1.Effect of chronic stress on long-term potentiation of hippocampus and neurotransmitter of amino acid in rats

BACKGROUND: Severe or prolonged stress is harmful to health and even induces many mental and physical disorders and then impairs cognitire functions of the organism.OBJECTIVE: To investigate the effects of stress on the changes in amino acid and chronic stress on long-term potentiation (LTP) of hippocampus of rats.DESIGN: Completely randomized controlled experiment.SETTING: Mental Health Center of Shantou University.MATERIALS: The experiment was completed in the Medical College of Shantou University in December 2000. Totally 16 adult male SD rats were divided randomly into control group and stress group with 8 in each group.METHODS: Mice in stress group were swum compulsively for 4 weeds.Chronic stress model was established, and isolated hippocampal slice(500 μm) combined with electrophysiological technique was used to observe the changes of LTP in hippocampal CA1 field. High-frequency stimulation (HFS) was applied to Schaffer collaterals in hippocampus CA3, then the changes of PS amplitude and fEPSPs slope in CA1 pyramidal neurons were observed. High performance liquid chromatography (HPLC) coupled with UV detection was used for the quantificative analysis of hippocampal amino-acid neurotransmitters.MAIN OUTCOME MEASURES: ① Population spike (PS) amplitude and field excitatory postsynaptic potentials (fEPSPs) slope were used to indicate the changes of LTP. ② Changes of amino acid of hippocampus.RESULTS: One rat in stress group died because of drowning in the second week of experiment was supplied, and others entered the final analysis. ① The increases of PS amplitude and fEPSPs slope after HFS in control group were significantly higher than those in stress group (P< 0.05). ② The levels of aspartate and glutamate in control group were significantly lower than those in stress group [(2.425±0.211) μmol/g,(6.016±0.677) μmol/g. P< 0.01; (4.746±0.609) μmol/g. (8.094±1.035) μmol/g.P < 0.01]; but there was no significant difference of γ-aminobutyric acid between control and stress groups [(4.229±0.449) μmol/g, (4.249± 0.463) μmol/g, P > 0.05].CONCLUSION: It is suggested that chronic stress can inhibit the development of LTP in hippocampal CA1 field and increase the levels of hippocampal aspartate and glutamate, but does not affect the level of γ-aminobutyric acid. The accumulation of hippocampal excitatory amino acids induced by chronic stress may be the neurobiochemical basis of impairments in learning and memory abilities.
2.Evaluation of left ventricular longitudinal axis systolic asynchrony using RT-3PE QTVI and STI in patients with dilated cardiomyopathy

Objective To explore left ventricular longitudinal axis systolic asynchrony by real-time triplane quanti-tativetissue velocity imaging(RT-3PE QTVI) and two-dimensional speckle tracking imaging(STI) in patients with dilated cardiomyopathy. Methods Thirty normal adults were included as the control group and thirty adults with DCM. The images from enough frame rates RT-3PE QTVI and STI were obtained from series long-axis views. Simp-son method was used to measure LVEF. Q-lab software was used to measure peak systolic tissue velocity(Vs) and peak systolic longitudinal strain (Ls),its time to peak point from each segment(Ts,Tls) and the maximal temporal difference of Ts,Tls(Ts-diff,Tls-diff),and the standard deviation of. Ts,Tls(Ts-SD,Tls-SD),which were served as systolic asynchrony indexes in assessing LV longitudinal axis systolic asynchrony. Results ① Compared the con-trol group and DCM group,there were significant differences in LVEDd,LVESd,LVEDL,LVESL,LVEF,FS (P<0.01). ② In comparison with the control group,the Ls and Vs of each segment were decreased in the DCM group (P<0.01). Compared with the control group,the parameters of Tls and Ts in DCM group had obvious ahead or de-layed(P<0.05). ③ Compared with the control group,the parameters of Ts-SD, Tls-SD, Ts-diff, Tls-diff,were significantly different in DCM group (P<0.01). Ts-SD,Tls-SD,Ts-diff,Tls-diff corrrlated negatively to LVEF(r=-0.67,-0.72,-0.75, -0.77,P<0.01). Conclusion The prevalence of left ventricular longitndinal axis systolic asynchrony exits in patients with DCM. There is a better technique to evaluate LV longitudinal axis systolic asynchrony by RT-3PE QTVI and STI than only one.
3.Study on the obtaining situation of osteoporotic fractures related knowledge and influencing factors of the nurses

Objective To know the situation of nurses obtaining osteoporotic fractures related knowledge ,and provid guiding basis for further education and relevant training on target people .Methods Questionnaire survey and semi -structured interviews methods were used to investigate 595 nurses ,9 clinical nurse workers and nurse managers who come from a tertiary comprehensive teaching hospital were interviewed personally .Results The scores of the related knowledge in nurses were 35 .70 ± 6 .36 ,score rate was 67 .36% ;Interview data analysis showed that lack of professional training ,training was not systematic ,the approach obtain knowledge was simple were result in lack of the related knowledge in nurses ;By the multiple liner stepwise regression ,The educa‐tion level and professional title were main factors which affect nurses acquired osteoporosis knowledge .Conclusion It is necessary to focus on improving the strengthen of related knowledge education for the nurses with low education background and low profes‐sional title ,At the same time ,related departments of the hospital should strengthen the training of professional knowledge .

5.The Investigation of the Relation Between the Expression of Podoplanin and the Lymphatical Metastasis in the Human Colonic Carcinoma

Objective To provide the morphology data for the study on the molecular mechanism of tumor metastasis,we observed the expression of podoplanin on the blood vessels and lymphatics in the human colonic carcinoma.Methods We observed the expression of podoplanin on the blood vessels and lymphatics in the colonic carcinoma from the patients of the operation in the defferent stages with the methods of HE staining and podoplanin immunohistochemistry staining. Results Podoplanin has not been stained,but podoplanin was positively expressed on the colonic carcinoma lymphatics.And, The podoplanin expression positive rate of the colonic carcinoma were increased along with the increase of tumor stages and occurence of lymph nodes metastasis. Conclusion Podoplanin choosed to express in lymphatics in the colonic carcinoma and could correctly distinguish the blood vessels and lymphatic vessels,could be regarded as the specific marker to mark lymphatic vessel. And, The podoplanin expression positive rate of the colonic carcinoma were increased along with the increase of tumor stages and occurence of lymph nodes metastasis.It indicates,It was correlated with colonic carcinoma lymphatical metastasis that the count of lymphatic vessels were increased in the colonic carcinoma along with the tumor advancing.
6.Clinical and imaging features of cerebral autosomal recessive arteriopathy with subcortical infarcts and leukoencephalopathy(report of one family)

Objective To investigate the clinical and imaging features of cerebral autosomal recessive arteriopathy with subcortical infarcts and leukoencephalopathy(CARASIL).Methods Clinical data of a Chinese Mongolian patient with CARASIL were analyzed retrospectively and pedigree investigation was carried out in the family.Results The proband's parents were cousin and her brother was a patient with CARASIL too.The patients had onset at 25 and 23 years old,respectively.Clinical manifestations included cerebral stroke,progressive motor and mental deterioration,seizures,alopecia,and ocular fundus arteriosclerosis.No common risk factors of cerebral stroke were found in the family.Brain MRI showed bilateral diffuse cerebral white matter lesion with multiple infarcts and O'Sullivan sign.Cervical vertebral MRI showed multiple protrusion of intervertebral disc and significant retrogression.Conclusions CARASIL is clinically characterized by young-age-onset cerebral stroke,cerebral arteriosclerosis,alopecia,cervical and lumbar spondylopathy.MRI shows multiple cerebral infarcts,leukoencephalopathy and retrogression of intervertebral disc.

8.Promoter methylation and expression of RKIP gene in patients with gastric carcinoma

Purpose To investigate the correlation between promoter methylation of RKIP and the clinicopathological features of gastric carcinoma and to analyze the correlation between promoter methylation of RKIP and expression of RKIP. Methods The protein ex-pression and promoter methylation of RKIP were detected in 45 cases of surgically resected gastric carcinoma specimens and 21 cases of corresponding normal tissues using the immunohistchemistry SP method and methylation-specific PCR ( MSP) . Results The positive rate of promoter methylation of RKIP was 48. 89% in gastric carcinoma tissues and 4. 8% in adjacent tissues (P<0. 05). The positive rate of the expression of RKIP protein was 42. 22% in gastric carcinoma tissues and 90. 48% in adjacent tissues (P<0. 05). The pro-tein expression and promoter methylation of RKIP were correlated with the degree of differentiation and lymph node metastasis ( P<0. 05) . Expression of RKIP protein in gastric carcinoma was associated with promoter methylation of RKIP. Conclusions Promoter abnormal methylation of RKIP may be one reason of the deletion of expression of RKIP protein, which correlates with occurrence and development of gastric carcinoma.
9.Timeliness law on the immediate analgesia on acute migraine treated with electroacupuncture at shaoyang meridian points.

OBJECTIVETo observe the differences in timely effectiveness of immediate analgesia on acute migraine between electroacupuncture (EA) at shaoyang meridian points and non-meridian points.
METHODSThe randomized controlled trial method was adopted. One hundred and ten cases of acute migraine were randomized into two groups. 55 cases in each one. In the observation group, EA was applied to Fengchi (GB 20), Waiguan (TE 5), etc. In the control group. EA was applied to non-meridian points. Only one treatment was given, and the nee dies were retained for 30 min in the two groups. Separately. at 10 time points, named before acupuncture, in 5 min, 10 min. 20 min and 30 min (at the moment of needle removal), 1 h, 2 h, 4 h, 6 h and 8 h after acupuncture, the pain intensity numerical rating scale (PI-NRS) was adopted to evaluate the pain severity and the adverse reac tion was recorded.
RESULTS(1) The effective rate of imnmediate analgesia was 87. 3% (48/55) in the observation group. significantly higher than 52.7% (29/55) in the control group (P<0.01). (2) NRS of each group was re duced at each time point, from 5 min to 8 h after acupuncture (all P<0.01). (3) NRS score at 30 min after acu puncture in the control group was obviously lower than that in the observation group (P<0.05), but the scores in 2 h, 4 h, 6 h and 8 h after acupuncture in the observation group were lower apparently than those in the control group (all P<0. 01). The differences in the scores in 5 min, 10 min, 20 min and 1 h after acupuncture were not significant (all P>0. 05). (4) For the reducing amplitude of NRS score at the same time point after acupuncture, the results in 20 min, 30 min and 1 h in the control group were higher apparently than those in the observation group (all P<0. 05). But, in 2 h, 4 h, 6 h and 8 h, the results in the observation group were higher apparently than those in the control group (all F<0.05). The differences in 5 min and 10 min after acupuncture were not significant (both P>0. 05). (5) In 24 h after acupuncture, the recurrence rate of migraine was 12. 7% (7/55) in the observation group, which was lower obviously than 34. 5% (19/55) in the control group (P<0. 05). The adverse reaction was not found in the two groups.
CONCLUSIONEA at the shaoyang meridian points achieves the definite immediate analgesia effect on acute migraine and presents the time effectiveness of analgesia.

10.Subxiphoid versus intercostal video-assisted thoracoscopic extended thymectomy for myasthenia gravis: A retrospective cohort study

Objective To compare the clinical efficacy of subxiphoid video-assisted thoracoscopic surgery (XVATS) and conventional intercostal VATS (CVATS) extended thymectomy for myasthenia gravis (MG). Methods The clinical data of MG patients who underwent extended thymectomy in the Department of Thoracic Surgery of Xuzhou Central Hospital from October 2016 to October 2021 and finished the follow-up were retrospectively reviewed. They were divided into an XVATS group and a CVATS group according to the procedure. The perioperative variables and clinical efficacy of the two groups were compared. Results A total of 84 patients were collected, including 43 males and 41 females, with a mean age of 52.3 years. There were 41 patients in the XVATS group and 43 patients in the CVATS group. There was no mortality, cardiopulmonary thrombosis, prolonged air leak, or mediastinal infection. Additionally, the CVATS group recorded 5 (11.6%) patients of conversion to open thoracotomy, 1 (2.3%) patient of postoperative MG crisis, 1 (2.3%) patient of bleeding in thorax, and 1 (2.3%) patient of chylothorax. The operation time (127.4±50.4 min vs. 122.9±38.6 min), intraoperative bleeding [46.9 (25.7, 79.2) mL vs. 45.7 (21.9, 92.1) mL], incidence of complications [0 vs. 7.0% (3/43)], chest tube duration (4.3±1.9 d vs. 4.8±2.8 d), follow-up time (19.1±8.5 months vs. 22.5±13.7 months), the proportion of residual mediastinal fat tissue [12.2% (5/41) vs. 4.7% (2/43)], and total MG remission rate [29.3% (12/41) vs. 51.2% (22/43)] were not statistically different between the two groups (P>0.05). However, the two groups showed significantly different incidence of conversion to open thoracotomy [0 vs. 11.6% (5/43), P=0.024], postoperative hospital stay time (8.2±3.3 d vs. 11.4±5.8 d, P=0.003) and total drainage volume [396.7 (173.8, 542.5) mL vs. 218.8 (102.1, 430.0) mL, P=0.038]. Conclusion XVATS extended thymectomy is technically safe and feasible; however, more evidence is warranted before the recommendation of this approach for the treatment of MG.
