preconfluent with no GJ formation), TFLC exhibited no effect on the cytotoxicity of OHP (P > 0.05).
CONCLUSIONTFLC increases the Cx43 expression and GJ function in normal TM3 Leydig cells, and the enhancement of GJ function reduces the cytotoxicity of OHP.

7.Pancreatic acinar cell carcinoma: a clinicopathological and immunohistochemical analysis of 8 cases

ObjectiveTo investigate the clinicopathological features,immunohistochemical phenotype of pancreatic acinar cell carcinoma.MethodsEight cases of pancreatic acinar cell carcinoma admitted to our hospital from January 2001to January 2011were retrospectively analyzed. The clinicopathological characteristics,and immunohistochemical staining for phenotype were analyzed,then the follow-up data were summarized.ResultsAll 8 patients with pancreatic acinar cell carcinoma was male,with a median age of 47 years old.Tumors were located in the pancreatic head in 4 patients,pancreatic body and tail in 4 patients.The average tumor size was 4.5 cm × 4.0 cm × 3.2 cm,the section appeared as gray or gray-red and presented as solid or cystic lesions.Larger tumors were often accompanied by hemorrhage and necrosis.Microscopically,the tumor cells arranged in acinic,cord,trabecular or solid nests.The cytoplasm was abundant and eosinophilic.The nuclear was round,oval,slightly atypia.lmmunohistochemical staining showed diffusely positive for CAM5.2,α-AT,α-ACT and focally positive for CA19-9,CEA,E-cad,β-cat and MUC-1 and only occasionally positive for AFP,NSE,Syn and CgA.Follow-up data showed there was one case of postoperative death due to postoperative pancreatic leakage with abdominal infection.Liver metastasis occurred in 4 cases,among whom,2 cases died.ConclusionsPancreatic acinar cell carcinoma is a rare epithelial malignant tumor of pancreas,with distinct phenotype characteristics.
8.Effect of oleuropein on IL-1β-induced rat chondrocytes

AIM:To investigate the effect of oleuropein on interleukin-1β( IL-1β)-induced SD rat articular chondrocytes .METHODS:The SD rat articular chondrocytes were isolated by 2 step enzyme digestions .The chondrocytes were cultured in vitro.Inverted microscopic observation was performed during the culture .Alcian blue staining and type II collagen immunohistochemical staining were used to identify the chondrocytes .The effects of oleuropein on the viability of chondrocytes were determined by CCK-8 assay.The cells in 3rd passage were pretreated with oleuropein at 10, 50 or 100 μmol/L and subsequently stimulated with IL-1βat 10 μg/L for 24 h.Production of prostaglandin E 2 ( PGE2 ) and ni-tric oxide (NO) were evaluated by the Griess reaction and an enzyme linked immunosorbent assay (ELISA).The mRNA expression of matrix metalloproteinase ( MMP)-1 and MMP-13 was measured by real-time PCR.The protein levels of in-ducible nitric oxide synthase (iNOS), cyclooxygenase-2 (COX-2) and nuclear factor-kappa B (NF-κB) were detected by Western blotting .RESULTS:The cell viability of chondrocytes was not significantly impaired by treating with oleuropein at concentration of 10, 50 or 100μmol/L for 24 h compared with control group .Pretreatment with oleuropein significantly in-hibited the production of PGE 2 and NO induced by IL-1β.Oleuropein also significantly decreased the IL-1β-stimulated MMP-1 and MMP-13 mRNA expression in articular chondrocytes .Pretreatment with oleuropein inhibited the IL-1β-media-ted activation of NF-κB by suppressing the degradation of its inhibitory protein IκBαin the cytoplasm .CONCLUSION:Oleuropein inhibits IL-1β-induced inflammatory gene expression by suppressing NF-κB activation at the transcriptional le-vel, suggesting a new mechanism for the anti-inflammatory effects of oleuropein as a novel agent on treating with osteoarthri-tis.
9.Yin-Dan Xin-Nao-Tong Capsule Combined with Routine Modern Therapies for Treatment of Type-2 Diabetics Mellitus Complicated with Coronary Heart Disease:A Systematic Review

This article was aimed to assess the effectiveness and safety of Y in-Dan Xin-Nao-Tong (YDXNT) Cap-sule combined with routine modern therapies for treatment of type-2 diabetes mellitus (T2MD) complicated with coronary heart disease (CHD). Databases including CBMDisc, CMAC, CNKI, VIP, Wanfang Data, PubMed, EMbase, and Web of Science were electronically searched from inception to December 2013. Randomized controlled trials (RCTs) on YDXNT capsule combined with routine modern therapies for treatment of T2MD complicated with CHD were included. Two reviewers independently screened literature according to the inclusion and exclusion criteria, ex-tracted data, and assessed the methodological quality of included studies. Then, meta-analysis was performed using RevMan 5.1.0 software. The results showed that a total of 9 RCTs with 667 T2MD with CHD cases were included. The results of meta-analysis indicated that the total effective rate of YDXNT capsule combined with routine modern therapies was significant higher than the routine treatment for T2MD complicated with CHD treatment with statistical significance (OR = 4.01, 95%CI [2.37-6.79], P< 0.000 01). It showed that YDXNT capsule combined with routine modern therapies can control angina pectoris. Due to limitation of included data, controlling of the duration of angina pectoris, the attack frequency of angina pectoris, and fasting blood glucose, may be better than the routine treatment group with significant differences. YDXNT capsule can improve hemorheology indices and reduce high condensation state among CHD with T2DM patients in order to relieve angina pectoris. It was concluded that on the basis of current clinical evidences, YDXNT capsule had better treatment effect than the routine treatment group in the treatment of CHD with T2DM. However, the results have certain limitations. It still required double-blind multi-center RCTs with high quality, large samples for further verification.
10.Myomectomy and mitral plasty for mitral abnormalities in hypertrophic obstructive cardiomyopathy

Objective analysis the pathogenesis and the treatment strategies of the hypertrophic obstructive cardiomyopathy (HOCM) with the concomitant mitral valve abnormalities. Methods From October 1996 to June 2009, 62 patients suffered from HOCM underwent surgical treatment. There were 41 males and 21 females with age ranging from 6 to 68 ( 34.05 ±15.26) years old. The body weight were 27 -83 kg [mean (60. 42 ± 12.71 ) kg]. Detected by preoperative echocardiography, all patients had the systolic anterior movement of the mitral leaflet ( SAM ) and 50 patients had mitral regurgitation (MR). Ventricular septal myomectomy was performed under general anesthesia and cardiopulmonary bypass (CPB) with a moderate systemic temperature and low volume blood flow. The concomitant operations included mitral valve replacement ( MVR 12 cases), mitral valve plasty ( MVP9 cases). During the perioperative period the patients were evaluated by echocardiography. Left ventricle ( LV ), left ventricular outflow tract ( LVOT), left atrium ( LA ), left ventricalar ejection fraction ( LVEF), mitral valve construction and function were evaluated. Results The time of CPB and the sortic occlusion were (104.23 ±47.14) (402290) min and (66.76 ±36.32) (20-195) min, respectively. The endotracheal intubation time was ( 13.23 ± 11.76 ) ( 5 -21 ) h and ICU stay was (42.53 ± 37.41 ) ( 11 ~ 183 ) h. Comparing with the parameters before operation: the sizes of the LA were (43.46 ± 7.21 ) mm vs. ( 34.56 ± 6.45 ) mm, pressure gradients through LVOT ( 103.84 ±44.04) mm Hg vs. (23.54±17.78) mm Hg and the thickness of the septal (26.93±5.23) mm vs. (17.12±5.67) mm.All parameters were significantly decreased( P <0.05 )postoperatively. All surviving patients had no or only trivial MR and SAM. Four patients (6.4% ,4/62 ) died during the perioperative period. The cause of death included severe low cardiac output, heart failure, severe ventricular arrhythmias and severe acute renal failure. The main complications were: left bandle branch block in 33 cases, intraventricular conduction block in 7, complete AV block in 6, anterior hemiblock in 5, type Ⅰ artioventricular conduction block in5, atrial fibrillation in 4. All surviving patients were in New York Heart Association functional class Ⅰ or Ⅱ during the follow-up from 1 month to 10 years. Cardiac symptoms were markedly relieved. Mitral valve construction and function were significantly improved. There were no death, no complications and none required additional mitral valve or myecomy surgery. Conclusion Ventricular septal myomectomy alone is sufficient to eliminate or significantly reduce the severity of MR and SAM. In patients with HOCM and MR due to the congenital mitral valve disease, concomitant MVP may be the first choice. Considering the inherent risks of prosthetic valves and anticoagulation complications, MVR should be the second choice.
